Exploratory study of effective chemotherapy to clear cell carcinoma of the ovary.

Although clear cell carcinoma of the ovary is considered to be a tumor with poor prognosis, the clinical characteristics has not been defined. The aim of this study was to evaluate the response of clear cell carcinoma of the ovary to first and second-line chemotherapy and explore effective chemotherapy. Fifty-three patients with clear cell carcinoma of the ovary were enrolled between 1988 and 1997 at our department. Since taxol was not available in Japan at that time, cisplatin-based combination chemotherapy has been exclusively used as a standard first-line chemotherapy. Retrospective analyses of clinical characteristics and the response to first or second-line chemotherapy were performed. Median age was 52 years (range 27-71 years). Tumors were 34% (18/53) stage I, 19% (5/53) stage II, 38% (20/53) stage III, and 9% (5/53) stage IV. All patients with I or II stage disease had optimal cytoreduction. Out of 25 patients with III or IV stage disease 20% (5/25) had negative residual tumor, 36% (9/25) had <2 cm residual tumor, and 44% (11/25) had >/=2 cm residual tumor. All patients received postoperative platinum-based chemotherapy. Of 23 patients with measurable residual tumor 8.7% (2/23) completely and 13% (3/23) partially responded to first-line chemotherapy consisting of cisplatin, adriamycin and cyclophosphamide (CAP) or cisplatin and cyclophosphamide (CP) by CT scan or second look laparotomy. Presence of endometriosis was 55% (29/53) but was not a prognostic factor. Although overall response rate of ovarian clear cell carcinoma to first-line chemotherapy by CAP or CP was about 22%, EP or EJ consisting of etoposide and cisplatin or carboplatin used as a second-line chemotherapy showed 29% response rate, while CPT-P consisting of CPT-11 and cisplatin showed 40% response rate. Clear cell carcinomas were frequently present at early stage, with association of endometriosis and with poor overall prognosis. Although patients with advanced ovarian clear cell carcinoma seemed to have better response to CPT-P than conventional platinum-based chemotherapy, further studies are required with larger number of patients to draw firm conclusions.

Telomerase activity in gynecological tumors.

The aim of this study was to define the clinical implications of semi-quantitative telomerase activity in gynecological tumors by comparing the telomerase activity of cancerous lesion and the adjacent non-cancerous lesion. In 118 cases of gynecologic tumors, including 41 uterine cervical tumors, 43 uterine body tumors and 34 ovarian tumors, telomerase activities were determined using TRAPeze telomerase detection kit for the extension reaction of the telomere sequence and the PCR reaction for amplification of the sequence, and using fluorecence-based telomere repeat amplification protocol (F-TRAP) method for the detection. In all gynecologic cancers examined, telomerase activity of the cancerous lesion was significantly higher than that of the non-cancerous lesion. Telomerase activity in the uterine cervix increased in the following order of the normal uterine cervix, cervical dysplasia and cervical cancer. Regarding the endometrial cancer, telomerase activity at the primary lesion in patients with lymph node metastases was significantly higher than that in patients without lymph node metastases. When telomerase activity was compared by histologic subtypes of the ovarian cancer, clear cell adenocarcinoma showed significantly lower telomerase activity than the other subtypes, especially endometrioid adenocarcinoma. In all gynecologic cancers examined, there was no clear correlation between the telomerase activity and age at diagnosis or age of menopause. Although all tumors with 100 units or more telomerase activity were cancerous, the sensitivity was 39% in cervical cancer, 41% in endometrial cancer and 21% in ovarian cancer, respectively. Cervical intraepithelial neoplasia (CIN) had already increased telomerase activity and endometrial cancer with lymph node metastases had also greater activity than that without lymph node metastases. Although telomerase activity in ovarian cancer tended to increase as stage advances, it is noteworthy that clear cell adenocarcinoma showed significantly lower telomerase activity than endometrioid adenocarcinoma.

Effect of graded coronary stenosis on regional myocardial blood flow and left ventricular function.

Dogs with 70% stenosis of the left circumflex coronary artery showed a reduction of myocardial blood flow with a minimal impairment of left ventricular wall motion. Eighty to ninety percent stenosis produced a marked reduction of flow and wall motion, and ST elevation close to 100% occlusion, but delayed antegrade run-off was found. The effectiveness of collaterals to protect against left ventricular dysfunction was supported.

Experience of two orbital MALT lymphomas treated with radiotherapy, and review of the literature.

Orbital MALT lymphoma is a relatively rare malignant disease, for which radiation therapy is effective. However, the optimal dose has not been determined. We experienced two cases of orbital MALT lymphoma. One patient was successfully treated, but the other developed a severe complication. We conclude that for safe treatment of orbital MALT lymphoma, a dose in excess of 40 Gy must not be delivered.

Case report of malignant splenic tumor.

Primary angiosarcoma and malignant lymphoma of the spleen are uncommon. We report such tumors in a 44-year-old-woman and a 51-year-old-man. The value of diagnostic imaging by US and CT is emphasized. These techniques can establish the solid nature of the mass and may be the first methods to suggest malignancy.

Quantitative determination of amines in wine by liquid chromatography.

A liquid chromatographic (LC) procedure is described for the determination by dansylation of the following 16 kinds of biogenic amines found in wine: monomethylamine (MM), ethylamine (EM), iso- and n-propylamine (Pr), iso- and n-butylamine (Bu), iso- and n-amylamine (Am), pyrrolidine (PY), 2-phenethylamine (PH), tryptamine (TR), putrescine (PU), cadaverine (CA), histamine (HI), tyramine (TY), and spermidine (SP). The amines in white and red wine were applied to a column of Amberlite CG-50 type I resin (Na-form) after the column had been washed with water and eluted with 1N hydrochloric acid. This eluate was evaporated to dryness under reduced pressure and derivatized with dansyl chloride (DNS). LC separations were performed on Finepak SIL C18S and LiChrosorb RP-8 columns with an acetonitrile-water elution gradient. In the survey of commercial wines by this method, most of the samples were found to contain 12 amines, including iso-Am, CA, PU, TY, and others. The highest levels of these amines were 4.84 micrograms PU/mL in red wine, and 5.11 micrograms iso-Am/mL in white wine. The total levels of amines in red wine were comparatively higher than in white wine.

Synergistic antitumor effect of interleukin-2 and irradiation on pancreatic cancer in Syrian golden hamsters.

The cooperative effect of interleukin-2 (IL-2) and radiotherapy was studied in pancreatic ductal adenocarcinoma implanted subcutaneously (s.c.) into Syrian golden hamsters. No cooperative antitumor effect was observed in animals given intraperitoneal injections of IL-2 (5 micrograms/hamster) from the 8th day after irradiation. Tumor regrowth was slightly inhibited in animals treated from the 15th and 24th day after irradiation, but the inhibition was not significant. In contrast, when IL-2 was injected intratumorally from the 18th day after irradiation, tumor regrowth was significantly inhibited. It was concluded that the antitumor effect of irradiation was enhanced by IL-2, especially by intratumoral injection after the restoration of immunological competence.

Superior mesenteric artery aneurysm secondary to infectious endocarditis.

A case of superior mesenteric artery aneurysm secondary to infectious endocarditis is presented. Although selective angiography was necessary to detect the shape and precise location of the aneurysm, non-invasive diagnostic techniques including ultrasonography and computed tomography were also useful in detecting the aneurysm and planning the management of this unusual condition. Mitral valve replacement and aneurysmorrhaphy with bypass grafting of the superior mesenteric artery were successfully performed.

Thrombus formation and structure and the evolution of mass effect in intracranial aneurysms treated by balloon embolization: emphasis on MR findings.

This study was designed to further assess the capabilities of MR as a tool for the diagnostic evaluation of patients with giant intracranial aneurysms, to determine MR's ability to define the degree of thrombosis present within giant aneurysms before and after treatment with balloon occlusion, and to delineate the MR characteristics of both spontaneous and induced thrombus within giant aneurysms. Nine patients with unclippable intracranial aneurysms treated by parent artery occlusion with detachable balloons were evaluated with MR, angiography, and CT. Pretreatment and posttreatment MR studies were evaluated for their ability to (1) define the size, configuration, and anatomic relationships of an aneurysm; (2) detect and characterize thrombus within an aneurysm; and (3) determine if treatment successfully caused complete aneurysm thrombosis. MR imaging does not replace angiography in either the pretreatment or the posttreatment evaluation of patients with giant intracranial aneurysms. Thrombus formation and dissolution is a complex, dynamic process. Active thrombus in incompletely thrombosed aneurysms differs from isolated organizing thrombus in completely thrombosed aneurysms. Induced and spontaneous thrombi differ in mechanisms of formation and in composition; their MR characteristics are also different. Reduction in mass effect is common after complete thrombosis of giant intracranial aneurysms.

Effect of local vibration on ventilatory response to hypercapnia in normal subjects.

We studied the effects of local high frequency mechanical vibration on ventilatory (VE) and occlusion pressure (P0.1) responses to CO2 rebreathing in twelve normal subjects. Three kinds of vibration procedures were employed: a) sustained vibration over the tendon of the quadriceps femoris near the knee, b) sustained vibration of the right 2nd or 3rd parasternal intercostal spaces and c) 'in-phase' chest wall vibration applied during inspiration on the right 2nd or 3rd parasternal intercostal spaces and during expiration on the right 9th or 10th intercostal spaces anterior to the midaxillary line. The slopes of VE response to hypercapnia (delta VE/delta PETCO2) were 2.05 +/- 0.26 (mean +/- SE), 2.48 +/- 0.24, 2.82 +/- 0.32 and 3.35 +/- 0.38 l.min-1/mmHg in the control state, during tendon vibration of quadriceps femoris, sustained chest wall vibration and 'in-phase' chest wall vibration, respectively. This sequential increase in slopes was significant compared to the control values. The effect of vibration on the P0.1 response to hypercapnia was similar to that of VE. We conclude that local mechanical vibration facilitates responsiveness to hypercapnia.

Simultaneous liquid chromatographic determination of residual synthetic antibacterials in cultured fish.

A liquid chromatographic method is described to determine simultaneously the following 11 synthetic antibacterial agents used in a fishery: nitrofuran derivatives furazolidone, nifurpirinol, difurazone, and furamizole; sulfa drugs sulfamerazine, sulfisozole, sulfamonomethoxine, and sulfadimethoxine; and, oxolinic, nalidixic, and piromidic acids. A Nucleosil C18 column was used with tetrahydrofuran-acetonitrile-phosphoric acid-water (29 + 1 + 0.06 + 69.94) as the mobile phase. Pretreatment of the fish meat sample with acetone extraction and alumina column cleanup gave good separation of the LC peaks without interference from any other components. Recovery of the antibacterial agents was ca 80%. The lower limit of detection of the drugs was 1-2 ng for 10 microL injection.
