Association of smoking with serum and dietary levels of antioxidants in adults: NHANES III, 1988-1994.

OBJECTIVES: This study examined the association of smoking with serum levels and dietary intakes of antioxidants in a nationally representative sample. METHODS: This study classified 7873 apparently healthy adults aged 17 to 50 years from National Health and Nutrition Examination Survey III (NHANES III) data as nonsmokers or as smokers if their serum cotinine levels were either lower than 14 ng/mL or 14 ng/mL or greater, respectively. SUDAAN software was used for the statistical analysis. RESULTS: Smokers of both sexes had significantly (P < .001) lower serum levels of vitamin C, alpha-carotene, beta-carotene, beta-cryptoxanthin, and lutein/zeaxanthin. Reduction in the serum vitamin E, lycopene, and selenium levels in smokers was slight. Smokers also had significantly lower dietary intakes of vitamin C and beta-carotene. A significant (P < .001) inverse relation was found between serum vitamin C and beta-carotene levels and cotinine levels independent of diet effect, and a positive relation (P < .001) was found between serum levels and dietary intakes. CONCLUSIONS: Antioxidants appear to have differing declines in serum levels as a result of reduced dietary intakes and the effects of smoking.

Regular overtime and cardiovascular functions.

It was concluded that there was sufficient evidence about a possible link between long working hours particularly exceeding 50 a week and the risk of significant health outcomes, including cardiovascular disease from literature review by Spurgeon et al.. This study was conducted to find out the single effect of regular overtime work on the cardiovascular functions through objective biological indices such as blood pressure or heart rate variability. We conducted a field survey of 238 male engineers who were working at the department of research & development of three electronics manufacturing companies in S. Korea. The field survey consisted of (1) self-report questionnaire (working hours and health conditions, and fatigue) and (2) measurements of blood pressure and heart rate variability. By multivariate analyses we could show the relationship between overtime work and some cardiovascular functions after controlling the effects of major confounders such as age and sleeping hours, which were pointed out by Iwasaki et al. (1998) and Sasaki et al. (1999). Especially, low frequency component (power in the low frequency range, 0.04-0.15 Hz) of the heart rate variability during work might be used as early objective biological indices for chronic effect of regular overtime work on cardiovascular functions. However, we should confirm those effect through the well-designed prospective study.

Long working hours and subjective fatigue symptoms.

Presuming that overtime work may associate with chronic fatigue and then decrease cardiovascular function, this report was prepared to find out the definite positive relationship between long working hours and subjective fatigue complaints. We analysed the data of a field survey of 238 men who were working at the department of research & development of three electronics manufacturing companies in S. Korea. The field survey consisted of self-report questionnaires on the working hours, health conditions, and fatigue. For data analysis, the subjects were divided into 3 groups on the criteria of 60 and 70 working hours per week: less longer (LLWH), longer (LWH), and more longer (MLWH) working hour groups. We compared the age-adjusted mean % scores of fatigue complaints among LLWH, LWH, and MLWH. The rate of complaints of subjective fatigue before going to work for LWH and MLWH tended to be significantly higher than those for LLWH. Hence, we conclude that the questionnaire on the subjective fatigue complaints is a good screening tool for early detection of cumulative fatigue due to chronic job stress such as long working hours.

Phase 2 and 3 clinical trial of oral bexarotene (Targretin capsules) for the treatment of refractory or persistent early-stage cutaneous T-cell lymphoma.

OBJECTIVES: To determine the safety and efficacy of oral bexarotene (Targretin capsules; Ligand Pharmaceuticals Incorporated, San Diego, Calif). DESIGN: The effects of 2 randomized doses of 6.5 mg/m(2) per day (with crossover for progression) vs 650 mg/m(2) per day (later modified to 300 mg/m(2) per day) were evaluated in an open-label, multicenter, phase 2 and 3 study conducted between February 1997 and November 1998. SETTING: Eighteen international cutaneous T-cell lymphoma clinics at academic referral centers. PATIENTS: Fifty-eight patients with biopsy-proven stage IA through IIA cutaneous T-cell lymphoma that was refractory to (or patients were intolerant of) treatment or had reached at least a 6-month response plateau under at least 2 forms of prior therapy (median of 3.5 prior therapies). INTERVENTION: Bexarotene (Targretin capsules) administered once daily with meal for 16 weeks or longer. MAIN OUTCOME MEASURES: Primary end point classification of overall response rate of complete and partial remissions determined by either the Physician's Global Assessment of Clinical Condition or the objective Composite Assessment of Index Lesion Severity. Body surface area, time to response, duration of disease control, time to disease progression, individual index lesion signs and symptoms, and quality of life parameters were secondary outcomes. RESULTS: Responses (> or = 50% improvement) were seen in 3 (20%) of 15 patients with an initial dose at 6.5 mg/m(2) per day (95% confidence interval [CI], 0%-40%), 15 (54%) of 28 patients at 300 mg/m(2) per day (95% CI, 35%-72%), and 10 (67%) of 15 patients at above 300 mg/m(2) per day (95% CI, 43%-91%). The rate of progressive disease was 47%, 21%, and 13% at the same dose levels, respectively. Eight (73%) of 11 patients crossing over from 6.5 mg/m(2) per day to higher doses subsequently responded. The median duration of response from start of therapy could not be estimated for the 15 patients at 300 mg/m(2) per day owing to low relapse rates in 2 patients (13%); at higher doses it was 516 days. The following drug-related adverse effects were reversible and treatable: hypertriglyceridemia (46 patients [79%]), hypercholesterolemia (28 patients [48%]), headache (27 patients [47%]), central hypothyroidism (23 patients [40%]), asthenia (21 patients [36%]), and leukopenia (16 patients [28%]). No cases of drug-related neutropenic fever, sepsis, or death occurred. Pancreatitis occurred in 3 patients with triglyceride levels higher than 14.69 mmol/L (1300 mg/dL), all of whom were taking 300 mg/m(2) or more of oral bexarotene per day. CONCLUSIONS: Bexarotene (Targretin capsules) (the first retinoid X receptor-selective rexinoid) was well tolerated and effective as an oral treatment for 15 (54%) of 28 patients with refractory or persistent early-stage cutaneous T-cell lymphoma at doses of 300 mg/m(2) per day. Hypertriglyceridemia and hypothyroidism require monitoring but are reversible and manageable with concomitant medication.

Control and separation of algae particles from WSP effluent by using floating aquatic plant root mats.

In this paper, the potential uses of water hyacinth and its root mats for separating algae particles in the effluent from waste stabilization ponds (WSPs) were discussed. Pilot-scale integrated processes consisting of WSPs and multiple WHPs (water hyacinth ponds) were operated in order to extract effects of the root mats on the reduction of algae concentrations. Root mats in the bottom of WHPs separated significant amount of the algae cells through attachment as the effluent from WSPs passed through them. Attachment of the algae particles to the surface of live roots was found to be similar to adsorption phenomena but it lasted even at saturation, probably due to the continuous reproduction of active attachment sites by detachment and growth of the roots. Additionally, this paper discusses attachment mechanisms and other issues concerning design and polishing of the WSPs effluent by WHPs.

Transmission of Mycobacterium tuberculosis among high school students in Korea.

SETTING: Screening of pulmonary tuberculosis (PTB) patients from high school students by chest radiography and sputum examination. OBJECTIVE: To investigate transmission of Mycobacterium tuberculosis among high school students. DESIGN: The restriction fragment length polymorphism (RFLP) profiles of M. tuberculosis isolated from PTB patients of high school students were analysed. RESULTS: Clustering cases were found in PTB patients screened among high school students. During this study, a single strain with a particular RFLP profile was found most frequently. This particular strain was also the one that was found to be most prevalent among Korean clinical isolates of M. tuberculosis during a nationwide TB prevalence survey previously done in 1995. This strain seems to be the most widely distributed one in Korea, and has thus been designated K1. In addition, by close examination of the RFLP profiles, it was also found that there existed 34 isolates that had RFLP profiles that were similar to the K1 strain. Therefore, K1 and K1-related strains (18.4% of strains, designated as the K family) seem to comprise the most dominant M. tuberculosis strains present in Korea. CONCLUSION: Clustering cases were found in PTB patients screened among high school students in Korea. The K family was found to be most prevalent among the clinical isolates of M. tuberculosis found.

Developing CD-ROM based multimedia digital textbook of 'San-Yin-Jiao(SP-6) pressure for reducing the labor pain and shortening the labor time'.

The computer-based textbook is a new educational tool that promises to play a prominent role in the coming years. Classical instructional technologies, such as video, stills, audio files and computer programs with a textbook orientation, have been merged into one multimedia computer system and have created additional opportunities for learning in medical, dental and nursing education. The authors developed a hypermedia textbook of â San-Yin-Jiao pressure for reducing the labor pain and shortening the labor time' using a personal computer with hypermedia software that contains texts, images, videos, audios and literature citations. The target population of this educational CD-ROM would be nurse-midwives, clinical nurses working for the obstetric units, and faculty members who teach the Maternity and Women's Health Nursing. Valuable and practical experiences were obtained and shared.

[Two cases of effective weekly paclitaxel administration for metastatic breast cancer].

We report two cases of effective weekly paclitaxel (TXL) administration for metastatic breast cancer. TXL (80 mg/m2) was infused over 1 hour after short premedication on an outpatient basis. Administration was continued for 3 weeks followed by 1 week rest. Case 1: A 66-year-old woman was treated with adriamycin, cyclophosphamide, doxifluridine (5'-DFUR), fadrozole, medroxyprogesterone acetate (MPA) and pamidronate for pleural, mediastinal and supraclavicular lymph nodes and skull metastases. She complained of hoarseness and supraclavicular and mediastinal lymph nodes had developed, for which she received weekly TXL administration. Metastatic lymph nodes had disappeared 3 months after administration. Case 2: A 61-year-old woman was treated with CMF therapy, 5'-DFUR, tamoxifen, MPA fadrozole and pamidronate for bone metastasis and local recurrence. She received weekly TXL administration for an axillary recurrence and contralateral supraclavicular lymph node metastases. Metastatic and recurrent lesions had disappeared 3 months after administration. The toxic events were leukopenia (grade 1) and peripheral neuropathy (grade 1). No major adverse effects were observed in either case.

Therapeutic sequence in the management of advanced cases in implant dentistry.

Osseointegrated implants have contributed significantly to the treatment of mutilated dentition. Many patients treated with removable prostheses have benefited from implant-supported fixed prostheses. Achieving successful osseointegration is now a predictable procedure and the successful integration of implants is no longer the main objective. Successfully restoring the form, function, and esthetics of the system, as well as maintaining osseointegration, is now a main objective. This goal depends on proper diagnosis and treatment planning, proper case selection, and an effectively and efficiently planned treatment approach.

Tissue-specific expression of rat IL-18 gene and response to adrenocorticotropic hormone treatment.

IL-18 is a pleiotropic cytokine also proposed to have a role in modulating immune function during stress. Initially found in immune cells, IL-18 mRNA is detectable in several tissues including the cells of the zona reticularis and the zona fasciculata of the adrenal cortex, where its levels are elevated by acute stress or adrenocorticotropic hormone treatment. In the present study, we compared the expression of IL-18 in the adrenal cortex with that of spleen and duodenum, two other IL-18-positive tissues. In situ hybridization showed that, in contrast to the adrenal cortex, in spleen and duodenum IL-18 is primarily localized to immune cells. In duodenum, IL-18 mRNA was also detectable in epithelial cells. Northern blot demonstrated that while the adrenal gland synthesized IL-18 mRNA of 1.1 kb, spleen and duodenum produced a 0.9-kb transcript. RT-PCR, sequencing, Western blot, primer extension, and rapid amplification of cDNA end analysis demonstrated that the three tissues synthesize IL-18 mRNAs containing the same coding region and produce the same IL-18 peptide, but differ in the length of their 5'-untranslated region, indicating tissue-specific usage of the promoter region. Finally, in contrast to the adrenal gland, adrenocorticotropic hormone treatment did not increase the levels of IL-18 mRNA in spleen and duodenum. These results demonstrate tissue-specific expression and promoter usage of IL-18 gene and suggest that the adrenal cortex and not immune cells may be the source of IL-18 during stress.

Genetic structure and functional implication of the fcb gene cluster for hydrolytic dechlorination of 4-chlorobenzoate from Pseudomonas sp. DJ-12.

The fcb gene cluster responsible for the hydrolytic dechlorination of 4-chlorobenzoate (4CBA) was cloned from the chromosomal DNA of Pseudomonas sp. DJ-12, and its nucleotide sequence analyzed. The gene cluster was organized in the order fcbB-fcbA-fcbT1-fcbT2-cbT3-fcbC, which is different from that reported in other bacteria. A promoter-like sequence (-35 and -10 region) is located upstream of the fcbB gene and putative ribosome-binding sequences were found upstream of the respective orfs. A stem-loop transcription terminator structure is found downstream of fcbC. This suggests that the six orfs are transcribed into a polycistronic mRNA. The FcbA, FcbB, and FcbC enzymes for dechlorination of 4CBA have a relationship in common with the enzymes involved in fatty acid metabolism on the basis of their deduced amino acid sequences. The proteins encoded by fcbT1, fcbT2, and fcbT3 show similarity to those encoded by dctP, dctQ, and dctM of Rhodobacter capsulatus respectively, which encode transporter proteins for C4-dicarboxylate. It is likely, therefore, that these proteins of DJ-12 play a role in transport of 4CBA into the cell.

Bradykinin activates phospholipase D2 via protein kinase cdelta in PC12 cells.

Bradykinin (BK) activates phospholipase D (PLD) and induces several responses such as catecholamine secretion, collapse of growth cones, and gene expression in PC12 pheochromocytoma cells. Although two distinct PLD isozymes, PLD1 and PLD2, have been cloned from mammalian cells, the regulatory mechanism for each PLD isozyme by BK is not clear. In our present study, we investigated the activation mechanism of PLD2 by BK in PLD2-overexpressing PC12 cells. BK stimulated PLD2 activity in a concentration-dependent manner within 1 min and this activation was inhibited by pretreatment of the cells with protein kinase C (PKC) inhibitor. PKCalpha and PKCdelta translocated from cytosol to membrane upon BK treatment, and rottlerin potently inhibited the activation of PLD2 by BK. These results suggest that BK activates PLD2 via PKCdelta in PC12 cells.

Sulfoxide-containing aromatic nitrogen mustards as hypoxia-directed bioreductive cytotoxins.

A series of diaryl and alkylaryl sulfoxide-containing nitrogen mustards were synthesized and evaluated for their hypoxia-selective cytotoxicity against V-79 cells in vitro as well as for their metabolism profiles with the rat S-9 fractions. In general, the diaryl sulfoxides (4, 5, and 7-9) showed much greater hypoxia selectivity (11-27-fold) than the alkylaryl sulfoxides (approximately 3-fold) (1 and 3). The fused diphenyl sulfoxides (10 and 11), on the other hand, showed very low hypoxia selectivity (1.3-3-fold). Compound 10 was highly cytotoxic under both aerobic and anaerobic conditions, while 11 showed low cytotoxicity under both conditions. The bioreduction of 8 by the rat S-9 fraction under anaerobic conditions was inhibited by menadione and enhanced by benzaldehyde, acetaldehyde, or 2-hydroxypyrimidine suggesting the involvement of aldehyde oxidase in the reduction of the sulfoxides. Bioreductive metabolism studies of selected model sulfoxides suggested that diaryl sulfoxides are better substrates for aldehyde oxidase than alkylaryl sulfoxides.

Identification of differentially expressed genes in normal and tumor human gastric tissue.

The search for differentially expressed genes in gastric cancer may help define molecular alterations and molecular diagnosis of gastric cancer. Using the differential display PCR technique, we identified 18 genes that are differentially expressed between normal and tumor human gastric tissues. Their expressions were verified with reverse Northern blot analysis and Northern blot analysis. Oxidative phosphorylation-related genes, antizyme inhibitor of ornithine decarboxylase, protein phosphatase-1beta, 35-kDa peroxisomal membrane protein, and cystic fibrosis transmembrane conductance receptor were highly expressed in tumor tissue, whereas pepsinogen A, Na-K ATPase alpha subunit, nerve growth factor receptor, and alpha-tropomyosin were highly expressed in normal tissue. In addition, 3 unknown genes were found to be differentially expressed in paired gastric tissues. These differentially expressed genes may provide significant opportunities for further understanding of gastric carcinogenesis and the molecular diagnosis of gastric cancer.
