Intrathecal administration of thrombin inhibitor ameliorates cerebral vasospasm. Use of a drug delivery system releasing hirudin.

The role of thrombin as a spasmogen after subarachnoid hemorrhage was evaluated using the intrathecally administered thrombin inhibitor hirudin, released from a drug delivery system (DDS) based on collagen in a canine vasospasm model. The DDS was implanted into the cisterna magna with autologous blood in the hirudin-treated group. The reduction in the angiographical diameter of the basilar artery was only 19% in the hirudin-treated group on day 7, showing a significant difference between hirudin-treated and nontreated groups (p < 0.01). These results suggest that thrombin is an important cause of vasospasm. The collagen DDS has great potential for treatment in the cerebrospinal fluid milieu.

Analysis of bone scintigram data using a speech recognition reporting system.

A total of 649 bone scintigram reports were stored using a voice pattern recognition system in a general-purpose medium-sized computer ACOS-650, and bone scintigraphy carried out in this institute was examined by analysing these data. The results showed that the introduction of the system made it possible to analyse all the data quickly, whereas previously the amount of information that could be analysed was restricted because of the complexity of the data. The results also showed that the system would be useful for understanding the examinations carried out in the whole hospital as well as for analysing metastatic tumours and the numbers of patients receiving examinations. Furthermore, it would be helpful in the logical analysis of reports prepared by doctors.

Analysis of bone scintigram data using speech recognition reporting system--data analysis with speech recognition system.

Five hundred eighty bone scintigram reports were stored using a voice pattern recognition system in a general-purpose, middle-sized computer (ACOS-650). Bone scintigraphy carried out in our institute was examined by analyzing these data. The results of the examination showed that the introduction of this system made it possible to analyze all the data quickly. Before the introduction of this system, the data able to be analyzed had been restricted because of their complexity. The results also showed that this system would be useful for understanding the examinations carried out in the whole hospital as well as for analyzing metastatic tumors and the number of patients receiving examinations. Furthermore, this system would be helpful in the logical analysis of reports prepared by doctors.

Patch angioplasty of the superior vena caval obstruction (case reports with long follow-up results).

Four cases of the superior vena caval obstruction caused by mediastinal tumor and fibrous mediastinitis are presented. Palliation was achieved by resection of tumor or scar tissue and placing a Teflon or autogenous vein patch to the resected defect of the superior vena cava. One of the patients died of metastasis at 8 months postoperatively, but patency of the patch graft was confirmed. The other three are alive and well at 14, 14 and 11 years, respectively. Continued patency has been confirmed by venographies in two of them.

Early clinical results of the modified human umbilical cord vein homograft (Dardik Biograft).

Fifty-six reconstructions using Biograft were performed in 45 cases with peripheral arterial occlusion. These consisted of 11 femoro-popliteal bypasses above the knee, 19 femoro-popliteal below the knee, 10 femoro-tibial or peroneal and 16 other miscellaneous procedures including aorto-femoral and extra-anatomic bypasses. Overall patency rates for each type of procedure were 81.8%, 78.9%, 30.0%, and 93.8%, respectively. The cumulative patency rate (calculated by the life table method) for the total group was 70.5% at 56 months. Early failure was thought to relate to technical factors in most instances. The majority of late failures was due to intimal hyperplasia at the distal anastomotic site. One of 9 late failures was successfully treated by patch angioplasty at 23 months following initial operation. Therefore, in order to attain better late results, early discovery of developing stenosis by noninvasive examination techniques and arteriography is important.
