[Molecular oncological study on DNA ploidy, Ki-ras point mutation, and p21 expression in colorectal cancer].

We studied DNA ploidy, point mutation of Ki-ras oncogene codon 12, and p21 expression using paraffin embedded materials from 42 cases of colorectal cancer. DNA ploidy was measured by the method of Hedley et al. flow cytometrically. Point mutation of Ki-ras oncogene was examined by the method of Bos et al. using a dot-blot screening procedure, and p21 expression was examined immunohistochemically. Incidence of aneuploidy, Ki-ras point mutation, and p21 expression was 71.4%, 26.2%, 40.5%, respectively. There was a very weak correlation between p21 expression and pathologic findings, but there was no correlation between pathologic findings and DNA ploidy, as well as Ki-ras point mutation. Patients who showed aneuploidy tended to have more point mutation of Ki-ras oncogene. There was no correlation between p21 expression and Ki-ras point mutation, as well as DNA ploidy. Although there was no correlation between Ki-ras point mutation and survival, a significant correlation between survival and DNA ploidy, as well as p21 expression was recognized. Patients who had tumors with diploidy or p21 expression tended to have better prognosis.

Primary structure and functional expression of a rat G-protein-coupled muscarinic potassium channel.

Parasympathetic nerve stimulation causes slowing of the heart rate by activation of muscarinic receptors and the subsequent opening of muscarinic K+ channels in the sinoatrial node and atrium. This inwardly rectifying K+ channel is coupled directly with G protein. Based on sequence homology with cloned inwardly rectifying K+ channels, ROMK1 (ref. 11) and IRK1 (ref. 12), we have isolated a complementary DNA for a G-protein-coupled inwardly rectifying K+ channel (GIRK1) from rat heart. The GIRK1 channel probably corresponds to the muscarinic K+ channel because (1) its functional properties resemble those of the atrial muscarinic K+ channel and (2) its messenger RNA is much more abundant in the atrium than in the ventricle. In addition, GIRK1 mRNA is expressed not only in the heart but also in the brain.

Primary structure and functional expression of a mouse inward rectifier potassium channel.

A complementary DNA encoding an inward rectifier K+ channel (IRK1) was isolated from a mouse macrophage cell line by expression cloning. This channel conducts inward K+ current below the K+ equilibrium potential but passes little outward K+ current. The IRK1 channel contains only two putative transmembrane segments per subunit and corresponds to the inner core structure of voltage-gated K+ channels. The IRK1 channel and an ATP-regulated K+ channel show extensive sequence similarity and constitute a new superfamily.

111In-labeled Mn-metalloporphyrin for tumor imaging.

We synthesized and developed a new tumor imaging agent, 111In-labeled metalloporphyrin (111In-ATN-10) which consists of a carrier (ATN-10) of the tumor imaging possessing both a non-radioactive manganese complex in the porphyrin ring and a bifunctional chelating group (DTPA) attached to its side chain. The images of the three kinds of tumors were delineated more clearly by 111In-ATN-10 than 67Ga-citrate. Moreover, there was no photosensitivity in ATN-10. 111In-ATN-10 is studied as a new tumor positive scintigraphic agent instead of 67Ga-citrate.

Introduction of a fluorescence-microscopic technique for the detection of eggs, egg shells, and mites in scabies.

A technique using the fluorescence microscope can prove helpful in the laboratory diagnosis of scabies. Specimens from fifteen patients with scabies were used in this study. All of the specimens were embedded with glycerine instead of potassium hydroxide (KOH) solution. The specimens were examined at 0 min, 30 min, 1 hr, 6 hrs, 24 hrs, and one week after mounting under light and fluorescence microscopes. Specimens embedded with non-fluorescent glycerine were not clear immediately after mounting but became so after about 1 hr. Eggs and egg shells were easily counted in the specimens under the fluorescence microscope but were very hard to identify under the light microscope. Mites were absent in half of the specimens; only eggs and egg shells were present in those specimens found by the fluorescence microscope. The above findings suggest that the detection of egg shells by the use of fluorescence microscope may be helpful for the diagnosis of scabies, in particular with mite negative specimens. Slides prepared with non-fluorescent glycerine were more stable and could be preserved for a long time. However, this method is time-consuming and requires expensive equipment.

Mouse mammary tumor virus with rearranged long terminal repeats causes murine lymphomas.

Mouse mammary tumor virus (MMTV) is a slowly transforming retrovirus associated primarily with the induction of mammary tumors. It is widely accepted that T-cell lymphomas of various mouse strains are associated with extra proviruses of MMTV. These extra proviruses showed site-specific rearrangements in the U3 region of long terminal repeats (LTRs), consisting of about 400 nucleotide deletions and occasional substitution resulting in unique tandem repeats. However, the question of whether these mutant MMTVs cause lymphomas has not been experimentally resolved. Here we present distinct evidence that they do. We constructed chimeric MMTVs by replacing the LTR of the recently constructed pathogenic MMTV provirus clone with rearranged LTRs of MMTV proviruses obtained from two DBA/2 mouse lymphoma cell lines, MLA and DL-8, and inoculated them into BALB/c mice. These mice developed lymphomas, but no mammary tumors, 4 to 11 months postinoculation, whereas the original pathogenic MMTV clone alone induced mammary tumors. These results showed that the tissue specificity of MMTV tumorigenesis is determined by the LTR structures.

[Effects of intravenous administration of nasaruplase, a plasminogen pro-activator, on left ventricular function and systemic hemodynamics in a canine model of acute myocardial infarction].

The thrombolytic effects and changes in left ventricular function after intravenous administration of nasaruplase were examined in a canine thrombus model of acute myocardial infarction. When 8 U/kg/min of nasaruplase was intravenously administered after 30 min of coronary arterial occlusion, coronary recanalization was achieved in 78.6% (11/14) of the animals with little change in plasma fibrinogen concentration. There was no further decrease in cardiac output, left ventricular ejection fraction or regional wall motion immediately after reperfusion, compared with 30 min after coronary arterial occlusion. However, all these parameters returned to pre-occlusion levels one week after reperfusion. Nasaruplase markedly reduced infarct size as well as cardiac hypertrophy following coronary arterial occlusion, demonstrating suppression of the development of ischemic myocardial damage. These results indicate that coronary thrombolytic therapy with intravenous administration of nasaruplase is useful for the treatment of acute myocardial infarction.

Chronobiology in cardiology.

In these years, circadian periodicity in the onset of myocardial ischemia, myocardial infarction, sudden cardiac death and ischemic stroke has been confirmed by several investigators. There has also been reported that there exists a significant circadian rhythm in ventricular arrhythmias in patients with myocardial infarction. These recent advances in chronocardiology depend on a development of a high quality built-in A/D converter in ambulatory ECG monitoring system and a remarkably developed sophisticated software. One of the most current topics in recent years is heart rate (HR) variability. As an index of HR variability, RR50 is frequently estimated by the cosine fitting technique. Next, we analyzed the so-called Lorenz plot for another index of HR variability, that is preceding R-R intervals and coupling intervals were plotted sequentially every 3 hours on the abscissa and ordinate, respectively. Finally, we investigated HR variability by power spectral analysis of R-R intervals both by the maximum entropy method and the fast Fourier transform. Mainly, in this paper, we introduced current topics in HR variability, but recent advances in ambulatory blood pressure (BP) monitoring system are also strikingly remarkable. Our newly developed monitoring system of physical activity donated quite a few informations evaluating episodic changes of HR and BP. This device was also useful for finding out the difference between the individual life styles, such as the eveningness versus morningness. In conclusion, these very recent advances in ambulatory monitoring system should be really available for the clinical practice from this time onward.

Comparison of reversed and in situ saphenous vein grafts for infragenicular bypass: experience of two surgeons.

A series of 241 consecutive autogenous vein bypasses for chronic lower-limb ischaemia performed by two surgeons since 1980 was reviewed. After 1985, in situ vein bypass was employed preferentially and was compared with reversed vein bypass. The two groups of patients had similar risk factors, indications and outflow. Of the 241 bypasses, 157 were to the below-knee popliteal artery and 84 to infrapopliteal arteries. The utilization rates of a single ipsilateral saphenous vein were 57.5% for reversed and 71.9% for in situ vein bypass. However, in situ vein bypass was impossible in 43 procedures and these were changed to the reversed operation with contralateral vein. The primary 5-year patency rates of reversed and in situ vein bypass grafts to the popliteal artery were 82.5 versus 74.5%, and the primary 4-year patency rates for infrapopliteal bypass 68.5 versus 80.0%. The respective secondary patency rates were 94.2 versus 92.1% and 85.7 versus 91.1%. The main cause of graft failure was vein graft stenosis (reversed vein bypass, 13.0%; in situ, 11.1%), which usually occurred in the first 2 years after surgery. Of 23 grafts revised for stenosis, 21 were salvaged and restenosis rarely occurred. Both reversed and in situ vein bypass grafts were equally effective, but careful surveillance for 2 years and aggressive revision were extremely important after either type of reconstruction.

[Chemo-endocrine therapy with high dose medroxyprogesterone acetate for recurrent or advanced breast cancer].

The endocrine therapy with high-dose medroxyprogesterone acetate (MPA) was applied for 19 patients with recurrent breast cancer and 3 with advanced breast cancer. According to the dominant lesion of the disease, the patients were divided into three groups; 13 with soft tissue lesion, 5 with visceral lesion, and 4 with osseous lesion. The serum MPA level was higher than 25 ng/ml in 14 cases out of 19 examined. The results of objective responses in second-line therapy with MPA were CR in two cases, and NC or PD in 20 cases. In 19 out of 22 patients with MPA therapy, the periods of survival were longer than the period of 50% survival of the patients without MPA therapy.

[UFTPM (UFT + CDDP + MMC) therapy for progressive stomach cancer. Research Association of Progressive Stomach Cancer].

We organized a cooperative research group consisting of 10 institutions and UFTPM therapy was given patients with unresectable and postoperatively relapsed stomach cancer. As the result, 20 cases were registered and the ratio of PR according to the criteria of stomach cancer chemotherapy was 4 out of 17 complete cases (23.5%), excluding 3 incomplete cases. The efficacy for the cases treated with more than 2 courses was 36.4% (4/11) and that for cases with 3 courses was 75% (3/4). One case with 3 courses was resectable and the cancer cells disappeared. Adverse effects of grade 3 or more according to the WHO criteria were observed in 20% of the cases.

[Resection of posterior mediastinal cyst under thoracoscopy].

A 52-year-old woman with left posterior mediastinal cyst underwent surgical removal using thoracoscopy. Under general anesthesia by use of one lung ventilation, thoracoscopy was introduced through a trocar at the mid-axillary line of the 7th intercostal space. Grasping forceps were introduced through a incision in the anterior-axillary line of 4th and 5th intercostal space. The lung was held with grasping forceps. The lung was fixed to the region of anterior-mediastinum with the weight of forceps. Then posterior-mediastinum was able to be observed well by the operator. The thin cyst-wall was biopsied and excised as much as possible. Residual cyst-wall was burned by electric knife. A pathological diagnosis probed that the tumor was bronchogenic cyst without malignancy. Post operative pain markedly reduced. The patient rapidly recovered and was discharged on the 6th postoperative day. Thoracoscopic resection of mediastinum cyst may be a useful approach.

[Clinical and pathological studies on superficial esophageal carcinoma by flow cytometric analysis of nuclear DNA content].

Correlation between histological type, depth of invasion, nodal involvement, macroscopic type, survival rate and nuclear DNA content was studied by flow cytometer using paraffin-embedded materials from 52 cases of superficial esophageal carcinoma. There were 23 cases (44.2%) of diploid and 29 cases (55.8%) of aneuploid. There was no correlation between DNA aneuploidy and depth of invasion or size of the carcinoma. Carcinoma with aneuploid had a high frequency of nodal involvement and well-differentiated histological type. Carcinoma with deep of highly elevated macroscopic type has a tendency to have higher DNA content. There was no correlation between survival rate and nuclear DNA ploidy patterns.

[Results of arterial reconstruction in Buerger's disease].

One hundred and eight bypass operations for Buerger's disease were performed between November 1976 and August 1990. The bypass procedures, prostheses, cumulative patency rate and the influence of smoking on the results were discussed. Five-year cumulative patency rate of Aorto-femoral bypass was 88.2%. Aorto-femoral bypass should be always indicated as a standard procedure for the iliac lesion in Buerger's disease, because the patency rate was very low in aorto-iliac bypass group or ilio-femoral bypass group. Five-year cumulative patency rate of infrainguinal bypass using autogenous vein was 64.8%, and that in the patients who quit cigarette smoking was as high as to 73.3%. We should use autogenous vein as much as possible for infraiguinal bypass and discontinuance of cigarette smoking is considered to be one of the important factors to improve the patency rate in Buerger's disease.

[Extensive bowel infarction caused by intraaortic balloon pumping].

Two cases of an extensive bowel infarction occurred during Intraaortic Balloon Pumping (IABP) are reported. In the first, an acute myocardial infarction was seen suddenly on the seventh postoperative day following arterial reconstruction of the right lower leg and his condition required IABP support. Five day after beginning of IABP, severe symptoms suggesting peritonitis appeared, and laparotomy revealed an extensive infarction of small bowel and sigmoid colon. In the second, the patient required IABP support for weaning from cardiopulmonary bypass after completion of coronary artery bypass grafting. A severe peritonitis was caused by an extensive infarction of small and large intestine during two days of IABP support. Cholesterol emboli were found in the small bowel arteries of the case 1 at microscopic examinations of the autopsy specimen. Both patients died of cardiopulmonary failure and renal failure respectively, though necrotic intestine were resected. Circulatory support using IABP should not be selected, when a patient has severe arteriosclerotic lesions of the aorta.
