Modified Method for Competitive Reverse Transcription Polymerase Chain Reaction for Rapid and Automated Quantitation of Messenger RNA in Multiple Samples.

Background: Competitive reverse transcription polymerase chain reaction (RT-PCR) has been used increasingly to quantitate messenger RNA (mRNA) levels; however, conventional competitive RT-PCR methods require four or five reactions per sample of RNA, employing serial dilutions of an internal competitor sequence, making analysis of multiple samples of tedious process. A modified method is described by which multiple samples and multiple RNA transcripts can be analyzed easily by an automated process. Methods and Results: Transforming growth factor beta-1 (TGF-beta-1) mRNA was assayed in total RNA extracted from cultured human skin fibroblasts. A standard solution of total RNA was first prepared by pooling RNA from several cell lines and stored in aliquots. A 270-bp competitor RNA molecule (RNA mimic) was prepared by in vitro transcription and was added to each reaction. PCR was performed with a fluorescent dye (Hex; Applied Biosystems, Foster City, CA)-labeled sense primer to amplify a 161-bp-long c DNA product of target TGF-beta-1 mRNA sequence and the RNA mimic. The PCR products were analyzed with an automated laser-scanned gel electrophoresis system and the area under the curve (AUC) was used for quantitation. The concentration TGF-beta-1 mRNA in standard RNA was determined by conventional competitive RT-PCR. Subsequently, equal amounts of RNA mimic were mixed with four serial dilutions of standard RNA and 0.1 µg of sample total RNA for RT-PCR. A standard curve was generated using the known dilutions of a standard target RNA solution and ratio of AUC for target to that for mimic for each dilution. The unknown sample was then quantitated by interpolation of its area under the curve ratio on the standard curve. This method had inter- and intra-assay coefficients of variation of less than 10%. Conclusions: This modification is highly reproducible for quantitation of mRNA and significantly reduces the number of PCR reactions required for each assay. It can be used to assay several RNA molecules in a given sample by designing RNA mimics and PCR primers to generate PCR products of different lengths so that they can be analyzed by the laser scanning of a single lane of electrophoretic gel.

The transcriptional repression of the human Cu/Zn superoxide dismutase(sod1) gene by the anticancer drug, mitomycin C(MMC).

The Cu/Zn superoxide dismutase(sod1) is one of the key enzymes that protects cells against oxidative stress. In order to investigate the effects of mitomycin C(MMC) on the induction of apoptotic cell death and on the sod1 transcription level, the CATs activity of HepG2 cells transfected with sod1 promoter-CAT(chloramphenicol acetyl transferase) fusion reporter was measured after MMC treatment. The CAT assay showed that exposure of HepG2 cells to MMC decreased the transcription level of the sod1 gene. The accumulation of p53 tumor suppressor protein by MMC treatment of HepG2 cells was noted. In order to investigate the p53-negative response element in its promoter region, a p53 cotransfection experiment with serially deleted sod1 promoter/CAT reporter constructs was performed. The results show a significant reduction of CAT activity in all deletion reporter constructs. The results show that MMC treatment inhibited sod1 gene transcription through p53-mediated transcriptional repression.

The mean A beta load in the hippocampus correlates with duration and severity of dementia in subgroups of Alzheimer disease.

Using image analysis techniques to quantify the percentage area covered by the immunopositive marker for amyloid beta-peptide (A beta), we examined subjects with combinations of either early-onset or late-onset Alzheimer disease (AD) and either familial Alzheimer disease (FAD) or sporadic Alzheimer disease (SAD). We measured the mean and maximum A beta loads, in the hippocampus of each subject. There were no statistically significant differences in the mean A beta load between familial and sporadic AD subjects. Although sample sizes were too small for statistical testing, subjects with the epsilon 4/epsilon 4 allele of the apolipoprotein E (ApoE) gene had higher mean A beta loads than those with the epsilon 3/epsilon 3 or epsilon 3/epsilon 4 alleles. Members of the Volga German families (recently linked to chromosome 1) all had high mean A beta loads, and one of the chromosome 14-linked subjects had the highest mean A beta load while the other had a relatively small load, but the sample was too small for statistical comparisons. The duration of dementia and neuropsychological test scores showed a statistically significant correlation with the mean A beta load in the hippocampus, but not with the maximum A beta load. This difference indicates that the mean A beta load may be a more useful feature than the maximum A beta load as an objective neuropathological measure for cognitive status. This finding may help to improve the established methods for quantitative assessment of the neuropathological changes in AD.

Recovery of pancreatic tissue perfusion and ATP tissue level after reperfusion in canine pancreas grafts preserved by the two-layer method.

We examined whether pancreas grafts preserved by the two-layer method maintained adenine nucleotides pool as substrates for ATP synthesis before reperfusion and had the ability to recover pancreatic tissue perfusion and synthesize ATP promptly after reperfusion. After preservation by the two-layer cold storage method using EC for 48 h (group 1) or simple cold storage in EC for 48 h (group 2), canine pancreas grafts were autotransplanted. In controls (group 3), canine pancreas grafts were autotransplanted without preservation. Tissue adenine nucleotide concentrations were measured using high-performance liquid chromatography, and pancreatic tissue perfusions were measured using the hydrogen gas clearance method. Graft survival rates were five of five, zero of five, and five of five in groups 1, 2, and 3, respectively. Total adenine nucleotide levels before reperfusion in group 1 were almost the same as in group 2. Furthermore, ATP tissue levels in group 1 before reperfusion were also the same as in group 2. However, tissue ATP levels in group 1 after 2 h of reperfusion (6.71 +/- 1.19 mumol/g dry weight) were significantly higher than group 2 (4.51 +/- 0.51 mumol/g dry weight; p < 0.05). On the other hand, pancreatic tissue perfusions in grafts preserved by the two-layer method (group 1) after 2 h of reperfusion were significantly higher than in group 2. We conclude that oxygenation of pancreas grafts during preservation by the two-layer method allows for ATP synthesis and leads to maintenance of the grafts' ability to recover ATP levels and tissue perfusions after reperfusion.

A methodology for evaluation of boundary detection algorithms on medical images.

Image segmentation is the partition of an image into a set of nonoverlapping regions whose union is the entire image. The image is decomposed into meaningful parts which are uniform with respect to certain characteristics, such as gray level or texture. In this paper, we propose a methodology for evaluating medical image segmentation algorithms wherein the only information available is boundaries outlined by multiple expert observers. In this case, the results of the segmentation algorithm can be evaluated against the multiple observers' outlines. We have derived statistics to enable us to find whether the computer-generated boundaries agree with the observers' hand-outlined boundaries as much as the different observers agree with each other. We illustrate the use of this methodology by evaluating image segmentation algorithms on two different applications in ultrasound imaging. In the first application, we attempt to find the epicardial and endocardial boundaries from cardiac ultrasound images, and in the second application, our goal is to find the fetal skull and abdomen boundaries from prenatal ultrasound images.

Programmable ultrasound imaging using multimedia technologies: a next-generation ultrasound machine.

High computational and throughput requirements in modern ultrasound machines have restricted their internal design to algorithm-specific hardware with limited programmability. We have architected a programmable ultrasound processing system, Programmable Ultrasound Image Processor (PUIP), to facilitate engineering and clinical ultrasound innovations. Multiple high-performance multimedia processors were used to provide a computing power of 4 billion operations per second. Flexibility was achieved by making our system programmable and multimodal, e.g., B-mode, color flow, cine and Doppler data can be processed. We have successfully designed and implemented the PUIP to fit within an ultrasound machine. It provides a platform for rapid testing of new concepts in ultrasound processing and enables software upgrades for future technologies. Current and future clinical applications include extended fields of view, quantitative measurements, three-dimensional ultrasound reconstruction and visualization, adaptive persistence, speckle reduction, edge enhancement, image segmentation, and motion analysis. The PUIP is a significant step in the evolution of ultrasound machines toward more flexible and generalized systems bridging the gap between many innovative ideas and their clinical use in ultrasound machines.

Vitamin B12 treatment for delayed sleep phase syndrome: a multi-center double-blind study.

The active form of vitamin B12 (methylcobalamin) has been reported to be effective on sleep-wake rhythm disorders. Previous studies, however, were performed under open trial, and the effect of vitamin B12 has not been properly evaluated. The aim of this double-blind study was to investigate the efficacy of methylcobalamin on delayed sleep phase syndrome (DSPS). Methylcobalamin (3 mg/day) or placebo was administered for 4 weeks. The subjects were 50 patients with DSPS aged 13-55 years (26.8 +/- 1.3), 27 of whom received the active drug while 23 received the placebo. No significant differences were observed between the 2 groups in subjective evaluations of mood or drowsiness during the daytime or in night sleep by sleep-log evaluation. These results indicate that 3 mg methylcobalamin administered over 4 weeks is not an effective treatment for DSPS.

The human cytomegalovirus UL97 protein is a protein kinase that autophosphorylates on serines and threonines.

The product of the human cytomegalovirus (CMV) UL97 gene, which controls ganciclovir phosphorylation in virus-infected cells, is homologous to known protein kinases but diverges from them at a number of positions that are functionally important. To investigate UL97, we raised an antibody against it and overexpressed it in baculovirus-infected insect cells. Recombinant baculovirus expressing full-length UL97 directed the phosphorylation of ganciclovir in insect cells, which was abolished by a four-codon deletion that confers ganciclovir resistance to CMV. When incubated with [gamma-32P]ATP, full-length UL97 was phosphorylated on serine and threonine residues. Phosphorylation was severely impaired by a point mutation that alters lysine-355 in a motif that aligns with subdomain II of protein kinases. However, phosphorylation was impaired much less severely by the four-codon deletion. A UL97 fusion protein expressed from recombinant baculovirus was purified to near homogeneity. It too was phosphorylated upon incubation with [gamma-32P]ATP in vitro. This phosphorylation, which was abolished by the lysine 355 mutation, was optimal at high NaCl and high pH. The activity required either Mn2+ or Mg2+, with a preference for Mn2+, and utilized either ATP or GTP as a phosphate donor, with Kms of 2 and 4 microM, respectively. The phosphorylation rate was first order with protein concentration, consistent with autophosphorylation. These data strongly argue that UL97 is a serine/threonine protein kinase that autophosphorylates and suggest that the four-codon deletion affects its substrate specificity.

Reversible cystic disease associated with pulmonary tuberculosis: radiologic findings.

PURPOSE: The authors describe radiologic findings of cystic lung disease associated with pulmonary tuberculosis. MATERIALS AND METHODS: Three immunocompetent women with pulmonary tuberculosis were seen with acute respiratory failure and diffuse bilateral pulmonary opacity depicted at admission chest radiography. The women did not have acquired immunodeficiency syndrome, or AIDS. They were aged 26, 27, and 52 years. RESULTS: Follow-up radiography and computed tomography showed multiple air-filled cystic lesions in both lungs, which disappeared almost completely after antituberculous chemotherapy (isoniazid, rifampin, ethambutol hydrochloride, and pyrazinamide). CONCLUSION: Pulmonary tuberculosis manifested with reversible multiple cystic lung disease in three immunocompetent patients.

Sleep and circadian influences on cardiac autonomic nervous system activity.

To assess the separate contributions of the sleep and circadian systems to changes in cardiac autonomic nervous system (ANS) activity, 12 supine subjects participated in two 26-h constant routines, which were counterbalanced and separated by 1 wk. One routine did not permit sleep, whereas the second allowed the subjects to sleep during their normal sleep phase. Parasympathetic nervous system activity was assessed with respiratory sinus arrhythmia as measured from the spectral analysis of cardiac beat-to-beat intervals. Sympathetic nervous system activity was primarily assessed with the preejection period as estimated from impedance cardiography, although the 0.1-Hz peak from the spectral analysis of cardiac beat-to-beat intervals, the amplitude of the T wave in the electrocardiogram, and heart rate were also measured. Respiratory sinus arrhythymia showed a 24-h rhythm independent of sleep, whereas preejection period only showed a 24-h rhythm if sleep occurred. Thus the findings indicate that parasympathetic nervous system activity is mostly influenced by the circadian system, whereas sympathetic nervous system activity is mostly influenced by the sleep system.

Alterations in brain and plasma concentrations of alpha-melanocyte-stimulating hormone during habituation of prey-catching behavior in toads.

Alpha-melanocyte-stimulating hormone (alpha-MSH) and adrenocorticotropin (ACTH) promote habituation of prey-catching behavior in toads. We tested the hypothesis that habituation is associated with alterations in the activity of alpha-MSH neurons in a toad, Bufo cognatus. We used immunocytochemistry and RIA to determine the organization and distribution of alpha-MSH neurons in the brain. In addition, we measured brain alpha-MSH and plasma concentrations in toads at different times before and during stimulus specific habituation of the turning response to a rotating prey dummy. Cell groups immunoreactive for alpha-MSH-/and ACTH were detected only in the infundibular hypothalamus. Projections from these cells innervated the olfactory nucleus, nucleus accumbens, septum, habenular nucleus, the preoptic area, the hypothalamus, the caudal thalamus, optic tectum and brain stem. The rank order of alpha-MSH concentrations in the brain of Bufo cognatus was caudal thalamus/hypothalamus (T/HT) > preoptic area (PO) > or = rostral telencephalon (Tel) > brain stem (BS) > optic tectum (OT). The only difference observed in tissue alpha-MSH levels between habituated and non-habituated toads was an elevation in T/HT alpha-MSH levels in control toads after 20 min in the testing apparatus. In contrast, many differences were observed in both groups as a function of time spent in the test apparatus. Concentrations of alpha-MSH in the T/HT and PO increased while concentrations in the Tel decreased over time for both control and habituated toads. Plasma alpha-MSH titers rose over time in control toads but not in habituated toads. Our findings suggest that habituation per se is not associated with dramatic alterations in brain or plasma alpha-MSH levels. On the other hand, prolonged confinement causes pronounced alterations in the activity of brain and pituitary proopiomelanocortin cells.

Confirmatory factor analysis of the personality disorder subscales from the Inventory of Interpersonal Problems.

The purpose of this study was to identify the best fitting hierarchical factor structure of the subscales for personality disorders developed from the Inventory of Interpersonal Problems (IIP). In earlier work, 5 subscales associated with a diagnosis of personality disorder (PD) had been developed. With data collected from 5 additional samples at 4 sites (N = 1004), relations among the IIP-PD subscales were investigated using confirmatory factor analysis. Several competing models were tested, and a second-order model with 1 second-order factor and 5 first-order factors provided the best fit to the data. The results support the hypothesis of a single latent construct reflecting general personality dysfunction. Measures of this construct may be useful for screening patients into yes versus no PD groups.

Growth hormone response to the hypothalamic somatostatinergic activity in acromegalic patients.

Oral glucose administration suppresses the TRH-induced TSH response by enhancing the hypothalamic somatostatinergic activity (HSA). We assessed the HSA in 13 acromegalic patients by measuring glucose-induced suppression of TRH-stimulated TSH secretion. The HSA showed wide variation with up to 64% suppression. The mean HSA of the patients (25 +/- 6%) did not differ from that in normal young men (19 +/- 4%) in our previous study. Six patients had normal or low HSA, and the other 7 patients had high HSA. The mean TRH-stimulated TSH levels of the patients with normal or low HSA was significantly lower than that of the patients with high HSA (5.13 +/- 0.10 vs. 11.30 +/- 2.80 mU/L). The HSA did not relate to sex, age, tumor size, basal GH levels, the paradoxical responses to TRH and GnRH, octreotide response, or the gsp oncogene. In the combined glucose-TRH test, glucose pretreatment completely suppressed the paradoxical increase in GH level at 30 min in 4 patients. However, it could suppress the paradoxical GH response by only 6-51% in the other 5 patients who also showed the paradoxical response in TRH test. The tumor diameter of patients with good response to the HSA was significantly larger than that of the patients with poor response (31 +/- 5 vs. 14 +/- 2 mm) as was the tumor grade (3.3 +/- 0.3 vs. 1.7 +/- 0.2). This study supports the idea that a reduction of HSA is not a primary cause of acromegaly, and the HSA seems to increase to suppress the autonomous secretion of GH from the pituitary adenomas. HSA as well as the response of tumors to HSA do not determine tumor growth.

Tuberculosis of the trachea and main bronchi: CT findings in 17 patients.

OBJECTIVE: The purpose of our study was to describe the CT findings of tuberculosis of the trachea and main bronchi. MATERIALS AND METHODS: Initial (n = 17) and follow-up (n = 7) CT scans were available from 17 patients (five men and 12 women; aged 25-82 years old) with tracheobronchial tuberculosis. The diagnosis of tuberculosis was based on bronchoscopic, microbiologic, or pathologic findings. RESULTS: The trachea (n = 6), the right main bronchus (n = 6), and the left main bronchus (n = 4) were involved in 10 patients with actively caseating tuberculosis. These airways showed irregular (n = 11) or smooth (n = 4) circumferential narrowing and occlusion (n = 1). Also, the trachea (n = 2), the right main bronchus (n = 1), and the left main bronchus (n = 6) were involved in seven patients with fibrotic tuberculosis. These airways showed smooth (n = 4) or irregular (n = 3) circumferential narrowing and occlusion (n = 2). Mediastinitis (increased densities in mediastinal fat) was seen in four of 10 patients with active tuberculosis but not in any of the seven patients with fibrotic disease. Tracheal tuberculosis, which has always been associated with bronchial disease, involved the distal trachea and exceeded 3 cm in length. After the seven patients underwent initial scans and antituberculous chemotherapy, serial CT scans showed improvement in seven of 11 lesion sites: normalized airways (n = 4) or smoothing from irregular narrowing (n = 3). The remaining four sites showed no change. CONCLUSION: On CT scans, actively caseating tracheobronchial tuberculosis showed circumferential and predominantly irregular luminal narrowing and mediastinitis. When fibrotic disease was found, a CT scan revealed equal distribution of smooth and irregular narrowing and less wall thickening than was seen in patients with active disease. On CT scans, tracheal tuberculosis involved the long segment of the distal trachea, a condition associated with bronchial tuberculosis.
