PURPOSE: To study the patterns of structural damage of the optic disk in patients with early focal visual field loss using a confocal scanning laser ophthalmoscope. METHODS: Thirty-nine subjects with repeatable early focal visual field loss were included. The Heidelberg Retina Tomograph (Heidelberg Engineering, Heidelberg, Germany) was used to obtain topographic optic disk measurements. For analysis of structural damage, the topographic measurements were divided into 36 10-degree sectors. Sector analysis was performed using sector rim area to sector disk area ratio. Patients were assigned to one of three groups based on the measurement of the optic disk damage: (1) diffuse disk damage; (2) focal disk damage; or (3) no detectable disk damage. RESULTS: Approximately half of the patients with early focal glaucomatous visual field loss showed diffuse optic disk damage, one quarter to one third had focal damage, and one sixth had no detectable damage. Optic disk area was smallest in the no-detectable-damage group (1.71 + 0.19 mm2), followed by the focal-damage group (2.06 + 0.54 mm2), and was largest in the diffuse damage group (2.29 + 0.48 mm2; P = .22). CONCLUSION: In patients with early focal glaucomatous visual field loss, observable optic disk damage can be diffuse, focal, or undetectable. The Heidelberg Retina Tomograph may be capable of detecting different patterns of diffuse or focal structural damage.
Chlorogenic acid, a dietary antioxidant, effectively inhibited the iron-induced lipid peroxidation of bovine liver microsomes in a concentration-dependent manner. In the Fenton-type reaction, chlorogenic acid inhibited the production of the hydroxyl radical by iron-EDTA or iron-ADP, while iron plus chlorogenic acid did not generate the hydroxyl radical. The formation of an iron complex with chlorogenic acid was demonstrated by UV/vis absorbance spectroscopic, ESR and 1H-NMR studies. The ferric complex with chlorogenic acid was in the ferric high-spin state near rhombicity, and had no radical scavenging activity. The results indicate that chlorogenic acid prevented the formation of the hydroxyl radical by forming a chelate with iron whose complex cannot catalyze the Fenton-type reaction.
Catalase catalyzed the peroxynitrite-mediated nitration of 4-hydroxyphenylacetic acid. The curve for the pH dependence of nitration was similar to that for the reaction between peroxynitrite and phenol. Cyanide, azide, and 3-amino-1,2,4-triazole inhibited the nitration in a dose-dependent way. When catalase was mixed with peroxynitrite, Compound I was detected as an intermediate. Because azide was an electron donor for the peroxidatic action of catalase, and because 3-amino-1,2,4-triazole inhibited catalase activity by binding with Compound I, peroxynitrite-mediated phenolic nitration was probably accompanied by Compound I formation. Both catalase and superoxide dismutase protected Escherichia coli from peroxynitrite toxicity.
Diarrhea, sudden death after short duration of diarrhea and sudden death without apparent signs were observed in a herd of breeder pigs. Five pigs that died suddenly with diarrhea (SDD pigs) and 6 pigs that died suddenly without signs (SD pigs) were examined. The average age of the pigs was about 28 days. Twelve pigs of age 10 to 14 days old showing diarrhea (D pigs) were also examined. Eleven of them recovered. Large numbers of Escherichia coli were detected in all organs of every SDD and SD pig and in feces of D pigs. All of the isolates were identified as enterotoxigenic E. coli (ETEC) by the polymerase chain reaction (PCR). Porcine reproductive and respiratory syndrome (PRRS) virus cDNA was also detected from the lung of every SD and SDD pig by the RT-PCR. High and low titers of antibodies to PRRS virus were found in 10-day-old and 1-month-old pigs, respectively. In an experiment, 3 ETEC were isolated from 9 healthy weaning pigs during the quiescent stage in the herd. These data showed that growth of the ETEC was not active in healthy weaning pigs; however, following infection with PRRS virus ETEC infection became systemic and caused peracute death in the weaning pigs. It suggested also that infection with PRRS virus in 10-day-old pigs were protected by the colostral antibodies, and fatal infection by ETEC did not occur as a result.
Intravenously injected ultrasonic contrast agents have improved sonographic visualization of blood flow. On enhanced color Doppler sonography using Levovist (Schering AG, Germany) and FS069 (Molecular Biosystems, USA), minute tumor blood flow in woodchuck hepatoma was clearly demonstrated as vascular flow around and within the tumors. Furthermore, on enhanced gray scale sonography using FS069, parenchymal flow was demonstrated as sonographic "tumor stain". However, larger doses of the agent provided shadowing that disturbed sonographic evaluation of deeper portions of the liver. With advances of second harmonic imaging, it may be possible to evaluate only blood perfusion in that it eliminates the signals of fundamental frequency.
PURPOSE: To evaluate efficacy of contrast-enhanced power Doppler imaging of the liver. MATERIALS AND METHODS: We administered two kinds of contrast agents, SHU-508 (Schering AG, Germany) and FS069 (MBI, USA), intravenously to woodchucks with hepatocellular carcinomas. Power Doppler imaging of the liver was performed. To obtain minute hemodynamic information of the liver, we tried continuous infusion of contrast agents and flash echo mode (intermittent scanning) on power Doppler imaging. RESULTS: When we administered continuously contrast agents, the enhancement increased gradually and lasted until the end of the infusion. To compare with bolus injection, we were able to detect blood flow of the liver more clearly and easily without saturation artifacts. On power Doppler flash echo imaging of three or five seconds suspension, signals from only arterial flow were obtained. In comparison on seven seconds suspension imaging, signals from both arterial and portal flow were obtained. With flash echo mode, we could acquire the information of blood perfusion to depict the signals from eliminated microbubbles. CONCLUSION: With continuous infusion and flash echo mode on contrast-enhanced power Doppler imaging, we could get more detailed information of blood flow and perfusion of the liver. This method is expected to be useful for the diagnosis of liver tumor in clinical settings.
We report here on treatment results of consecutive CCLSG NHL studies (NHL855, 1985-1989; NHL890, 1989-1996). The NHL855 protocol consisted of an induction phase of five drugs (VCR, PRD, CPM, DXR, and high-dose MTX) and a maintenance phase of 7 drugs. The probabilities of EFS at 7 years were 78% (SE, 10%) for the patients with localized disease, and 38% (SE, 7%) for those with advanced disease. In the NHL 890 protocol, the patients were assigned to two different treatment groups according to their histology and received different consolidation therapy; non-lymphoblastic subtype was treated almost identically to NHL855 while LASP and VP-16 were newly added for the lymphoblastic subtype. The 7-year EFS improved to 91% (SE, 6%) for localized disease, and 61% (SE, 6%) for advanced disease. A remarkable improvement was particularly evident for lymphoblastic type with mediastinal mass. Optional trial of high-dose sequential chemotherapy and peripheral blood progenitor cell auto grafting resulted in an unfavorable outcome. The 7-year EFS according to main histological subgroups were as follows: 84% (10%) for large cell type, 67% (11%) for Burkitt's-type, 58% (10%) for lymphoblastic type. Secondary cancer occurred in two of the 163 patients studied. Both patients were AML (M0/M4) and MLL rearrangement was detected in the M4 case.
This study evaluated the usefulness of multiple overlapping stenting for the treatment of a diffuse lesion in a single and small coronary artery. We studied 48 consecutive patients with a lesion of a vessel with reference diameter < 3.0 mm who received Wiktor stent implants due to suboptimal result or threatened closure after plain old balloon angioplasty (POBA). The patients were divided into 2 groups: Group D, 18 patients with a diffuse lesion with length > or = 15.0 mm who were treated with 2 overlapping Wiktor stents; and Group S, 30 patients with a discrete lesion who were treated with a single Wiktor stent. Two patients with a discrete lesion were not included in Group S because of failure of stenting. Stenting was successful in all patients in Group D. One patient in Group S developed subacute thrombosis after stenting. Follow-up angiography was performed after 6 months. Before treatment, the lesion lengths in Groups D and S were 24.2 +/- 10.0 and 9.2 +/- 4.4 mm, the reference diameters were 2.56 +/- 0.32 and 2.69 +/- 0.33 mm, and the minimal lumen diameters were 0.42 +/- 0.34 and 0.53 +/- 0.40 mm, respectively, with no significant difference in the latter 2 parameters. Restenosis occurred in 72% and 30% (p = 0.01) and revascularization of the target lesion was successful in 67% and in 30% (p = 0.03), in Groups D and S, respectively, both significantly higher in Group D. Follow-up study revealed that the minimal lumen diameter and the net gain were significantly greater in the overlapping portion of the 2 stents than in the proximal and distal portions in Group D, with no significant difference in restenosis rate between these 3 portions. Multiple overlapping stenting using Wiktor stents is useful for diffuse lesions with unfavorable results after POBA even if the affected vessel is smaller than 3.0 mm. However, patients should be carefully observed because restenosis occurs frequently.
Phenolic non-flavonoid compounds in diets, such as chlorogenic acid and caffeic acid are widely recognized to be antioxidants. However, it is not known how these phenolics scavenge reactive species of oxygen and nitrogen. We determined the rate constants of the reactions between the phenolics with superoxide and hydroxyl radical with a pulse radiolysis. The second-order rate constants of the reactions of chlorogenic acid with superoxide and hydroxyl radical were 1.67 +/- 0.14 x 10(6) M(-1) s(-1) and 3.34 +/- 0.19 x 10(9) M(-1) s(-1), respectively, while those of caffeic acid with superoxide and hydroxyl radical were 0.96 +/- 0.01 x 10(6) M(-1) s(-1) and 3.24 +/- 0.12 x 10(9) M(-1) s(-1), respectively. By scavenging peroxy radical chlorogenic acid inhibited the initiation of chain lipid peroxidations by organic free radical. The second-order rate constant of the reaction of chlorogenic acid with peroxy radical was estimated to be 1.28 +/- 0.11 x 10(5) M(-1) s(-1). Chlorogenic acid was rapidly oxidized by peroxynitrite in concentration- and pH-dependent manners and its rate constant was determined to be 1.6 +/- 0.7 x 10(5) M(-1) s(-1), using competitive inhibitions by glutathione and methionine.
We examined the allelic association between the dopamine D2 receptor (DRD2) gene and alcoholism in 100 biologically unrelated Japanese alcoholics and 93 unrelated controls. Genomic DNA was prepared from peripheral white blood cells using the phenol-chloroform method. A 310-bp region surrounding the TaqA site at the DRD2 locus was amplified by polymerase chain reaction (PCR), and the PCR product was incubated with TaqI. The A1 allele remained intact while the A2 allele was cut. The frequency of the A1/A1 genotype and the frequency of the A1 allele were higher in early-onset alcoholics than in controls, P < 0.05 and P < 0.01, respectively. Moreover, the frequency of the A1/A1 genotype and the frequency of the A1 allele were higher in early-onset alcoholics with family histories of alcohol dependence than in controls, P < 0.01 and P < 0.01, respectively. The results indicate that the DRD2 gene is associated with susceptibility to early-onset alcoholism, and that each additional A1 allele shifts onset of alcoholism to an earlier age.
The mutant Cu/Zn superoxide dismutase (SOD1) associated with familial amyotrophic lateral sclerosis (FALS) with a 2 bp deletion was produced in two protein expression systems. The mutant SOD1, expressed as a fusion protein in E. coli, had immunoreactivity to an anti-human SOD1 antibody but no SOD activity. It was more susceptible to proteolysis and its immunoreactivity decreased more rapidly than the wild type. The mutant SOD1, expressed in Cos1 cells, was not detected by either SOD activity staining or Western blot analysis, although expression of its mRNA was confirmed. These results suggest that the mutant SOD1 is seriously unstable in mammalian cells.
Heparin extensively inhibited infection of MDBK cells by equine herpesvirus 1 (EHV-1) strains adapted to bovine cells or hamsters, while the reagent merely reduced infectivity of strains passaged only in equine cells. The gC of two strains adapted to non-equine cells seemed to have higher affinity for heparin, although the reagent bound to both the gC and gB of all strains tested. Amino acid substitutions of the gC of the EHV-1 strains adapted to non-equine cells converged on the hydrophilic regions, amino acid residues 92 to 175, resulting in the glycoprotein becoming more cationic. These results indicate that these hydrophilic regions of the gC may be responsible for binding to heparin.
To develop a strategy for extended primary culture of human hepatocytes, we placed human hepatocytes between two layers of collagen gel, called a "collagen gel sandwich." Maintenance of hepatocellular functions in this system was compared with that of identical hepatocyte preparations cultured on dry-collagen coated dishes or cocultured with rat liver epithelial cells. Human hepatocytes in a collagen gel sandwich (five separate cultures) survived for more than 4 wk, with the longest period of culture being 78 d. They maintained polygonal morphology with bile canaliculuslike structures and high levels of albumin secretion throughout the period of culture. In contrast, hepatocytes on dry-collagen became feature-less, and albumin secretion could not be detected after 14 d of culture. This loss of albumin secretion was partially recovered by overlaying one layer of collagen gel. Ethoxyresorufin O-deethylase activity, associated with cytochrome P450 1A2, was detected basally up to 29 d in collagen gel sandwich culture. These activities were induced four- to eightfold after induction with dibenz(a,h)anthracene. Cocultures also maintained basal activity up to 29 d. However, their inducibility was lower than that of hepatocytes in collagen gel sandwich. No ethoxyresorufin O-deethylase activity was detected in hepatocytes cultured on dry-collagen at 7 d. Thus, the collagen gel sandwich system preserves differentiated morphology and functions of human hepatocytes in primary culture for a prolonged period of time. This system is a promising model for studying human hepatocellular function, including protein synthesis and drug metabolism in vitro.
BACKGROUND: The "neural capacity (NC)" of high-pass resolution perimetry (HRP) may be correlated to the neuroretinal rim area of the optic disc. The authors compared HRP with conventional differential light sensitivity (DLS) perimetry in terms of correlation with glaucomatous optic disc changes. METHODS: Thirty-seven eyes of 37 normal-tension glaucoma (NTG) patients were examined. The authors used an Ophthimus system for HRP and a Humphrey Field Analyzer (HFA) for DLS perimetry. The total rim area and the area of temporal, superior, nasal, and inferior quadrants were determined using a confocal laser tomographic scanner. Quadrant NC (QNC) was calculated using the resolution thresholds corresponding to a given quadrant. Quadrant mean deviation (QMD) was calculated from the total deviations of HFA. RESULTS: In all eyes, NC and MD showed significant correlations with total rim area (Spearman rank correlation coefficient [rs] = 0.375, P < 0.05 for NC; rs = 0.336, P < 0.05 for MD). A significant correlation was observed in 21 eyes with early visual field changes (rs = 0.469, P < 0.05), but not in 16 eyes with advanced visual field changes. Both QNC and QMD showed significant correlations with inferior rim area (rs = 0.575, P < 0.01 for QNC; rs = 0.431, P < 0.01 for QMD). However, no significant correlations were found between QNC or QMD and superior rim area. CONCLUSIONS: The NC may relate to glaucomatous optic disc changes better than does MD in NTG, especially in eyes with early-stage visual field changes.
Transient transfection and in vitro infection experiments were performed to characterize replication and antigen synthesis of the hepatitis B virus (HBV) in human hepatocyte lines HH29 and HHY41, derived from normal liver tissue. These liver cell lines are capable of supporting HBV replication and gene expression at levels similar to the human hepatoma cell line HuH-7. Strikingly, a very tight adhesion of HBV to the outer cell membrane of HH29 and HHY41 was observed under conditions that removed HBV to undetectable levels from HuH-7 hepatoma cells. However, no productive HBV infection could be established in these cells as determined by the absence of viral transcripts and de novo antigen synthesis. In conclusion, the human hepatocyte cell lines HH29 and HHY41 may be useful to study important aspects of late steps in the replication of HBV, but appear to lack certain cellular components that play a pivotal role during early steps of the viral life cycle.
Gray scale second harmonic imaging (2.5 MHz/5.0 MHz) was evaluated in preliminary animal studies with a new ultrasound contrast agent (FS069). FS069 was administered intravenously in 10 rabbits (6 with normal liver, and 4 with implanted VX-2 tumors) and two woodchucks with hepatocellular carcinomas. The vasculature (including tumor vessels) and liver parenchyma were clearly enhanced at a low dosage (optimal dose was from 0.1 to 0.2 mL/kg) in all cases. Enhancement was reproducible and generally dose-dependent. Tumors were enhanced well during the early phase and tumor enhancement disappeared earlier than that of surrounding normal liver. Arterial phase and portal phase were easily distinguished and patterns of enhancement were diagnostic of the tumors. Gray scale second harmonic imaging is useful in the detection of hepatic tumors and in diagnosis of their hemodynamics.
RATIONALE AND OBJECTIVES: The authors evaluate the efficacy of SHU 508A, a galactose-based contrast agent used in gray-scale second harmonic imaging in vitro and in vivo experiments. METHODS: A Toshiba prototype harmonic imaging system (2.5/5.0 MHz) was used with SHU 508A in a phantom experiment and to image the liver in five healthy rabbits and one rabbit that had VX-2 tumors in the liver. RESULTS: In the second harmonic imaging, most of the fundamental components of the backscattered echo were eliminated, and good images with high contrast were obtained in the phantom experiment. Liver parenchyma was enhanced clearly in all rabbits at 0.3 mL/kg, an effect that lasted for 90 seconds. The tumor, which was mostly necrotic, was depicted clearly as a negative enhanced area surrounded by enhanced healthy liver. CONCLUSIONS: Gray-scale second harmonic imaging is a promising new method for visualization of perfusion of organs.
