Clinical significance of simple heart rate-adjusted ST segment depression in supine leg exercise in the diagnosis of coronary artery disease.

To evaluate the clinical significance of simple heart rate-adjusted ST segment depression (delta ST/delta HR) in the diagnosis of coronary artery disease, 42 patients with stable exertional angina underwent supine leg exercise testing and cardiac catheterization. During exercise, heart rate, a multilead electrocardiogram, and pulmonary artery wedge pressure were recorded. The sensitivity and accuracy of the delta ST/delta HR criteria (greater than or equal to 3.0 microV/beat/min) were significantly greater than the conventional analysis of ST segment depression criteria (greater than or equal to 0.2 mV) for detecting three-vessel coronary artery disease at a matched specificity of 72% (100% versus 46%, 81% versus 64%, p less than 0.01). A significant linear correlation was found between maximum pulmonary artery wedge pressure increments during exercise (delta PAWP) or Gensini score and the delta ST/delta HR (delta PAWP: r = 0.51, p less than 0.001; Gensini score: r = 0.47, p less than 0.001). There were no statistically significant differences in the delta PAWP or Gensini score between patients with three-vessel disease who had delta ST/delta HR greater than or equal to 3.0 microV/beat/min and those with one- or two-vessel disease who had delta ST/delta HR greater than or equal to 3.0 microV/beat/min (delta PAWP: 18.1 +/- 2.0 versus 21.9 +/- 3.3, p = NS; Gensini score: 68.5 +/- 6.6 versus 66.3 +/- 11.3, p = NS). These findings demonstrate that delta ST/delta HR is more useful than a conventional analysis of ST segment depression for identifying not only anatomically severe coronary artery disease but also functionally severe coronary artery disease.

The micronucleus assay with mouse peripheral blood reticulocytes using acridine orange-coated slides.

Ultra-vital staining with acridine orange (AO) is introduced into the micronucleus assay with mouse peripheral blood cells. Peripheral blood was stained vitally by dropping whole blood on an AO-coated slide and covering the sample with a coverslip. With this method, reticulocytes are identified easily by their red fluorescing reticulum structure. The distinction between young and mature erythrocytes was clearer and less subjective than the distinction between polychromatic and normochromatic erythrocytes by Giemsa staining or by conventional AO fluorescent staining. Although the induction of micronucleated peripheral reticulocytes (MNRETs) was delayed by about 12 h compared to that of micronucleated polychromatic erythrocytes (MNPCEs) in the bone marrow, the frequencies of MNRETs and MNPCEs were almost identical at each optimal sampling time. It is concluded that bone marrow cells can be replaced by peripheral blood as material for the micronucleus assay.

Lack of toxicity/carcinogenicity of monosodium succinate in F344 rats.

The toxicity/carcinogenicity of monosodium succinate, a food additive, was examined in F344 rats. The oral LD50 was greater than 8 g/kg body weight. In a 13-wk subchronic oral toxicity study, the only toxicological finding was suppression of body-weight gain in groups given greater than or equal to 2.5% monosodium succinate in the drinking-water. Histological examination revealed no toxic lesions specifically caused by the compound in any organs of any of the treated rats. The maximum tolerated dose was determined to be 2-2.5% on the basis of body-weight depression. In a long-term (2-yr) toxicity/carcinogenicity study, monosodium succinate was given ad lib. in drinking-water (distilled water) at levels of 0, 1 or 2% to groups of 50 male and 50 female rats. No toxic lesion specifically caused by long-term administration of monosodium succinate was detected. No dose-related increase was found in the incidences of tumours in any organ or tissue except for C-cell tumours of the thyroid gland of females. The incidence of these tumours in females given the 2% dose was higher than that in controls but not significantly so, and a positive trend for this tumour was noted in females. C-Cell tumour is one of the most commonly observed spontaneous tumours in ageing female rats of this strain and occurs at a variable incidence. There was no difference between the female control and treated groups in the incidence of preneoplastic change of the thyroid gland. Furthermore, the incidence of C-cell tumours in the female control group was lower than that in our historical controls. It is concluded that the increase in C-cell tumours in the female high-dose group and the detection of a positive trend for this tumour in females were probably a function of experimental variability and were not related to treatment. The results indicate that monosodium succinate had neither toxic nor carcinogenic activity in F344 rats when it was given continuously at levels of 1 or 2% in the drinking-water for 2 yr.

Long-term toxicity/carcinogenicity of musk xylol in B6C3F1 mice.

The long-term toxicity/carcinogenicity of musk xylol, a synthetic nitro musk, was examined in B6C3F1 mice of both sexes. Musk xylol was administered at dietary levels of 0 (control), 0.075 or 0.15% for 80 wk. The overall tumour incidences in all treated groups of both sexes were significantly higher than those in the corresponding controls. Combined malignant and benign liver cell tumours were clearly increased in both sexes, and in males a positive significant trend was also noted for the occurrence of hepatocellular carcinomas. In males the incidence of Harderian gland tumours was also significantly greater in treated groups than in controls. Some other neoplasms, such as lung tumours in both sexes and Harderian gland tumours and lymphomas in females, occurred in greater numbers in the treated groups, although the differences were not statistically significant in comparison with the controls. In addition, the incidences and total numbers of malignant tumours were significantly increased in treated groups of both sexes, although the increases were not dose dependent. The results demonstrated that musk xylol is carcinogenic in B6C3F1 mice when given at dose levels of 0.075 or 0.15% in the diet for 80 wk.

Effect of ethylene glycol monomethyl ether and diethylene glycol monomethyl ether on hepatic metabolizing enzymes.

Glycol ethers have been extensively used in industry over the past 40-50 years. Numerous studies on the toxicity of glycol ethers have been performed, however, the effects of glycol ethers on the hepatic drug metabolizing enzymes are still unknown. We studied the changes of the putative metabolic enzymes, that is, the hepatic microsomal mixed function oxidase system and cytosolic alcohol dehydrogenase, by the oral administration of diEGME and EGME. Adult male Wistar rats were used. DiEGME was administered orally; 500, 1000, 2000 mg/kg for 1, 2, 5 or 20 days and EGME was 100, 300 mg/kg for 1, 2, 5 or 20 days. Decreases in liver weights were produced by highest doses of diEGME (2000 mg/kg body wt/day for 20 days) and EGME (300 mg/kg body wt/day for 20 days). DiEGME increased hepatic microsomal protein contents and induced cytochrome P-450, but not cytochrome b5 or NADPH-cytochrome c reductase. The activity of cytosolic ADH was not affected by diEGME administration. On the other hand, EGME did not change cytochrome P-450, cytochrome b5 or NADPH-cytochrome c reductase. The activity of cytosolic ADH was increased by repeated EGME treatment. Therefore it is suspected that the enzyme which takes part in the metabolism of diEGME is different from that of EGME, although diEGME is a structural homologue of EGME.

Prevention of human rotavirus infection with chicken egg yolk immunoglobulins containing rotavirus antibody in cat.

A study was made on the passive protection against rotavirus-induced diarrhea. Chickens were immunized with bovine rotavirus (serotype 1) and the egg yolk immunoglobulins containing a high titer anti-rotavirus neutralizing antibody (CEYI) was obtained. The CEYI was then orally administered to specific-pathogen-free cats, and the cats were infected with human rotavirus. The cats treated with the CEYI remained clinically healthy after challenge, whereas diarrhea occurred in the placebo-fed cats as control. Virus antigens were detected in feces in all the diarrheal cases in the placebo-fed cats but were only sporadically detected in the CEYI-fed cats. However, the cats were only protected against rotavirus infection by the presence in the gut at the time of infection of the antibody. These results suggested that continuous administration of the CEYI is capable of preventing children from diarrhea induced by human rotavirus infection and viral shedding.

[Experimental stomach-ulcer on pika].

Experimental gastric ulcer formation was performed in the pika and compared with that in the rat. Gastric ulcers were formed in pika that were subjected to water restraint for 4-5 days for 2 hours each day. Gastric ulcers were also formed under conditions of 1-4 days for 3 hours each day and 1-2 days for 5 hours each day. The severest (widest) ulcers were obtained under the condition of 5 hours' water restraint. Histopathologically, the ulcers were mostly erosions, but those formed under 5 hours' restraint reached the tunica muscularis mucosae. In addition, inflammatory changes were recognized. In contrast, while gastric ulcers in the rat formed within a short time, they were histopathologically less severe than those in the pika. Therefore, water restraint for 4 hours performed 4-5 times is suitable to obtain gastric ulcer formation in the pika and may result in more severe gastric ulcers than in the rat. Compared with the rat, the pika showed differences in the appearance and degree of gastric ulcers formed by the injection of serotonin and reserpine.

Clinical studies on the occurrence and the pathogenesis of optociliary veins.

We reviewed retrospectively 20 cases of optociliary vein over the past 5 years. Fifteen of the 20 cases (75%) were over 40 years of age. There was no sex differences (9 males and 11 females). Causative ocular diseases were: central retinal vein occlusion (14 cases, 70%); optic disc drusen (2 cases, 10%); and optic nerve sheath meningioma, high myopia, glaucoma, congenital anomaly (1 case each, total 20%). The number of patients with an optociliary vein was high in central retinal vein occlusion (14/190 cases, 7.4%). We concluded that occurrence of optociliary vein is not restricted to cases with optic nerve sheath meningioma and this shunt vessel may occur under other conditions in which central retinal venous return is seriously disturbed. The most common cause of optociliary vein is central retinal vein occlusion.

SF2487, a new polyether antibiotic produced by Actinomadura.

A new antibiotic SF2487 has been isolated from the culture broth of Actinomadura sp. SF2487. The structure of antibiotic SF2487 was determined by spectroscopic analyses of the sodium salt and X-ray diffraction analysis of the silver salt. The antibiotic represents a new member of polyether group antibiotics known as the acyltetronic acid type 4. The antibiotic is weakly active against Gram-positive bacteria and exhibits antiviral activity against influenza virus in vitro.

[Studies on the teratogenic potential of 2,2'-methylenebis (4-methyl-6-tert-butylphenol) in rats].

2,2'-Methylenebis (4-methyl-6-tert-butylphenol), an antioxidant, was given orally to pregnant Wistar rats by stomach intubation at the dose levels of 93.5, 187 or 375 mg/kg body weight during days 7 to 17 of pregnancy, and the effects of the compound on dams and fetal developments were examined. In the dams at the two higher doses of 187 and 375 mg/kg, toxic signs such as hair fluffing and diarrhoea were observed, and their body weight gain and food consumption were suppressed. Two dams, which showed marked diarrhoea in the highest dose group, died. However, there was no evidence of fetal malformation attributable to treatment with the compound in any of the dose groups treated, although a slight increase in fetal death was found in the highest dose group. It is concluded that 2,2'-methylenebis (4-methyl-6-tert-butylphenol) has a weak lethal effect on fetal development but not a teratogenic effect in the rat.

Spreading of cultured corneal epithelial cells on fibronectin and other extracellular matrices.

Previous investigations demonstrated that fibronectin is an essential adhesive glycoprotein for the corneal epithelial cells. Fibronectin mediates attachment and spreading of the corneal epithelial cells. However, it seems to be important to know the changes in cellular receptor activity in order to understand the interactions of corneal epithelial cells and underlying extracellular matrix. In this investigation, we studied the effects of various culture times and conditions on the receptor activity of rabbit corneal epithelial cells for fibronectin. The cultured cells were placed on tissue culture plates that were coated with various concentrations of four extracellular matrices: fibronectin, laminin and collagen types I and IV. Freshly isolated corneal epithelial cells without culture did not spread on fibronectin and laminin; they spread only on collagen types I and IV. When the epithelial cells were cultured for 12 h or more, they spread on fibronectin. However, the spreading of the cells on collagen types I and IV was the same regardless of the culture period (up to 20 h). Only a small number of epithelial cells spread on laminin at the highest concentration examined after culture for 12 h or more. Thus, the corneal epithelial cells responded differently to fibronectin, collagen types I and IV, and laminin. Perhaps the receptor for fibronectin appears when epithelial cells are cultured, but the receptor(s) for collagen types I and IV are always present on the cell surface of corneal epithelial cells.

[A case of epidural hematoma occurring on the opposite site of craniotomy after clipping surgery performed on internal carotid giant aneurysm].

Postoperative epidural hematomas remote from the operating field are sometimes seen as a complication after ventricle drainage, ventricle-peritoneal shunt or suboccipital craniotomy. Reported here is a very rare case of epidural hematoma which occurred on the opposite site of craniotomy after clipping surgery performed on internal carotid giant aneurysm. A 43-year-old woman was admitted to our hospital because of progressive visual disturbance in her right eye for twelve months. Precise examinations of her right eye revealed deterioration of visual acuity (0.02) right temporal-hemianopsia and an optic disc atrophy. A computed tomography scan (CT) showed a suprasellar round mass which was homogeneously well enhanced. Right carotid angiogram disclosed a large internal carotid artery aneurysm directed supramedially. The aneurysm was explored in June 1988. The neck was clipped with Sugita's ring clips through right frontotemporal craniotomy. The patient recovered fully and extubation was performed soon after the operation. Neurological examinations revealed no abnormal findings. Two days after the operation, she gradually developed impairment of consciousness and nausea. CT scan showed mass effect caused by epidural hematoma over the left temporoparietal region contralateral to the craniotomy site. Evacuation of the hematoma was carried out urgently. She had a good clinical course and postoperative angiogram demonstrated disappearance of the giant aneurysm. She was discharged and returned home without new neurological deficits. We review literature, and discuss presumptive pathogenesis responsible for such unexpected postoperative epidural hematomas.

[Biological features of the skin of the pika, Ochotona rufescens rufescens--concentration of histamine in the skin].

The quantity of histamine and the number of mast cells in the skin of the pika were measured and compared with rabbits, guinea pigs and rats. The ranking of regional histamine levels in the skin of the pika was: perianal region greater than abdomen greater than interscapular region = back greater than lumbus greater than head greater than auricle, and the average value of the 7 regions was 22.6 micrograms/g. The level of histamine in the 6 regions, except the auricle, was 2-5 times that of rabbits and guinea pigs. In the auricle of each of the 4 kinds of animal (pika, rabbit, guinea pig and rat), the levels were almost identical. With respect to histamine levels, those in the pika resembled those in rats. The number of mast cells in the skin of the pika was less than in rats, and was greater than that in rabbits and guinea pigs. The average value was 9.9/mm2.

[Randomized study of initial treatment with radiation.MCNU or radiation.MCNU.interferon-beta for malignant glioma. Hiroshima Brain Tumor Study Group].

The efficacy of radiation.MCNU (MR group) or radiation.MCNU.interferon-beta (IMR group) for malignant glioma was studied by a randomized trial at numerous medical facilities. MR group was irradiated with 50-60 Gy and intravenously injected with 2 mg/kg of MCNU on the initial day of irradiation and 6 weeks later. IMR group was also given intravenous administration of interferon-beta at the dose of 2 x 10(6) IU/m2 for 5 serial-days every eight weeks. There was no difference in background between the two groups. The response rate in MR group and IMR group was 44.4% (4/9) and 30.0% (3/10), respectively, showing no significant difference. The resected tumor volume before the start of these regimens seemed to correlate the response to the treatment in both groups. The major toxicity was myelosuppression, especially using MCNU with interferon-beta. These results indicated that this combined therapy is effective for malignant glioma, and should be executed further trials and follow up study.
