[Indication and effect of bromocriptine in euprolactinemic amenorrhea--comparison with clomiphene].

Effect of bromocriptine on induction of ovulation and occurrence of pregnancy was examined in patients with normoprolactinemic ovulatory disturbances. The patients under study (53 in number) were divided into two groups, A and B, depending on the effect clomiphene had on the ovulatory responses and the occurrence of pregnancy. Of the group A, patients (21 in number), who had failed to show any ovulatory response to clomiphene, 8 patients ovulated on bromocriptine alone. Three out of 13 patients who complained of sterility became pregnant. Three out of 8 patients who had not ovulated on bromocriptine therapy alone ovulated on a combination of bromocriptine and clomiphene, and 2 out of these 3 cases became pregnant. Of the group B patients (31 in number), who had failed to become pregnant in spite of ovulatory responses to previous clomiphene therapies, 7 out of 23 became pregnant, who complained of sterility when on bromocriptine alone. Out of 12 cases who had failed to get pregnant on the bromocriptine alone, 2 patients became pregnant on the combination of bromocriptine and clomiphene. These data may indicate that the bromocriptine or the combined therapy of bromocriptine and clomiphene is useful for the treatment of patients with ovulatory disturbances or sterility who do not respond to the clomiphene therapy.

Changes in intracranial pressure (ICP) pulse wave following hydrocephalus.

The pulse pressure (PP) of the intracranial pressure pulse wave (ICPPW) was measured in experimental and clinical hydrocephalus: namely, obstructive and kaolin-induced hydrocephalus as an experimental study, and noncommunicating and communicating hydrocephalus as a clinical study. At approximately the same ICP level, the PP was much higher in the obstructive hydrocephalus group and slightly higher in the kaolin-induced hydrocephalus group than in the controls. Also the PP level in noncommunicating hydrocephalus patients [3.19 mm Hg (SD: 1.04)] was significantly (P less than 0.05) higher than that in the communicating hydrocephalus patients [1.88 mm Hg (SD: 0.60)]. The results of our research suggest that the stronger the disturbance of the communication of the CSF between cranial cavity and spinal cavity the higher the PP of ICPPW. This results from the distensibility of the spinal dural sac and exerts a great influence on the elasticity and the PP of the cranial system. It is reasonable, therefore, to suggest that a marked increase in the ratio of the PP to the ICP in the normal or slightly high ICP hydrocephalus may indicate disturbance of the CSF flow, especially loss of CSF communication between the cranial and spinal compartments.

[A case of bilateral metastatic breast carcinoma from gastric carcinoma].

The patient was a 48-year-old woman who had undergone radical surgery for gastric carcinoma in 1981. Seventeen months after the operation, recurrence of gastric carcinoma at the anastomotic stoma was found, and total gastrectomy was performed. Fourteen months after the second operation, she was revealed to have bilateral breast tumors and underwent exploratory excision. Histological examination suggested primary malignant tumor of the breast. Bilateral standard radical mastectomy was done seven days after the excision. Final histological examination revealed metastatic breast cancer originating from the stomach. She died from general metastasis seven months after the radical mastectomy in spite of adjuvant immunochemotherapy.

[RI angiography with 99mTc and 201T1-chloride scintigraphy in determining the effectiveness of intra-arterial infusion in chemotherapy of locally advanced and recurrent breast cancer].

Eleven patients with local advanced and recurrent breast cancer were treated preoperatively with intraarterial infusion of chemotherapeutic agents. After the treatment, nine patients underwent radical mastectomy. RI angiography with 99mTc is a useful procedure for determining the correct position of the tips of an inserted cannula. The results of 201Tl-chloride scintigraphy have been found to be closely consistent with the results of the resected specimen histologically. Therefore, 201Tl-chloride scintigraphy is also a quite useful procedure for determining the effects of intraarterial infusion in cancer chemotherapy.

[Medical images; the clinical significance of the medical images of thyroid cancer].

Supplementary diagnosing methods for thyroid cancer were discussed, with particular reference to medical images. Although palpation is, of course, essential in the examination of nodules, we have also used soft X-rays and I-123 scintigraphy, as well as Tl-201 scintigraphy, ultrasonography and aspiration biopsy. In addition, various diagnosing methods have been employed to determine the degree of invasion in advanced cancer patients, such as CT scan for observing invasion of the surrounding tissues, naso-tracheoscopy for invasion of the trachea and a flow meter for vascular tracts. When the tumor size is under 2 cm, we use soft X-rays, ultrasonography and sometimes try additional aspiration biopsy. When the tumor is smaller, aspiration biopsy is employed with ultrasonography. For tumors larger than 2 cm, we use soft-X-rays, I-123 scintigraphy and ultrasonography. Tl-201 scintigraphy is employed to determine operative indication, following which aspiration biopsy and CT scan are performed. CT scan, tracheofiberscopy and doppler flow studies are carried out for advanced cancer cases to determine the degree of invasion of carcinomas.

[Advanced thyroid carcinoma--possible induction of the host's antitumor immunological response by 131I therapy].

UNLABELLED: The patient was a 76-year-old man. When he was 31, he had undergone right lobectomy for thyroid cancer. Following this, had repeated recurrences, and finally reached total-thyroidectomized status. Consequently, he was given thyroxin and PS-K thereafter. When he was 75, 131I therapy was undertaken for lung metastases. The changes of serum anti-microsomal antibody (AMA), immunosuppressive substance (ISS) and thyroglobulin (TG) were as follows: three months after the therapy, AMA was 40(2), although it had always been negative before. Nine months later, ISS and TG markedly decreased. CONCLUSION: Under the administration of immunomodulating agents, 131I therapy might induce antitumor immunological responses by the effect of enhancement of cancer cell antigenicity.

[Systems analysis of pressure transmission in intracranial cavity].

Systems analysis of intracranial pressure pulse waveform was carried out in 6 dogs during normal condition (normocapnia and intracranial normotension) in order to explore how to transmit the pressure pulse through the intracranial cavity. Pulses originated in the heart travel in the intracranial cavity through the vessels, transmit into the cerebral tissue and finally reach to the dural surface. It can be speculated that the epidural pressure pulse waveform might be influenced by the property of intracranial components. The intracranial cavity which consists of brain parenchyma, vessels, cerebrospinal fluid, blood, pia, arachnoid, dura and skull is considered as a kind of system. The systems analysis method was applied to it. The common carotid arterial pressure pulse wave was used as the input signal of system, and the epidural pressure pulse wave was used as the output. The intracranial pressure was measured by an epidural pressure transducer attached to the dural surface through a parietal burr hole. The common carotid arterial pressure pulse wave and the epidural pressure pulse wave were recorded on FM analog tape. The characteristics of transmission are evaluated by the exact transfer function of system. We consider that the exact transfer function of system is obtained by the randomized input signal caused by the cardiac pacemaker or the aorta balloon. The power spectrum of input signal in this method didn't have a flat profile like completely random wave (or white noise) but a gentle slope between 1 Hz and 20 Hz. Therefore, the result in our method may be reliable only on the waves between 1 Hz and 20 Hz.(ABSTRACT TRUNCATED AT 250 WORDS)

Ovarian steroidogenesis in Japanese patients with polycystic ovary syndrome.

Gonadotropin and steroid hormone levels in both peripheral and ovarian venous blood were measured in samples obtained from 20 Japanese patients with polycystic ovary syndrome (PCOs) and 10 normal women in early follicular phase (normal women) by radioimmunoassay. The change in the amount of steroid hormone following intravenous human menopausal gonadotropin (HMG) or dexamethasone administration was investigated. The mean concentration in patients with PCOs was significantly higher than the concentrations found in normal women for LH (p less than 0.001), but not for FSH in peripheral blood. Significantly elevated ovarian venous steroid hormone levels in PCOs were found for 17 alpha-hydroxypregnenolone (p less than 0.05), progesterone (p less than 0.05), 17 alpha-hydroxyprogesterone (p less than 0.01), 4 delta-androstenedione (p less 0.01), testosterone (p less than 0.01), estrone (p less than 0.01) and estradiol (p less than 0.05), but not for dehydroepiandrosterone-sulfate (DHEAS). The ovarian dehydroepiandrosterone (DHEA) level was slightly elevated in PCOs. The concentration of ovarian 4 delta-androstenedione in PCOs reached twelve times as much as that in normal women. After the administration of HMG, all of the ovarian venus steroid hormone levels were elevated slightly and without significance in the short observation time for 10 min. The DHEAS level was suppressed while the ovarian DHEA level remained high in PCOs following dexamethasone administration. These findings seem to indicate there is no adrenal involvement and no adrenal-like component in the ovary of PCOs, and no evidence of 3 beta-hydroxysteroid dehydrogenase and/or aromatase deficiency in this study. The increase in the steroid hormone secretion in PCOs is explained by the increase in ovarian production in polycystic enlarged ovaries.

Lack of response of peripheral blood mononuclear cells to thyroid microsomal antigen in nontoxic nodular goiters.

The production of interferon gamma (IFN gamma) by peripheral blood mononuclear cells (PBMC) from patients with nontoxic nodular goiter (NTNG), autoimmune thyroid disease (AITD), and normal persons (NL) has been studied in vitro, with the express object of comparing NTNG and AITD. PBMC were cultured with thyroid microsomal antigen (TMc), thyroglobulin (Tg), and liver microsomal antigen (LMc) at concentrations of 0, 1, 10, 100, and 1000 ng/ml for two days. IFN gamma was measured by a spot-ELISA technique which determines the number of cells secreting IFN gamma. There were no significant differences in the basal numbers of IFN gamma secreting cells between NL, AITD, and NTNG PBMC in this study. In AITD PBMC, TMc antigen caused a significant increase in the number of IFN gamma secreting cells, compared with the numbers in unstimulated AITD PBMC (percent IFN gamma secreting cells), whereas Tg and LMc antigen caused a significant decrease in all groups. The maximal percentage change in IFN gamma secreting cells (i.e., compared to unstimulated PBMC) did not increase significantly in NTNG in response to TMc antigen, in contrast to the AITD PBMC. Based on these findings, it is concluded that NTNG PBMC are not sensitized to the thyroid antigens tested; this is unlike the findings from AITD PBMC.
