[Recurrence of acute uncomplicated cystitis--criteria for the evaluation of recurrence after antimicrobial chemotherapy].

UNLABELLED: The recurrence of female acute uncomplicated cystitis was investigated clinically. The criteria for the evaluation of recurrences were proposed, as follows; PATIENTS: Target infection is acute uncomplicated cystitis (AUC) which had satisfied the specifications of AUC Criteria by the UTI Committee of Japan and showed the excellent effects of an antimicrobial agent after a definite period of administration. Treatment period: Seven days; after 3 days' administration to evaluate the drug efficacy, patients shall take an additional 4 days' treatment. Interval of follow up proposed was 7 days. Evaluation of recurrence: Parameters of criteria are pyuria and bacteriuria. Recurrence: Pyuria greater than or equal to 10 WBCs/hpf and bacteriuria greater than or equal to 10(4)/ml. Evaluation of the day of recurrence: Evaluation should be made 14 days after the start of treatment. Urine sampling: After 7 days of treatment, midstream urine is collected and in cases with positive findings, catheterized urine should then be collected. Using these criteria it will be possible to evaluate and compare the ability of various antimicrobial agents to cure acute uncomplicated cystitis.

[Pyeloureteritis cystica: report of a case].

A 68-year-old female patient was admitted for the examination of duplication of right ureter and right hydronephrosis. Antegrade pyelography showed multiple, small, round defects in the ureter from the upper pole of right kidney. Ureteroureterostomy was performed under the diagnosis of ureteritis. Including our case, 25 cases of pyeloureteritis cystica have been reported in Japan.

[A case of megaureter].

The terminology of megaureters, their histological findings and etiology vary greatly in the literature. We encountered an obstructed megaureter in a 39-year-old man. He complained of epigastral and left flank pain. X-ray examination showed a right marked hydronephroureter which compressed the bladder and left ureter. We performed right nephroureterectomy. Postoperative course was uneventful and his clinical condition improved.

[A case of pathological dilatation of the seminal duct end].

A case of azoospermia due to pathological dilatation of seminal duct end is reported. A 34-year-old man visited our clinic complaining of infertility. He was diagnosed as having azoospermia by semen analysis. Cystic dilatation of seminal duct end was improved after transurethral resection of the cystic wall. His wife became pregnant on the 27th day after his operation.

Reduction in brain immunoreactive corticotropin-releasing factor (CRF) in spontaneously hypertensive rats.

The brain CRF concentration of spontaneously hypertensive rats (SHR) and normotensive Wistar Kyoto rats (WKY) was examined by rat CRF radioimmunoassay. Anti-CRF serum was developed by immunizing rabbits with synthetic rat CRF. Synthetic rat CRF was also used as tracer and standard. The displacement of 125I-rat CRF by serially diluted extracts of male Wistar rats hypothalamus, thalamus, midbrain, pons, medulla oblongata, cerebral cortex, cerebellum and neurointermediate lobe was parallel to the displacement of synthetic rat CRF. In both WKY and SHR the highest levels of CRF immunoreactivity were shown by the hypothalamus and neuro-intermediate lobe, and considerable CRF immunoreactivity was also detected in other brain regions. The CRF immunoreactivity in the hypothalamus, neurointermediate lobe, midbrain, medulla oblongata and cerebral cortex was significantly reduced in SHR and it may suggest that CRF abnormality may be implicated in the reported abnormalities in the pituitary-adrenal axis, autonomic response and behavior of SHR.

Stability of rat (human) corticotropin-releasing factor in serum, urine and tissue incubation medium.

The stability of rat (human) CRF in serum, urine and tissue incubation medium was examined using Sephadex gel filtration and CRF radioimmunoassay with anti-rat (human) CRF serum. Human serum after incubation with rat (human) CRF for 1 h at 37 degrees C showed two peaks of CRF immunoreactivity on a Sephadex G-50 fine column. Most of the immunoreactivity coeluted with the rat (human) CRF marker. When rat (human) CRF was incubated with rat liver, kidney or hypothalamus, only 3.1-14.9% of the CRF was recovered at the rat (human) CRF position on gel filtration, and two to four CRF-immunoreactive peaks appeared after the rat (human) CRF marker. When rat (human) CRF was incubated with human urine (pH 6.0) for 24 h at room temperature, one peak of CRF immunoreactivity coeluted with the rat (human) CRF marker on Sephadex gel filtration. The urine extracts of normal rats showed some small peaks of CRF-like immunoreactivity on the Sephadex column, with the main peak appearing after authentic CRF. These results suggest that rat (human) CRF is relatively stable in serum and urine, but is easily degraded by tissue enzymes, with the degraded CRF fragments being excreted in the urine.

[Clinical and statistical study on recent cases of suprapubic prostatectomy].

Results of operation were analyzed statistically in 123 patients who underwent suprapubic prostatectomy for benign prostatic hypertrophy from 1974 through 1981. Age of the patients ranged from 48 to 89 years and the mean age was 70.1 years. The average amount of blood loss during operation was 590 ml. Amount of blood loss in patients operated under epidural anesthesia was less than that in the patients operated under general anesthesia. Furthermore, significant correlation was observed between amount of blood loss and the weights of enucleated prostate glands. The average weight of enucleated prostate glands was 33.4 g and the weight increased as the patient's age increased. Histological examination of the enucleated specimens revealed occult carcinoma in 4.1% of the patients. Association of bladder neck sclerosis was observed in 27% of the patients whose prostate weighed less than 20 g. Significance of prophylactic vasectomy for the prevention of postoperative epididymitis was considered to be low. Incidence of postoperative bacteriuria was higher in patients who had bacteriuria preoperatively than those without preoperative bacteriuria. Therefore, eradication of preoperative bacteriuria seemed important in the prevention of postoperative bacteriuria. No significant correlation was observed between postoperative residual urine volume and the weight of the enucleated prostate glands.

HLA-A and B antigens in patients with Peyronie disease.

Using microdroplet lymphocyte cytotoxicity test, as a method of HLA typing, 9 patients with Peyronie disease were studied from a viewpoint of relationship between this disease and HLA-A and B antigens. The frequencies of each antigen in the patients were compared with those of healthy controls. No significant difference of distribution in HLA antigens was observed between the patients and the control group.

[Treatment of complicated urinary tract infections caused by fungi].

The predisposing factors of funguria and the clinical efficacy of treatment with ketoconazole, a new oral imidazole antifungal drug were studied in 10 patients with complicated urinary tract infection due to fungi. Preceding antibiotic therapy, presence of indwelling catheter and malignant tumor were regarded as the important factors that predisposed to the fungal urinary tract infections. Patients received 200 mg of ketoconazole once a day for 5 to 14 days. Of the 9 patients in whom clinical efficacy of ketoconazole was evaluated, excellent response was obtained in 4 patients, moderate in 2 and poor in 3 patients. Of the 9 strains of fungi isolated before treatment, 6 strains or 67% were eradicated after treatment. Neither subjective nor objective side effects were observed in any of 10 patients receiving ketoconazole, and laboratory tests showed elevated BUN and serum creatinine in 1 patient. From the results obtained in this study, ketoconazole was regarded as useful in the treatment of complicated urinary tract infection due to fungi.

[Effect of porcine calcitonin on pain caused by cancer-induced bone destruction].

The analgesic effect of porcine calcitonin (PCT) was studied in 32 patients with cancer-induced bone destruction. Twenty patients (62.5%) showed some response among them 10 of 13 patients who had suffered from uncontrollable pain despite other treatment methods such as radiotherapy, and or the administration of other analgesics. The effect was observed about 10 days after the administration of the drug. Combined therapy with PCT and other modalities such as radiotherapy, chemotherapy and hormone therapy had a marked effect in controlling the pain.

Studies on the phagocytic function of urinary leukocytes.

Urine specimens from patients with urinary tract infection (UTI) were examined to determine the rate of phagocytosis and viability of urinary leukocytes. The phagocytic function of urinary leukocytes was also studied in vitro. The mean rate of viable urinary leukocytes was 83 per cent and the phagocytic potency was confirmed by light and electron microscopic studies. In 99 per cent of 113 patients with UTI, urinary leukocytes were shown to have phagocytized bacteria. The rate of phagocytosis in chronic UTI was higher than that in acute UTI. Urinary osmotic pressure and the positive or negative of antibody coated bacteria were supposed to be factors influencing phagocytic potency of urinary leukocytes.
