[Study on intractable factors in urinary tract infections--multiple regression analysis].

Intractable complicated urinary tract infections (UTI) are caused by host and/or bacterial factors which predispose to persistent infections and recurrent infections. It is still unknown what kind of factors are responsible for the intractable complicated UTI. With the increase of compromised host or cases with complicated UTI, the factors involved in intractable and recurrent UTI are diversified. Accurate diagnosis of the factors affecting the therapeutic effect will have more importance. The factors affecting the therapeutic effect was subjected to multiple regression analysis from both aspects of underlying disease in the urinary tract (complicated factors) and systemic conditions (compromised factors) in one hundred and ninety patients of complicated UTI admitted to our clinic. Sex, presence or absence of hydronephrosis and indwelling catheter and volume of residual urine as complicated factors and age, serum creatine value, peripheral neutrophil count, peripheral lymphocyte count, diabetic or not, whether the patient underwent major operation within 1 week or not, and serum albumin value, an indicator of malnutrition, as compromised factors were analyzed by multiple regression analysis. The presence of indwelling catheter, residual urine more than 50 ml and hypoalbuminemia less than 3 g/dl were the most determinant of the clinical efficacy in cases with complicated UTI. Interestingly, presence of residual urine more than 50 ml is considered an equally intractable factor with the presence of indwelling catheter. These factors proved to be important also as recurrent factors. In the host with these factors, UTI are often caused by resistant bacteria and indicated to be intractable also bacteriologically.

Bladder carcinoma producing granulocyte colony-stimulating factor.

A patient presented with grade 3 transitional cell carcinoma of the bladder showing a marked increase in granulocyte colony-stimulating factor (G-CSF). G-CSF was identified in the carcinoma by immunohistochemical procedures. The serum and urinary levels of G-CSF were persistently elevated during his clinical course. These results suggest that the carcinoma produced G-CSF and was responsible for its high levels in serum and urine.

[A questionnaire survey on urinary incontinence and urinary disturbances in the institutionalized elderly with senile dementia].

We carried out a questionnaire survey concerning urinary disturbances, among nursing home patients. The answers were obtained from 1,038 elderly including 355 males and 683 females. Ages, spanned 50-99, with an average age of 79.1. Of the 1,038 respondents which we obtained through our survey for management of urination, 35.8% of the total said that they are able to urinate without incontinence. Those able to urinate with incontinence accounted for 23.6% of the total. However, 40% of all patients required an adult diaper throughout the day to control their urinary functions. Patients suffering from neurological disorders accounted for 70% of respondents, and a correlation was seen between the extent of dementia and ADL, and excretory control. Urinary functioning in both men and women was found to grow increasingly difficult with age, and medical problems involving urinary difficulty appear to increase with the advance of the aging process. The representative groups for this survey were limited to elderly people in nursing homes, many of whom suffer from neurological disorders such as cerebral infarction. It was found that both male and female patients experience a variety of urinary disturbances.

[The effect of subjective urinary symptoms on quality of life (QOL)--results of a field survey for prostatic diseases].

We investigated the effect of subjective urinary symptoms on quality of life (QOL) in 536 males who participated in a field survey for prostatic diseases in Hokkaido from 1991 to 1993. To quantitatively estimate the subjective urinary symptoms, we used a questionnaire that included questions about irritative symptoms such as "diurnal frequency", "nocturia", "sensation of residual urine", "urgency" and "frequent urination within 2 hrs.", and obstructive symptoms such as "difficulty to start urination", "intermittency during urination", "strain or pushing", "weak stream" and "stopping or starting urination". The incidences of participants with poor QOL increased as irritative and obstructive symptoms became apparent (p < 0.001). However, some participants, who had only mild urinary symptoms, could not tolerate the situation and felt impairment of QOL. In contrast, others did not feel a decrease of QOL in daily life in spite of severe symptoms. Thus, subjective urinary symptoms did not always correlate with QOL in some individuals. These results indicated that we should keep in mind not only subjective urinary symptoms but their effect on QOL whenever we determine an indication for medical or surgical intervention for patients having urinary symptoms.

[Study of sexual function of male diabetics].

We studied the actual general state of sexual dysfunction in 201 male diabetics (age range: 22-76 years) who were commuting outpatients, using the Sapporo Medical University's Sexual Function Questionnaire. The control group consisted of 6,426 healthy male subjects. The principal parameters taken into account by the Questionnaire were the libido and ability to achieve/maintain an erection, with consideration given to the factor of the subject's age. The results were as follows. 1. The diabetic patients who were experiencing neuropathy showed a decrease in the ability to achieve/maintain an erection at an earlier age than the patients without neuropathy. In addition, the number of patients with a decreased ability to achieve/maintain an erection was found to be 30 (45%) of 67 cases with neuropathy compared with 24 (18%) of 134 cases without neuropathy. The severity of that decreased ability was also greater in the cases with neuropathy. 2. In comparison with the healthy male control subjects, the diabetics showed decreases in libido and the ability to achieve/maintain an erection which were not very severe at younger ages, but became striking after the age of about 60 years. Thus, the degree of dysfunction accelerated with increasing age. After the age of 60 years, the erection score was found to decrease in 31.7% of the patients without neuropathy but in 61.1% of the patients with neuropathy. 3. Weighted regression analysis showed that the most important factor involved in the ability to achieve/maintain an erection was the subject's age (contribution rate: 27.2%), followed by neuropathy (7.4%). These two factors represented the explanatory factor of erectile dysfunction in approximately 1/3 of the diabetics.(ABSTRACT TRUNCATED AT 250 WORDS)

[Changes in the voiding condition with aging in elderly males--results of a field survey for prostatic diseases].

We investigated changes in the voiding condition with aging in 1476 males 50 years and older, who participated in a field survey for prostatic diseases conducted from 1982 to 1991 in Hokkaido. Voiding conditions were evaluated subjectively with a symptom index (weak stream, intermittency, nocturia, urge incontinence), and objectively with maximum flow rate. Digital rectal examination (DRE) revealed 10.3% (572 participants) of total evaluable participants to have moderate or marked enlargement of the prostate. The incidence of enlargement of the prostate apparently increased with aging. Subjective urinary symptoms, both obstructive and irritative, increased in the incidence and degree with aging. The maximum flow rate and maximum flow rate-nomogram also became worse with aging the incidences participants having urinary symptoms or decreased maximum flow rate increased in parallel with the size of the prostate on DRE. For each size of the prostate on DRE, the incidence of participants having urinary symptoms increased with aging. The maximum flow rate showed a similar tendency, decreasing with aging, irrespective of the size of the prostate. These results indicate that aging as well as the size of the prostate may affect the voiding condition in males aged 50 years and older.

[Etiological analysis of male diabetic erectile dysfunction with particular emphasis on findings of vascular and neurological examinations].

We studied the nocturnal penile tumescence (NPT) value obtained by simple method and the vascular and neurological findings closely related to the ability to achieve/maintain an erection in order to investigate the causative factors of organic erectile dysfunction seen in diabetes. Studies were carried out on 62 male diabetics (32-78 y.o.). The results were as follows. 1. There were no cases of a decrease in the nocturnal penile tumescence (NPT) value among eight subjects who showed no erectile dysfunction. However, a decrease in the NPT value was recorded in 28 (85%) of 34 subjects who showed complete failure to achieve/maintain an erection. It was, therefore, surmised that there is a large organic contribution to the erectile dysfunction seen in diabetes. 2. We determined the penile blood pressure index (PBPI), the reactions of papaverine tests, conduction velocity of the dorsal nerve of the penis and the latency of the bulbocavernosus reflex. 42 (67.7%) of the 62 subjects had penile vascular disorders and the cases with severe vascular disorders were skewed to the high age brackets. There were also many cases with neuropathy involved in the ability to achieve/maintain an erection: 38 (61.2%) of the 62 subjects. Moreover, many of the subjects were found to have damage to both the blood vessels and the nerves relating to an erection, and these forms of damage were surmised to be deeply involved in the decrease in the NPT value. 3. Multivariate analysis indicated that the most important factor involved in the decrease in the NPT value was vascular damage (contribution rate: 30.8%), followed by neuropathy (6.9%). These two factors represented the most important explanatory factors of the organic erectile dysfunction seen in diabetes. 4. The contribution rate of vascular damage to the decrease in the NPT value was 18.8% for diabetic subjects under the age of 60 years, but 45.1% for subjects aged 60 years or more. The contribution rate was thus higher in the elderly subject group. On the other hand, the contribution rate of neuropathy to the decrease in the NPT value was 7.7% for diabetic subjects under the age of 60 years, but the rate decreased to 4.7% for subjects aged 60 years or more.

[Study on nocturnal penile tumescence (NPT) in healthy males study on age-related changes of NPT].

Nocturnal penile tumescence (NPT) occurs in all healthy males as a physiological phenomenon. Male sexual function can be evaluated by NPT. However, the age dependent changes and the significance of NPT have still remained unknown. Thus, we measured NPT throughout the night in healthy 189 subjects, ranging from 3 to 84 years old, and studied the relationship between this phenomenon and age. The parameters obtained by NPT appeared to be greater in men over about 10 years old. This increase of the parameters was related to the maturation of the hypothalamo-pituitary-testicular axis, which starts rapidly at puberty. A significant decrease in the total tumescence time in men over 30 years of age was related to a decrease of androgen. In men over 55 years of age, the maximum penile circumference change and the maximum increase rate in penile circumference were significantly lower than those of men in their twenties. These results suggest that the decrease in the parameters of NPT in men over 55 years of age had a relationship with the decrease of androgen, and organic changes in the corpus cavernosum and penile vascular system that occurred with aging. In conclusion, NPT may be related to the sexual maturation and decline in men. We also provide standard reference parameters for NPT in Japanese, which could be useful in the diagnosis and treatment of male sexual dysfunction.

[Study on nocturnal penile tumescence (NPT) in healthy males. Study on the relationship between the serum free testosterone level and NPT].

We studied the correlation of nocturnal penile tumescence (NPT) and serum free testosterone, which were evaluated in 123 healthy males and in 6 cases of primary hypogonadotropic hypogonadism before and after treatment with human chorionic gonadotropin (hCG). In healthy subjects 3 to 84 years old, the serum free testosterone levels had a relationship with 4 parameters revealed by NPT: total tumescence time, mean tumescence time, maximum penile circumference change and maximum increase rate in penile circumference. In men who showed the peak value for each NPT parameter and those who were younger, the serum free testosterone levels were correlated with all 6 parameters revealed by NPT: total tumescence time, NPT frequency, mean tumescence time, circumference in flaccid penis, maximum penile circumference change and maximum increase rate in penile circumference. In 6 cases of the primary hypogonadotropic hypogonadism, total tumescence time, mean tumescence time, maximum penile circumference change, and maximum increase rate in penile circumference significantly increased after hCG treatment. This indicated that an increase of the serum free testosterone level was responsible for the increase of the parameters of NPT. In men who were older than the subjects showing the peak value for each NPT parameter, the serum free testosterone levels were correlated with 4 parameters revealed by NPT: total tumescence time, mean tumescence time, maximum penile circumference change and maximum increase rate in penile circumference. These results suggest that the free testosterone level may be involved in the decrease of maximum penile circumference change as well as in age-related changes of the penile vascular system that were the major factor responsible for the decrease. This is the first report that studied the relationship between NPT and androgen in any group. NPT as a parameter for the erectile function was related to androgen levels.

[Clinical study on female patients with bladder irritability--comparison between cystitis and urethral syndrome].

We analysed the clinical background of female who had no underlying disease but consulted out urology outpatient clinic with a chief complaint of symptoms of bladder irritation. Overally, 50.1% (102/200 cases) of the cases had urethral syndrome, without pyria or bacteriuria. Plotting of the age distribution of the urethral syndrome cases showed a bell-shaped curve, with the peak at the 40-49 year-old age bracket. The age distribution for the cystitis cases showed two peaks: at 20-29 years of age and 60-69 years of age. These results indicate that the cases of urethral syndrome involve multiple factors with increasing age. Examination of the external genitalia revealed the urethral smear specimens to be positive for leukocytes in 48.9% (88/180 cases) of the total urethral syndrome cases and in 72.7% (16/22 cases) of the urethral syndrome cases positive for urethral bacteria. Accordingly, it was surmised that there is considered to be a precursor of cystitis-as a cause of urethral syndrome cases. In addition, the rates of detection of chlamydia trachomatis from the vagina and urethra of the urethral syndrome cases in the reproductive age range of (2/34 cases) for the urethra. However, these rates were about the same as those determined for healthy volunteers. Nevertheless, it was surmised that urethritis or vaginitis caused by C. trachomatis was one of the causes of the symptoms of bladder irritation in some of the cases. Accordingly, it was concluded that infection due to C. trachomatis cannot be ignored as a possible cause of urethral syndrome.

[Expression of squamous cell carcinoma-associated antigen in bladder cancer cells--a comparative study with the expression of proliferating cell nuclear antigen (PCNA)].

We investigated the expression of squamous cell carcinoma-associated antigen (SCC-Ag) and proliferating cell nuclear antigen (PCNA) in bladder carcinoma cells using immunohistochemical techniques. Tissues with 10% or more cells stained for SCC-Ag were defined as positive for the antigen. All grade 1-transitional cell carcinomas (TCCs) and 87% of grade 2-TCCs were negative for cytoplasmic SCC-Ag. However, 57.6% of grade 3-TCCs not associated with squamous metaplasia and 80% of those associated with squamous metaplasia showed strongly positive cytoplasmic SCC-Ag. Of squamous cell carcinomas, the well or moderately differentiated carcinomas showed a higher incidence (80-100%) of the antigen in the cytoplasm than the poorly differentiated ones (25%). Nuclear SCC-Ag was found in 63.6% of grade 1-TCCs, 46.3% of grade 2-TCCs and 54.9% of grade 3-TCCs not associated with squamous metaplasia. However, grade 3-TCCs with squamous metaplasia and squamous cell carcinomas were negative for nuclear SCC-Ag. The positive rate for PCNA increased from grade 1 to grade 2, grade 3 without squamous metaplasia and grade 3 with squamous metaplasia in the TCCs. All squamous cell carcinomas showed a high positive rate, irrespective of the differentiation. Multiple regression analysis revealed a closer association between the expression of cytoplasmic SCC-Ag and the histological grade of TCCs.

Pyogenic psoas muscle abscess: report of three cases.

We report three cases of primary psoas muscle abscess. Two of these cases were treated by open drainage through a lumbar incision and the other was by ultrasonography (USG)-guided percutaneous aspiration of the abscess. Antibiotic therapy was also instituted in all cases. Post-operatively, the patients recovered well with no sign of recurrence. USG-guided percutaneous drainage combined with appropriate antibiotic therapy appears to be the first choice procedure for treatment of the primary psoas abscess if correct diagnosis is promptly made and the procedure can be performed.

[Study of clinical usefulness of an antiandrogen, TZP-4238, as a drug for treatment of benign prostatic hypertrophy--its influence on sexual function].

Using chlormadinone acetate (CMA) as the control drug, a double-blind study was carried out to examine not only the effects of 17 alpha-acetoxy-6-chloro-2-oxa-4,6-pregnadiene-3,20-dione (TZP-4238;) on subjective urinary symptoms but also, especially, that on the sexual function in patients with benign prostatic hypertrophy (BPH). The clinical efficacy rate in relation to the subjective urinary symptoms was about the same in the two treatment groups; 45.9% in the TZP-4238 group and 50.0% in the CMA group. However, the incidence of adverse effects on the sexual function showed a marked difference between the two groups. The TZP-4238 group revealed a lower suppressive effect on the function than the CMA group (45.1% vs. 71.4%). In relation to their causation of other adverse effects, there were no differences between the two drugs. Accordingly, in consideration of the facts that TZP-4238 with less influence on the sexual function affords a superior quality of life to elderly patients, while achieving effective treatment by means of a convenient single daily administration, TZP-4238 was surmised to be a more useful drug than CMA as an antiandrogen for the treatment of BPH.

[Changes in the salivary testosterone level in aged].

Measurement of the level of free testosterone is important in the evaluation of testicular function. Because most of the testosterone in the saliva is in the free form, measurement of the salivary testosterone level is considered to be effective for the evaluation of testicular function. In the present study, a commercial kit was employed to measure the salivary testosterone level, and the change in the salivary testosterone level with age was investigated. The subjects of this study were 76 males, 20-89 years of age, with no endocrinological diseases. The concentration of testosterone in the saliva was measured for each of the subjects, and for 34 of the subjects the concentrations of total testosterone and free testosterone in the serum were also measured at the same time. Standard serum was diluted to have a testosterone concentration of 50 pg/ml, and the results indicated that it was sufficiently possible to measure the salivary testosterone. The coefficient of correlation between the salivary testosterone concentration and the serum total testosterone concentration was 0.479 (p < 0.01), while the coefficient of correlation between the salivary testosterone concentration and the serum free testosterone concentration was 0.732 (p < 0.001). These findings thus indicate that the correlation between the salivary testosterone concentration and the serum free testosterone concentration is better than that between the salivary testosterone concentration and the serum total testosterone concentration. It was also demonstrated that the salivary testosterone concentration decreased significantly with aging after the fifth decade of life. The correlation coefficient for that relationship was -0.606 (p < 0.001), and the change was similar to that seen in the serum free testosterone concentration as a function of aging. The findings suggested that measurement of the salivary testosterone concentration with the commercial kit is useful for the evaluation of testicular function.

[Re-examination of the criteria for clinical evaluation on bacterial prostatitis--analysis of the data of the clinical study of temafloxacin].

The clinical efficacy of temafloxacin (TMFX) was evaluated in the patients with acute prostatitis (AP: 31 cases) and chronic prostatitis (CP: 75 cases), and the criteria for clinical evaluation on bacterial prostatitis by the Japanese UTI Committee were re-examined. The clinical efficacy of TMFX on prostatitis was examined. The number of the evaluable cases by doctors in charge were 30 and 68 for AP and CP, respectively. The efficacy rates were as high as 96.7% for AP and 80.9% for CP. In the patients whose efficacy could be evaluated by the Committee, the efficacy rates were as high as 100% (15/15) for acute bacterial prostatitis (ABP) and 66.7% (18/27) for chronic bacterial prostatitis (CBP). The bacteriological eradication rates were 100% (15/15) for ABP and 81.5% (22/27) for CBP. Adverse drug reactions were observed in 8 of 100 cases (total evaluable cases) but they clinically did not consist any problems. Abnormal laboratory test findings were observed in 9 of 71 cases (total evaluable cases), whose changes were all slight and did not clinically cause any problem. Therefore, TMFX was concluded to be highly useful in the treatment of ABP and CBP. The criteria for clinical evaluation on bacterial prostatitis were re-examined. Concerning the treatment period, comparison of the evaluation at Day 7 with that at Day 14 in ABP revealed that the efficacy was evaluable at Day 7. In the same way, comparison of the evaluation at Day 14 with that at Day 28 in CBP revealed that the efficacy was evaluable at Day 14. Concerning the pathogens, in ABP, the major pathogens were considered to be GNR as well as E. coli. On the other hand, in CBP, GNR and E. faecalis, that were generally authorized to be pathogens, were detected in less than half of the patients, and CNS were detected in the other patients. Among the CNS cases, there were some cases that showed no relationship between improvement of clinical symptoms and bacterial response. Therefore, it is necessary to perform further studies on this matter. In conclusion, regarding the criteria for clinical evaluation on bacterial prostatitis proposed by the Japanese UTI committee, the treatment period for evaluation of efficacy was appropriate, and it is necessary to perform further studies in order to determine the pathogens in bacterial prostatitis.

[Lobectomy under thoracoscopic guidance].

From September '92 to November '93, 15 lobectomies were performed under thoracoscopic guidance. After a thoracoport was inserted through the seventh intercostal space to conduct the operation utilizing a monitor, a six to ten cm incision was made in the fifth intercostal space. The pulmonary arteries and veins were divided after stapling with an Endo GIA or Roticulator 30-VIII. The lobar bronchus was stapled with a Roticulator 30-3.5, 30-4.8 or an Endo TA and affected lobes were resected. In cases of lung cancer, lymph node dissection has become possible as well due to the improvement of endoscopic instrumentation.

Expression of squamous cell carcinoma-associated antigen in grade 3 pT1 transitional cell carcinoma of the bladder and prediction of its progression and intravesical recurrence.

BACKGROUND: Among superficial transitional cell carcinomas (TCC) of the bladder, Grade 3 pT1 disease is associated with a higher risk of progression and intravesical recurrence. The authors determined whether or not squamous cell carcinoma-associated antigen (SCC antigen) could predict clinical behavior of Grade 3 pT1 disease. METHODS: SCC antigen was immunohistochemically stained in Grade 3 pT1 TCC of the bladder that had been fixed in formaldehyde solution and embedded in paraffin. Patients were followed up, and disease progression and recurrence were identified. Disease progression was defined as histologically verified muscle invasion or clinically detectable distant metastasis, including pelvic lymph node metastasis. RESULTS: SCC antigen in the cytoplasm was positively identified with immunohistochemical staining in 21 of 55 patients with the disease. Of 21 patients with positive SCC antigen in the cytoplasm, 8 experienced disease progression, whereas there was progression in 5 of 34 patients with negative antigen. The nonprogression rate of Grade 3 pT1 carcinomas with positive antigen in the cytoplasm was significantly lower than that for those with negative antigen. The disease-free rate also was clearly lower in the positive carcinoma than in the negative. Multivariate analysis confirmed that SCC antigen in the cytoplasm was the only significant variable independently affecting progression and intravesical recurrence of the disease. CONCLUSIONS: The results suggested that immunohistochemical expression of SCC antigen in the cytoplasm is closely linked with a higher risk for progression and intravesical recurrence of Grade 3 pT1 disease. Detection of the antigen can help to more accurately predict the clinical course of the disease.

Expression of ganglioside GM3 and H-2 antigens in clones with different metastatic and growth potentials isolated from Lewis lung carcinoma (3LL) cell line.

In view of the evidence that cell expression of gangliosides in several tumors is positively involved in the metastatic phenotype, Lewis lung carcinoma (3LL) cell line, expressing GM3 as the major ganglioside, was analysed for the cell surface expression of GM3. An indirect immunofluorescence assay, using a M2590 monoclonal antibody recognizing GM3, was used for this purpose. Since the parental 3LL cells consist of heterogenous subpopulations differing in the degrees of GM3 expression, we have developed clones of this cell line with different degrees of metastatic potentials by using an in vitro non-selective procedure in order to investigate whether the expression of GM3 is associated with metastatic potential. The degree of cell surface expression of GM3 among the clones correlated well with their total cellular content of this ganglioside. However, we were unable to confirm the report of increased level of GM3 in high metastatic 3LL clones, nor did a decreased level correlate with weak metastatic ability. In our recent work, an inhibitor of glucosylceramide synthase, D-threo-1-phenyl-2-decanoylamino-3-morpholino-1-propanol (D-PDMP), was found to decrease the levels of all cellular glucosphingolipids and cause the accumulation of the precursors of glucosylceramide. The present study does not, however, rule out the possible involvement of this lipid family in metastatic dissemination, since treatment of 3LL cells with D-PDMP resulted in significant inhibition of their experimental metastatic potential. Clones expressing very low GM3 grew slowly in culture dishes, suggesting that GM3 may have a regulatory role in cell proliferation. The low metastatic clones expressed high levels of H-2Kb antigen, while the expression of the same antigen on the high metastatic clones was relatively low, confirming the previous observation of this tumor system. Moreover, a clone showing the lowest tumorigenic potency revealed both a high cell surface expression of H-2Kb and a high H-2Kb/H-2Db ratio.
