Peritoneal lymphomatosis: CT findings.

BACKGROUND: Diffuse peritoneal tumor infiltration is well recognized on computed tomography (CT) and is usually associated with carcinomatosis. The purpose of this investigation was to analyze the CT findings of peritoneal spread from primary gastrointestinal lymphomas. METHODS: Abdominal CT scans in eight patients with peritoneal lymphomatosis were retrospectively reviewed. Patients were 12-75 years old (mean = 48 years); with six patients were male and two were female. Pathologic evidence of primary lymphoma was available by colonoscopic biopsy of the terminal ileum in seven cases and by gastroscopic biopsy of the stomach in one case. All patients had non-Hodgkin's lymphoma. We analyzed CT findings in view of presence or loculation of ascites, abnormal patterns of mesentery and omentum, presence of peritoneal enhancement, presence of low attenuation and location of lymph nodes, and primary gastrointestinal lymphoma. RESULTS: Although ascites was present in all patients, there was no loculation. The involvement of mesentery was present in seven patients, and the stellate pattern was the common type (4/7). The involvement of omentum was present in seven patients, and the common type was omental cake (3/7). Peritoneal enhancement was present in six patients. Enlarged lymph nodes were present in six patients, mainly at the retroperitoneum and mesentery, and showed centrally low attenuation in half the patients. CONCLUSION: Patterns of tumor involvement of mesentery, omentum, and peritoneum seen in peritoneal lymphomatosis are indistinguishable from those seen in peritoneal carcinomatosis or tuberculous peritonitis. However, ascites without any loculation or septation and diffuse distribution of enlarged lymph nodes were helpful signs of peritoneal lymphomatosis.

An analysis of the subjective experience of schizophrenia.

To examine how varieties of the subjective experience of schizophrenia (SES) can be categorized, a self-report questionnaire consisting of 150 items from the SES in Japanese was administered to 237 Japanese schizophrenic inpatients, and the results were analyzed through a multivariate method. The first factor represents loss of focus and adequacy of thoughts, behavior, and interpersonal perception; it resembles negative symptoms and coincides with the basic symptoms of schizophrenia reported by anthropological psychiatrists. The second factor represents automatic and excessive thoughts and affective loading, and overlaps "the tremble" of Conrad and the mental automatism of de Clerambault. Both signify different combinations of experiences from conventional descriptive symptoms, and may represent more suitable ways to describe the SES.

Subjective experience and related symptoms in schizophrenia.

We had previously extracted two types of subjective experience of schizophrenia (SES); first, a feeling of inadequacy in stream of speech, thought, and action, associated with a distorted sense of self, and second, a feeling that excessive thoughts are filling and sticking to one's head, causing negative affective burden such as misery and oppression. This study tried to validate their content using conventional symptom clusters as external validators. Subjects were 63 patients from two hospitals in Tokyo meeting ICD-10 criteria for schizophrenia. Positive, negative, and depressive psychopathology were measured by the Brief Psychiatric Rating Scale (BPRS), Scale for the Assessment of the Negative Symptoms (SANS), and Hamilton's Depression Scale (HDS). The two types of SES were measured by an original scale. The first type of SES correlated significantly with the negative symptoms of alogia, avolition, and attention, whereas the second correlated with positive and depressive symptoms. To analyze how schizophrenia is experienced by patients, qualitative and comprehensive descriptions, such as indicated by our subjective factors, will be useful.

Insight and clinical correlates in schizophrenia.

It is not clear to what extent poor insight is inherent in schizophrenic psychopathology or is related to clinical factors. It is clinically important to elucidate this point to know how some insight remains intact in schizophrenia. Sixty-three ICD-10 schizophrenics were recruited. Insight was measured by David's three-dimensional scale, which deals with the awareness of the need for treatment, of the illness, and of psychotic experiences. Clinical factors that may influence insight were grouped into objective psychopathology, subjective experience, and treatment settings, according to the normal development of insight starting from objective illness, through subjective perception of it, to seeking treatment. Psychopathology was measured by the Brief Psychiatric Rating Scale (BPRS) and the Scale for the Assessment of Negative Symptoms (SANS), and the subjective experience was measured by a checklist of our own. Positive symptoms, especially disordered thought, hallucinations, and delusions, had a modest inverse correlation with awareness of illness and of psychotic experiences, but these were not correlated with alogia or other negative symptoms. Later age at the first onset was significantly correlated with poor awareness of psychotic experience, but no correlation of subjective experience with insight emerged. Awareness of the need for treatment was significantly higher in outpatient than in inpatient groups, and was not correlated with any other variables. Poor awareness of illness and of psychotic experience seems to be a trait of the acute rather than the chronic psychopathology of schizophrenia. Poor awareness of psychotic experience was related to later onset. Awareness of the need for treatment seems to improve, even in the absence of awareness of illness or of psychotic experience, in outpatient settings--the effects of social interaction may be relevant to this.

The survey of PCDDs and PCDFs in the ambient air of the urban and industrial sites in Korea, 1998-99.

A comprehensive quality survey for PCDDs/PCDFs in ambient air has been studied in Korea for two years. Monitoring was undertaken at two locations throughout Seoul (urban site) and Incheon (industrial site) in winter and spring. The periods are during 16 to 17 March 1998, 21 to 22 January 1999 and 28 to 29 April 1999 (Seoul) and 25 to 26 March 1998, 18 to 19 January 1999 and 26 to 27 April 1999 (Incheon). The values of PCDDs/PCDFs in ambient air were expressed as concentrations and international TEF values. The contents of PCDDs/PCDFs in urban site (Seoul) were lower than in industrial site (Incheon). The measurements in wintertime were higher than in springtime. The contents of 2,3,4,7,8-PeCDF with half toxicity of 2,3,7,8-TCDD were high. Thus we assumed to have an interest to the contents of 2,3,4,7,8-PeCDF. In this study, sample size was small (sample number was only 18) and sampling period was limited by 24 h. Therefore, we need more widespread and long-term surveys in order to estimate the human exposure by inhalation.

Levels of PCDDs and PCDFs in two kinds of fast foods in Korea.

We initiated an attempt to gather preliminary Korean fast food data on the concentrations, whole weight and international toxic equivalent (TEQ), in two kinds of fast foods, hamburger and fried chicken. The levels of polychlorinated dibenzo-p-dioxins/polychlorinated dibenzofurans (PCDDs/PCDFs) in hamburgers are detected to be lower than in fried chickens. Total TEQ level of PCDDs in fried chicken is 47.45 times higher than in hamburger. In case of PCDFs, it is 48.61 times. Even though these are compared to the only one brand and three samples each, we think that these high levels of PCDDs/PCDFs in fried chicken are very important data because of the potentially increasing daily intake. If we continue to survey for PCDDs/PCDFs in fast foods more even, especially fried chicken, these data will be useful to calculate the estimated daily intake of TEQ levels of PCDDs/PCDFs in Korea.

PCDD and PCDF exposures in workers and controls living near an industrial waste incinerator.

This study measured the levels of 17 congeners of PCDDs/PCDFs in serum to compare the levels between potentially exposed workers at an industrial waste incinerator and any residents with no known exposures. The 1,2,3,6,7,8- and 1,2,3,7,8,9-HxCDD were detected in serum of workers but in controls. Likewise, 1,2,3,7,8-PeCDF, 1,2,3,6,7,8- and 1,2,3,7,8,9-HxCDF were detected only in serum of workers. The international toxic equivalent (TEQ) levels of PCDDs/PCDFs in sera of workers are much higher than in controls. Among PCDDs, the proportion of total concentration and TEQ level is dominated predominantly by 1,2,3,6,7,8- and 1,2,3,7,8,9-HxCDD. We need extensive studies to estimate human exposure and are continuing this investigation.

Effect of Rehmannia glutinosa on immediate type allergic reaction.

We investigated the effect of aqueous extract of Rehmannia glutinosa steamed root (RGAE) on the allergic reactions in vivo and in vitro. RGAE dose-dependently inhibited systemic allergic reaction induced by compound 48/80. When RGAE was pre-treated at the same concentrations with systemic allergic reaction test, the plasma histamine levels were reduced in a dose-dependent manner. RGAE dose-dependently inhibited skin allergic reaction activated by anti-dinitrophenyl (DNP) IgE. RGAE also dose-dependently inhibited the histamine release from the rat peritoneal mast cells (RPMC) by compound 48/80 or anti-DNP IgE. Moreover, RGAE had significant inhibitory effect on anti-DNP IgE-induced tumor necrosis factor-alpha production of RPMC. These results indicate that RGAE may be beneficial in the regulation of immediate type allergic reaction.

S-[2-(4-azidosalicylamido)ethylthio]-2-thiopyridine: radioiodinatable, cleavable, photoactivatible cross-linking agent.

S-[2-(4-Azidosalicylamido)ethylthio]-2-thiopyridine (AET) contains a 2-thiopyridyl moiety, which permits cysteine-specific incorporation into protein through a cleavable disulfide bond, and a 4-azidosalicylamido moiety, which permits radioiodination and photoactivatible cross-linking. In contrast to the related compound S-[2-[N-[4-(4-azidosalicylamido)butyl]carbomoyl]ethylthio]-2 -thiopyridine [APDP; Zecherle, G., Oleinikov, A., and Traut, R. (1992) Biochemistry 31, 9526], AET contains a relatively short linker arm between the 2-thiopyridyl moiety and the 4-azidosalicylamido moiety. In a previous paper, it was shown that AET could be used in site-specific protein-protein photocross-linking to identify nearest-neighbor protein domains within a multiprotein complex [Chen, Y., Ebright, Y., and Ebright, R. (1994) Science 265, 90]. In this paper, the synthesis, radioiodination, and incorporation into protein of AET are described.

On predicting multi-route and multimedia residential exposure to chlorpyrifos and diazinon.

This paper formulates regression models and examines their ability to associate exposures to chlorpyrifos and diazinon in residences with information obtained from questionnaires and environmental sampling of the National Human Exposure Assessment Survey Arizona (NHEXAS-AZ) database. A knowledge-based list of 29 potential exposure determinants was assembled from information obtained from six questionnaires administered in the course of the study. This list was used to select the independent variables of each model statistically and electronically. Depending on the data type of dependent and independent variables, four classes of regression models were developed to determine desired associations. Route-specific exposures were estimated using the indirect method of exposure estimation and measurements from the NHEXAS-AZ field study. The stepwise procedure was used to construct regression models. Significance level at P=0.10 was used for entry and retention of independent variables in a model. Twelve significant regression models were formulated to quantify associations among exposures and other variables in the NHEXAS-AZ database. Route-specific exposures to pesticides associate significantly with questionnaire-based variables such as preparation of pesticides, use of pesticide inside the house, and income level; and with concentration variables in three media: dermal wipe, sill wipe, and indoor air. Models formulated in this study may be used to estimate exposures to each of the pesticides. Yet, the use of these models must incorporate clear statements of the assumptions made in the formulation as well as the coefficient of determination and the confidence and prediction intervals of the dependent variable.

Applications of computerized microscopic image analysis in infectious diseases.

Advances in computerized microscopy have resulted in image analysis systems that rapidly and precisely measure various aspects of cellular morphology and physiology. These systems-composed of a microscope and attached photomultiplier tube or camera, an image processor, and a computer-have been used to measure lysosomal enzymes, pH, and calcium within phagocytes; to detect viral nucleic acids in in situ hybridization preparations; and to quantitate rates of cellular movement. These experiments have shown that (1) the intracellular proliferation of virulent microorganisms is associated with reductions in acid phosphatase, beta-glucuronidase, and lysozyme activity; (2) virulent Toxoplasma gondii, Legionella pneumophila, and Nocardia asteroides inhibit phagosomal acidification; and (3) changes in intracellular calcium movement affect phagocytic function. These methods have also been used to detect the AIDS virus within cultured lymphocytes and to measure cellular chemotaxis and chemokinesis. Further advances in technology should produce improved microscopic image analysis systems with wider applications for the investigation of infectious diseases.

Effect of rifampin and oxacillin on the ultrastructure and growth of Staphylococcus aureus.

The minimal inhibitory concentrations (MICs) of rifampin for 12 strains of Staphylococcus aureus ranged from 0.005 microgram/ml to 0.02 microgram/ml, and the minimal antibiotic concentrations (i.e., those that inhibited growth by 90% in comparison with control values) ranged from one-fifth to one-half the MICs. Rifampin alone produced a 1-log (90%) decrease in the staphylococcal population after incubation for 6.5 hr, and the inhibitory effect after removal of the antibiotic lasted for 5 hr. Regardless of whether rifampin was used simultaneously with, before, or after exposure to oxacillin, the latter drug partially or totally prevented the overgrowth of rifampin-resistant staphylococci. The use of this combination in therapy is therefore justified. Staphylococci exposed to rifampin at concentrations equal to three or 20 times the MIC developed a cell wall that was four times thicker than normal and had a peripheral undulated dense layer. The cross walls were also two to 10 times thicker than normal but had a normal central dense zone. Exposure of staphylococci to oxacillin produced large cells with multiple cross walls that underwent autolysis when the organisms were transferred to drug-free medium. When exposure to oxacillin was preceded by exposure to rifampin, cells did not become enlarged. When the large cells produced by oxacillin were transferred to medium containing rifampin, the cross walls did not lyse.

Effect of shear rates on protein adsorption in the total artificial heart.

Plasma protein adsorption is the first event in blood-material interaction and influences subsequent platelet adhesion and thrombus formation. Thromboembolic events are strongly influenced by surface characteristics of materials and fluid dynamics inside the blood pump. In vitro flow visualization, and an animal experiment with a moving actuator total artificial heart (TAH), were performed to investigate fluid dynamic effects on protein adsorption. The different levels of shear rate inside the ventricle were determined by considering the direction of opening of the four heart valves in the implanted TAH, and the visualized flow patterns as well. Each ventricle of the explanted TAH was cut into 12 segments according to the shear rate level. The adsorbed protein on each segment was quantified using an ELISA method after soaking in 2% (w/v) SDS/PBS for 2 days. Adsorbed protein layer thicknesses were measured by the immunogold method under transmission electron microscopy. Scanning electron microscopic observation showed that the right ventricle, immobilized with albumin, displayed different degrees of platelet adhesion on each segment, whereas the left ventricle, coated with polythyleneoxide-sulfonate, indicated nearly the same platelet adhesion behavior, regardless of shear rates. The surface concentrations of adsorbed proteins in the low shear rate region are higher than those in the high shear region, which was confirmed statistically.

Albumin immobilized polyurethane and its blood compatibility.

Surface pretreatment with albumin on a blood contacting material inhibits platelet adhesion, activation, and subsequent thrombus formation. Although adsorbed albumin improves blood compatibility, rapid desorption occurs when this surface is exposed to circulating blood. In this study, human serum albumin was immobilized on a polyurethane (PU) surface to investigate its blood compatibility and extended effects on a blood-material interface. The PU surface was treated with hexamethylene diisocyanate (HMDI), and the PU-HMDI was further grafted with albumin to produce an albumin immobilized PU surface (PU-albumin). The PU-albumin surface was characterized by attenuated total reflection infrared electron spectroscopy for chemical analysis, scanning electron microscopy, and dynamic contact angle. Blood compatibility was evaluated by in vitro protein adsorption, platelet adhesion, and occlusion time in an ex vivo rabbit arterio-arterial shunt. Immobilization of albumin was confirmed by the disappearance of the -NCO peak observed at 2,250 cm-1 on the PU-HMDI surface by infrared spectroscopy and the existence of sulfur atomic percent by electron spectroscopy for chemical analysis. The concentration of PU-albumin was approximately 5.8 micrograms/cm2. The PU-albumin also showed a slight increase in hydrophilicity on the Wilhelmy plate method, and there was less fibrinogen adsorption than a PU control. In addition, PU-albumin had less platelet adhesion, platelet activation, and thrombogenicity. The ex vivo occlusion time of untreated PU was 50 min, that of PU-albumin was extended to 150 min, indicating that a PU-albumin surface has better blood compatibility than PU alone.

The spectrum of eosinophilic lung disease: radiologic findings.

PURPOSE: Eosinophilic lung disease includes various disease entities. Each disease manifests different radiologic findings. The purpose of this review is to present the radiologic findings of the spectrum of eosinophilic lung disease. METHOD: We reviewed the radiologic, histologic, and clinical findings of the spectrum of eosinophilic lung disease from the previous reports and our experiences. RESULTS: Simple pulmonary eosinophilia is characterized by transient and migrating opacities on chest radiography. Acute eosinophilic pneumonia is characterized by acute clinical symptoms and signs and rapid changes of radiographic diffuse reticular lesions. Chronic eosinophilic pneumonia, with more prolonged symptom duration, history of asthma, occurrence of relapse, and radiologic features of subpleural consolidation, can be differentiated from acute eosinophilic pneumonia. Allergic bronchopulmonary aspergillosis presents with bilateral central bronchiectasis with or without mucoid impaction. Although these diseases show specific radiographic findings, some show overlapping radiographic features. High-resolution CT enables characterization of parenchymal lesions further by showing internal and marginal features and the exact extent of the lesions. Extrapulmonary organs are involved in Churg-Strauss syndrome and idiopathic hypereosinophilic syndrome. Asthma is associated with Churg-Strauss syndrome, allergic bronchopulmonary aspergillosis, chronic eosinophilic pneumonia, and bronchocentric granulomatosis. CONCLUSION: Integration of clinical, laboratory, and radiologic findings enables initial and differential diagnoses of various eosinophilic lung diseases.

Diffuse micronodular lung disease: HRCT and pathologic findings.

PURPOSE: The purpose of our study was to compare the findings seen on HRCT with those from pathologic examination in patients with diffuse micronodular lung disease (DMLD). METHOD: Forty consecutive patients with biopsy-proven DMLD (each nodule being <5 mm in diameter and occupying more than two-thirds of lung volume on chest radiograph) were included. High resolution CT (HRCT) scans were analyzed with particular attention to the location of nodules in and around the secondary pulmonary lobule (centrilobular, perilymphatic, and random) and the zonal distribution. The findings were compared to pathology. RESULTS: CT scans showed centrilobular nodules in the patients with diffuse panbronchiolitis (n = 4), infectious bronchiolitis [n = 4; Hemophilus influenzae (n = 3) and Mycoplasma pneumoniae (n = 1)], hypersensitivity pneumonia (n = 3), bronchogenic disseminated tuberculosis (n = 3), pneumoconiosis (n = 1), primary lymphoma of the lung (n = 1), and foreign body-induced necrotizing vasculitis (n = 1). They demonstrated perilymphatic nodules in the patients with pneumoconiosis (n = 5), sarcoidosis (n = 2), and amyloidosis (n = 2). They demonstrated micronodules of random distribution in the patients with miliary tuberculosis (n = 9) and pulmonary metastasis (n = 5). Upper and middle zonal predominance was seen in two of two patients with sarcoidosis and in two of six patients with pneumoconiosis. The CT location of nodules in the secondary pulmonary lobule represented the location and nature of the lesion on pathologic examination. CONCLUSION: By showing the distribution of micronodules in and around the secondary pulmonary lobule, HRCT enables the narrowing of the differential diagnosis of DMLD. CT findings reflect gross morphologic features of pathologic examination.
