[Clinical features, diagnosis and treatment of solitary vertebral arch tuberculosis].

OBJECTIVE: To explore clinical features of vertebral arch tuberculosis, and to discuss the diagnosis and treatment of this illness. METHOD: Seventeen of 3,825 cases of spinal tuberculosis who were hospitalized and treated from 1956 to 1996 were reviewed, and their clinical features, diagnosis and treatment were analyzed and reported. RESULT: Sixteen cases of vertebral arch tuberculosis were in adolescence and prime of life, and another one was a 2-year-old child. Their symptoms in early stages were untypical, including local fixed pain and tenderness, and later, abscess, fistula and symptoms of nerve system appeared. All the cases were diagnosed through summarizing of their history, signs, imaging examination and bacteriology tests, and only one case was misdiagnosed. In one of 17 cases, antituberculosis chemotherapy was conducted alone and non-operation was performed. In the other 16 cases, focal debridement of vertebral arch was performed on the basis of antituberculous chemotherapy, and in 4 cases of them with symptoms of nerve system, laminectomy and spinal decompression was performed homochronously. All the cases were healed and no relapse occurred during follow-up. CONCLUSION: It is difficult to diagnose vertebral arch tuberculosis in early stages due to its occult symptoms, but the symptom of confined pain and manifestation in X-ray, CT and MRI are much useful for early diagnosis. Abscess puncture and acid-fast staining smear microscopy can usually be used to confirm diagnosis. And surgical intervention is an important measure for its treatment.

[The value of cephalometric analysis in diagnosis and treatment of obstructive sleep apnea syndrome].

OBJECTIVES: To study the value of cephalometric analysis in the diagnosis, treatment option and evaluation of prognosis. METHODS: A detailed cephalometric analysis was performed using lateral X-ray films from 50 cases with obstructive sleep apnea syndrome (OSAS) and 30 age- and sex-matched controls. RESULTS: Statistical results showed that OSAS patients were different from controls in the following aspects: 1. Their uvula, soft palate and tongue were significantly enlarged. 2. The hyoid bone was displaced inferiorly and posteriorly. 3. The posterior airway space (palatopharyngeal plane) was narrow. 4. The hard palate was elongated. 5. The mandibular body was retropositioned. Fifty unselected consecutive OSAS patients(mean AHI 51.42 +/- 3.96, Medilog SAC 847, Oxford UK) underwent LAUPP using CO2 laser. Twenty-five cases were followed-up for 4-8 months. 10/25 had obtained a reduction in AHI > 50%, 15/25 had AI < 20 times. The success rate was compared with cephalomentric findings. CONCLUSION: Cephalometry has an important diagnostic and predictive value in the clinical investigation of patients suspected of having OSAS.

[Maxillary sinus lifting, bone graft, and simultaneously placement of implants].

OBJECTIVE: To study the effect of maxillary sinus floor lifting by bone graft for immediate implantation. METHODS: 12 maxillary sinus underwent sinus floor bone graft and simultaneously 24 implants olacement. The mean time of follow up was 14 months. RESULTS: There were no implant loose or lost, no clinical complaint of the maxillary sinus area, and X-ray exam showed well osseointegration in 6 months postoperatively. Abnormal sensitivity of chin area were found in 8 patients after bone graft taken from chin, but the sensitivity became normal spontaneously in 3-12 months postoperatively. CONCLUSION: Successful maxillary sinus floor bone graft with simultaneously placement of implant is predictable.

[Prognostic value of tumor vascularity in gastric carcinoma].

OBJECTIVE: To determine whether tumor angiogenesis correlates with progression of gastric carcinoma. METHOD: Microvessel counts (MVC) of 131 specimens resected from patients with gastric carcinoma were investigated by immunohistological staining with a monoclonal antibody against F-VIII RAg, counting the mean number of microvessels in the five areas of highest vascular density under 200 times magnification microscopy. Correlation among MVC, various clinicopathologic factors, and prognosis was studied. RESULT: MVC increased with TNM stage, the MVC of patients with advanced stage disease was significantly higher than that of early stage patients (P < 0.01). MVC was significantly higher in tumors with deep penetration (P < 0.05), lymph node metastasis (P < 0.001) and distant metastasis (P < 0.05) than in those without invasion and such metastasis. The recurrence rate after curative resection in hypervascular group (MVC > or 16) was 58.4%, which was significantly higher (P < 0.05) than that in the hypovascular group (MVC < 16). Moreover, the prognosis of patients with a high MVC was significantly poorer than that of those with a low MVC. The probability of 5 year survival rate was estimated by P = e(1.6231-0.1036 x MVC)/1 + e(1.6231-0.1036 x MVC). Multivariate analysis indicated that MVC is an independent prognostic factor in gastric cancer. CONCLUSION: Angiogenesis within gastric cancer is an indicator of tumor behavior and may identify patients at higher risk for recurrence and poorer prognosis.

[Extraction and amplification of DNA from formalin-fixed human tissues].

A method was developed for the extraction of DNA from formalin-fixed human tissues prepared for routine histopathological examination. The extracted DNA was not intact. By increasing the formalin fixation time the necessary amount of intact DNA decreased. The procedure reported here made the final product suitable for quantitative and qualitative analysis and for DNA-typing by PCR technique.

[PCR-based detection of HLA-DQ alpha and polymarker loci and their application in forensic science].

Studies were conducted to evaluate the forensic applicability of multiplex amplification of HLA-DQ alpha and PM loci. These loci could be simultaneously typed by reverse dot blot approach where allele specific oligonucleotide probes were immobilized on a nylon membrane strip. Allele and genotype frequencies for HLA-DQ alpha and PM loci were determined in Chinese Han population. The frequency data can be used in individual identification and paternity test.

[PCR-based analysis of the D1S80 locus and its application in paternity test].

Allelic data for the D1S80 locus was obtained by using the PCR and subsequent analysis with high-resolution horizontal PAGE technique. In a sample of 184 unrelated Chinese, the D1S80 locus had 75 phenotypes and 20 allelic. The distribution of genotype was in agreement with expected values according to the Hardy-Weinberg equilibrium. Analysis of the D1S80 locus by the PCR has been successfully applied in 150 cases of questioned paternity.

[The design of a circuit for detecting respiration signal with impedance method].

A respiration signal detecting circuit with impedance method is introduced. In this circuit, a four electrode arrangement is used, which can reduce movement artifact in using two electrodes. In signal processing, a series of methods such as filtering and amplyfing are used to overcome the heart beat and bloodflow signals. The circuit has successfully been applied in the blocking physiological parameters monitor that we have developed.

[Isolation, purification and physico-chemical properties of immunoactive constituents from the fruit of Lycium barbarum L].

Three glycoconjugates, LbGp3, LbGp4 and LbGp5, were isolated from the fruit of Lycium barbarum L. Molecular weights of LbGp3, LbGp4 and LbGp5 were 9.25 x 10(4), 21.48 x 10(4) and 2.37 x 10(4), respectively. Carbohydrate contents of LbGp3, LbGP4 and LbGp5 were 93.6%, 85.6%, 8.6%, respectively. LbGp3 was composed of Ara and Gal in a molar ratio of 1:1. LbGp4 was composed of Ara, Gal, Rha and Glc in a molar ratio of 1.5:2.5:0.43:0.23. LbGp5 was composed of Rha, Ara, Xyl, Gal, Man and Glc in a molar ratio of 0.33:0.52:0.42:0.94:0.85:1. Elemental analysis of N contents: LbGp3 0.83%, LbGp4 1.72%, LbGp5 9.58%. The linkage between the glycan and protein may be of O-linkage in LbGp4.

[Influence of dextran 40 and dextran 70 on the adhesion of erythrocytes to endothelial cells].

Adopting the flow chamber realtime microrecordation system, we studied the dynamic effects of dextran 40(DX40) and dextran 70(DX70) on the adhesion of erythrocytes to monolayer of cultured human umbilical vein endothelial cells (HUVEC). The results demonstrated that erytherocytes pretreated with DX40 adhered to HUVEC less than the erythrocytes of the control group did, but DX70 increased the number of erythrocytes adhering to HUVEC. The higher the concentration of DX70 was, the more the erythrocytes pretreated with DX70 adhered to HUVEC. When shear stress increased, the decrease rate of the number of erythrocytes (pretreated with DX70) adhering to HUVEC declined with the increase of DX70 concentration. These indicate that DX40 as an expander of blood plasma is better than DX70.

Hemodynamics of ophthalmic artery and central retinal artery and correlation with other factors in patients with primary open angle glaucoma.

OBJECTIVE: To investigate the hemodynamics of ophthalmic artery (OA) and central retinal artery (CRA) in patients with primary open angle glaucoma(POAG) and analyse the correlation between hemodynamics of OA and CRA and other factors. METHODS: The hemodynamics of OA and CRA in normal persons (102 eyes) and glaucomatous patients (102 eyes) were measured by Colour Doppler Image (CDI). The linear correlation analysis was made between OA and CRA about each of the following parameters: the peak systolic flow velocity (Vmax), the end diastolic velocity (Vmin). The multiple stepwise regression analysis was taken to investigate the correlation between each of the following hemodynamics of OA and CRA: Vmax and Vmin in 74 eyes with POAG and each of the following related factors in hemorrheology: whole blood apparent viscosity at low, medium and high shear rate, plasma viscosity and hematocrit. The multiple linear regression was applied to investigate the correlation between each of the following hemodynamics of OA and CRA: Vmax and Vmin in 69 eyes with POAG and each of the following blood vessel filling time: the arm-retinal artery (A-AT) and retinal artery-venous (A-VT) of the fundus fluorescein angiography(FFA). RESULTS: The Vmax, Vmin and time-averaged maximum velocity (Vmean) of OA and CRA in patients with POAG were lower than those in normal persons, but the resistance index (RI) of OA and CRA was higher than those in normal. There was a positive correlation between OA and CRA in Vmin, but a negative correlation between Vmin of OA and plasma viscosity. The whole blood apparent viscosity at low shear rate was clodely related to Vmax and Vmin of CRA. Also there was a negative correlation between Vmax of CRA and A-AT of the FFA. CONCLUSION: There was abnormal hemodynamics, which is influenced by blood viscosity, of OA and CRA in patients with POAG. Abnormal hemodynamics can affect the blood supply to optic disc and retina in POAG.

Concentrations of carotenoids, tocopherols, and retinol in paired plasma and cervical tissue of patients with cervical cancer, precancer, and noncancerous diseases.

Paired blood (collected after an overnight fast) and cervical tissue (cancerous, precancerous, and noncancerous) samples were obtained from 87 patients (age, 21-86 years) who had a hysterectomy or biopsy due to cervical cancer, precancer (cervical intraepithelial neoplasia I, II, and III), or noncancerous diseases. The samples were analyzed using high-performance liquid chromatography for 10 micronutrients (lutein, zeaxanthin, beta-cryptoxanthin, lycopene, alpha-carotene, beta-carotene, cis-beta-carotene, alpha-tocopherol, gamma-tocopherol, and retinol). The results indicated that: (a) among the three patient groups, the mean plasma concentrations of all micronutrients except gamma-tocopherol were lowest in the cancer patients; however, the mean tissue concentrations of the two tocopherols and certain carotenoids were highest in the cancerous tissue; and (b) among the 10 micronutrients, only the concentrations of beta-carotene and cis-beta-carotene were lower in both the plasma and tissue of cancer and precancer patients than in those of noncancer controls. These results suggest that: (a) not all of the micronutrient concentrations in plasma reflect the micronutrient concentrations in cervical tissue; thus, in some cases, it may be necessary to measure the tissue micronutrient concentrations to define the role of the micronutrients in cervical carcinogenesis; and (b) maintaining an adequate plasma and tissue concentration of beta-carotene may be necessary for the prevention of cervical cancer and precancer.

[Telomerase activity in association with the pathological differentiation of prostate cancer].

We analyzed the telomerase activity in association with the pathological differentiation of prostate cancer. Furthermore, to verify whether the detection of telomerase in prostate needle biopsy samples could be valuable for diagnosis, we assayed the telomerase activity in needle biopsy samples. Telomerase activity was examined by PCR-based telomeric repeat amplification protocol (TRAP) assay. Among 31 primary prostate cancer tissues, 28 samples (90%) displayed telomerase activity, in which of relative levels were associated with pathological differentiation. High activity of telomerase in primary and biopsy samples was more frequently detected in poorly differentiated adenocarcinoma. These studies suggest that determination of telomerase activity in prostate cancer might be an useful examination for detecting prostate cancer and evaluating the biological malignant potentials.

Newly developed diabetic retinopathy and its preceding changes in biological markers.

To disclose the chronological changes prior to the manifestation of diabetic retinopathy (DR), we analyzed the time course of biological markers among apparently healthy diabetic subjects in a case-control study of 21,579 adults who had undergone comprehensive health examinations for > or = 10 years. We identified 54 cases who had newly developed DR, and selected 108 adults without fundus abnormalities, matching them for sex, age, and fasting plasma glucose (FPG) at the onset of the patient group's retinopathy as a referent group from the same population. In a multivariate analysis, a high average FPG (> 175 mg/dl) and a final-year FPG reduction (< -3%) were significantly associated with a 5.4 (95% CI, 1.8-15.7)- and 5.0 (95% CI, 1.0-24.7)-fold increased risk of DR, respectively. Thus, we surmised that sustained hyperglycemia and a subsequent drop in FPG might promote retinopathy in non-insulin dependent diabetes mellitus.

Contribution of activated macrophages to the process of delayed xenograft rejection.

BACKGROUND: When hyperacute rejection, involving natural xenoreactive antibodies (XAb) and/or complement (C), can be prevented, xenografts (Xgs) undergo delayed xenograft rejection associated with a progressive mononuclear cell infiltration. We have previously shown that XAb formation can be totally suppressed in leflunomide (LF)-treated, T-deficient nude rats receiving hamster hearts. Hence, this model was well-suited to study a role played by other factors, e.g., natural killer (NK) cells and macrophages (Mphi). The relative contribution of Mphi to delayed xenograft rejection was investigated. METHODS: In addition to LF (20 mg/kg/24 hr p.o.), anti-asialoGM-1 serum (1 mg/48 hr i.v.) and N omega-nitro-L-arginine methyl ester (L-NAME; 100 mg/kg/24 hr i.v.) were given. Graft-infiltrating cells, deposition of cytokines (interferon-gamma [IFN-gamma] and tumor necrosis factor-alpha [TNF-alpha]), IgM and C, and expression of endothelial cell (EC) P- and E-selectins were investigated by immunohistochemistry. In some cases, rat rTNF-alpha or anti-TNF-alpha antibodies were injected intravenously. RESULTS: Xgs rejected after 3 days by LF-treated rats showed an absence of IgM, C, and T cells, but the infiltration of NK cells and Mphi, together with the presence of IFN-gamma and TNF-alpha. Addition of NK cell depletion resulted in a significantly prolonged survival of Xgs (6 days; P<0.001) in which NK cells and IFN-gamma had disappeared, but Mphi were still prominent. Additional blockade of Mphi nitric oxide (NO) with L-NAME further prolonged Xg survival (11 days; P<0.001). In these rejected Xgs, Mphi, TNF-alpha, and EC expression of P- and E-selectins was still found, together with platelet thrombi, neutrophil-EC adhesion, and vessel intima lesions. The role of TNF-alpha in initiating this Xg rejection was further demonstrated by the acceleration of Xg rejection after injection of rTNF-alpha and by a synergism between L-NAME and anti-TNF-alpha antibodies in hampering the acceleration of Xg rejection seen after transfer of sensitized Mphi. CONCLUSION: In the absence of XAb, T cells, and NK cells, Mphi can still reject Xgs. Both NO-dependent and NO-independent mechanisms are involved. In the latter case, Mphi-derived, TNF-alpha-associated EC activation may play a role.

Impaired secretion of very low density lipoprotein-triglycerides by apolipoprotein E- deficient mouse hepatocytes.

To explore mechanisms underlying triglyceride (TG) accumulation in livers of chow-fed apo E-deficient mice (Kuipers, F., J.M. van Ree, M.H. Hofker, H. Wolters, G. In't Veld, R.J. Vonk, H.M.G. Princen, and L.M. Havekes. 1996. Hepatology. 24:241-247), we investigated the effects of apo E deficiency on secretion of VLDL-associated TG (a) in vivo in mice, (b) in isolated perfused mouse livers, and (c) in cultured mouse hepatocytes. (a) Hepatic VLDL-TG production rate in vivo, determined after Triton WR1339 injection, was reduced by 46% in apo E-deficient mice compared with controls. To eliminate the possibility that impaired VLDL secretion is caused by aspecific changes in hepatic function due to hypercholesterolemia, VLDL-TG production rates were also measured in apo E-deficient mice after transplantation of wild-type mouse bone marrow. Bone marrow- transplanted apo E-deficient mice, which do not express apo E in hepatocytes, showed normalized plasma cholesterol levels, but VLDL-TG production was reduced by 59%. (b) VLDL-TG production by isolated perfused livers from apo E-deficient mice was 50% lower than production by livers from control mice. Lipid composition of nascent VLDL particles isolated from the perfusate was similar for both groups. (c) Mass VLDL-TG secretion by cultured apo E-deficient hepatocytes was reduced by 23% compared with control values in serum-free medium, and by 61% in the presence of oleate in medium (0. 75 mM) to stimulate lipogenesis. Electron microscopic evaluation revealed a smaller average size for VLDL particles produced by apo E-deficient cells compared with control cells in the presence of oleate (38 and 49 nm, respectively). In short-term labeling studies, apo E-deficient and control cells showed a similar time-dependent accumulation of [3H]TG formed from [3H]glycerol, yet secretion of newly synthesized VLDL-associated [3H]TG by apo E-deficient cells was reduced by 60 and 73% in the absence and presence of oleate, respectively. We conclude that apo E, in addition to its role in lipoprotein clearance, has a physiological function in the VLDL assembly-secretion cascade.

Circulating activated endothelial cells in sickle cell anemia.

BACKGROUND: The vascular wall participates in the pathogenesis of sickle cell disease. To determine whether the endothelium is activated in this disease, we studied the number, origin, and surface phenotype of circulating endothelial cells in patients with sickle cell anemia. METHODS: We used immunohistochemical examination of buffy-coat smears to enumerate circulating endothelial cells, and we evaluated the surface phenotype by applying preparations of circulating endothelial cells. An immunofluorescence microscopy panel of antibodies was used, including a specific anti-endothelial-cell antibody, P1H12. RESULTS: Mean (+/-SD) numbers of circulating endothelial cells in normal blood donors, patients with sickle cell trait, and patients with hemolytic anemias not due to hemoglobin S were 2.6+/-1.6, 3.0+/-2.6, and 2.0+/-0.8 per milliliter of whole blood, respectively. Patients with sickle cell anemia who presented with acute painful episodes had 22.8+/-18.2 circulating endothelial cells per milliliter of blood (P<0.001 for the comparison with normal donors), and patients with no such events within one month before or after blood sampling had 13.2+/-11.8 circulating endothelial cells per milliliter of blood (P=0.002 for the comparison with normal donors and P=0.019 for the comparison with patients with acute events). Serial observations of three patients showed a tendency toward higher levels of circulating endothelial cells at the onset of acute painful crises. The average viability of circulating endothelial cells was 66+/-30 percent. In patients with sickle cell anemia, regardless of clinical status, the circulating endothelial cells were predominantly microvascular in origin (CD36-positive), and most of the cells expressed four markers of endothelial-cell activation: intercellular adhesion molecule 1, vascular-cell adhesion molecule 1, E-selectin, and P-selectin. CONCLUSIONS: Our studies suggest that the vascular endothelium is activated in patients with sickle cell anemia, regardless of the patients' clinical status. Adhesion proteins on activated endothelial cells may have a role in the vascular pathology of sickle cell disease.
