[A case of postoperative airway obstruction by Quincke edema].

A 42-year-old female was scheduled for removal of brain tumor under general anesthesia with nitrous oxide, oxygen and isoflurane. Two days after operation, airway obstruction by increased swelling around the neck was observed. The first neck X-ray films and CT-scans after operation were not indicative of the hematoma or cyst but suggestive of the neurovascular edema (Quincke). It is necessary not to overlook postoperative airway obstruction by Quincke's edema.

[Effects of Celosia argentea and Cucurbita moschata extracts on anti-DNP IgE antibody production in mice].

We have already reported that the Perilla frutescens extract (PFE) suppressed anti-DNA IgE antibody production in mice. In this study, we prepared extracts of Celosia argentea L. (CAE) and Cucurbita moschata Duch (CME), which are Chinese herbal medicines like Perilla frutescens, and examined the effects on anti-DNP antibody responses in mice. To examine the effects of CAE & CME on primary antibody responses, CAE & CME were intraperitoneally injected the day before primary immunization of DNP-ovalbumin. Anti-DNP antibody production was markedly suppressed. Then, we examined the effects on secondary antibody responses. CEA & CME were injected only the day before secondary immunization. Anti-DNP IgE production was markedly suppressed, but IgG responses were not affected. It was also found that mitogenic activity occurred in CAE & CME dose dependently in vitro. These effects of CAE & CME were superior to that of PFE. These results suggest that CAE & CME may be more useful than PFE for the suppression of IgE antibody in certain allergic disorders.

Inhibition of cholinergic transmission by opiates in ileal myenteric plexus is mediated by kappa receptor. Involvement of regulatory inhibitory G protein and calcium N-channels.

Opiates are known to influence intestinal motility via modulation of cholinergic transmission. The aims of this study are to characterize the opioid receptor subtype that modulates cholinergic transmission and to investigate the intracellular mechanism responsible for inhibition of acetylcholine (ACh) release by opiates using longitudinal muscle-myenteric plexus preparations of the guinea pig ileum. The kappa-receptor agonist U50488H and the mu-receptors agonist [D-Ala2,N-Me-Phe4, Gly5-ol]enkephalin, inhibited the release of ACh evoked by electrical stimulation (0.2 and 2 Hz) in a dose-dependent fashion, whereas the delta-receptor agonist DPDPE, had no effect. ACh release evoked by depolarization with veratridine, which was more analogous to high frequency stimulation, was inhibited only by U50488H. Pertussis toxin abolished the inhibitory effect of U50488H on veratridine-induced ACh release suggesting that the principal mechanism by which opiates inhibit cholinergic transmission is via activation of an inhibitory regulatory G protein. Veratridine-stimulated release of ACh was antagonized by omega-Conotoxin GVIA (a preferential N channel blocker) but was not affected by nifedipine (an L channel blocker) or nickel (a T channel blocker). U50488H did not produce further inhibition of veratridine-evoked ACh release in the presence of omega-Conotoxin GVIA. These results suggest that both kappa- and mu-agonists can modulate cholinergic transmission, although the kappa-agonist appears to be more potent. The kappa receptors modulate ACh release by inhibition of N-type voltage-sensitive Ca++ channels via a pertussis toxin-sensitive G protein in guinea pig ileum.

[An adult case of congenital esophagobronchial fistula complicated with pyothorax].

A 64-year-old woman with congenital esophagobronchial fistula complicated with pyothorax is reported. She was admitted to our hospital because of high fever and cough. Chest X-ray showed a cavity with niveau in the right lower lobe. The next day she noticed right chest pain. Chest X-ray showed right pleural effusion. The diagnosis of left esophagobronchial fistula with pyothorax was confirmed by esophagography, esophagoscopy, chest X-ray and chest CT. After treatment of the pyothorax with antibiotics and drainage, fistulectomy and right lower lobectomy were performed. The postoperative course was uneventful. Histologically, the fistula had esophageal epithelium with a submucosal muscle layer and demonstrated a transitional zone between squamous epithelium and bronchial epithelium.

[Clinical utility of Gd-DTPA subtraction MR imaging for spinal bone metastasis].

Based on reports that Gd-DTPA contributes to the detection of tumors, we used it in 31 cases (97 lesions) of spinal bone metastases. The result was that Gd-DTPA increased the intensity of tumors and the surrounding bone marrow to almost the same level in 53%. To show the metastases clearly, an existing subtraction command system was utilized. The technique included the pixel-by-pixel method, to obtain a Gd-DTPA T1-weighted image (T1WI) subtracted by the original T1WI. The detectability of the subtraction image was improved up to 96%, but was less than the original T1WI (99%). Because of the different imaging rationale between two methods, a means to assess the quality of diagnosis must be proposed. To check the normal background, the same kind of postprocessing was performed in 21 patients without malignancy. Gd-DTPA perfusion was highest in the paravertebral veins, moderate in muscles and epidural fat, and lowest in the spinal cord, intervertebral disk and bone cortex. Gd-DTPA enhanced subtraction MR imaging provides a new diagnostic tool to detect and to assess bone metastasis.

Effect of absorption promoters on subcutaneous absorption of human epidermal growth factor in rats.

Subcutaneous administration of human epidermal growth factor (hEGF) to rats gave a significantly smaller value of area under the curve (AUC) of concentration in plasma of immunoreactive hEGF versus time than intravenous administration, probably because the slow entry rate into the blood circulation and consequently the enzymic degradation of hEGF at the injection site. In the present study, absorption promoters such as sodium caprate, N-acylamino acids, disodium ethylenediamine-tetraacetate (EDTA), and sodium glycocholate were used because they were expected to inhibit the enzymic degradation of hEGF at the injection site and to facilitate the entry of hEGF into the blood circulation. Coadministration of an absorption promoter with hEGF significantly increased the entry rate and AUC value of immunoreactive hEGF compared with the case without the absorption promoter. The enzymic degradation of hEGF in the supernatant of the rat subcutaneous tissue homogenates and in the buffer solution containing leucine aminopeptidase or protease was markedly inhibited by the presence of the absorption promoters except EDTA. On the other hand, only EDTA increased the initial entry rate of FITC-dextran (M(r), 4000), which is not metabolized at the injection site, although all absorption promoters including EDTA markedly increased the extravasation of Evans blue. Thus, the increased subcutaneous bioavailability of hEGF in the presence of absorption promoters (except EDTA) was mainly attributed to the inhibitory effect of absorption promoters against the enzymic degradation of hEGF at the subcutaneous tissues.

Multifocal intraductal papillary adenocarcinoma of the pancreas: report of a case.

This paper describes a case of a mucin-producing tumor of the pancreas, histologically diagnosed as multifocal intraductal papillary adenocarcinoma. The patient had a markedly dilated main pancreatic duct, detected by ultrasound (US) and computed tomography (CT), and endoscopy showed a papillary tumor projecting from the patulous orifice of the enlarged papilla of Vater. Several biopsies taken from the tumor indicated a diagnosis of papillary adenocarcinoma. Total pancreatectomy was thus performed, revealing a multifocal papillary tumor growing along the main duct in the head, body, and tail of the pancreas. Microscopically, these multiple tumors were found to be mucin-producing papillary adenocarcinoma of the main and subsidiary pancreatic ducts with nodular and microfocal periductal invasion. The surgical treatment of multifocal tumors, may therefore necessitate total pancreatectomy to achieve curative resection, in some cases.

Correlation between serotonergic measures in cerebrospinal fluid and blood of subhuman primate.

The relationship between the concentration of serotonin (5-hydroxytryptamine; 5-HT), its precursor; tryptophan (Trp) and the main metabolite 5-hydroxyindoleacetic acid (5-HIAA) in cerebrospinal fluid (CSF) and blood of monkey have been studied. 5-HT, Trp and 5-HIAA underwent circadian changes in both CSF and blood. Significant correlations were found between 5-HT, 5-HIAA and Trp in CSF and blood. The significance of these findings and their relationship to the use of peripheral serotonergic system as a functional model of the central nervous system are discussed.

Augmentation of tumour delivery of macromolecular drugs with reduced bone marrow delivery by elevating blood pressure.

Effects of angiotensin II (AT-II)-induced hypertension on the distribution of macromolecules to Walker carcinoma and to bone marrow of SMANCS [poly(styrene-co-maleic-acid)-neocarzinostatin conjugate] were investigated in rats. AT-II-induced hypertension from about 100 to 150 mmHg significantly increased the accumulation of the macromolecular drug SMANCS and 51Cr-labelled bovine serum albumin ([51Cr]BSA), representatives of macromolecular drugs, in tumour tissue. At 1 h after i.v. administration, intratumour concentrations of [51Cr]BSA and SMANCS were elevated by 1.2-1.8-fold. The higher drug accumulation in the tumour that was produced by the artificial hypertension was retained even 6 h after administration. This observation indicates an additive effect to that under normotensive conditions where intratumour macromolecular drug concentrations increase steadily during this period. Furthermore, distributions of these drugs in the bone marrow and the small intestine decreased during artificial hypertension to 60-80% of those in the normotensive state. Therefore, the drug concentration ratios of tumour/bone marrow and tumour/small intestine were increased by 1.8-2.4-fold. A decreased distribution of SMANCS to normal tissues under hypertensive conditions was also confirmed by the significant reduction of its toxicity e.g. leukopenia, diarrhoea, and body weight loss, even at a lethal dose. On the contrary, [3H]methylglucose showed no remarkable difference in tumour or bone marrow accumulation under this hypertensive condition. These results show the advantages of macromolecules over small molecules for AT-II-induced hypertension chemotherapy.

Inhibitory effect of the water extract of spikes of Miscanthus sinensis on IgE formation in mice.

The effects of four different plant extracts of the spikes of Miscanthus sinensis, Phragmites communis, and Imperata cylindrica var. major, and of the spikelets of Coix lachryma-jobi on IgE antibody formation were investigated in mice. The IgE antibody titer was tested by the passive cutaneous anaphylaxis (PCA) method in rats. Of these, the water extract of M. sinensis showed an appreciable inhibitory effect on IgE formation. Furthermore, an undialyzable fraction of the water extract with a relative molecular mass of more than 50,000 d, designated as MSIS, showed the most potent inhibitory activity on IgE formation. MSIS was also a potent inhibitor of IgE formation when given intraperitoneally or intranasally to mice the day before injection of dinitrophenyl (DNP)-ovalbumin (OVA) antigen in both the primary and secondary immune responses. Moreover, MSIS clearly suppressed on-going IgE antibody formation in both primary and secondary immune responses.

The suppressive effect of gelatin-conjugated superoxide dismutase on disease development and severity of collagen-induced arthritis in mice.

We studied the effect of superoxide dismutase (SOD) on murine collagen-induced arthritis (CIA), an animal model of human rheumatoid arthritis (RA). Among SOD derivatives studied, only gelatin-SOD conjugate which has prolonged half life in vivo was effective to suppress the development of CIA, while native SOD or gelatin carrier alone was ineffective. Interestingly, pyran polymer-conjugated SOD which also has a long half life showed no suppressive effect on the disease. No significant effect on immune response against type II collagen (CII) was found in any of the experimental groups. In addition, induction of suppressor cells was not detected in spleen or lymph node cells of the gelatin-SOD-treated group. Therefore, these results suggest that oxygen radicals may have an important role in the effector phase of the immune response to manifest this chronic autoimmune polyarthritis. Thus, the use of appropriate antioxidants for the treatment of human RA may be rationalized.

[A case of brain abscess due to Streptococcus sanguis in association with multiple pulmonary arteriovenous fistulas].

A case is reported here of brain abscess due to Streptococcus sanguis in association with multiple pulmonary arteriovenous fistulas. A 19-year-old male who had been diagnosed in 1988 as pulmonary arteriovenous fistulas was admitted to our hospital for repeated epistaxis, headache, and vomiting. A brain abscess which was diagnosed using various roentogenologic examinations, worsened despite conservative therapy. Additional surgical drainage was performed; S. sanguis was isolated from the drainage fluid. After drainage, clinical symptoms gradually improved with no subsequent neurological deficits. We have formulated the following hypothesis regarding the mechanism of abscess formation in this case: S. sanguis invaded from a ruptured nasomucosal vein, forming a septic emboli in the blood flow, which passed through a pulmonary arteriovenous shunt and led to the formation of a brain abscess which established a metastatic presence in the cerebral tissue.

Promoting effects of bile acid load on the occurrence of cholangiocarcinoma induced by diisopropanolnitrosamine in hamsters.

This study was performed to clarify the promoting effects of primary or secondary bile acid load on the occurrence of cholangiocarcinoma, using Syrian golden hamsters. These hamsters received subcutaneously diisopropanolnitrosamine (DIPN) once weekly for 10 weeks, and simultaneously were given a standard pellet diet (control group) containing taurocholic acid (TCA group) or deoxycholic acid (DCA group). The rates of cholangiocarcinoma at 20 weeks were 23% in the control group, 60% in the TCA group and 59% in the DCA group. There were significant differences between the control and the TCA or DCA groups (p < 0.05). The rates of proliferation of bile ductules or hyperplasia of the bile duct epithelium and the bromodeoxyuridine labeling indices of bile duct epithelial cells were high in both groups treated with bile acids, compared with those in the control group. Regarding the composition of bile acids in the intraductal bile, the TCA and DCA groups revealed a decrease in primary bile acids and an increase in DCA. These results suggest that both TCA and DCA given orally promote the occurrence of DIPN-induced cholangiocarcinoma.

Measurement of the secretion of a small peptide at the single cell level by the cell immunoblot assay: thyroidectomy increases the number of substance P-secreting anterior pituitary cells.

The present study developed a technique for quantification of a small peptide secreted by single endocrine cells and investigated the cellular basis for the effects of endocrine environments on their secretion by using this technique. To quantify substance P (SP) secreted by monodispersed rat anterior pituitary cells and to estimate the relative abundance of SP-secreting cells, the cell immunoblot assay was improved by several modifications, including the use of incubation chambers and addition of immunostaining procedures of fixation by paraformaldehyde and elimination of nonspecifically stained blots. Single pituitary cells formed SP-specific blots on protein-blotting transfer membranes that constituted a floor portion of the incubation chamber. A combination of microscopic image analysis and SP standard curve made it possible to quantify the amount of SP stained as blots. SP secretion from single pituitary cells showed a large variation ranging from 0.14-144 fg/cell.h. The mean SP secretion and the number of SP-secreting cells were increased in an incubation time-dependent manner, with plateau levels of 20.3 fg/cell and 30.2 in a chamber area of 8 x 8 mm, respectively, at 3 h. Thyroidectomy significantly increased the number of SP-secreting cells and the total amount of SP secretion but decreased the mean SP secretion from single pituitary cells. This decrease in the mean SP secretion in the thyroidectomized rats was found to be due to a relative abundance of cells that secrete small amounts of SP. The present study demonstrates that the cell immunoblot assay is useful for quantifying peptide secretion at the single cell level and suggests that thyroidectomy-induced increase in SP secretion from anterior pituitary cells is not due to an increase in SP secretion per cell but due to an increase in the number of SP-secreting cells.

Nucleotide sequence of the Clostridium stercorarium xynA gene encoding xylanase A: identification of catalytic and cellulose binding domains.

The nucleotides of the xynA gene of Clostridium stercorarium were sequenced. The structural gene consists of an open reading frame of 1533 bp encoding 511 amino acids with an M(r) of 56,519. The signal peptide cleavage site was identified by comparison with the N-terminal amino acid sequence of the enzyme produced by a recombinant Escherichia coli. Xylanase A consists of a catalytic domain belonging to family G at the N-terminus and two direct repeats of about 90 amino acids with a short spacing at the C-terminus. Deletion analysis showed that the repeated sequences were responsible for binding the enzyme to Avicel and were not essential for catalytic activity. The catalytic domain of this enzyme is highly homologous to xylanase A of Clostridium acetobutylicum (identity: 69%) and xylanase B of Bacillus pumilus (identity: 64%).

Impaired fibrinolysis in hypertension and obesity due to high plasminogen activator inhibitor-1 level in plasma.

In order to elucidate the influence of the risk factors of coronary heart disease on the fibrinolytic activity, relationships between blood pressure, body mass index (BMI), plasma lipoprotein (a) (Lp(a)) level and the plasma levels of tissue plasminogen activator (tPA) and plasminogen activator inhibitor-1 (PAI-1) were analyzed in the subjects with mild hypertension. Systolic blood pressure showed a positive correlation with total PAI-1 and free PAI-1. Diastolic blood pressure showed no correlation with these proteins involved in the fibrinolytic system. BMI had a positive correlation with total PAI-1, free PAI-1 and euglobulin clot lysis time (ECLT). Plasma Lp(a) level showed correlation with neither blood pressure nor fibrinolytic parameters, but it showed weak negative correlation with body mass index (BMI). These results suggest that high blood pressure and obesity tend to increase free PAI-1 which reduces fibrinolytic activity. Lp(a), however, seems not to influence directly the fibrinolytic system but may work to decrease fibrinolytic activity only in conjunction with other risk factors. The effects of daily drinking of alcohol and smoking on the fibrinolytic system were also investigated in the present study and we obtained the results that habitual drinking increased plasma levels of both tPA and PAI-1 whereas smoking did not affect fibrinolytic activity. These results suggest that risk factors for coronary heart disease such as hypertension and obesity are closely related to the impaired fibrinolysis.

Progressive diffuse arteriovenous malformation--case report.

A rare diffuse arteriovenous malformation (AVM) occurred in an 11-year-old boy with a 4-year history of transient ischemic attacks and weakness of the left extremities. Postcontrast computed tomography (CT) showed cord-like enhancement in the corpus callosum and pineal region. Magnetic resonance (MR) imaging demonstrated atrophy of the right hemisphere and multiple signal voids in the sulci. Cerebral angiography showed a diffuse AVM fed by the bilateral anterior cerebral and right middle cerebral arteries. The AVM nidus consisted of scattered nidi. Several draining veins appeared as multiple signal voids on MR images. No surgery was possible because of the scattered nidi. Left hemiparesis and mental retardation worsened, with progressive atrophy of the right hemisphere on serial MR images. Follow-up radiological examinations showed that the number of nidi increased, the draining vein enlarged, and the gyri calcified. The gyriform calcification on CT scans strongly resembled that seen in Sturge-Weber syndrome.

Effects of the muscarinic receptor agonist carbachol and/or antagonist pirenzepine on gastric mucus secretion in rats.

The effects of the muscarinic agonist carbachol on the secretion and accumulation of gastric mucus glycoprotein (mucin) were examined. Gastric mucin obtained from the soluble mucus, adherent mucus gel, and surface mucosal and deep mucosal layer was isolated and quantified. One hour after the subcutaneous administration of carbachol (0.08-80 micrograms/kg body weight) the deep corpus mucin content had decreased significantly (75% of control), corresponding to an increase (120% of control) in the soluble mucin content with 0.8 microgram/kg of carbachol treatment. These changes were counteracted by 10 mg/kg of pirenzepine (selective M1 antagonist) pretreatment. On a single administration of 10 mg/kg of pirenzepine, deep corpus mucin tended to increase, and soluble mucin decreased significantly (49% of the control). These two drugs failed to cause any significant change in the mucin content of the surface and antral deep mucosa or the adherent mucus gel. The muscarinic agonist and the M1 antagonist are thus shown to accelerate the secretion and accumulation, respectively, of mucin in the deep corpus mucosa. Thus intrinsic M1 receptor may possibly be involved in the secretion of mucin in the gastric deep corpus mucosa.
