Establishment of IL-5-dependent early B cell lines by long-term bone marrow cultures.

We established two different IL-5-dependent Ly1+ early B cell lines in long-term bone marrow culture system. One of them (J-87) is stromal cell (ST2) dependent and the other (T-88) is ST2 independent. Both J-87 and T-88 are B220+, Ly1+, sIgM-, Ia-, Thy1-, and IL-2R+, and respond to IL-3 and IL-5 in the presence of ST2. The T-88 can proliferate only in response to IL-5 in the absence of ST2. Southern blot analysis using JH probe revealed that configuration of IgH gene of both cell lines shows rearranged pattern. Binding assay for radiolabeled IL-5 to T-88 demonstrated that T-88 has two classes of IL-5 binding sites (low and high affinity) on the membrane. These data strongly suggest that there are IL-5-sensitive stages at both stromal cell-dependent and stromal cell-independent phases in early B cell development.

[Effective evaluation of combination chemotherapy in stage I-II of non-Hodgkin's lymphoma--a comparison of treatment with multivariate analysis].

Sixty-one patients with non-Hodgkin's lymphoma (Stage I and II) were treated solely with radiotherapy or in combination with chemotherapy. A multivariate analysis was used to identify the prognostic factors that significantly affected the treatment outcome. The variables examined included: stage of disease, according to the Ann Arbor classification, sex, age, pathological criteria, according to the LSG, the presence or absence of systemic symptoms, the sites of disease involvement, the LDH, the bulk of the disease, and the combination chemotherapy. Variables achieving a p-value in the 0 to 0.10 range with a multivariate analysis for the overall survival were the stage of disease, the sites of the disease involvement, the bulk of the disease, and the combination chemotherapy. Finally, the overall survival was affected by bulk of the disease (p less than 0.01) and the combination chemotherapy (p less than 0.01). The 5-year survival rates were 84.4% for stage I and 50.7% for stage II. The survival rates were 47.5% for radiotherapy treatment alone and 75.3% for combination chemotherapy. It thus was concluded that combination chemotherapy was effective for treatment of stages I and II of non-Hodgkin's lymphoma.

[Undescended testis cancer: report of two cases--review of the literature of 179 cases in Japan].

Recently, we experienced two cases of testicular tumor in patients with cryptorchidism. Case 1: A 35-year-old man was admitted because of right inguinal mass. He had a tumor of right undescended testis and underwent right orchiectomy. Case 2: A 45-year-old man, who was pointed out to have bilateral cryptorchidism was admitted because of right groin mass. He underwent right orchiectomy and left orchiopexy. Histological examination of two cases were seminoma. Testicular tumor in undescended testis was made with 179 reported cases from the Japanese literature.

[The mechanism of resistance and its circumvention of cisplatin-resistant human ovarian cancer cells].

In this study, we attempted to elucidate the mechanisms of cisplatin resistance and circumvent the resistance with calmodulin antagonists using a cisplatin-resistant human ovarian cancer cell line (KFr). It was assumed that the cisplatin resistance resulted from impairment of cisplatin transport systems. In particular, we considered that a reduced cisplatin transport function caused by some amino acids had an effect on the resistance mechanism. Reduced uptake of cisplatin as well as cisplatin analogues to KFr cells was observed. Four cisplatin analogues used in this study had cross-resistance to cisplatin. However, the concentration of drugs incorporated into cells was not always correlated with the degree of resistance. Therefore, it is possible that, in addition to the reduced influx function, an other mechanism is involved in the resistance. Combined treatment with cisplatin and calmodulin antagonists significantly increased the uptake of cisplatin by KFr cells and maintained the cellular level. Thus, a combination therapy of cisplatin and calmodulin antagonists seemed to be of clinical use.

[Effects of lymphokine-activated killer cells and interleukin-2 on the ascites formation and the survival time of nude mice bearing human ovarian cancer cells].

Effect of intraperitoneal instillations of interleukin-2 (IL-2) and/or lymphokine-activated killer (LAK) cells on the ascites formation and the survival time was examined by using a nude mice model with malignant ascites by intraperitoneal inoculation of human ovarian cancer cells (HRA) derived from ascites of a patient with serous cystadenocarcinoma of the ovary. On 28 days after tumor inoculation, all nude mice in both untreated and spleen cells only treated groups formed ascites. Two of 10 nude mice treated with IL-2 only after tumor inoculation survived without forming ascites during the experimental period. On the other hand, all nude mice treated with LAK cells only formed ascites by 14 days after tumor inoculation. When LAK cells and IL-2 were combined 5 of 10 mice survived without forming ascites during the experimental period. The survival time of the IL-2 only treated group was significantly prolonged, compared to that of medium only, spleen cells only and LAK cells only treated groups. When administration of LAK cells was followed by IL-2, the survival time was further prolonged.

[A case of metastatic tumor of the spermatic cord from gastric carcinoma].

A case of metastatic tumor of right spermatic cord in a 54-year-old man is reported. He visited our department with the complaint of a painless mass in right inguinal lesion. The initial diagnosis was a right spermatic cord tumor, and then orchiectomy was performed. Since pathological findings showed a poorly differentiated adenocarcinoma, further examinations were done, postoperatively. An upper gastrointestinal series revealed the presence of gastric carcinoma. Thus, these results suggested that the spermatic cord tumor was a metastatic lesion from the gastric carcinoma. Forty eight cases of metastatic tumor of the spermatic cord have been reported in our country. In 26 of these cases, gastric carcinoma metastasized to the spermatic cord.

Renal adenoma. Clinicopathological and histochemical studies.

We report the results of clinicopathological and histochemical studies on 64 renal adenomas found in 22 patients. We selected typical renal adenomas for these studies, discarding borderline lesions. The ages of the patients ranged from 42 to 84 years, with an average of 61 years. Male to female sex ratio was 6.3:1. The background conditions included renal cell carcinoma, long-term dialysis, chronic glomerulonephritis, pyelonephritis, hydronephrosis and polycystic kidney. The sizes of the tumors ranged from 0.1 to 2.3 mm in maximum diameter, with an average of 0.9 mm. All the tumors were located in the cortex, especially in the superficial one-third. The cytoplasm of the tumor cells was predominantly eosinophilic, and the tumor structure was predominantly papillary. Histochemical study of 19 adenomas from 13 patients demonstrated that all of the adenomas from 13 patients demonstrated that all of the adenomas were positive for EMA, and for at least one marker of the distal tubulus, i.e., DBA, PNA, SBA and PKK1, and that 9 adenomas were positive for at least one marker of the proximal tubulus, i.e., LM1 and LTA. Renal adenomas had predominant histochemical features of the distal tubulus, suggesting differentiation to a distal tubulus-like histology.

[New methods for measurement of peripheral airway resistance].

We have developed two new methods to evaluate the peripheral resistance of the lung. The first is a method for continuous measurement of the frequency dependence of dynamic compliance (Cdyn, f.d.) and pulmonary resistance (RL) from a single-breath cycle by Fourier-series analysis. In this method, we have found that procaterol my first dilate the central airway and then gradually dilate the peripheral airway in bronchial asthma. The second is a method for the direct measurement of peripheral resistance by the anterograde catheter system using a catheter tip micromanometer. The catheter tip was inserted into the 3 mm diameter small airway anterogradely and intrabronchial pressure was measured simultaneously with transpulmonary pressure and mouth flow. We have found that the values of peripheral resistance in patients with COPD are four to seven times higher than that of normal subjects. These results suggest that the behavior of the peripheral and central airway may differ between patients and that the peripheral airway may be important to examine the pathogenesis of COPD.

[A case of urachal cyst prolapsed from external orifice. Review of the literature of 172 cases in Japan].

A case of urachal cyst prolapsed from external orifice in a 15-year-old female is reported. She was admitted to the hospital with complaints of macrohematuria and palpation of a thumb's head size tumor prolapsed from external urethral orifice. An urachal cyst was suspected from the cystogram, CT and the cystoscopy. We extirpated the tumor with the bladder wall. Pathological examination demonstrated a urachal cyst.

[Transplantation of embryonic cerebella into adult rat cerebella].

Cerebellar primordia of rat embryos (embryonic days of 14-20) were transplanted into mature cerebella of normal adult rats (Fischer 344). After 1-12 months, animals were sacrificed, and the development of grafts and the interaction between host and graft were examined by light and electron microscopy. Grafts from E14 rat cerebella showed the highest survival rate (85%), which decreased with increasing donor age. In some experiments, the donor tissue developed and differentiated to form folia with the trilaminar organization of the cerebellar cortex. Synaptic connections between neuronal elements in the graft showed basically the normal pattern. Thus, mossy terminals formed synaptic contacts with dendrites of granule cells, and axons of basket cells made synaptic contacts with somata of Purkinje cells. Many spines of Purkinje dendrites were contacted with parallel fibers, while others were surrounded by processes of astroglia. Migration of donor Purkinje and granule cells in the host molecular layer was frequently observed. Some donor granule cells, implanted in the superficial aspect of the host molecular layer, migrated down along processes of Bergmann glia, which resembled the migration pattern of the normal development. Other granule cells attached to neuroglial processes which ensheathed capillary walls. While the migration of granule cells was found to be glial-dependent, the glial role in the migration of Purkinje cells was not clear. Purkinje cells probably migrated into the host molecular layer under favorable conditions where the loss of host Purkinje cells had been occurred to some extent and consequently followed by the increase of free axon endings of parallel fibers and interneurons of the host.(ABSTRACT TRUNCATED AT 250 WORDS)

[Experience in treating esophageal carcinoma with multiple fractions per day (MFD)].

We have experienced treating 29 patients of esophageal carcinoma with multiple fractions per day (MFD) examined the effectiveness of this method. A dose of 2.0 Gy was delivered to the conventional field and 1.1, 1.15, or 1.2 Gy to the gross disease at 4- to 6- hour interval for 20 days, 4 days per week. The total tumor doses were 62, 63 or 64 Gy/40 f/5 w. The following results were obtained: 1) CR rates were 44.8% on barium examination and 55.2% by endoscopic evaluation; 2) the two-year actuarial, loco-control, and disease-free survival were 49.7%, 75.5%, and 52.3%, respectively; and, 3) the acute complications were tolerable, though esophageal stricture was found to have increased in patients treated with 64 Gy.

[Pulmonary hemodynamics during exercise in anesthetized dogs with inspiratory loading and in patients with chronic obstructive pulmonary disease].

We studied 1) the effects of inspiratory elastic loading on pulmonary hemodynamics and pulmonary gas exchange with and without electrically induced hindlimb exercise in 8 anesthetized dogs, 2) pulmonary hemodynamics at rest and during single stage exercise on a supine cycle ergometer in 6 patients with chronic obstructive pulmonary disease (COPD) and 3) changes in P wave height in lead II of electrocardiogram during progressive exercise on a treadmill in 9 COPD patients. The pulmonary arterial pressure-flow relationship from resting to exercise was not affected by inspiratory elastic loading, but elastic loading accompanied by exercise increased alveolar-arterial O2 tension difference (AaDO2). The pulmonary arterial pressure-flow relationship in COPD from rest to exercise showed higher pulmonary vascular resistance than that in control group. Moreover, we found a greater increase in P wave height during exercise in COPD with decrease in PaO2 during exercise than in COPD without decrease in PaO2 during exercise. This increase in P wave height during exercise was inhibited by oxygen inhalation. We speculated that 1) increased AaDO2 during exercise with elastic loading may be due to increased shunt effects with low ventilation-perfusion ratio and 2) increased P wave height during exercise in COPD may be correlated with hypoxia during exercise and may be useful to detect latent cor pulmonale in COPD.

[A case of the management of developed collateral vessels through left thoracotomy of recurrence of right bronchiectasia with hemoptysis].

A 40-year-old woman, with a previous history of right S1, S2 segmentectomy and right middle lobectomy for bronchiectasis with hemoptysis at the age of 24, was admitted for frequent hemoptysis. As a result of a chest roentgenogram, a CT scan and bronchographic examination, a recurrence of bronchiectasis in the right ventral segment of the upper lobe (S3) and the right superior segment of the lower lobe (S6) was diagnosed. The bronchial arteriogram and aortogram showed four vessels going into this region and a shunt lesion was located on located on the proximal side of the pulmonary artery. If a right pneumonectomy had been done, pulmonary function would have gotten worse. If a right thoracotomy had been performed, there would have been much bleeding from the entering vessels. Therefore, a left thoracotomy was preformed and those vessels ligated. The patient is doing well one year after the operation without hemoptysis.
