[Effect of Astragalus Angelica mixture on serum lipids and glomerulosclerosis in rats with nephrotic syndrome].

OBJECTIVE: To confirm the renal protective action of Astragalus Angelica mixture (AAM). METHODS: Effect of AAM was observed in puromycin amino-nucleoside induced nephrotic syndrome model of rat, and pravachol, an effective lipid lowering medicine, was used as positive control. RESULTS: AAM could not only reduce the serum levels of total cholesterol and triglyceride as pravachol did, but also could lower levels of low density lipoprotein and apolipoprotein B100 (P < 0.05). Light microscopic and immuno-histochemical examination revealed that in the same time of lowering serum lipids, this mixture could also cause reduction of glomerular sclerosing index (P < 0.01) and less accumulation of extracellular matrix (P < 0.01). CONCLUSIONS: AAM has the effects of regulating lipid metabolism, attenuating renal damage and maintaining renal function in rats with nephrotic syndrome.

[Role of substance P in pressor response of lateral hypothamus-perifornical region to glutamate].

Substance P(SP)-immunoreactive cell bodies, axon terminals and SP receptors were widely scattered in most pressor areas. Lateral hypothalamus (LH) contains SPergic neurons; and SPergic fibers-SP receptors are present in LH projection areas; hence the role of SP in LH-pressor response was studied. The results showed that: (1) microinjection of glutamate into the lateral hypothalamus-perifornical region (LH/PF) or SP into LH projection areas: the locus coeruleus (LC), periaqueductal gray matter (PAG) or nucleus parabrachialis (NPB), respectively, all could induce pressor response; (2) preinjection of DPDPDT (a substance P antagonist) into the LC, PAG (but not NPB) reduced the LH/PF pressor response; (3) intra-RVL (rostral ventrolateral medulla) injection of phentolamine, propranolol or atropine also markedly attenuated the LH/PF pressor response. Taken together with our previous findings that alpha-, beta- and M-receptors in the RVL mediated the pressor response to excitation of LC, and alpha-, beta-receptors mediated the PAG-pressor response, it is suggested that the LH/PF pressor response may be brought about via the activation of LC- and PAG-RVL systems by SPergic fibers from the LH/PF.

[Treatment of sensorineural hearing loss with delayed onset of vertigo (report of 13 cases)].

Sensorineural hearing loss with delayed onset of vertigo (SHL-DV) is a syndrome consisting of preexisting SHL and subsequent episodes of vertigo. The possible underlying pathophysiology is delayed endolymphatic hydrops. This paper details our clinical experience with 8 patients with SHL-DV caused by otitis media and 6 patients following sudden deafness and hearing loss due to an unknown cause. The average period of onset of vertigo after the occurance of the hearing loss was 22.6 years. The average period of vertigo was 6.2 years. Surgical treatment was performed in 13 patients (14 ears) because of disabling vertigo. Endolymphatic sac decompression was performed in 2 ears, transmastoid labyrinthectomy in 3 ears, gentamycin perfusion of the tympanic cleft in 3 ears, streptomycin perfusion of the labyrinth in 6 ears. Follow-up for 13 patients who had surgery averaged 2.3 years, all patients were cured of their episodic vertigo. Three patients underwent surgical labyrinthectomy suffering with total hearing loss, but aminoglycoside antibiotics induced labyrinthectomy preserving some hearing. We conclude that chemical labyrinthectomy is the therapy of choice for the severe SHL-DV who has serviceable hearing.

[Fluconazole versus ketoconazole in systemic fungal infection: a double-blind randomized study].

We carried out a multiple-center study, including a double-blined randomized clinical trial on fluconazole (Flu, group A) and ketoconazole (Keto, group B) and an open trial on Flu only for evaluating the efficacy and safety of Flu in treating deep and shallow fungal infection. 222 patients participated in the study and most of them had severe underlying diseases. The dosage and therapeutic duration were as follows: Flu 200-400 mg daily for 1-8 weeks in 34 patients with fungal infection and 150 mg as a single dose in 30 patients with fungal vaginitis; Keto 400 mg daily for 1-8 weeks in 30 patients with fungal diseases and for 5 days in 30 patients with fungal vaginitis. In the trial 124 patients were randomized to receive either Flu (64 patients) or Keto (60). The cure rate in group A and B was 81.3% (52/64) and 58.0% (35/60) respectively (P < 0.05). The fungal eradication rates of the two groups were 85.7% and 70.0% (P > 0.05) respectively. 98 patients entered the open trial. They included fungal infection of the respiratory tract and urinary tract, cryptococal meningitis, fungal sepesis and systemic dissemination of mycosis and fungal vaginitis. 84 (85.7%) were cured. The fungal eradication rate of this group was 92.9%. The side-effect rates of the three groups were 3.1% (2/64), 5.0% (3/60) and 6.1% (6/98) respectively. They were mild and transient vomiting, nausea, and anorexia. In group B and the open group however, there was one case of ALT elevation in each group (P > 0.05). This study suggests that Flu is a safe, effective and useful drug for the treatment of severe fungal infection.

[Lipid-lowering effect of pravastatin on glomerulosclerosis protection and treatment].

The lipid-lowering effect of pravastatin on glomerulosclerosis was observed in nephrotic syndrome model, which was induced in rats by using repeated puromycin injections. The histologic changes of the model were similar to those of human focal segmental glomerulosclerosis. Rats were divided into three groups, namely, normal control, nephropathy and nephropathy with pravastatin. Pravastatin was administered through gastric tube 6mg.kg-1.d-1 for 12 weeks. The lipoprotein profile and histologic development were observed. The results showed that pravastatin-treated rats had significantly lower cholesterol, triglycerides, very low density lipoprotein and high density lipoprotein than the nephropathy group at 7 weeks (P < 0.05). Moreover, the renal damage was also less marked in pravastatin-treated rats. Light microscopic and immunohistochemical examination revealed that lipid-lowering therapy was associated with significant lower level of glomerular sclerosing index and less accumulation of extracellular matrix including collagen III, IV, laminine and fibronectin as compared with the nephropathy group. However urinary protein and serum albumin levels were similar in both groups. It is suggested that lipid-lowering thyrapy may retard the progression of glomerulosclerosis and thus preserve renal function. The results imply that hyperlipoidemia plays an important role in the pathophysiological progression of renal disease.

[Relationship between insulin resistance and coronary heart disease in non-insulin-dependent diabetes mellitus patients].

Recent clinical studies showed that an independent association between hyper-insulinaemia and coronary heart disease (CHD) in men with normal glucose tolerance. In non-insulin-dependent diabetes mellitus (NIDDM) it is less clear. Therefore, we evaluated insulin-sensitivity index, plasma glucose, insulin, lipoproteins and apolipoproteins in 40 male NIDDM patients with CHD, and compared them with 36 male NIDDM patients without CHD. The insulin-sensitivity index determined by the reverse of fasting plasma glucose and insulin product. The results showed that the subjects with CHD had significantly lower insulin-sensitivity index (P < 0.01) compared to those without CHD. Using step-wise logistic regression analgsis, we found insulin-sensitivity indexes (OR 0.237, 95%CI 0.0909-0.6167, P = 0.0032) were negatively associated with CHD, and were independent from other cardiovascular risk factors such as age, body mass index, hypertension, and hyperlipemia. We conclude that in NIDDM patients with CHD are more insulin resistant compared to those without CHD. Insulin resistance is associated with CHD, and independent from other cardiovascular risk factors.

[Splenic artery embolization for the treatment of traumatic splenic rupture].

We used splenic artery gelfoam embolization to save the spleen Seldinger's method in 20 patients with traumatic rupture of the spleen. All patients had the spleen reserved by single embolizing hemostasis satisfactorily. We delineate the method, clinical value, indication and experience of embolism-hemostasis in detail.

[The effect of epithelium on inhibition of histamine-induced contraction of guinea-pig isolated trachea by potassium channel opener Rp 52891].

To evaluate whether the relaxant effect of Rp 52891 on isolated normal or sensitized guinea-pigs trachea is influenced by the removal of airway epithelium, twenty-four normal and sensitized guinea-pigs were randomly divided into two groups: epithelium-intact trachea and epithelium-denuded trachea. The effect of Rp 52891 on contraction elicited by histamine in epithelium-intact and epithelium-denuded trachea was investigated, and it was compared with that of aminophylline or isoprenaline. The results revealed that there was no significant difference in histamine-induced contractile response between normal and sensitized isolated trachea. The inhibitory effect of Rp 52891 on the contractile response of histamine was significantly reduced by removing the epithelium but that of aminophylline or isoprenaline was not changed. These results suggested that Rp 52891 may directly relax the airway smooth muscle of guinea-pigs in vitro, the epithelium of guinea-pigs trachea may modulate the relaxant effect of Rp 52891.

[Detection of ras gene mutation in various stages of gastric cancer by PCR/RFLP SSCP and DNA sequencing].

Investigate mutation of ras gene family in various stage of gastric cancer in China. PCR-RFLP, PCR-SSCP and PCR-DNA sequencing were used to detect mutation rates of H-ras, K-ras and N-ras gene. Mutation rates of H-ras at 12 codon in metaplasia, atypical hyperplasia, and progressive gastric cancer is 16.7% (6/36), 31.2% (15/48), 34.7% (25/72), respectively. In groups of superficial gastritis and normal control, no mutation were found. Mutations of H-ras 61 codon and N-ras 12 codon in various groups were the same as normal. Only 2 cases of K-ras 12 codon mutation were detected in gastric cancer by PCR-SSCP, but it was not identified by DNA sequencing. It may be of polymorphism. All H-ras 12 codon mutation were G-->T mutation. There are significant difference between groups of metaplasia, dysplasia, and gastric carcinoma comparing with group of normal control (P < 0.05, P < 0.01, P < 0.01). H-ras 12 codon mutation maybe an early event and maybe play important role in gastric carcinogenesis. Although K-ras mutation rate is high in colon cancer and leukemia it seemed to be relationship with gastric cancer. High frequency of H-ras 12 codon mutation maybe the characteristic of gastric cancer and associate with high incidence of gastric cancer in China. Three methods used in this experiment were compared that SSCP method is more sensitive than RFLP and cold SSCP is simpler and likely to be used in clinic.

[The effects of adrenergic antagonists on epinephrine determined by the assay of adenosine 3',5'-monophosphate in bovine trabecular cells in vitro].

OBJECTIVE: To compare and assay the effects of various beta-adrenergic antagonists at effective concentrations on epinephrine (EPI). METHOD: Cyclic adenosine 3',5'-monophosphate (cAMP) in trabecular cells was measured by protein binding assay with 3H-cAMP. RESULTS: (1) The effect of EPI increasing cAMP in bovine trabecular cells (BTC) in vitro did not obviously recover at the fifth hour after the removal of 10(-5) mol/L timolol, and partly recovered at the fifth hour after the removal of 10(-6) mol/L timolol. (2) There was only part recovery for the effect of EPI at the fifth hour after the removal of ICI 118, 551 (10(-5) mol/L, and 10(-6) mol/L). (3) There was an obvious recovery for the effect of EPI at the fifteenth minute after the removal of 10(-5) mol/L bisoprolol. CONCLUSION: The effect of timolol and ICI 118, 551 for blocking EPI is strong and sustained; bisoprolol produces a smaller blockade to the effect of EPI, lasting a short time. Our results also presume that BTC contains beta-adrenergic receptors, and beta 2-adrenergic receptors are dominant.

[Study on the inclusion compound of phyllostachysin A and DMF].

The crystal structure of inclusion compound phyllostachysin A[1] and DMF, C20H26O6.C3H7ON, was determined by X-ray diffraction. A colorless block crystal with dimensions of 0.6 x 0.6 x 0.8 mm was used for analysis. It shows orthorhombic, and it's space group is P2(1)2(1)2(1) with a = 10.895(3), b = 13.237(2), c = 15.373(3) A, V = 2217.0(3) A3 Z = 4, Dc = 1.302 g cm-3. Direct method was applyed to determine the structure, and the parameters were refined by least square method. The final Rf factor is 0.060 and Rw = 0.083. The result shows that the crystal is formed by Phyllostachysin A and DMF by inclusion interaction.

[Studies on all-spectrum analysis for X-ray diffraction of Chinese herbal medicine calculus bovis].

Investigation on famouse Chinese herbal medicine-Niu huang (calculus bovis) was carried out by all-spectrum X-ray diffraction analysis. Diffraction spectrums, as well as the specific symboling peaks of calculus bovis, artificial bezoar, bile ductstone, human gallstone and hog gallstone, were recognized. The error distribution curves of d-delta d for specific symboling peaks was also obtained by calculation under diffrent testing conditions, by which we identified successfully three samples provided by a pharmaceutical factory. This article shows that all-spectrum X-ray diffraction analysis can be used to identify Chinese traditional crude drug, and provides for morphological and microscopical study.

[Ameloblastic carcinoma and malignant amelobastoma: histological reunderstanding].

Twenty-four cases of malignant ameloblastoma were reported, among them were nineteen cases followed up for 5 to 30 years. Histologic characteristics include predominated proliferation of the peripheral cells with papillary protruding to stroma, replacement of the stellate reticular cells in the follicular center by sarcomatous cells, as well as large and deep-stained or alveolar nuclei with moderate atypia, and high mitosis of the tumor cells. Clinico-radiologically, the tumor showed malignant features such as fast growing recently, pain, anesthesia, cauliflower ulcer, limited mouth opening, and large multilocular radiolucent area with ill-defined border. The biological behavior, nomenclature of the ameloblastic carcinoma or malignant ameloblastoma, and differential diagnosis from other tumors were also discussed.

[The effects of pressure on cultured bovine trabecular meshwork cells].

OBJECTIVE: To observe the effects of pressure on trabecular meshwork cells. METHODS: Bovine trabecular meshwork cells were cultured and submitted to different amounts of hydrostatic pressure. Cellular morphology and phagocytic function were observed under inverted phase-contrast microscope, light microscope and electron microscope. RESULTS: Compared with the control group, the cells under 2.0 kPa or 2.67 kPa for 48 hours had no remarkable difference in criteria observed. Those under 4.0 kPa for 24 hours showed slight changes in structure and a mild decrease in phagocytic function. The damage appeared more severe if the pressure was higher or lasted longer. CONCLUSION: Trabecular meshwork cells can only bear pressure below a certain level. They may be destroyed structurally or impaired functionally by pressure over this level.

[Molecular cloning of the imipenem resistant gene in bacteroides fragilis].

Imipenem, as a representative of-cabapenem, is one of the most effective beta-Lactam agents against Bacteroides, but recently there have been a few reports on the resistant strains of Bacteroides. To explore the mechanism of imipenem resistance (IMPr) at the molecular level, we extracted chromosome DNA of the resistant strains and acquired a 1.7 kb fragment of resistant gene of imipenem. Recombinant plasmid was stably reserved in generation after generation in E. coli DH5. Resistant gene of imipenem was labeled with Dig-11-dUTP by way of random prime, and 40 strains of clinically isolated Bacteroides were detected by the prepared DNA probe. Two IMPr strains showed hybridization signal, whereas 38 IMPr strains did not show such a signal. The result suggests that the Dig-labeled IMPr gene probe can be used in molecular epidemiology for investigating IMPr strains.

[Relationship of primary pulmonary carcinoma with P16 protein expression].

This study sought to address the relationship of primary pulmonary carcinoma (PPC) with P16 protein expression. The expression of P16 protein in 65 cases of PPC was examined with immunohistochemical staining. The rate of loss of P16 protein expression in PPC was 36.92% +/- 18.45%. The rate of loss of P16 protein expression in adenocarcinoma (AC) was 28.47 +/- 16.33%, that in squamous cell carcinoma (SCC) was 35.95% +/- 17.36%, and that in adenosquamous cell carcinoma (ASCC) 57.88% +/- 10.18%. There significant differences were in the rate of loss of P16 protein expression between those aged < 60 with SCC and those aged > 60 with SCC, between low differentiated PPC and intermediate differentiated PPC, between AC and ASCC, and between SCC and ASCC (P < 0.05). There were no significant differences in the rate of loss of P16 protein between males and females with PCC, between left and right PPC, and between PPC and metastatic pulmonary carcinoma of lymph node (P > 0.05). These suggest that P16 protein expression may be one of the important standards in estimating the prognosis of patients with PCC.

[Expression of metastasis-suppressor gene nm23H1 product in nasopharyngeal carcinoma].

Using the specific monoclonal antibody of nm23H1 gene product and the immunohistochemical technique, we studied the expression of DNPK/nm23H1 and its correlation with lymph-node metastasis in nasopharyngeal carchnoma (NPC). The results revealed that, in a total of 31 cases of NPC, 13 showed positive expression with a positive rate of 41.9%. Among 21 cases of NPC with no lymph-node metastasis, 11 (52.3%) showed positive expression while only 20% (2/10) of those with lymph-node metastasis were positive, and there was significant difference between them (P < 0.01). Meantime the expression was very low or not detectable in the cancerous cells of the match lymph-node metastasis and all 10 cases showed negative expression. Statistical significant difference was found between the group of NPC with no metastasis and the group of lymph-node metastasis (P < 0.01). The above results indicated that high expression of DNPK/nm23H1 existed in the group of NPC with no metastasis while low expression existed in those with metastasis and lymph-node metastasis. It suggests that the expression of nm23H1 is inversely associated with lymph-node metastasis. The DNPK/nm23H1 may play a role in polymerization of microtubulin protein and participate in the process of cancer metastasis.
