Protective effects of manidipine on spermatogenesis in stroke-prone spontaneously hypertensive rats.

1. We evaluated the protective effects of manidipine on spermatogenic damage induced by the hypertensive vascular changes in stroke-prone spontaneously hypertensive rats (SHRSP). 2. Blood pressure reached more than 250 mmHg in SHRSP at 15 weeks of age, and thereafter the hypertensive changes in testicular arterioles developed. Manidipine reduced both the blood pressure and the hypertensive vascular changes. 3. Although immature and mature spermatids greatly diminished in SHRSP at 23 weeks of age, manidipine could preserve almost normal spermatogenesis even at 23 weeks of age. Transferrin concentration in testicular cytosol, which was considered to be indicative of the Sertoli cell function, in SHRSP with manidipine administration was significantly higher than that in SHRSP with no treatment at 23 weeks of age. 4. In conclusion, manidipine could prevent the development of the hypertensive changes in intratesticular arterioles and maintain normal Sertoli cell function. As a result, manidipine protected spermatogenic damage in SHRSP.

Interferon-gamma and interleukin-2 suppress the experimental metastasis of mouse renal adenocarcinoma.

While cytokines such as interferon-alpha (IFN-alpha), interferon-gamma (IFN-gamma) and interleukin-2 (IL-2) are partially effective in the treatment of metastatic renal cell carcinoma, these cytokines have not been investigated for their ability to suppress the development of metastasis. We investigated whether they suppressed experimental pulmonary metastasis by subline-2 of streptozotocin-induced mouse renal adenocarcinoma (MRAC-PM2). IFN-gamma (1 x 10(4) and 1 x 10(5) U/mouse) and IL-2 (1 x 10(4) and 1 x 10(5) U/mouse) suppressed the experimental pulmonary metastasis of MRAC-PM2 in a dose-dependent manner, but 1 x 10(5) IU/mouse of IFN-alpha did not. The combination of 1 x 10(5) U IFN-gamma with 1 x 10(5) U IL-2 had the most powerful inhibitory effect on pulmonary metastasis without any side-effects. In addition, the combination of 1 x 10(5) U IFN-gamma with 1 x 10(5) U IL-2 had a synergistic inhibitory effect on the growth of subcutaneous tumors. These results indicate that either IFN-gamma or IL-2, or their combination, produce a more favorable effect than IFN-alpha on the metastatic development of renal adenocarcinoma.

Serum prostate-specific antigen in a community-based population of healthy Japanese men: lower values than for similarly aged white men.

OBJECTIVE: To determine whether the age-specific reference ranges for serum prostate-specific antigen (PSA) concentration generated for white men are applicable to other races. PATIENTS AND METHODS: Three-hundred and thirty-five healthy Japanese men, aged 40-79 years, residing in the small fishing village of Shimamaki-mura, Japan, agreed to enter this prospective, community-based study. All underwent a detailed clinical evaluation that included a serum PSA determination, a digital rectal examination and a transrectal ultrasound. Two-hundred and eighty-six (85%) completed the prostatic evaluation and had no evidence of prostate cancer by any one of the three diagnostic tests; these men formed the study population on which all analyses were performed. RESULTS: The serum PSA concentration correlated directly with patient age (r = 0.33; P < 0.001) and prostatic volume (r = 0.57; P < 0.001). PSA density (PSAD) also was directly proportional to age (r = 0.30; P < 0.001). Adjusting for age, the serum PSA concentration was lower for Japanese men than for white men (P < 0.001). Thus, the recommended age-specific reference ranges (95th percentile) for serum PSA for Japanese were lower as well: 0.0-2.0 ng/mL for 40-49 years; 0.0-3.0 ng/mL for 50-59 years; 0.0-4.0 ng/mL for 60-69 years; and 0.0-5.0 ng/mL for 70-79 years. Based on transrectal ultrasound-volume determinations, the lower serum PSA concentrations in Japanese men are due in large part to their smaller prostate glands as compared with white men (P < 0.001). CONCLUSIONS: These findings confirm the earlier observations that serum PSA, prostatic volume and PSAD are age-dependent. However, because of physiological differences among the two races, partly due to the size of the prostate gland, the age-specific reference ranges for serum PSA are lower for Japanese men than for white men. Because of these racial differences, it is now crucial to conduct a similar investigation among black men.

[Study of pathogenicity of various serovars of Enterococcus faecalis].

Using a mouse experimental UTI (urinary tract infection) model, a study was conducted to find the pathogenicity of various serovars of E. faecalis. On the basis of studies employing serovar-specific factor sera prepared with E. faecalis type strains, serovar 2, 3, 4 and 10 strains showed a high incidence of involvement in pyelonephritis: 90.3%, 85.7%, 85% and 73.3%. Serovar 1, 6 and 7 strains each showed a 63.6% incidence of involvement in pyelonephritis, indicating that they have a moderate pathogenicity. The pathogenicity of the other serovar strains was not very strong, with a low incidence of 40-59.1%. These results were thus in good agreement with the findings of the study using the mouse experimental UTI model infected with clinical isolates. Serotyping was performed of E. faecalis clinical isolates obtained from patients with pyelonephritis or urosepsis. Serovars 2 and 4 comprised 75.1% of these isolates. It was surmised that E. faecalis serovars 2 and 4 tend to have strong pathogenicity. Thus, there were quite a few differences in pathogenicity of E. faecalis according to each kind of serovar.

[Study of distribution of serovars of Enterococcus faecalis clinical isolates].

Serotyping was conducted of E. faecalis strains isolated from various clinical specimens, and the distribution of the serovars was investigated. To date, 21 serovars of E. faecalis have been identified. The most common serovars in Japanese 770 strains were types 2 (30.6%), 7 (13.9%), 1 (12.2%) and 4 (6.9%), and these four serovars accounted for 63.6% of the total strains. The most common serovars in USA 200 strains were types 1 (22.5%), 4 (14.5%) and 2 (14.0%). The most common serovars in UK 65 strains were types 2 (24.6%), 4 (13.8%) and 9 (7.7%). The distribution of E. faecalis serovars differ as a function of the country of origin. Next, serological classification was performed for E. faecalis strains isolated from various Japanese clinical specimens. For the strains obtained from the urine, the most common serovars were types 2 (32.7%) and 7 (14.6%). Similarly, the most common serovars of the strains isolated from the various other clinical specimens were as follows: types 7 (35.0%) and 2 (15.0%) from pus; types 7 (44.4%) and 2 (33.3%) from sputum; types 2 (22.3%) and 1 (13.1%) from blood; and types 2 (34.9%) and 1 (27.9%) from the vaginal smear. It had no definite pattern for the distribution of serovars. The distribution of serovars varied in accordance with isolated years. Accordingly, the data obtained in these studies revealed that the distributions of E. faecalis serovars differ as a function of the geographical area of origin, the clinical specimen of origin and the year of origin. This approach is useful for serological classification of E. faecalis strains, and it is thought that it will be useful for epidemiological studies of this bacterium.

[Clinical efficacy of lomefloxacin (100 mg or 300 mg) single-dose therapy in female acute uncomplicated cystitis].

Female acute uncomplicated cystitis responds relatively well to antimicrobial chemotherapy. In particular, new quinolones are suited for use as antimicrobial agents in single-dose therapy of female acute uncomplicated cystitis since they have a long serum half-life and express potent antimicrobial activity against the causative microbes of this infection. Lomefloxacin (LFLX) is one such new quinolone which shows a long serum half-life, expresses potent antimicrobial activity against Escherichia coli (E. coli) and maintains an effective urinary drug concentration for approximately three days after a single administration. The authors carried out a comparative investigation of the clinical efficacy of single doses of 100 mg and 300 mg of LFLX in the treatment of female acute uncomplicated cystitis. The clinical efficacy rates with these doses, evaluated on the 3rd day after administration, were 98.2% (56/57 cases) for the 100mg-LFLX dose and 100% (62/62 cases) for the 300-mg LFLX dose. When the evaluation was performed on the 7th day after administration, the clinical efficacy rates were 91.3% (42/46 cases) for the 100-mg LFLX dose and 95.8% (46/48 cases) for the 300-mg LFLX dose. In addition, the microbial eradication rates were 73.7% (42/57 cases) for the 100-mg LFLX group and 75.8% (47/62 cases) for the 300-mg LFLX group on the 3rd day after administration, and 71.7% (33/46 cases) for the 100-mg LFLX group and 83.3% (40/48 cases) for the 300-mg LFLX group on the 7th day after administration. Although there were no statistically significant differences between the two LFLX dosage groups for these parameters at either of the evaluation times, the rates for the 300-mg LFLX dose were slightly superior. The investigators judged the efficacy of the LFLX treatment as having been insufficient in 12 patients, and urological examinations performed on six of those cases determined that there were mild underlying diseases in four cases, such as stenosis of the urethral meatus. On the basis of the findings described above, it is clear that a single 100-mg dose of LFLX provided sufficient clinical efficacy in the treatment of female acute uncomplicated cystitis, but the efficacy of the 300-mg dose of LFLX was even better. In addition, it was surmised that performance of detailed urological examinations provides an opportunity to detect mild underlying diseases that may be the cause of the intractability in female acute uncomplicated cystitis cases showing an insufficient response to treatment with antimicrobial agents such as LFLX.

[A study on the role of Chlamydia trachomatis in chronic prostatitis--analysis of anti-Chlamydia trachomatis specific IgA in expressed prostate secretion by western-blotting method].

Opinion among researchers is divided as to whether Chlamydia trachomatis is the causative organism of chronic prostatitis. In this study, using enzyme immuno assay, we examined the positive rate of anti-C. trachomatis IgA according to the leukocyte count in expressed prostate secretion (EPS), in a total of 169 patients including 92 patients who showed chronic prostatitis-like symptoms. And we confirmed, by western-blotting, the presence or absence of anti-heat shock protein (HSP: 60 KDa) IgA in EPS, and we investigated the role of C. trachomatis in chronic prostatitis. The total positive rate of anti-C. trachomatis IgA in EPS was 26.0% (44/169). According to the leukocyte count in EPS, the positive rate for the group with more than 10 WBC/hpf was 29.0% (20/69), for the group with 5-9 WBC/hpf was 37.9% (11/29), for the group with less than 5 WBC/hpf was 18.3% (13/71). Accordingly the positive rate of anti-C. trachomatis IgA in EPS was not higher in direct correlation to the severity of the inflammation at the infected site (p = 0.09). But the positive rate for the group with more than 5 WBC/hpf was higher than for the group with less than 5 WBC/hpf. On the other hand, the total positive rate of anti-HSP IgA in EPS was 17.6% (6/34), and according to the leukocyte count in EPS, the positive rate for the group with more than 10 WBC/hpf was 38.5% (5/13), for the group with 5-9 WBC/hpf was 0% (0/12). Accordingly the positive rate of anti-HSP IgA in EPS was higher in direct correlation to the severity of the inflammation at the infected site (p = 0.13). From this results, it was suggested that C. trachomatis was concerned with 29.0% (20/69) of chronic prostatitis with more than 10 WBC/hpf, and also C. trachomatis is the causative organism of chronic prostatis.

[Long-term monitoring of female acute uncomplicated cystitis cases after lomefloxacin single-dose therapy].

Female acute uncomplicated cystitis responds relatively well to antimicrobial chemotherapy, but this is also a disease which shows a high frequency of recurrence. However, there have been no published reports regarding long-term monitoring of the course of this disease after therapy has been administered. Accordingly, using primarily a questionnaire, the authors carried out long-term monitoring (for a mean of 242 days) of the natural course of cases of female acute uncomplicated cystitis after single-dose therapy with lomefloxacin (LFLX), a new quinolone antimicrobial agent. The subjects of this study were female patients diagnosed as having acute uncomplicated cystitis with pain upon urination, pyuria (> or = 10 WBCs/hpf) and bacteriuria (> or = 10(4) cfu/ml). LFLX was orally administered as a single dose of 100 mg or 300 mg, and the therapeutic efficacy was evaluated on the 3rd and 7th days thereafter. In principle, the evaluation of cure was performed on the 7th day after LFLX administration, and monitoring was conducted to detect early recurrence during the next 7 days (i.e., through the 14th day after treatment). Then the subjects were monitored for late recurrence during a mean follow-up period of 242 days by means of a questionnaire. Confirmation of recurrence was carried out to the greatest extent possible. It was possible to carry out long-term monitoring of the natural course of 101 cases of female acute uncomplicated cystitis in which the clinical efficacy on the 3rd day after LFLX treatment had been evaluated as good or excellent.(ABSTRACT TRUNCATED AT 250 WORDS)

Mass screening for prostate carcinoma: a study in Hokkaido, Japan.

Although there has been controversy about the early detection of prostate carcinoma by mass screening, we carried out an investigational study of such screening, using digital rectal examination (DRE) and serum prostate-specific antigen (PSA) determination in Hokkaido, Japan. We determined the detection rate and some profiles of the carcinoma detected by screening in Japanese men 50 years old and older. The detection rate of carcinoma was 1.10% in 1,639 participants. The rate linearly increased with aging from 0.25% for men in their fifties to 2.29% for those in their seventies. Two thirds of the carcinomas were T1(c) or T2, the frequency of which seemed to be higher than that of patients who visited medical institutions. The simultaneous use of both DRE and serum PSA determination showed a higher positive predictive value than either alone. Our current results indicated that the detection rate of carcinoma was higher than we previously anticipated for Japanese men, and that simultaneous use of DRE and serum PSA determination yielded a higher positive predictive value than either alone.

[The preventive effect of indeloxazine hydrochloride to the sexual dysfunction caused by anti-androgenergic agent (allylestrenol)].

Anti-androgenergic agents are usually used for patients with benign prostatic hypertrophy (BPH). However steroidal anti-androgenergic agents tend to suppress the sexual function. This side effect is very significant in middle-aged men. Therefore we studied the preventive effect of indeloxazine hydrochloride (INDX), which induces an increase of the dopamine level in the brain, on the sexual dysfunction induced by an anti-androgenergic agent (allylestrenol: ALE). Thirty-six patients with BPH were classified into two groups, one used ALE only, and the other ALE with INDX. For the subjective evaluation of the sexual function, a self assessment questionnaire method was employed before and after administration. We especially studied 3 questions, "morning erection", "erectile capacity" and "frequency of sex". For the objective evaluation of the sexual function, nocturnal penile tumescence (NPT) was measured using an erectometer. NPT occurs in healthy males as a physiological phenomenon and it shows the erectile capacity objectively. The levels of LH, total testosterone and free testosterone were also determined. In the ALE only group, sexual dysfunction was found subjectively and objectively, but in the ALE with INDX group, it was not found. Levels of LH, total testosterone and free testosterone were decreased in the both groups. There was no significant difference between the two groups. We hypothesized that the sexual dysfunction due to ALE is related with not only to the decrease of androgen, but also to suppression of the central nervous system; for example, the suppression of the area of the brain mediating sexual behavior.(ABSTRACT TRUNCATED AT 250 WORDS)

[Study of sexual function in male hemodialysis patients--a sexual function survey using a questionnaire].

Sexual dysfunction is a well-documented complication among male hemodialysis patients. To evaluate it in detail, we conducted a sexual function survey using the Sapporo Medical University Sexual Function Questionnaire. Two hundred five male patients receiving outpatient hemodialysis were entered into this study. Patients with diabetes mellitus and severe anemia (Hb value < 8.0 g/dl) were excluded. The control group consisted of 3,462 healthy male subjects. Of male hemodialysis patients, 33.7% showed deterioration of libido, while 44.4% showed deterioration of potency, both of them decreased greatly with age. Ejaculatory ability correlated with potency. We compared the frequency of sexual intercourse in male hemodialysis patients with that in normal males. Twelve point nine percent of hemodialysis patients (HP) ranging from 30 to 39 in age versus 3.5% of age-matched normal males had no sexual intercourse and 22.4% of HPs from 40 to 49 versus 3.0% of age-matched normal males, 52.2% of HPs from 50 to 59 versus 7.5% of age-matched normal males, and 89.3% of HPs from 60 to 69 versus 18.0% of age-matched normal males had no sexual intercourse. These results revealed that the sexual frequency in hemodialysis patients decreased more than that in normal males in all age groups and with age.

[Study of sexual function in male hemodialysis patients--analysis of the cause of erectile dysfunction].

Erectile dysfunction is a well documented complication among male hemodialysis patients. The cause has been reported to be related to multiple factors, including neurological, endocrinological and vasculogenic elements. The purpose of this study was to identify the factors which most greatly determine erectile dysfunction in hemodialysis patients. Male hemodialysis patients without diabetes mellitus and severe anemia (Hb value < 8.0 g/dl) were entered into the study. We measured nocturnal penile tumescence (NPT) values in these patients and carried out neurological studies (measurement of the penile dorsal nerve conduction velocity and the bulbocavernosus reflex (BCR) latency). a vasculogenic study (measurement of the penile blood pressure index (PBPI) and endocrinological studies (measurement of serum free testosterone levels and serum prolactin levels before hemodialysis). NPT values (maximum penile circumference changes) in hemodialysis patients decreased compared with those in healthy males. In both hemodialysis patients and healthy males, NPT values decreased with age. NPT values in hemodialysis patients were significantly lower than those in healthy males in the fifties and sixties. 32.2% of hemodialysis patients had severe penile neurological disorder. 55.6% of them showed abnormal NPT. PBPI was low in only 10.0% of hemodialysis patients. However, there was a significant correlation between PBPI and the NPT value (r = 0.387). Serum free testosterone levels in hemodialysis patients were significantly lower than those in healthy males. There was a significant correlation between the serum free testosterone level and the NPT value (r = 0.328). However, there was no correlation between the serum prolactin level and the NPT value. To identify the factors which most greatly determine erectile dysfunction in hemodialysis patients, we carried out multivariate analysis. The criterion variables in this analysis were NPT values. The coefficient of determination was highest for a neurological disorder (30.7%), followed by an endocrinological disorder (a reduction in the serum free testosterone level) (11.6%) and a vasculogenic disorder (a reduction in PBPI) (4.2%).

[Clinical studies of spermatogenic damage in patients with testicular cancer].

We investigated the spermatogenic function of patients with testicular cancer, and the influences of anti-cancer chemotherapy on testicular function in these patients. Fifty-one patients with testicular cancer were selected for evaluation of their testicular function, including spermatogenesis and endocrinological function, before and after chemotherapy with anti-cancer agents. Before chemotherapy with anti-cancer agents, 22 of 49 patients (44.8%) had a sperm concentration of less than 20 x 10(6)/ml, and 8 patients (16.3%) showed azoospermia. The mean sperm concentration of the patients with testicular cancer was 29.0 x 10(6)/ml before therapeutic chemotherapy with anti-cancer agents, but within 3 months after chemotherapy, it decreased to 3.86 x 10(6)/ml (p < 0.01). Fifteen of 19 patients (73.7%) were revealed to have azoospermia. Damage to spermatogenesis became more severe with the number of chemotherapy treatment. No patients had a detectable sperm count at the completion of 3 or more courses of chemotherapy. But some patients who received 3 courses or more of chemotherapy showed recovered sperm counts after 2 or more years. Thus, a lack of sperm after chemotherapy for cancer did not necessarily indicate inability to recover spermatogenesis over 2 years after chemotherapy. Serum FSH levels of the patients were 5.62 +/- 3.43 mIU/ml before chemotherapy, and 19. 70 +/- 17.06 mIU/ml (p < 0.05) at the time of its completion. Serum FSH levels could reflect damage to spermatogenesis in these cases. Cases in which spermatogenesis did not recover may have higher serum FSH levels than those with recovery of spermatogenesis.

[Effects of anti-cancer agents on cultured rat Sertoli cells].

We performed primary immature Sertoli cell culture to investigate whether or not anti-cancer agents would have a direct effect on rat Sertoli cells. Sertoli cells, isolated from testes of 18-day-old rats, were cultured in pellets with medium for 5 days. The concentration of transferrin in cultured medium were measured as the function of Sertoli cells. The anti-cancer agents cis-diamminedichloroplatinum (CDDP), adriamycin and vinblastine were selected for this study, and added to the culture medium. CDDP decreased the level of transferrin concentration in cultured medium, namely 0.5 microgram/ml of CDDP resulted in 54.9% of the transferrin concentration in the medium compared with that without any anti-cancer agents (p < 0.05), and 1.0 micrograms/ml of CDDP produced transferrin concentrations of 57.5% and 46.2% (p < 0.05), respectively. Adriamycin (0.4 microgram/ml) and vinblastine (0.5 microgram/ml) produced transferrin concentrations of 35.2% and 31.3% in cultured medium (p < 0.05), respectively. These findings revealed that anti-cancer agents have direct damaging effects on rat Sertoli cells.

[Clinical efficacy of neoadjuvant M-VAC chemotherapy for invasive bladder cancer].

We clinically evaluated the efficacy of neoadjuvant M-VAC (methotrexate, vinblastine, doxorubicin and cisplatin) therapy followed by radical cystectomy. A total of 21 patients with locally invasive bladder cancer received neoadjuvant M-VAC therapy with an average of 2.4 courses (range 2-4). Of the 21 patients, 4 had stage T2N0M0, 11 had stage T3aN0M0, 5 had stage T3bN0M0 and 1 had stage T4N0M0 at diagnosis. Of the 21 patients, 3 had clinically complete responses (cCR) and 12 had partial responses (cPR), for an overall response rate of 71.4%. The patients who responded to the chemotherapy (cCR + cPR) had a 2-year disease free rate of 80.0% in contrast to 50.0% for the remaining patients who did not respond. A pathological response (pT0, pT1) was achieved in 6 (28.6%) of the 21 patients. The 5 patients in this group remain free of disease for 15 to 79 months (mean 58.8 months). These preliminary results suggest that the patients who achieve either a complete or partial response, in particular those had pathological stage of pT0 or pT1, may have a favorable clinical course.

[Studies of spermatogenic damages induced by anti-cancer agent and anti-androgenic agents in rat testes].

To investigate the mechanisms responsible for and recovery process of spermatogenic damage induced by anti-cancer agent and anti-androgen agents, we experimented with rats injected with those agents. Cis-diamminedichloroplatinum (CDDP) was selected as the anti-cancer agent causing spermatogenic damage. Flutamide and cyproterone acetate were selected as anti-androgen agents. Both of these agents induced histopathological spermatogenic damage. The influences of CDDP on spermatogonia and spermatocytes were remarkable, compared with cyproterone acetate and flutamide, which damaged mainly spermatids. CDDP had severely damaging effects on the DNA-synthesizing activity of spermatogenesis, based on findings using monoclonal antibody proliferating cell nuclear antigen (PCNA), compared with anti-androgen agents. We also studied Sertoli cell function of drug-induced spermatogenic damage in rat testes. Measuring the contents of transferrin in rat testes indicated the damage to Sertoli cell function. High doses of CDDP (8 mg/kg) affected testicular transferrin concentration, but 4 mg/kg and 2 mg/kg of CDDP had no significant effect on Sertoli cell function. Cyproterone acetate and flutamide had severely damaging effects on Sertoli cell function not only at a high dose (30 mg/kg) but at low doses of 15 mg/kg and 7.5 mg/kg. These results showed that an anti-cancer agent primarily affects the DNA synthesizing activity of spermatogonia and spermatocytes, but high doses of these agents also have damaging effect on Sertoli cells, and anti-androgen agents mainly affect Sertoli cells.

[Effect of follicle-stimulating hormone (FSH) on protection or acceleration to recovery from spermatogenic damage induced by anti-cancer agents].

We investigated spermatogenic damage induced by anti-cancer agents in previous studies. The results showed that the damage to spermatogenesis induced by anti-cancer agents could be due to not only direct damaging effects on DNA synthesis but also a direct effect on Sertoli cell function. Thus, stimulating Sertoli cell function could protect against the damaging effects of anti-cancer agents on spermatogenesis, or stimulate recovery from impair spermatogenesis. We experimented with rats injected with the anti-cancer agents cisdiamminedichloroplatinum (CDDP) and adriamycin, which impair spermatogenesis, in order to examine this hypothesis. Follicle-stimulating hormone (FSH), which should stimulate Sertoli cell function in vivo, was selected, and the effects of FSH in protecting against damage or stimulating recovery from damage were investigated. The injection of FSH resulted in no significant differences in the histopathological findings of spermatogenic damage induced by anti-cancer agents or testicular transferrin concentration as compared to those observed without FSH injection. But, in the protocol of the experiments on acceleration of recovery from spermatogenic damage induced by anti-cancer agents, the spermatid index without FSH was 1.02 +/- 0.24, that with FHS 75 IU/kg was 1.50 +/- 0.15, and that with FSH 150 IU/kg was 1.62 +/- 0.39, a significant difference (p < 0.05). Also, the PCNA labeling index and testicular transferrin concentration were significantly different with FSH from those without FSH.

[A study on the erectile response with the vacuum constriction device compared with intracavernous injection of a vasoactive drug].

Many types of vacuum constriction devices (VCDs) are used for the treatment of impotence, but the VCDs made in the USA are too large for Japanese males, and air leakage occurs. Therefore, we examined the erectile response of 47 impotent men to a vacuum constriction device that is made in Japan and can be applied suitably for Japanese men, and compared the response to intracavernous injection of prostaglandin E1 (PGE1). When the 47 cases were divided into two groups by nocturnal penile tumescence, twenty of the impotent men were regarded to have fewer organic factor (group A) while 27 were regarded to have more organic factor (group B). All (100%) of the 20 cases of group A achieved a complete erection with the VCD, but only 11 (55%) of them achieved a complete erection with PGE1. Twenty-three (85%) of the 27 cases of group B achieved a complete erection with the VCD, but only nine (33%) achieved a complete erection with PGE1. Two of the four patients who did not achieve a complete erection with the VCD stopped pumping the VCD because of penile pain, and the other two patients had a penile-brachial pressure index (PBPI) of under 0.65. It was suggested that patients with no severe vascular disease can show a good erectile response with the VCD.
