[Treatment of avascular necrosis of talus with vascularized bone graft].

In order to study the curative effect of vascularized bone graft in the treatment of avascular necrosis of talus, 24 patients were treated with vascularized bone grafts, in which 9 cases had received 1st cuneiform bone graft with a malleolaris anteriomedialis, 4 cases with the 1st cuneiform bone graft with the medial tarsal artery and 11 cases with vascularized cuboid bone graft with the lateral tarsal artery. All of the patients were followed up for 3-5.5 years. The clinical observation and X-ray examination showed that function of the ankle joint was completely or almost normal in 16 cases, and the bone repair was excellent. There was slight pain in the ankle joint in 4 cases. The efficiency rate of the treatment was 83.3%. It could be concluded that vascularized bone graft might be an effective method in the treatment of avascular necrosis of talus.

[A comparison of chromosome fragile sites in elderly and young people].

In this comparative study, the authors investigated the chromosome aberrations and expression of fragile sites in the lymphocytes which were collected from 45 elderly people and 29 young people and were cultured with FUDR for 24 hours. The results showed that the aberration rate was 24.75% for elderly people, but 4.22% for young people (P < 0.01): that the expression of fragile sites was 45.94% for elderly peoples, but 23.02% for young people (P < 0.01): and that the relation of aberrations with fragile sites was 79.98% for elderly people, and 92% for young people. These findings suggest that chromosome aberration is in close association with fragile site.

[Total cavo-pulmonary connection for complex congenital heart disease].

Between April 1994 and January 1997, 10 patients with complex congenital heart disease were treated with total cavo-pulmonary connection(TCPC). They included 7 males and 3 females, aged 2-17 years(mean 10.1 years). These cases were single ventricle in 5, tricuspid atresia in 4, severe Ebstein anomaly in 1. For construction of the right atrial lateral tunnel, Gore-Tex patch was used in 9 cases and autologous right atrial wall in another, 4 of the 10 patients underwent TCPC without aortic crossclamping. There were no operative deaths. The follow-up is 2-36 months(mean 12.3 months). All of these patients did well and were in NYHA class I. The advantages, operative indication and technique of TCPC and the experience of improvement of surgical results are briefly discussed in this paper.

[Supraglottic laryngectomy with or without one arytenoid in epiglottic carcinoma. A report of 40 cases].

OBJECTIVE: Supraglottic laryngectomy with or without one arytenoid is a functional laryngectomy suitable for treatment of epiglottic squamous carcinoma. This procedure consists of resection of the thyroid cartilage, epiglottis and the entire preepiglottic space. METHODS: Between 1990 and 1996, supraglottic laryngectomy was used to treat 40 patients with epiglottic carcinoma. The data were collected by a review of patient records and follow-up. The cancers were in stages T1(17), T2(17) and T4(6) according to the 1987 UICC cancer staging criteria. RESULTS: Twelve ipsilateral and one bilateral functional neck dissection were performed simultaneously of which 7 patients had metastasis in lymph nodes. No patient died postoperatively. Only 8(20%) had slight aspiration before the 20th day. All patients had decannulated. Twenty-nine cases received radiotherapy and chemotherapy. A follow-up analysis showed survival rate of 74% at 3 years. CONCLUSION: We propose supraglottic laryngectomy for the surgical treatment of early supraglottic carcinomas, which could acquire almost normal laryngeal function.

[The relationship of vascular endothelial growth factor and angiogenesis to the progression of gastric carcinoma].

OBJECTIVE: To investigate the relationship of vascular endothelial growth factor (VEGF) and angiogenesis to the progression of gastric carcinoma (GC). METHODS: VEGF protein expression and microvessel density (MVD) in 56 cases of human GC and VEGF mRNA expression in some cases were examined by use of immunohistochemical staining and in situ hybridization technique respectively. MVD and VEGF expression were analysed together with their relation to histologic types, depth of invasion, growth pattern, lymph node metastasis, distant metastasis and relapsing rate of GC. RESULTS: MVD was significantly higher in VEGF-positive GC than in VEGF-negative GC (P < 0.01). MVD and VEGF positively correlated with depth of invasion (P < 0.01), lymph node metastasis (P < 0.05) and distant metastasis (P < 0.05), but their relation to the histologic types and growth pattern of GC was not significant. The 5 year survival rate was significantly lower in GC patients with positive expression of VEGF or with MVD > or = 43. The expression of VEGF mRNA was consistent with that of VEGF protein, but the distribution is different. CONCLUSION: VEGF is highly related to angiogenesis of GC and promotes growth, invasion and metastasis of GC. VEGF expression or MVD is one of the predictors for the biological behavior of GC.

[Effect of Zhongfeng Naodeping Granule on hippocampal excitatory amino acid and neuron of stroke-prone spontaneously hypertensive rats with hemorrhagic apoplexy].

OBJECTIVE: To observe the effects of Zhongfeng Naodeping Granule (ZFNDPG) on hemorrhagic apoplexy. METHODS: The stroke-prone spontaneously hypertensive rats (SHRsp) were used to study effects of ZFNDPG on hemorrhage apoplexy. Excitatory amino acid (EAA) concentration in hippocampus sector, neuronal density and ultrastructural changes in hippocampal CA1 sector were measured. RESULTS: In pathological model group glutamate (Glu) and aspartate (Asp) concentration elevated obviously. With the ZFNDPG treating SHRsp of hemorrhagic apoplexy, Glu and Asp concentration in hippocampal sector could be markedly inhibited, compared with model group, P < 0.05-0.01. Neuronal morphology was observed: neurone injury was mild and neuronal density increased in hippocampal CA1 sector of treatment group, compared with model group, P < 0.01. Electron microscopy showed: edema, degeneration and necrosis caused by hemorrhagic apoplexy were improved after the ZFNDPG treatment. CONCLUSIONS: Effects of protecting neurone for SHRsp on hemorrhagic apoplexy might be associated with that ZFNDPG inhibited concentration of EAA.

[An antimicrobial experimental study on the traditional drug Anuqitabiao used by the Yi minority nationality].

The result of experimental study on the antimicrobial effect in vitro of the decoction, water-soluble ingredients and liposoluble ingredients of Anuqitabiao shows that decoction and water-soluble ingredients have certain inhibitory effect on all the tested bacteria, while the liposoluble ingredients do not, indicating the antimicrobial ingredients to be water-soluble. The result of antimicrobial experiment in vivo shows that neither different concentrations nor different separated parts of the decoction have any obvious protective effect on the pneumonococci-infected or staphylococciaureus-infected mice.

[Reconstruction of circumferential defect of cervical esophagus with platysma myocutaneous flaps after resection of hypopharyngeal carcinoma].

OBJECTIVE: To show the results of reconstruction of circumferential defect of cervical esophagus with platysma myocutaneous flaps after resection of hypopharyngeal carcinoma in 11 cases. METHODS: Total larynx, pharynx and cervical esophagus were resected and a permanent tracheostomy was made in the patients, because the cancer had invaded the larynx and cervical esophagus. Two platysma myocutaneous flaps were designed and dissected at both anteror-lateral neck, which was about 3.5-4 cm x 7-10 cm respectively. The flaps were made into a skin tube. The upper edge of the skin tube was sutured to the cut end of the pharynx and the lower sutured to the end of the esophagus. RESULTS: All of the flaps survived and there was no death after operation. There was fistula in 2 cases. The fistula healed after 14 days and 18 days respectively. All cases were normal in swallowing food. CONCLUSIONS: The operative methods, advantages and experiences are also descussed.

[Ultrasonographic diagnosis of peritoneal mesothelioma].

OBJECTIVE: To investigate the sonographic features of peritoneal mesothelioma and its relationship with the gross pathology. METHODS: The ultrasonographic results of 14 cases with peritoneal mesothelioma were summarized, the sonographic features were compared with the gross pathology. RESULTS: With ultrasound examination a sheetlike mass was seen in two patients and a localized irregular mass was seen in one. Involvement of "cakelike omentum" was present in five. Pelvic mass was seen in eight and two were also with abdominal mass; one presented abdominal mass only. Bowel adhesion was found in one. Ascites was presented in all patients except one. CONCLUSIONS: It is possible for ultrasonography to suggest the diagnosis of peritoneal mesothelioma, when the sheetlike or irregular mass of peritoneum was founded. Ultrasound guided biopsy was helpful in diagnosis of peritoneal mesothelioma before operation.

[Reoperative treatment of recurrent retroperitoneal tumor].

OBJECTIVE: To improve the resection rate and survival rate for recurrent retroperitoneal tumor (RRT). METHOD: We analysed 34 patients with RRT who were treated in our hospital from 1987 to 1995. One patient had benign tumor, the others malignant. Because 7 patients were operated on over one time, we performed 53 operations including complete resection in 42 patients, palliative resection 8 and biopsy 3. RESULT: The 1-and 2-year survival rates for complete resection cases were 71.2% and 65.3%. The patients who received palliative resection and biopsy died within one year. CONCLUSION: The diagnosis, preoperative preparation and intraoperative management of RRT are important to operative safety and survival rate. We emphasize the importance of reoperation and complete resection for RRT.

[Local amyloidosis of stomach].

OBJECTIVE: To improve the diagnostic rate of local amyloidosis of stomach. METHOD: One case of local amyloidosis of stomach was reported and the literatures were reviewed on the etiopathology, clinical features, diagnosis, treatment and prognosis. RESULT: The patient was negative for family history and other chronic disease. Total gastrectomy was taken and was eventually found to have localized amyloidosis of stomach and regional lymph nodes. The amyloidosis was found to be of AA amyloidosis. Multiple biopsy specimens taken from esophagus, duodenum, rectum and bone marrow showed no amyloid deposition. CONCLUSION: Amyloidosis should be considered in the differential diagnosis of gastric tumors, and also should be removed by operation if complications are to be prevented.

[Surgical treatment of the hypopharyngeal and cervical esophageal carcinoma].

OBJECTIVE: To review the experience of surgical treatment of hypopharyngeal and cervical esophageal carcinoma. METHOD: Seventy-six patients with squamous-cell carcinoma of the hypopharynx and cervical esophagus underwent surgery in our department from 1980 to June 1997. The site of the tumour was in hypopharynx in 31 patients and in cervical esophagus in 45. RESULT: The resectability rate of hypopharyngeal carcinoma (HPC) was 98.6% (30/31). The types of the esophageal reconstruction included pharyngo-esophageal anastomosis (3 patients) platysmamyocutaneous flaps reconstruction (PMFR) (12) free jejunal interposition (3) and gastric tube transposition following total pharyngolaryngo esophagectomy (12). The resectability rate of cervical esophageal carcinoma (CEC) was 91% (41/45). The esophageal construction applied gastric tube after esophagectomy without thoracotomy except one. The postoperative morbidity was 36.5% for gastric tube reconstruction (including one died of heart failure), 33.3% (1/3) for free jejunal interposition and zero for PMFR. The overall follow-up was from 2 to 108 months (median: 56.5 months) and the 1-, 3-, 5-year survival rates were 79.3%, 60% and 31.6% respectively for HPC and 68.3%, 9.5%, 0% respectively for CEC. CONCLUSION: Our data demonstrated that the resectability rate of the HPC and CEC was quite high but the long-term result of HPC was better than that of CEC. The PMFR after pharyngolaryngo-cervicalesophagectomy was a safe, effective operation with low morbidity and excellent long-term results.

[Treatment and prognosis of 33 patients with recurrent retroperitoneal soft tissue sarcomas].

OBJECTIVE: To investigate the treatment and the prognosis of patients with recurrent retroperitoneal soft tissue sarcomas. METHOD: The clinical data of 33 patients, treated for recurrent retroperitoneal soft tissue sarcomas from 1972 to 1996 were analyzed retrospectively. RESULT: Complete tumor resection was performed for 17 patients (51.5%) with tumor recurrence for the first time. 2, 3, 1 patients underwent multiple organs resection respectively when the sarcoma relapsed initially, secondly and thirdly. Fourteen patients were given adjuvant radiotherapy and chemotherapy postoperatively. Twenty-nine patients were followed for up for 6 to 12 months. Fifteen (51.7%) of them died. Seven of them survived beyond 5 years and 2 over 10 years. The 1-, 3-, and 5-years survival rate was 85.7%, 54.9% and 42.3% respectively. CONCLUSION: Surgery is effective in improving the survival rate of patients with recurrent retroperitoneal soft tissue sarcomas, including those with repeatedly recurrent sarcomas. Multiple organ resection could be beneficial to the curative resection rate. Adjuvant radiotherapy and chemotherapy might improve the local control of relapsed sarcomas to some extent.

[Cytotoxic effect of mitomycin C on the nonpigmented epithelium of ciliary body in rabbit eyes].

OBJECTIVE: To observe pathomorphological changes of nonpigmented epithelial cells (NECs) of ciliary body after local single application of mitomycin C (MMC) during sclerectomy in rabbit eyes, and to explore cytotoxic effect of MMC on ciliary body. METHODS: A single five minutes intraoperative exposure to the agent MMC (0.2 mg/ml, 0.1 ml) was used during sclerectomy in rabbit eyes. The treated area was rinsed with 20 ml normal saline. The NECs of ciliary body at the operated area were observed under light microscope and transmission electron microscope on the 7th and 28th day postoperatively. RESULTS: On the 7th day postoperatively, the NECs were swollen, and the intracellular abundant mitochondria were swollen and vacuolized. String pearls-like membrane-encapsulated vacuoles were present in the intervals of beta-cell membrane of the NECs. On the 28th day postoperatively, these damages were gradually recovered. CONCLUSIONS: These findings suggest a toxic effect of MMC on the NECs of ciliary body, possibly that can decrease the aqueous production and intraocular pressure, even causing persistent hypotony.

[Light sensitivity in central visual field].

OBJECTIVE: To examine the difference in the light sensitivity in the central visual field between the normal subjects and glaucoma patients with or without the early visual field damages and propose the criteria to identify the possible early disturbance in the visual field. METHODS: The light sensitivity at the 76 locations in the central field was measured in 108 normal eyes of 75 normal volunteers aged from 20 to 78 years and 82 eyes of 53 patients with early open-angle or angle-closure glaucoma aged from 30 to 70 years on the Humphrey Field Analyzer (Model 610) by using the Central-30-2 program. RESULTS: No sex and eye differences in the light sensitivity were found between the two groups. The significant difference in the light sensitivity existed between the age of 20 - 45 years and the age of 46 - 78 years in the normal subjects. The lower limit of 80% normal range for each location in the central field of the two age groups was calculated. The light sensitivity decreased by 2 - 3 dB at every 10 degree from the eccentricity. The light sensitivity in the inferior field was 1 - 2 dB higher than that in the superior. There was no statistical difference between the nasal and the temporal field. For the patients with the field defects the light sensitivity of many locations decreased by 4 - 8 dB, and by 1 or 2 dB in the patients without the field defects as compared with the normal control. CONCLUSIONS: Based on the comparison with the normal control and the lower limit of 80% normal range, we propose the following criteria for classification of damages in the glaucoma visual field. Grade I: light sensitivity decreases by 1 - 3 dB at a cluster of locations in the field; Grade II: light sensitivity reduces by 4 - 6 dB at a cluster of locations in the field; Grade III: light sensitivity reduces by more than 6 dB at a cluster of locations in the filed.

[Relaxing peripheral retinotomy with endolaser in treatment of retinal detachment with severe anterior proliferative vitreoretinopathy].

OBJECTIVE: To investigate the effects of relaxing peripheral retinotomy with endolaser for treatment of complicated retinal detachment with severe anterior proliferative vitreoretinopathy. METHODS: 44 eyes of complicated retinal detachment with severe anterior proliferative vitreoretinopathy underwent relaxing peripheral retinotomy with endolaser treatment and silicone oil injection. Outcome measures included retinal reattachment, visual acuity, intraocular pressure as well as these after silicone oil removal. RESULTS: After minimum follow-up of 3 months, the retina was completely attached in 44 eyes (8 of them with folds), 38 eyes achieved improvement of visual acuity. The silicone oil was removed in 22 eyes, the retina kept attached in 21 eyes of them. Hypotony (intraocular pressure < 10 mmHg) was seen in 8 of the 22 eyes with reattached retina. CONCLUSION: Relaxing peripheral retinopathy with endolaser in the surgery for complicated retinal detachment with severe anterior proliferative vitreoretinopathy is an effective procedure. Subretinal membrane peeling benefits a complete retinal reattachment. Hypotony occurred in some cases, therefore indications should be limited.

[Sulfation of polysaccharides isolated from Indocalamus tesselatus and their anticytopathic effect on human immunodeficiency virus type I].

For preparation of sulfated polysaccharides isolated from bamboo leaves (Indocalamus tesselatus) with chlorosulfonic acid-pyridine, the experimental conditions as the molar ratio of reactants and temperature and time of reaction were discussed. A homogeneous sulfation method with good yield of more than 80% was obtained and the sufur content was found to be 21.9%. Conformation change in sulfated polysaccharide with changes of intrinsic viscosity, polarimetery and CD spectra were found to be related to the negative charge. The polysaccharide sulfates were shown to have better activity to inhibit cytopathic effect of human immunodeficiency virus(HIV) infection than native polysaccharides. The effective concentration was 10 micrograms.ml-1 while the cytotoxic concentration was 2.5 mg.ml-1.

[Isolation of novel expression sequences of C2H2 type zinc finger protein gene from human brain tissue according to the conservation of zinc finger motif].

By low stringency PCR amplification of genomic DNA using the primers designed based on the conservation of zinc finger motif, we got 8 gradient eletrophoretic bands. After recovery of the second and third bands, the DNA fragments in them were cloned and sequenced. Compared to the GenBank database, among these 60 segments containing zinc finger motif, 23 segments were novel zinc finger genes' genomic segments. Then the human brain tissue cDNA library was screened, using these segments as probes, and 44 positive clones were obtained. Rescreening 28 of them, we got 20 rescreened clones. All of them were sequenced and sent to the GenBank DNA database for sequence analysis, the results showed that 16 were novel C2H2 type zinc finger protein cDNA segments. The cDNA segments encoding the novel C2H2 type zinc finger proteins provide the basic materials for cloning of full length cDNA of valuable novel zinc finger protein genes.
