Allergic contact dermatitis from gardenia fruit.

A 41-year-old man, with severe itchy dermatitis on the left leg, had treated a bruise on the left knee with a topical folk remedy called Sokujikoh, which contains about 16% powdered gardenia fruit. After 4 days, an exacerbation of symptoms appeared, with edema, erythema, and vesicles on the treated area. Patch testing indicated sensitivity to the seeds of gardenia fruit and, to a lesser extent, the pericarp (fruit wall). The allergens could be extracted into ethanol and, less readily, into water.

S-adenosyl-L-methionine ameliorates reduced local cerebral glucose utilization following brain ischemia in the rat.

The average values of the local cerebral glucose utilization (LCGU) were approx. 35-110 mumols/100 g/min in various nuclei in the brain of sham-operated animals. The values, however, were decreased by 25-75% in all areas examined at 24 hr after 4-vessel occlusion (forebrain ischemia) for 0.5 hr. Particularly, the LCGU values in the forebrain nuclei such as the cerebral cortices and thalamus were severely reduced, while that in the nuclei in the midbrain and hindbrain such as the red nucleus, chochlear nucleus and vestibular nucleus were slightly reduced. In rats treated with S-adenosyl-L-methionine (SAM, 100 mg/kg, i.p.) every 1 hr for 6 times from the recirculation, however, the LCGU values were increased in all brain structures by 35-195% in the forebrain ischemic rat. A significant increase was observed in the cerebral neocortices, caudate-putamen, lateral septal nucleus, thalamic nuclei, substantia nigra, cerebellum and some other nuclei. In some forebrain structures whose LCGU were mildly reduced to approx. 60-70% by the ischemia, SAM recovered the LCGU to more than 90% of the value in the sham-operated group. Thus it was concluded that SAM ameliorated widely ischemia-induced reduction of LCGU in the rat.

Two types of glomerular lesion in non-amyloid immunoglobulin deposition disease: a case report of IgD-lambda myeloma.

A 60-year-old man with nephrotic syndrome and renal failure was found to have a IgD-lambda myeloma. Autopsy revealed advanced nodular glomerulopathy with codeposition of delta heavy and lambda light chains. Glomerular nodules were of two types: One (type I lesion) is caused by nodular expansion of the mesangium by deposition of abnormal immunoglobulin. The second type (type II lesion) is peripherally lamellated and is ascribed to repeated local mesangiolysis superimposed upon type I lesion. These type I and type II lesions correspond to the two types of nodules in diabetic glomerulosclerosis.

[Non-invasive assessment of mitochondrial function in rat brain during and after hemorrhagic shock].

Brain oxygen metabolism was studied noninvasively by the simultaneous measurement of hemoglobin (Hb) oxygenation, and cytochrome oxidase (aa3) redox state in the brain of a rat, using our near-infrared (NIR) transmission spectrophotometer. Redox state of cyt. aa3 was measured using a wavelength pair of 830-940 nm. The changes of oxy- and total (oxy + deoxy) Hb content in the rat brain could be monitored quantitatively by expressions of -1.15 delta A750 + 1.39 delta A780 and -0.29 delta A750 + 0.59 delta A780, respectively (delta A750, delta A780: absorbance changes at wavelength 750 and 780 nm). An application of NIR-Spectrophotometry for the studies of brain metabolism in acute hemorrhagic shock in rats was presented. Tissue hypoxia was induced in 20 rats by rapid withdrawal of arterial blood to a mean blood pressure of 30 mmHg. This level of shock pressure was maintained for 30 minutes in group A (n = 8), and for 70 minutes in group B (n = 10), then the removed blood was returned by infusion. Blood oxygenation level, relative blood volume and cyt. aa3 redox state in the rat brain were monitored during shock, and for two more hours after the reinfusion. Group A had a high survival rate while group B had a poor survival rate. In both groups with the onset of hypotension, there occurred a rapid deoxygenation of brain blood and a rapid and progressive shift of cyt. aa3 towards a more reduced state.(ABSTRACT TRUNCATED AT 250 WORDS)

[Bronchiectasis with normal paranasal sinus roentgenogram].

Bronchiectasis has come to be considered as a type of sinobronchial syndrome in Japan, but there exist some cases without chronic sinusitis. We studied the clinical features of 14 cases of bronchiectasis with definitely normal paranasal sinus roentgenogram, diagnosed during the past ten years. There were eleven middle-aged women and three men. Ten patients (71%) complained of hemoptysis, one (7%) of dry cough, one (7%) of productive cough, and the two (14%) had no complaint. In seven patients (50%) CT and bronchography showed localized cylindrical bronchiectasis in the right middle lobe and/or left upper lobe lingular division. They were considered to be middle lobe lingular syndrome. Three patients (22%) with localized varicose or cystic bronchiectasis had a history of pneumonia or pertussis in their infancy, so their bronchiectasis were considered secondary to infantile bronchopulmonary disease. Two patients (14%) had diffuse cystic bronchiectasis and were almost asymptomatic. They might be cases congenital bronchiectasis or Williams-Campbell syndrome. Pulmonary function tests were normal in most of the cases and sputum culture revealed no cases of persistent bacterial infection. These clinical features are quite different from those of bronchiectasis reported as sinobronchial syndrome, in which chronic productive cough, poor pulmonary function, persistent bacterial infection, etc. are significant. So we conclude that there are two distinct groups in bronchiectasis.

[DBMP-85 was effective at diagnosis and LVP was effective at relapse in a case of acute mixed leukemia].

A 16 year-old boy was admitted to our hospital in April 1985, because of bilateral submandibular swellings. Hematological examination revealed Hb was 7.3 g/dl, WBC was 89,000/microliters (76% blast), and platelet was 154,000/microliters. His bone marrow was hypercellular and consisted with 91% blasts. Myeloperoxidase staining was positive for 38% of blasts. Auer rods were seen in some of blasts. Thus, the diagnosis was M1 according to FAB classification. Cytogenetic studies of 20 marrow cells were performed and all cells had 46, XY, -1, -7, 3q-, 7q-, 17q+, +2mar. Eighty five percent of blasts expressed HLA-DR and 43% of blasts expressed CD2 and CD13 simultaneously. Thus, this leukemia was considered as the hybrid type of acute mixed leukemia by surface marker analysis. DBMP-85 regimen, the chemotherapy for AML, was started after admission and complete remission (CR) was attained in June 1985. After 4 courses of post remission chemotherapy, he discharged in December 1985 and was followed at our outpatient clinic without chemotherapy. His disease was relapsed in June 1986, and the combination chemotherapy with mitoxantrone, etoposide and Ara-C was applied to him but failed to attain CR. Then, LVP protocol, the chemotherapy for ALL, was started and CR was achieved. The blasts at relapse had morphologically myeloid features, and expressed HLA-DR, CD2 and CD13 as well as at diagnosis. Cytogenetic studies at relapse showed some karyotype except gaining 12p- anomaly. Therefore, same blasts were considered to emerge at relapse. Our case suggests that LVP therapy may be effective for AML expressing myeloid and lymphoid surface markers.

[A case of perianeurysmal fibrosis treated by the thrombo-exclusion technique].

A very rare case of perianeurysmal fibrosis was presented. A 65-year old man was admitted with the complaint of abdominal pulsatile mass and intermittent claudication of right lower limb. Aortography revealed fusiform, calcified aneurysm of the infrarenal abdominal aorta and severe stenosis of right common iliac artery. CT scan demonstrated a soft tissue density structure around the aortic aneurysm which was enhanced after contrast medium injection. At surgery, the aneurysm had a thick, firm, smooth wall which was shiny white in appearance and adherent perianeurysmal fibrosis was marked. In order to avoid injury to surrounding tissue such as the duodenum or vena cava, thrombo-exclusion method was chosen. Histological examination of the aneurysmal wall showed fibrous tissue infiltrated with mononuclear cells such as lymphocytes and plasma cells. The postcontrast enhancement of aortic wall and perianeurysmal sheet disappeared on CT on the post-operative 57th day and no recurrence has been observed.

A cDNA fragment of hepatitis C virus isolated from an implicated donor of post-transfusion non-A, non-B hepatitis in Japan.

Recently, a cDNA from the hepatitis C virus (HCV) RNA genome has been isolated in the USA from a chronically infected chimpanzee. In order to isolate HCV cDNA derived from human material, RNA was extracted from plasma of a Japanese blood donor implicated in post-transfusion non-A, non-B hepatitis and HCV cDNA was synthesized and amplified by the PCR method using HCV-specific oligonucleotide primers. The cDNA fragment, 583 nucleotides long, showed 79.8% homology at the nucleotide level and 92.2% homology at the amino acid level compared with the prototype HCV cDNA. These results provides further evidence to show that HCV is closely associated with the development of post transfusion non-A, non-B hepatitis.

[An assessment of therapeutic effects by transcatheter arterial embolization (TAE) with degradable starch microspheres (DSM) for hepatic malignant tumors].

Transcatheter arterial embolization (TAE) by using degradable starch microspheres (DSM) as embolic material, 40 microns in diameter and degraded by serum amylase within one hour, was carried out for thirteen cases with hepatic malignancies including eleven cases with hepatocellular carcinoma (HCC), one case with cholangiocarcinoma and one case with metastatic liver cancer. DSM were mixed with anticancer agents and administered through the catheter, which was introduced by Seldinger's method, via the hepatic artery immediately after hepatic angiography in ten cases and through subcutaneously implanted drug delivery system (Port-A-Cath) in three cases. The dose of DSM was 900 mg/body and adriamycin 30-40 mg/m2 or 12-14 mg/m2 were used. The former was administered through the catheter immediately after angiography and the latter through Port-A-Cath for HCC and cholangiocarcinoma. A same dose of DSM and mitomycin C 15-16 mg/m2 was administered for metastatic liver cancer through the catheter immediately after angiography. The administration was repeated weekly in three cases in which Port-A-Cath was implanted, and at five weeks' interval through the catheter immediately after hepatic angiography in the other ten cases. Therapeutic effects were assessed and pharmacokinetics of adriamycin were studied. Results were as follows; 1) Partial response (PR) was obtained in five cases out of eleven cases with HCC (45.5%) and there were three cases with minor response (MR) in the other six cases. Totally, decrease of tumor size was demonstrated in eight cases out of eleven cases (72.7%). 2) In nine cases in which AFP was positive, the titer of AFP was decreased in seven cases (77.8%). 3) No change (NC) was obtained in cholangiocarcinoma.(ABSTRACT TRUNCATED AT 250 WORDS)

Spectroscopic and kinetic evidence for the thiolate anion of glutathione at the active site of glutathione S-transferase.

Ultraviolet difference spectroscopy of the binary complex of isozyme 4-4 of rat liver glutathione S-transferase with glutathione (GSH) and the enzyme alone or as the binary complex with the oxygen analogue, gamma-L-glutamyl-L-serylglycine (GOH), at neutral pH reveals an absorption band at 239 nm (epsilon = 5200 M-1 cm-1) that is assigned to the thiolate anion (GS-) of the bound tripeptide. Titration of this difference absorption band over the pH range 5-8 indicates that the thiol of enzyme-bound GSH has a pKa = 6.6, which is about 2.4 pK units less than that in aqueous solution and consistent with the kinetically determined pKa previously reported [Chen et al. (1988) Biochemistry 27, 647]. The observed shift in the pKa between enzyme-bound and free GSH suggests that about 3.3 kcal/mol of the intrinsic binding energy of the peptide is utilized to lower the pKa into the physiological pH range. Apparent dissociation constants for both GSH and GOH are comparable and vary by a factor of less than 2 over the same pH range. Site occupancy data and spectral band intensity reveal large extinction coefficients at 239 nm (epsilon = 5200 M-1 cm-1) and 250 nm (epsilon = 1100 M-1 cm-1) that are consistent with the existence of either a glutathione thiolate (E.GS-) or ion-paired thiolate (EH+.GS-) in the active site. The observation that GS- is likely the predominant tripeptide species bound at the active site suggested that the carboxylate analogue of GSH, gamma-L-glutamyl-(D,L-2-aminomalonyl)glycine, should bind more tightly than GSH.(ABSTRACT TRUNCATED AT 250 WORDS)

Non-traumatic dissecting aneurysms of the intracranial vertebral artery. Report of six cases.

We present 6 cases with dissecting aneurysm of the intracranial vertebral artery who developed subarachnoid haemorrhage (SAH). The following procedures were performed in this series; trapping of the involved artery in 2, proximal occlusion of the vertebral artery with detachable balloon in 2, and proximal clipping of the vertebral artery in 2. Proximal occlusion of the vertebral artery in 4 and trapping of the vertebral artery in one gave excellent results. We believe the treatment of choice is proximal occlusion of the vertebral artery, either by open surgery or by interventional neuroradiological procedures.

Aminopeptidases in human retroplacental sera: purification and characterization of two enzymes.

Ten different maternal serum samples were analyzed for the hydrolysis of S-Bz-Cys-pNA (substrate for CAP) and Ala-pNA. The results showed clear differences in the activities in individual sera. Similar S-Bz-Cys-pNA hydrolysis activity was detected for all sera. However, Ala-pNA hydrolysis activity differed remarkably. Serum exhibiting low Ala-pNA hydrolysis activity contained only CAP, and that exhibiting high Ala-pNA hydrolyzing activity contained CAP and AAP. The two aminopeptidases were independently purified to a homogeneous state through the same purification procedures and some of their biochemical properties were compared. The enzymes were quite different with respect to molecular mass, the substrate specificities for some aminoacyl-pNA substrates, and the effects of inhibitors. Among various natural peptides tested for hydrolysis, the enzymes hydrolyzed Met-enkephalin most rapidly, but their modes of action were different. Furthermore, only CAP degraded oxytocin and AAP exhibited a high kinin-converting activity.

Development of ion channels and neurofilaments during neuronal differentiation of mouse embryonal carcinoma cell lines.

1. an embryonal carcinoma cell line, PCC4-Aza1-ECA2, was induced to differentiate to neurones by two different procedures: an addition of retinoic acid to the culture medium or a reduction of serum concentration. The changes in membrane currents during differentiation were studied by the whole-cell variation of the patch-clamp technique and the change in neurofilament expression was studied immunohistochemically. 2. Stem cells showed the outward K+ current which inactivated slightly, but no inward currents were observed. These cells did not express neurofilament. 3. Three days after an addition of 10(-7) M-retinoic acid, neurofilament-positive round cells without processes began to appear. The inward currents observed in these cells were the Na+ current and fast-inactivating Ca2+-channel current. Four days after an addition of 10(-7) M-retinoic acid, the cells began to extend processes and showed an intense neurofilament expression. The inward currents were the Na+ current and slow-inactivating Ca2+-channel current, while the fast-inactivating Ca2+-channel current observed previously had almost disappeared. The amplitude of the outward K+ current was larger than that in the stem cell and it did not show clear inactivation. 4. By reducing the serum concentration in the medium from 10 to 0.1%, cells with processes were observed after 6 days. They were neurofilament-positive and had the Na+ current, both fast- and slow-inactivating Ca2+-channel currents, and the outward K+ current which inactivated slightly. 5. The properties of these ionic currents observed in induced neurones were studied. The Na+ current was blocked by 0.1 microM-tetrodotoxin at any stage. The Na+ current was evoked by a depolarization pulse to a level above -40 mV with a maximum amplitude at around -10 mV. The fast-inactivating Ca2+-channel current was evoked by a depolarization to a level above -50 mV with a maximum amplitude at around -15 mV. It was resistant to 50 microM-Cd2+. The slow-inactivating Ca2+-channel current was evoked by a depolarization pulse to a level above -30 mV with a maximum amplitude at around +5 mV. It was blocked by 50 microM-Cd2+. It showed slight inactivation, which was not voltage-dependent but current-dependent. It was enhanced by 1 microM-Bay K 8644. The outward K+ current was blocked by replacing intracellular K+ with Cs+. 6. Another embryonal carcinoma cell line, P19, was induced to differentiate to neurons by adding 10(-6) M-retinoic acid to the medium.(ABSTRACT TRUNCATED AT 400 WORDS)

Endothelium-dependent relaxation of human basilar arteries.

We studied the effects of acetylcholine and calcium ionophore A23187 on human basilar artery rings. Among 11 arteries, both agents produced significant relaxations in five, A23187 but not acetylcholine caused a response in six, and neither agent was effective in four. After rubbing off the endothelium, the relaxations induced by both agents were significantly attenuated. Indomethacin (a cyclooxygenase inhibitor) and AA861 (a specific inhibitor of 5-lipoxygenase) did not but SKF525A (an inhibitor of cytochrome P450-dependent monooxygenase) did significantly inhibit the acetylcholine-induced relaxation in human basilar arteries. On the other hand, AA861 inhibited while neither indomethacin nor SKF525A had any effect on the A23187-induced relaxation. Our results suggest that different mechanisms may be involved in acetylcholine and A23187-induced relaxations in human basilar arteries.
