[Bronchial hyperreactivity in patients with mitral stenosis and therapeutic effect of inhaled corticosteroids].

OBJECTIVE: To assess the role of bronchial hyperreactivity (BHR) in patients with mitral stenosis and to evaluate the effect of inhaled corticosteroids in treating these patients. METHODS: Histamine bronchial provocation testing was performed in 23 patients with mitral stenosis. Patients with BHR were randomized into beclomethasone treatment and placebo group. Patients without BHR were also treated with beclomethasone as controls. RESULTS: 15 patients showed BHR. His PD20-FEV1 were significantly higher after 6 weeks treatment (P < 0.05), and symptom scores significantly lower(P < 0.05) in beclomethasone treatment group while remained unchanged in placebo group(P > 0.05). CONCLUSIONS: BHR is commonly found in patients with symptomatic mitral stenosis. Beclomethasone can significantly decrease bronchial reactivity to histamine and improve symptoms in patients with mitral stenosis.

[Intraoral distraction osteogenesis].

OBJECTIVE: To study the application of intraoral distraction osteogenesis in correction of various dentofacial deformities. METHODS: From 1997, 12 cases with various dentofacial deformities were corrected by using 6 kind of intraoral distractor. 3 cases of them were the patients with cleft palate who have severe secondary maxillary hypoplasia. 4 cases with hemifacial microsomia, 2 cases with micrognathia and 3 cases with partial mandibular defect. The maxillae were elongated from 10 mm to 15 mm, the mandibles were elongated from 15 mm to 25 mm, the vertical mandibular distraction was from 10-15 mm. RESULTS: All cases were corrected satisfactorily and have no any complications. In 1 case the distractor did not work during distraction and was replaced. Another case the mandibular margin fractured after distraction, the rigid fixation was performed for it. CONCLUSIONS: The intraoral distraction osteogenesis provides a new method with many advantages for those dentofacial deformities that are difficult correct by using ordinary orthognathic surgery.

[Distraction osteogenesis for correction of maxillary constriction].

OBJECTIVE: To study the methods and effects of distraction osteogenesis for correction of maxillary constriction. METHODS: 10 cases (5 males, and 5 females, aged 16-33 years, average 19.6) were treated by distraction osteogenesis for correction of maxillary constriction. Le Fort I osteotomy and palatal suture osteotomy for the upper jaw were made before opening of the Hyrax screw expander. Totally four times of opening the screw a day were performed. 2 cases were expanded unilaterally. The AP head films were analyzed using Image and Database Analysis System (IDAS) before and after expansion. RESULTS: 1. Expansion of upper arch was achieved done within a few days, and the maxilla, maxillary dental base and the posterior teeth were expanded as much as 3.84 mm, 3.25 mm, and 5.61 mm respectively. 2. The new bone was formed between the opened suture in two weeks, and filled the suture in 2 to 3 months. 3. Maxillary teeth tipped bucally a little bit. CONCLUSIONS: Distraction osteogenesis rapid maxillary expansion could get a good result following Le Fort I and palatal suture osteotomy for correction of maxillary constriction.

[Correcting unfavourable intermaxillary relationship in partial edentulous area and simultaneous implant].

OBJECTIVE: This study was to evaluate the result of correcting the unfavourable intermaxillary relationship with orthognathic surgery in the partial edentulous area and simultaneous implant. METHODS: 9 cases with jaw discrepency underwent orthognathic surgery for correcting the intermaxillary relationship and simultaneous implant surgery. The orthognathic operations were Le Fort I osteotomy with mandibular SSRO in 3 cases; simple mandibular SSRO in one case, subapical osteotomy of mandibular front area in 4 case; posterior maxillary osteotomy in one case, 25 implants were simultaneously placed in the edentulos and partial edentulos area. RESULTS: Result showed all 9 cases got the normal intermaxillary relationship post-operation, and implant prosthesis 8 months later, 3 cases with implant supported partial denture, one case with implant supported overdenture, 5 cases with ceramic crowns. Follow-up means 11 months, no implant lost. CONCLUSIONS: Base on this work, the authors believe combining orthognathic surgery and implant surgery is a feasible and reliable method in correcting unfavourable intermaxillary relationship in the implant related area for indicated patients.

[Miniplate osteosynthesis of mandibular fracture].

OBJECTIVE: To evaluate the reliability and effectiveness of miniplate osteasynthesis in mandibular fractures. METHODS: 51 mandibular fractures in 31 cases treated with titanium miniplate osteosynthesis was analysed. RESULTS: No infection was found 28 cases had satisfied occlusion, 2 cases had unfavourable local malocclusion. The mouth opening of 28 cases were over 37 mm, only 2 cases under 37 mm 3 months postoperatively. CONCLUSIONS: The miniplate osteosynthesis has the advantages over conventional intermaxillary fixation and wire fixation.

[Application of distraction osteogenesis in functional mandibular reconstruction].

OBJECTIVE: To study the application of distraction osteogenesis in mandibular functional reconstruction. METHODS: Three kinds of mandibular distractors were used in 6 cases who had different mandibular defects because of tumor resection. For two cases with defect of most part of ramus specially desinged distractors were used in order to keep the normal height of the ramus. In one case the trifoci distraction principles were used and a trifoci distractor was desinged. In three cases the vertical distractors made in Germany or China respectively were used. The average age of 6 patients was 31.5 years old (from 8 to 54). RESULTS: The mandibular lengthenings in 5 cases were fully successful according to the direction and distance desinged before distraction. In one case the trifoci distraction principle was applied and a proximal 23 mm mandibular defect was corrected successfully, however, the 23 mm distal mandibular defect had not corrected because the screws for fixation of the transport block was loosened. No infection and dysosteogenesis were happened in all cases. CONCLUSIONS: Distraction osteogenesis can successfully be used in mandibular functional reconstruction and has much more advantages than traditional techniques. The most favorable time for distraction osteogenesis in mandibular reconstruction is at the same stage of tumor resection.

[The toxic effects of high-dose Bcl-2 antisense phosphorothioate oligodeoxynucleotides incubation on cell line].

OBJECTIVE: To investigate the toxic effects of high-dose Bcl-2 antisense phosphorothioate oligodeoxynucleotides (AS-PS-ODN, ASPO) incubation on HL60 cells and to understand the relationship between the dose of ASPO and the toxicity. METHODS: Cellular viability and toxicity were detected by trypan blue exclusion, colony-forming unit HL60 cells and MTT assay. The expression of Bcl-2 protein was determined by immunocytochemistry and flow cytometry analysis. The proportion of apoptosis cells was tested by acridine orange (AO) standing. The sense (PS-ODN, SPO) was chosen for the toxic control of independent sequence. The curves of dose-effect and dose-toxicity were transfered into straight line and the reguession analysis was done by computer with the SPSS soften-ware. RESULTS: When the dose of Bcl-2 PS-ODN was higher than 20 micromol/L, the non-specific effect of suppressing cell proliferation could occur. When the dose of Bcl-2 PS-ODN was more than 160 micromol/L, the non-specific effect (toxic effect) increased significantly. When the dose of Bcl-2 ASPO was higher than 80 micromol/L, the specific effect of inhibitting the expression of Bcl-2 proteion increased slowly. When the dose of PS-ODN reached 262 micromol/L, the toxic effect had no significant differences between Bcl-2 ASPO and Bcl-2 SPO. In the presence of ASPO or SPO at 160 micromol/L, the expression of Bcl-2 protein in the two groups of HL60 was decreased to 28% and 78% respectively, after 72 hours of incubation. The expression of Bcl-2 protein in the groups of HL60 which were passaged after incubation with ASPO or SPO was reinereased to 99.6% and 97.8%, respectively. CONCLUSION: Although ASPO is a kind of low-toxic drug, the toxic effects of Bcl-2 ASPO seem to be dependent on the dose increasing to a certain extent. It is clear that it can be used over a wide range of doses, however. Our results may also provide some referring concentrations of Bcl-2 ASPO for the pharmacodynamic and toxicological study in vivo in future.

[Separation of thoracopagus conjoined twins].

OBJECTIVE: To sam up the experience in diagnosis and separation of a thoracopagus conjoined twins. METHODS: Thoracopagus twin boys were admitted for evaluating possible separation and repair at age of 21 days. X-rays, CT, MRI, and ultrasound scan of torso showed separate and normal gastrointestinal and biliary tracts, little shared vasculature, but shared pericardium, diaphragm and liver parenchyma. The chest junctions were at the sternums and the most inferior two pairs of the ribs. One twin (twin A) had a protruding heart with VSD and ASD. RESULTS: The infants were separated successfully at age of 45 days. The chest defect of twin A was closed with aid of a polyester pericardial patch graft and a silicon supporter. Managing anesthesia met some difficulty in understanding of the anatomical and physiological consequences of the complex anomaly. They are had brain edema and multiple system organ failures after separation, and were cured under intensive care. They are alive and well 6 months after surgery. CONCLUSIONS: Careful preoperative evaluation proper anesthesia and surgical technique, and postoperative care are essential to the successful separation of the conjoined twins.

[A clinical study on fresh amniotic membrane transplantation for treatment of severe ocular surface disorders at acute inflammatory and cicatricial stage].

OBJECTIVE: To investigate the safety and evaluate the effect of fresh amniotic membrane transplantation to reconstruct the ocular surface with severe inflammation and scarring. METHOD: Amniotic membrane transplantation (38 cases, 46 eyes), or combined with either limbal transplantation (9 cases, 9 eyes) or lamellar keratoplasty (8 cases, 8 eyes) was performed on ocular surface burns (5 cases, 6 eyes), recurrent Mooren's ulcer (8 cases, 8 eyes) and severe symblepharon (38 cases, 46 eyes). All the patients were followed-up for 6 - 18 months (mean, 11 months). Impression cytology was examined in 21 eyes with normal tear secretion post-operatively. RESULTS: No acute rejection was observed after fresh amniotic membrane transplantation. No progressive melt and perforation were seen in five out of six eyes with burns in reconstructed ocular surfaces. Neovascularization, pseudopterygium and iris atrophy were not found during the follow-up. Visual acuity was improved in various degrees. Corneal ulceration was not noted in 8 cases (8 eyes) with frequently recurrent Mooren's ulcer. Ocular surface reconstruction was successful in 46 of 49 eyes with severe symblepharon. Symblepharon occurred in the remaining three eyes but much less than pre-transplantation in degree. Amniotic epithelia have been examined in about three months on the transplanted eyes with normal tear secretion. CONCLUSION: Fresh amniotic membrane can be used as a graft to reconstruct ocular surface. Such transplantation can effectively reduce neovascularization, fibrosis and inflammation. Our data indicated that complete removal of pathological tissues and perfect fixation of amniotic membrane graft are crucial steps in the reconstruction of ocular surface.

[Relationship between human herpesvirus 6 infection and idiopathic thrombocytopenic purpura].

OBJECTIVE: To investigate the pathogenic role of human herpesvirus 6 (HHV-6) in idiopathic thrombocytopenic purpura (ITP). METHODS: HHV-6 DNA was examined by polymerase chain reaction (PCR) in bone marrow mononuclear cells (BMMNC) of 105 ITP patients. Human cytomegalovirus (HCMV) and parvovirus B(19) DNA were also examined in some cases. Platelet-associated antibodies (PAIg) were measured by competitive ELISA in 66 ITP patients. Serum IgG titer to HHV-6 was observed by indirect immunofluorescence assay in 19 ITP patients. RESULTS: (1) HHV-6 DNA positivity was 41.0% for ITP patients, significantly higher than that for control group (P < 0.05). HHV-6 DNA positivity for adult ITP patients, especially adult chronic cases was significantly higher than that for childhood patients (P < 0.025). Positivities for parvovirus B(19) and HCMV DNA were 24.1% and 9.7%, respectively. (2) HHV-6 DNA positivity for patients with abnormal level of PAIgG was significantly higher than that for patients with normal level of PAIgG (P < 0.05). (3) Patients coinfected with HHV-6 and B(19) or HCMV had more severe symptoms or poorer prognosis. CONCLUSION: HHV-6 infection might be associated with excessive PAIgG. Coinfection with HHV-6, B(19) or HCMV may cause more severe symptoms in ITP patients.

[Validation studies on the analysis of the HLA-DQA1 locus using PCR and its application in forensic science].

Validation Studies were conducted to evaluate the forensic applicability of HLA-DQA1 locus. The locus was typed by a reverse dot blot approach and allele specific oligonucleotide probes were immobilized on a nylon membrane strip. Allele and genotype frequency for HLA-DQA1 locus was determined in shanghai Han population. The frequency data can be used in individual identification and paternity test.

[Dental caries in 5-year-old children in Yunnan in 1995].

OBJECTIVE: To investigate the deciduous tooth caries experience of 5-year-old children in Yunnan province. METHODS: Random samples of 5 year-old children from three cities and three counties of Yunnan were surveyed. RESULTS: The caries incidence of 5-year-old children in Yunnan is 75.75%. The mean dmft is 4.44. The filling rate is only 6.57%. CONCLUSION: The deciduous tooth caries incidence of 5-year-old children is high in Yunnan. There are two factors to be considered: 1. The dental health care education to the parents and teachers should be enforced. 2. In order to get the earlier diagnosis and treatment, regular examination and prevention method should be conducted.

[Cloning and expression of Cysticercus cellulosae antigen cC1 in E. coli].

OBJECTIVE: To clone and express Cysticercus Cellulosae antigen cC1 in E. coli. METHODS: cC1 cDNA fragment was cloned to BamHI and PstI sites of pGEM-3Z vector. After alteration of the restriction sites, the fragment was cloned to EcoRI and XhoI sites of pGEX-5T with a synthetic linker to construct recombinant expression vector pGEX-5T-cC1. RESULTS: The clone produced the largest yield of cC1 protein expression when incubated in 2YT culture medium for 3 h or induced by IPTG for 6 h. Detected by scanning optical densitometry, cC1 constituted 57% of the total bacterial proteins. Western blotting analysis revealed that the GST-cC1 fusion protein exhibited a specific reactive band. CONCLUSION: High level expression of Cysticercus cellulosae antigen cC1 was obtained in E. coli.

Microsurgical treatment of Mooren's ulcer.

OBJECTIVE: To evaluate effect of microsurgery of lamellar keratoplasty (LKP) on Mooren's corneal ulcer. METHODS: The effect, postoperative astigmatism, postoperative vision, ulcer recurrence of postoperation, and surgical complications of two groups of consecutive Mooren's ulcer inpatients treated respectively by non-microsurgery of LKP and microsurgery of LKP were analyzed. RESULTS: There were significant differences of the effect, postoperative astigmatism, postoperative vision, ulcer recurrence of postoperation, and surgical complications between the two groups. The effect and postoperative vision of the microsurgery-treated group were better than those of the non-microsurgery-treated group. The postoperative astigmatism, ulcer recurrence of postoperation, and surgical complications of the microsurgery-treated group were less than those of the nonmicrosurgery-treated group. CONCLUSION: Microsurgery of LKP of Mooren's corneal ulcer can greatly improve the ulcer healing and postoperative vision, and reduce the surgical complication and the ulcer recurrence of postoperation.

[Otogenic hypertrophic cranial pachymeningitis associated with edema of the temporal lobe and organic mental disorder-case report].

OBJECTIVE: Hypertrophic cranial pachymeningitis(HCP) is a rare disease which might be misdiagnosed. One typical case of otogenic hypertrophic cranial pachymeningitis (HCP) associated with edema of the temporal lobe and organic mental disorder was reported. Literatures associated with HCP were reviewed for reference. METHODS: In July, 1997, a 26-year-old man was admitted for right-sided severe headache, dizziness, nausea, vomiting, facial and abducens nerve palsy. MRI and CT revealed inflammation in the temporal bone and locally thickened dura mater with obvious enhancement as a band in the petrous apex and nearby tentorium cerebellum. The patient then underwent operation including decompression of the facial nerve, labyrinthectomy, and eradication of the inflammatory cells in the mastoid and petrous portion. Postoperatively, the clinical signs disappeared except hearing loss. Seven months later, the patient suffered from mental disorder with interrupted excitement to hit and abuse the family members. He was then treated in a psychosis hospital but no improvement could be seen. On May 13, 1998, MRI and the enhanced MRI demonstrated diffuse thickened tentorium cerebellum and dura mater in the middle cranial fossa. The inferior portion of the temporal lobe was obvious edema. The internal carotid artery was partially occluded due to the thickened wall of cavernous sinus. RESULTS: Hence, the diagnosis of HCP associated with edema of temporal lobe and organic mental disorder was established. The patient was cured by high dosage of penicillin. CONCLUSION: MRI is an essential method for diagnosis of HCP. Antibiotic was an effective treatment.

[CT anatomic measurement of the optic canal and its clinical significance].

OBJECTIVE: To probe into the normal anatomy of the optic canal and its clinical significance. METHODS: The optic canals of 200 normal subjects were examined by thin layer CT scan in axial nd coronal positions. The distance and anatomic relationship with circumferentral structure were measured. RESULTS: In average: (1) The transverse diameter of the optic canal was (3.57 +/- 0.61) mm, the longitudinal diameter was (4.82 +/- 0.38) mm, the length of the inner wall was (12.62 +/- 2.59) mm, the length of the outer wall was (10.18 +/- 2.19) mm, and the length of the optic canal was (11.46 +/- 2.35) mm. The angle between optic canal and the middle line of the skull was 34.34 degrees +/- 6.48 degrees, the angle between optic canal and the intraorbital optic was 10.69 degrees +/- 6.48 degrees. The thickness of the inner wall was (0.68 +/- 0.46) mm. It showed no significant difference between the two sides of the optic canal in diameters(P > 0.05). (2) The optic canal, the superior orbital fissure and the inferior orbital fissure were respectively shown as track, crack and gorge type in CT axial pictures, while in the coronal pictures, they were respectively shown as butterfly eyes, horizontal fissure and vertical fissure type. (3) The relationship of the posterior ethmoid sinus and the optic canal was: anterocanal(42.00%), semicanal(27.5%), whole-canal(18.75%), sella turcica(9.75%) and circumcanal(2.00%). (4) The relationship of the position of the sphenoidal sinus and the optic canal was: I degree sphenoidal sinus(20%), II degree (63%) and III degree (17%). (5) The incidence of the bony defect of the inner optic canal wall was 25.5%. The range of the defect was (5.07 +/- 2.52) mm. CONCLUSION: It's a reliable way to show the relation of the optic canal and circumcanal by CT, and it is of important value in operation.
