A malignant pheochromocytoma with ileus, polyuria and hypercalcemia: a case of recurrence 17 years after the initial operation.

A case of malignant pheochromocytoma, with a recurrence 17 years after the initial diagnosis of benign pheochromocytoma, was presented. The autopsy revealed multiple metastases of pheochromocytoma to the bone marrow of the thoracic and lumbar vertebrae. Of particular note is the fact that the patient was associated with paralytic ileus, polyuria and hypercalcemia and that he died of hypercalcemia crisis. Cases like this appear to be very rare.

Cushing's syndrome presenting the coexistence of a pituitary corticotrophic cell hyperplasia and a unilateral functional adrenal adenoma.

A very unusual case of Cushing's syndrome is presented. Most of the preoperative biochemical and radiological examinations were compatible with Cushing's syndrome owing to a right adrenal adenoma. Exceptional findings include normal concentrations of adrenocorticotrophin (ACTH) in plasma as well as a disturbance of its circadian rhythmicity and a significant adrenocortical responsiveness to exogenous ACTH. Secretory patterns of ACTH did not change even after right adrenalectomy. Studies in vitro revealed that the adenoma itself, but not the surrounding normal adrenal, was the source of cortisol secreted in response to ACTH. Post mortem examinations disclosed unexpectedly a hormonally inactive left adrenal adenoma and a focal hyperplastic lesion of the anterior pituitary with an ACTH concentration 53 times higher than that of the remaining tissue of the gland. It is a possibility that this case may have represented a transition between pituitary-dependent adrenocortical hyperplasia and adrenal adenoma to this date reported in only one similar case.

Role of multiple cerebral microthrombosis in symptomatic cerebral vasospasm: with a case report.

In our studies of the pathogenesis of cerebral vasospasm, we have been emphasizing that microthrombosis may play an important role in the induction of cerebral ischemic symptoms or cerebral infarction. In this report, multiple microthrombi are demonstrated histologically to be a major cause of cerebral infarction in the autopsied brain of a 63-year-old woman who died from typical cerebral vasospasm that occurred after the rupture of an anterior communicating artery aneurysm. We discuss the significance of thrombus formation and subarachnoid perivascular acidosis in vasospasm.

[An experimental production of suppurative otitis media in dog, and a trial to evaluate the therapeutic effect of cefmetazole on this otitis media].

An experimental suppurative otitis media in dog was produced to evaluate the therapeutic effects of cefmetazole (CMZ) in this study. In order to induce a definite case of otitis media, 1.0 mg of dexamethasone was administered intramuscularly to dog, 12 hours before a bacterial inoculation into the middle ear of it. For inoculation, 5 x 10(8) live bacteria of S. aureus, strain 571 (UOP) was prepared from its 18 hour-bouillon culture and added 0.9 ml of 5% mucin solution for a total inoculation fluid of 1.0 ml. This fluid was injected into the middle ear on one side by inserting a 'mouse syringe' with a swollen tip down the external auditory meatus to pierce the tympanic membrane. On the opposite side, only 1.0 ml of micin solution was injected. In this way, suppurative otitis media which definitely lasted until the 15th day following the bacterial inoculation was realized. Since in each positive controls, the onset of suppurative otitis media had been confirmed on the 4th day after inoculation, the intravenous administration of CMZ (three times a day; 9 : 30, 15 : 30, 21 : 30) was started at the end of the 4th day and continued through the 7th day. The dogs thus treated were sacrificed on the 10th day after inoculation and the live bacteria count within the middle ear, signs of inflammation on the tympanic cavity, and accumulation of pus were examined. The therapeutic effect of CMZ was rated as 'effective' in cases where S. aureus-571 was found to be negative. All the animals administered a daily dose of 30 mg/kg or above it, were found to be S. aureus-571 free, thus the satisfactory therapeutic effect of CMZ was demonstrated. Especially, at the daily dose of 120 mg/kg the membrane of the tympanic cavity in all cases exhibited a picture of a radical cure to almost the same degree as the control side membranes, and any signs of inflammation were hardly observed. The last mentioned phenomena had been observed too by the authors on the treatment of the experimentally induced pyometra in bitches, with CMZ. Thus it is assumed that in treating some suppurative diseases with CMZ, it may be advisable to initiate the administration at relatively large doses in order to achieve generally better and quick effect.

Study on the ultrastructure and acetylcholinesterase activity in von Recklinghausen's neurofibromatosis.

The cutaneous nodules obtained from seven patients with von Recklinghausen's neurofibromatosis were investigated by electron microscopy, and ultrastructural localization of acetylcholinesterase activity was demonstrated in the nerve fibers of this tumor for the first time using Karnovsky's thiocholine method. The enzymatic activity was mainly found in unmyelinated fibers, exactly associated with their axonal membranes, the interspace between the apposing axonal and Schwann cell membrane, and some different mesaxons, which indicated their cholinergic nature. Almost all myelinated fibers and some unmyelinated fibers did not possess the activity. The relationship between axon and Schwann cell was quite similar to that of normal peripheral nervous system, but two striking alterations of the nerves existed: One is the dissociation of unmyelinated fibers, and the other is the degenerative changes of the axon and the myelin sheath. As the evidence of schwannian proliferation, onion bulb formations and collagen pockets were observed. Some signs of fibroblastic proliferation were also found. From the present study and the review of the literature, the probable histogenesis of this disease was discussed.

Light and electron microscopic studies of the bone marrow and blood cells in chronic panmyelosis including polycythemia vera and primary thrombocythemia.

Both bone marrow and peripheral blood was investigated light and electron microscopically in 3 cases with polycythemia vera, 2 cases with primary thrombocythemia and 1 case with panmyelosis. In 5 cases the peripheral blood showed persistent increase in cells of two or three hematopoietic systems. Giant thrombocytes in the peripheral blood were seen in 3 cases. Erythroblasts, granulocytic young forms, and megakaryocytes were often observed in the blood. Histologic bone marrow examination showed prominent proliferation of all 3 hematopoietic cells in every case. Cytological and electron microscopical examinations of the bone marrow revealed many mitotic figures, morphological abnormalities, and unbalanced nucleocytoplasmic maturation in various hematopoietic cells. These findings suggested that the proliferation of all 3 hematopoietic cells in the bone marrow was not simply reactive in nature, but an idiopathic progressive process. It is considered that these disorders and primary myelosclerosis represent no separate entities and must be unified as "chronic panmyelosis".

Heat of sorption induced by sweating affects thermoregulatory responses during heat load.

To investigate the effects of heat of sorption per se induced by sweating on thermoregulatory responses in clothed subjects, two experiments were carried out. In experiment 1 [ambient temperature (Ta) 27.2 degrees C, 50% relative humidity (r.h.)], seven female subjects immersed their lower-legs in a water bath at a temperature raised between 35-41 degrees C during 70 min wearing garments made for the experiment of either 100% cotton (C) or 100% polyester (P). Skin blood flow at the forearm was significantly greater in C than in P (P < 0.05). The increase of mean skin temperature (Tsk) and clothing surface and clothing microclimate temperatures were significantly higher in C than that of P after the onset of sweating (P < 0.05). Furthermore, these responses were accompanied by warmer and more uncomfortable sensations in C than in P despite a lower rectal temperature in C compared with P and identical mean body temperature in both sets of garments. In experiment 2, to simulate the clothing microclimate after the onset of sweating, C and P garments were exposed in the climate chamber to r.h. raised from 50% to 95% at a constant Ta of 27.2 degrees C. The clothing surface temperature rose by 2.2 degrees C for C and by 0.5 degree C for P after the increase of r.h. These results clearly showed a more marked increase in heat of sorption in C than in P. These results indicated that the heat of sorption per se after the onset of an increasing clothing microclimate vapour pressure, mimicking the onset of sweating, enhanced thermoregulatory responses such as skin blood flow, (Tsk) and subjective voting in C.

Significance of p27 as a predicting marker for medroxyprogesterone acetate therapy against endometrial endometrioid adenocarcinoma.

We reported that p27 induced by medroxyprogesterone acetate (MPA) may be involved in the progestin-induced growth suppression of human endometrial adenocarcinoma cells. This study aimed at investigating whether p27 expression could be a predicting marker to evaluate the effectiveness of MPA therapy. The clinical responses of 15 patients with endometrial carcinoma treated with MPA were examined. p27 expression was evaluated by immunohistochemical staining. Percentage of positive nuclear staining was expressed as a strongly positive (SP) labeling index (LI). Before MPA treatment, SP LIs in the effective and noneffective groups were 22.6 +/- 14.3% and 9.1 +/- 9.2%. At 1-6 weeks in the MPA treatment, SP LIs increased in both groups and were significantly higher than those before the therapy. At 7-12 weeks, SP LIs in both groups decreased to the level of pretherapy. At 13-18 weeks, SP LIs in the effective group were 14.9 +/- 5.7%, whereas in the noneffective group, 1.1 +/- 2.0%. The former was significantly higher than the latter. p27 expression could predict the effectiveness of MPA treatment for endometrial carcinoma at an early stage of the 4-month period in MPA therapy and could be a useful predicting marker for MPA.

Clear cell adenocarcinoma of the endometrium is a biologically distinct entity from endometrioid adenocarcinoma.

Clear cell adenocarcinoma (CCA) of the endometrium has a poor prognosis, although the biologic features of this rare tumor are not clear. In this study, we analyzed the expression of biologic markers relating to carcinogenesis, tumor growth, and progression. Thirteen cases of CCA were compared with cases of endometrioid adenocarcinoma (EMA) of the endometrium. Immunohistochemical staining for p53; Ki-67; cyclins A, D1, and E; E-cadherin; progesterone receptor (PR)-A and PR-B; P-glycoprotein; MLH1; and MSH2 was performed. Labeling indices of p53, Ki-67, and cyclins A, D1, and E in CCA were 46.4 +/- 24.3%, 52.1 +/- 20.5%, 37.9 +/- 21.4%, 12.3 +/- 27.9%, and 8.2 +/- 22.9%, respectively. E-cadherin was expressed in only 1 case (7.7%) of CCA, as compared to 39 cases (61.0%) of EMA. No CCAs were positive for PR-A and PR-B. P-glycoprotein was detected in seven cases (53.8%). Loss of either MLH1 or MSH2 expression occurred in eight cases (61.5%). High-level expression of p53, cyclin A, and P-glycoprotein, and low-level or no expression of cyclin E, E-cadherin, PR-A, and PR-B was observed in CCA compared with EMA. The mechanism of cell-cycle regulation in endometrial CCA is different from that in EMA and may influence its malignant potential. Endometrial CCA is a distinct entity from EMA.
