Acceleration of barrier ontogenesis in vitro through air exposure.

Immaturity of the epidermal barrier in the preterm infant may have serious clinical consequences. However, regardless of the degree of prematurity, the barrier rapidly matures such that by 2 wk all infants display a competent barrier. To determine whether the change from an aqueous (intrauterine) to a xeric environment might be the stimulus for this accelerated maturation, we examined the effects of air exposure on cutaneous barrier formation in vitro. Skin explants from d 17 fetal rats were incubated either submerged or at the air-medium interface. As previously reported, a competent barrier formed under submerged conditions after 3-4 d, precisely mirroring the time course of maturation in utero. In contrast, barrier maturation was accelerated in air-exposed explants, with functional, histologic, and structural markers of barrier formation observed after only 2 d of incubation. A water-impermeable membrane blocked the acceleration of barrier formation, resulting in a developmental time course comparable to that for submerged explants. In contrast a water vapor-permeable membrane did not block the acceleration. Glucocorticoids and thyroid hormone, which accelerate barrier formation in utero or in vitro under submerged conditions, did not further accelerate barrier formation in the air-exposed model. These data indicate that: 1) air exposure accelerates barrier ontogenesis, suggesting that water flux may be an important signal for the accelerated barrier formation that occurs in premature infants; and 2) factors which accelerate barrier formation in utero may not further accelerate barrier formation in neonates.

Hypothyroidism delays fetal stratum corneum development in mice.

The epidermal permeability barrier, required for terrestrial life, is localized to lipid-enriched lamellar membranes in the extracellular spaces of the stratum corneum (SC). Immaturity of the SC is a significant contributor to morbidity and mortality in premature infants. Previous studies have shown that supraphysiologic concentrations of thyroid hormone accelerate epidermis/SC ontogenesis. Here we studied SC development in Hyt/Hyt mice who are genetically hypothyroid due to a mutation in the TSH receptor. In control mice on d 18 of gestation (term 19.5 d), only focal areas displayed a mature SC membrane pattern. By 19 d of gestation there was a mature multilayered SC with lamellar unit structures filling the extracellular spaces similar to that seen in mature mice. In Hyt/Hyt mice SC development was delayed at both 18 and 19 d of gestation. In both strains of mice, within the first day after birth there were no differences in epidermal or SC appearance, and the SC was fully mature. These findings indicate that thyroid hormone plays a physiologic role during normal intrauterine development of the SC. However, normal SC maturation ultimately occurs, indicating that thyroid hormone is not absolutely essential. Previous studies have shown that glucocorticoids accelerate SC development in euthyroid rats, and in the present study we demonstrate that glucocorticoids also accelerate SC ontogenesis in euthyroid mice. In contrast, in Hyt/Hyt mice glucocorticoids did not accelerate or normalize SC development, indicating that the glucocorticoid effect on SC maturation requires a euthyroid state or that glucocorticoids act via thyroid hormone. These studies demonstrate that thyroid hormone status is an important regulator of fetal SC development.

Long-term changes in bone mineral and biomechanical properties of vertebrae and femur in aging, dietary calcium restricted, and/or estrogen-deprived/-replaced rats.

To study the long-term effect of aging, low calcium diet (LCD) and/or ovariectomy (OVX), and estrogen replacement therapy (+E) on rat bone quality of both trabecular and cortical bone, 150 female Wistar rats of 4.5 months were divided into baseline, sham-operation (sham), sham + LCD, OVX, OVX + E, OVX + LCD, OVX + LCD + E, and were observed for 3, 6, and 9 months postsurgery. The bone mineral density (BMD) of the lumbar spine L1-L4, the femoral neck, the midshaft, and the distal metaphysis were determined using dual-energy X-ray absorptiometry (DXA) in vitro. Biomechanical tests of the L1 vertebral body and the left femur were performed. The right femoral midshaft and neck were processed undecalcified for determining cross-sectional moments of inertia (CSMIs). BMD in all groups increased rapidly with aging in the femoral midshaft composed only of cortical bone at 3 months post-OVX and stabilized or decreased thereafter, but decreased at all observation periods in the distal femoral metaphysis, consisting mostly of trabecular bone. L1 maximum compressive strength and stiffness increased as a function of aging in sham and sham + LCD but not in OVX and OVX + LCD. The order of loss in BMD at all sites and in L1 strength and stiffness was: OVX + LCD > OVX > LCD. LCD reduced while OVX improved the total femoral area, CSMIs in the femoral midshaft, and the torsional strength. Estrogen treatment preserved BMD and prevented OVX-induced loss in L1 strength. The BMD and biomechanical properties were greater in OVX + E than in OVX + LCD + E. Loss in BMD and CSMIs was greater in the femoral neck than in the midshaft. The data suggest that rat cortical bone might not be matured until 7.5 months of age. It would be more appropriate to consider rats at peak bone mass as a model of mature rat and to perform OVX at that time. LCD and OVX have a great potential for weakening the bone quality of cortical bone and trabecular bone, respectively, and have an additive effect when combined. Estrogen prevents only OVX-induced bone loss.

[The best timing of silicone oil removal].

OBJECTIVE: The study was designed to approach the best timing of silicone oil removal after its injection into the vitreous cavity in vitrectomy for reattachment of retinal detachment. METHODS: The silicone removal was performed on 100 eyes of 96 patients, and the incidence of silicone complications was analyzed statistically. RESULTS: The complication incidence increased with the time of silicone oil removal delayed. There are statistically significant differences in the complication incidence not only between the group with silicone oil removal within 3 months and the group with the removal later than 5 months, but also between the group with the removal within 5 months and the group with the removal later than 5 months. The complication incidence was 100% in the group in which the silicone oil was removed later than 18 months after its injection. CONCLUSIONS: The silicone oil should be removed no longer than 6 months after its injection, and the best timing to remove the oil is 2 to 3 months.

[A short-term clinical study on a new aqueous humor drainage implant for refractory glaucoma].

OBJECTIVE: To investigate the therapeutic effect of a new aqueous humor drainage implant made in China for treatment of refractory glaucoma. METHODS: The implant was designed by ourselves and manufactured by Beijing Rubber Products Design & Research Institute. From July 1993 to September 1995, we observed 40 cases (41 eyes) with refractory glaucoma treated with the implantation surgery, including neovascular glaucoma (NVG) 16 eyes, non-neovascular glaucoma (NNVG) 25 eyes. RESULTS: The mean period of postoperative follow-up was 9.2 (range 3-27 months) months, the IOP was lowered from preoperative 7.20 +/- 1.51 kPa to postoperative 2.40 +/- 1.33 kPa in NVG group and from preoperative 6.27 +/- 1.07 kPa to postoperative 2.00 +/- 2.93 kPa in NNVG group. The success rate was 81% in NVG group and 84% in NNVG group. The common postoperative complications were flat anterior chamber, hyphema, choroidal detachment and so on. CONCLUSIONS: Our newly designed aqueous humordrainage implant provides an economic implant in China. The method of implantation is simple, is one of the effective methods for treatment of refractory glaucoma and can be widely used in clinics.

[The association of HLA-DR4 gene subtypes with Vogt-Koyanagi-Harada syndrome].

OBJECTIVE: To investigate the association of HLA-DR4 subtypes with Vogt-Koyanagi-Harada (VKH) syndrome and to clarify immune genetic mechanism underlying the susceptibility/resistance to VKH syndrome. METHODS: HLA-DR4 alleles of 54 patients with VKH and 106 healthy controls were amplified and subtyped by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP). RESULTS: A selective amplification of the DR4 alleles of 263 bp was higher in patients with VKH than in the controls. The frequency of DR4-DRB1 alleles was 70% (38/54) in VKH group and was 19% (20/106) in the control group, relative rish (RR) = 10.21, P < 0.005. The genotyping of DR4 alleles showed that DRB1 * 0405 was more frequent in VKH group (35/38, 92%) than in the control group [6/20, 30%, RR = 27.2, P = 0.0,000,017, corrected pvalue (Pc) = 0.0,000,103]. CONCLUSIONS: It is suggested that DRB1 * 0405 be a susceptible subtype to VKH and the specific amino acid residue Ser (serine) at position 57 play an important role in the development of VKH syndrome. The molecular biological technique we used may be useful in studying the immunogenetic mechanism of VKH.

[Cataract extraction and intraocular lens implantation after vitrectomy].

OBJECTIVE: To study the characteristics of cataract extraction and surgical techniques in cases following vitrectomy. METHODS: Extra-capsular extraction/phacoemulsification of cataract and intra-ocular lens implantation were performed under vitreous irrigation. RESULTS: Of 37 cases (40 eyes), 31 eyes (77.5%) had obtained post-operative visual acuity > or = 0.1, 12 eyes (30.0%) > or = 0.3. The complications included nuclear dislocation into vitreous cavity (1 eye) and vitreous hemorrhage (1 eye) in the operation, and rubeosis iridis (1 eye) and pupillary capture (1 eye) after the operation. CONCLUSION: Anti-hypotony must be emphasized in the operation of cataract extraction after vitrectomy, and the vitreous irrigation is a good way against hypotony, extra-capsular extraction is more suitable than phacoemulsification in cases with hard nucleus.

[Experiences of treatment of mass burn casualties].

A fire in Karamay, Xinjiang on December 8, 1994, took a toll of 323 deaths at the site of the disaster, including 208 children, 75 teenagers and 40 adults. 135 burn patients were admitted to the hospital (including 41 children, 62 teenagers, and 32 adults). They were classified as extremely serious 9, serious 35, moderate 64 and mild 27. Among them 77 patients were complicated by inhalation injury, and tracheostomy was performed for moderate serious cases within 24 hours. Escharectomy and skin-grafting were done for III degree burn wound of hands, and early wound infection was effectively controled. Because of efficient organization, the rescue work and treatment were successful.

[Experience and lessons of lung transplantation].

In the last 2 years, four hospitals performmed cooperatively single-lung transplantation for 3 patients, 1 left lung and 2 right lung. The patients had pulmonary fibrosis, COPD with cancer or tuberculosis, infection in some extent before transplantation. The patients died from severe infection 9, 48 and 43 days after the operation. We discussed the selection of donors and recipients, operative procedures, postoperative management, especially monitoring, differential diagnosis and treatment of infection and rejection. HLA compatibility and transbronchial lung biopsy were important to the success of lung transplantation. We believe that rejection and infection are important causes of short-term death that should be given more attention.

[Ocular echographic evaluation of human aqueous drainage implant].

OBJECTIVE: To determine after implantation of a glaucoma aqueous drainage device whether fluid is present around the equatorial plate and to measure the bleb size around the plate. METHODS: 41 plates in 41 patients (41 eyes) with refractory glaucoma who had undergone human aqueous drainage (HAD) implantation underwent standardized echography (both A- and B-scan examinations) after a mean postoperative follow-up period, 5.2 months. According to the bleb height and the fluid underlying and/or overlying the plate, the blebs were divided into five grades and two types to evaluate their functions. RESULTS: Of the 41 cases examined with echography, 35 (85%) were associated with posterior blebs, 2 (5%) had no associated posterior blebs, and the posterior bleb in 4 (10%) cases was not certain. The percentage of the presence of filtering bleb around the plate was 15%, 12%, 29%, 37%, and 7% in Grade 0 to IV group, and 11% and 89% in type A and B, respectively. There was no significant correlation between the bleb size and the level of intraocular pressure control. CONCLUSION: Standardized ocular echography is helpful in the postoperative management of failed cases who have undergone HAD implantation because it can demonstrate the presence or absence of blebs and characterize them.

[Mutagenicity and carcinogenicity of nabumetone].

To study the mutagenicity and carcinogenicity of Nabumetone, we conducted Ames test (TA97, TA98, TA100, TA102), micronucleus test(MN) in mice marrow, chromosomal aberration assay(CA) in CHL cells in vitro, CA in germ cells from testes of mice, and cell transformation test of Syrian hamster embryo(SHE) cells. The maximum concentration was 500 micrograms/plate in Ames test with and without S9 mix. The mice were treated orally(gavage) daily for 4 days in 3 doses in which the maximum dose was 60% LD50 and sampled at the 5th day in MN. The maximum concentration was the dose that the growth of 50% of cells was inhibited in CA of CHL. Cells were harvested after recultured for 18 hours in fresh medium after treatment for 6 hours in the test with S9 mix, and after treatment for 24 or 48 hours in the without S9 mix. The mice were treated orally(gavage) daily for 5 days in 3 doses in which the maximum dose was 1/4 LD50 and sampled at the 6th day in CA of germ cells from testes of mice. 2 micrograms/ml was chosen as the maximum concentration in the cell transformation test of SHE cells, and result was observed after treatment for 9 days, All the tests obtained the same negative result as that reported by other investigators.

[Oxidative modification of serum LDL, VLDL and HDL induced by fed on high cholesterol diet in vivo in rabbits].

Many lines of evidence suggest that LDL is oxidized in vivo and that ox-LDL is present in the artery wall. But the oxidation of VLDL and HDL in vivo has not been reported yet. A study on effects of high cholesterol diet on oxidative modification of LDL, VLDL and HDL in rabbits was made. The control group (n = 8) was fed on routine diet, and the experimental group (n = 8) on high cholesterol diet (routine diet supplemented with 5% lard, and 0.5 g cholesterol a day for each rabbit) for 12 weeks. The serum LDL, VLDL and HDL were isolated by the density gradient ultra centrifugation. The oxidative modification of LDL, VLDL and HDL was identified by agarose gel electrophoresis, absorbance at 234 nm and fluorescence of TBARS. The results showed that serum TC, TG and TBARS in the experimental group were significantly higher than these of the control group (P < 0.01). The electrophoretic mobility of LDL, VLDL and HDL was increased, and absorbance at 234 nm and TBARS of LDL, VLDL and HDL in the experimental group were significantly higher than these of the control group (P < 0.01). These results suggest that not only LDL but also VLDL and HDL were oxidatively modified in vivo in rabbits after fed on high cholesterol diet.

[Expression of metastasis-suppressor gene nm23H1 product in nasopharyngeal carcinoma].

Using the specific monoclonal antibody of nm23H1 gene product and the immunohistochemical technique, we studied the expression of DNPK/nm23H1 and its correlation with lymph-node metastasis in nasopharyngeal carchnoma (NPC). The results revealed that, in a total of 31 cases of NPC, 13 showed positive expression with a positive rate of 41.9%. Among 21 cases of NPC with no lymph-node metastasis, 11 (52.3%) showed positive expression while only 20% (2/10) of those with lymph-node metastasis were positive, and there was significant difference between them (P < 0.01). Meantime the expression was very low or not detectable in the cancerous cells of the match lymph-node metastasis and all 10 cases showed negative expression. Statistical significant difference was found between the group of NPC with no metastasis and the group of lymph-node metastasis (P < 0.01). The above results indicated that high expression of DNPK/nm23H1 existed in the group of NPC with no metastasis while low expression existed in those with metastasis and lymph-node metastasis. It suggests that the expression of nm23H1 is inversely associated with lymph-node metastasis. The DNPK/nm23H1 may play a role in polymerization of microtubulin protein and participate in the process of cancer metastasis.

[Experimental study on supraeruption of the unopposed miniature pig molars].

Supraeruption of the molars in the absence of antagonist molars or without occlusal contacts was observed in a miniature pig. Tetracycline was used to label the new regenerated cementum and alveolar bone associated with the supraeruption. The results showed that the molar through root canal treatment migrated the same level as the molars with pulp, which means that the hydrodynamic and hydrostatic forces arising from the blood in pulp vessels look doubtful. Cementum and alveolar bone deposition was seen around the apical, cervical regions and under the furcation area. The thickness of the deposition layers was almost the same as the supraerupted volume from the model measurement.

[Clinical evaluation of ceramic inlay made by CAD/CAM for 3 year]

In this study, ceramic inlays made by CAD/CAM system were used to restore posterior tooth defects. After three years of clinical observation on 21 prostheses no secondary caries was found and patients satisfied with their restoration on aspects of color match and resistance to wear etc.Five cases of inlay fracture and of obvious space between inlay margin and Cavowall were found. The main reason was the insufficient thickness of the inlays meanwhile the selection of bonding agent was not appropriate. The clinical successful rate is 76.20%.As CAD/CAM is a sophisticated technique induced into prosthodontic area just in 1980's,it has still some deficiency to be improved and perfected.

[Dynamic changes of serum antibody titer and liver ultrastructural pathology in acute sporadic hepatitis E virus infected patients].

7 acute hepatitis E patients were tested for anti-HEV IgM and IgG antibodies by EIA. It revealed that anti-HEV IgM and IgG antibody titers were the highest at day 10-45 of the illness, and anti-HEV IgM declined gradually and totally lost within 2 months. Anti-HEV IgG had a similar condition to anti-HEV IgM, but at 7th month there still had 3 cases (40%) remained to be antibody positive. One case was examined for ultrastructural pathology. The swelling of liver cells on the fortieth day of illness was still to be found, the vacuolation of cytoplasm and condensation of mitochondria were present in liver cells.

[Diagnosis of hepatitis E virus infection in sporadic acute hepatitis].

The solid-phase enzyme immunoassay (EIA) was used for detecting anti-HEV immunoglobulin G (IgG) and M (IgM) by using a recombinant chimeric antigen (ORF3-2) from the open reading frame (ORF) 2 and 3 of hepatitis E virus gene. These sera were collected from the patients with sporadic acute viral hepatitis. The positive rate for anti-HEV IgG was 15.22% (7) among 46 patients with acute hepatitis, 5 of the 7 IgG positive sera were also positive (71.4%) for anti-HEV IgM. 91.8% (93) and 76.4% (78) were positive separately for anti-HEV IgG and IgM among 102 patients of suspected HEV infection (these patients were negative for anti-HAV, anti-HBV and anti-HCV), about 2/3 patients of HEV infection detected were simultaneously HEV-IgG and IgM positive. In control group of 98 normal persons, 4 (4.08%) were positive for anti-HEV IgG, but none were IgM positive. Paired sera of the patients in the acute phase and convalescent phase were detected, the titers of anti-HEV IgG with 4-16 fold decrease were seen in 6 cases and with 2-fold rise in 1 case.
