A technique to correct frontal sinus hypertrophy is presented. The outer table of the frontal sinus is removed and divided into three pieces with an oscillation saw. The pieces are trimmed at the margin, fixed with two microplates, and regrafted. Bilateral lateral thick portions of the supraorbital rim are shaved with a surgical burr. At the same time, a forehead lift is performed. A satisfactory result with a flat and wide forehead can be obtained by employing this technique.
The antiarthritic activity of a novel thiazole derivative, SM-8849, was compared with that of indomethacin and D-penicillamine, in mice with collagen-induced arthritis. SM-8849 reduced the incidence and severity of disease in collagen-immunized mice, as assessed by clinical observation. This efficacy was also confirmed by radiographic and histologic studies. Indomethacin produced an apparent reduction of the clinical score, but had only a marginal effect on bone destruction. D-penicillamine did not produce any improvement. Unlike indomethacin and D-penicillamine, SM-8849 reduced the serum levels of anti-type II collagen antibodies. Flow cytometric analysis of spleen cells from arthritic mice revealed an increase in T cells expressing activation antigens (class II antigens) in comparison with normal mice. Treatment with SM-8849, but not indomethacin or D-penicillamine, prevented the increase in Ia-bearing T cells. The results suggest that an effect of SM-8849 on immunocompetent cells may be responsible for the antiarthritic activity of the compound, and this would distinguish its action from that of traditional antirheumatic drugs.
A 7-year-old girl with cytochrome c oxidase deficiency who had no neurologic deficits in infancy suddenly developed ophthalmoplegia, ptosis, and respiratory arrest. She recovered almost completely 80 days after onset, suggesting that acute onset and rapid remission are observed in patients with cytochrome c oxidase deficiency. It is also possible that early initiation of therapy in cytochrome c oxidase deficiency with coenzyme Q10 may hasten and enhance the therapeutic effect.
A microdamage event, either bone microfracture or microcrack propagation, releases energy. Some of this energy is in the form of acoustic waves. We measured acoustic emission (AE) in normal and osteopenic vertebral bodies during compression loading to confirm the microdamage accumulation. The 2nd to 4th lumbar vertebrae taken from the embalmed cadavers of a 37-year-old male and a 75-year-old female were used. Bone mineral densities (BMDs) were measured by dual energy X-ray absorptiometer (DEXA). The male vertebrae had normal BMDs, while the female vertebrae had lower BMDs than the normal range and were graded osteopenic. Mechanical parameters (maximum load, maximum stress, stiffness, strain at maximum load, and apparent elastic modulus) and AE event count rates in the load-deformation curve were measured during quasi-static compression loading (deformation rate 0.1 mm/min). For all mechanical parameters, the normal vertebrae had higher values than the osteopenic vertebrae. Cumulative AE event counts until maximum load of the osteopenic vertebrae were much greater than that of normal vertebrae. The vertebrae which were well compressed to the plastic range in the load-deformation curve displayed dome-shaped fracture lines just above the end plates. These results are consistent with the hypotheses that microcracks of osteopenic vertebral bodies are generated and accumulate at lower strains than those of normal vertebrae at a specific site.
A method for estimation of the absolute renal activity within background activity in renal planar scintigraphy will be presented. This method corrects for oversubtraction of background activity with consideration of background activity of the kidney volume, and employs the depth-independent buildup factor (DIBF) method for the correction of attenuation and scatter of photons. This method requires the depth and thickness of the organ and the thickness of background for a background activity correction. The transmission factor (TF) for a volume source is derived from integrating TF for a thin source over the thickness of the organ for the DIBF method. To validate this method, phantom studies with various uniform background activity concentrations were performed and the data were compared with conventional background subtraction that do not consider the organ's volume. The results showed that with the conventional background subtraction method at all depth activity was underestimated with errors of 5%-30% for organ/background concentration ratios of 5-40, while this method estimated the true count rate with errors of less than 5%. More accurate quantifications of renal functions such as renal uptake, glomerular filtration rate (GFR) and renogram may be obtained by this proposed method on the planar images.

PURPOSE: The value of the thin slice direct oblique coronal technique, which is parallel to the anterior cruciate ligament (ACL), was assessed in the evaluation of ACL injury in comparison with conventional oblique sagittal and coronal images. MATERIAL AND METHODS: A thin slice direct oblique coronal technique was developed and applied clinically to 62 patients after conventional oblique sagittal and coronal images had been obtained. MR images of these 62 patients (24 with tears and 38 without tears) with an arthroscopic correlation were evaluated by three radiologists who were unaware of the arthroscopic results. The diagnostic accuracy of these new images was compared with that of oblique sagittal and coronal images by ROC analysis. RESULTS: Conventional oblique sagittal and coronal images for the diagnosis of ACL tears revealed accuracies of 82%, 84%, and 84%, sensitivities of 92%, 92%, and 96% and specificities of 76%, 79%, and 76% for the three reviewers, respectively. On thin slice direct oblique coronal images, specificities of 97%, 97%, and 97%, sensitivities of 96%, 96%, and 96%, and accuracies of 97%, 97%, and 97% were obtained, respectively. Diagnostic ability was significantly better with direct oblique coronal images (mean area under the ROC curve [Az]=0.99) than with conventional oblique sagittal and coronal images (Az=0.91) (p<0.05). CONCLUSION: The addition of thin slice direct oblique coronal images significantly improved specificity and accuracy in the diagnosis of ACL tears.
Four lesions in three patients with so-called vanishing tumor of the stomach are described. Two patients had one lesion each and the third patient two lesions. The lesions were localized in the gastric cardia, angle, and antrum. Radiological findings of these tumors showed a round tumor with smooth surface and regular margins. The edges of the tumors were gently sloping, while the mucosa surrounding the tumor was edematous. The time interval until these tumors disappeared was five days, 20 months, 12 months and seven months, respectively. When the soft appearance of a gastric tumor is suspected in UGI examination, the patient should be re-examined after a month or more, to avoid unnecessary surgery.
The initial stage of status spongiosus in Ammon's horn of two brains with panencephalopathic type of Creutzfeldt-Jakob disease (PE-CJD) was examined electron microscopically. The results obtained in this examination were as follows: initial formation of large vacuoles or cavities seemed to be the result of distension within cell processes, mainly astrocytes, and these vacuoles and cavities might gradually form status spongiosus. A very important finding in the present study was that all kinds of glial cells showed degenerative changes. It is concluded that glial insufficiency might be one of the pathogenetic mechanisms effective in panencephalopathic Creutzfeldt-Jakob disease.
An eleven year old patient with TAPVD, ASD and VSD with left to right shunt who underwent successful total correction is presented. Diagnosis, operative technique and the pertinent literature are briefly discussed.
Mechanical right ventricular assistance has never been percutaneously instituted. We invented a new method for administering right ventricular assistance without thoracotomy (RAWT) and evaluated its effects in acute experiments using dogs. To institute RAWT, an outflow cannula was inserted into the pulmonary artery from the femoral vein, and an inflow cannula was inserted into the right atrium from the right jugular vein. A Swan-Ganz catheter inserted into the outflow cannula led it to the pulmonary artery under blood pressure monitoring only. A Sarns' centrifugal pump was used as a blood pump. We applied this system to biventricular failed hearts supported by a left ventricular assist device. We could increase cardiac output from 53.3 +/- 31.8 to 77.8 +/- 31.7 ml/min/kg, and mean aortic pressure from 43.5 +/- 12.3 to 57.5 +/- 11.2 mm Hg with RAWT. The right ventricular unloading effect of RAWT was not disturbed by regurgitation through the pulmonary or tricuspid valve.
