The frequency of serum amyloid A2 alleles in the Japanese population.

The incidence of serum amyloid A2 alleles (SAA2 alpha and SAA 2 beta) in the Japanese population was analyzed by PCR-RFLP analysis. The SAA2 alpha allele was predominant (approximately 90%) in both healthy controls and adult patients with rheumatoid arthritis. There was no significant association of allele patterns with AA amyloidosis in the patient group. These results are discussed in comparison with those of previous reports from other areas.

[Severe late effects of esophageal cancer treated by simultaneous intraluminal hyperthermo-brachytherapy: a case report].

Severe late effects occurred after the treatment of esophageal cancer by external irradiation and simultaneous intraluminal hyperthermo-brachytherapy. This is the first case report on late side effects. A 73-year-old man was treated with radiotherapy alone according to his decision. The stage was T2N0M0 according to the 1987 UICC. External irradiation of 40 Gy/20 fr was delivered followed by simultaneous intraluminal hyperthermo-brachytherapy once weekly for four weeks (16 Gy/4 fr). Complete response was achieved. Eight months later, local recurrence and ulcer were found, and surgery was performed. The specimen showed healing of an ulcer acknowledged before surgery and another deep ulcer next to the tumor that penetrated the adventitia, which was close to perforation. The histopathologic specimen showed severe stenosis of muscular arteries in the adventitia, which was caused by fibrosis of the endothelium. The findings suggested that enhancement might be occurred with this combined therapy.

[The influence of the physician's speciality on ambulatory "essential laboratory tests"].

The purpose of this study was to examine the influence of a physician's speciality on the choice of "essential laboratory tests" and their response to the test results. A retrospective case-control study was conducted in divisions of the Department of Internal Medicine in Jichi Medical School Hospital. Twenty-seven parameters were evaluated in 213 outpatients (mean age 47 +/- 17 years; 53% female). Forty-three percent (2443 of 5751) of the tests were conducted by the physician under study and 10.5% (257 of 2443) of their tests were abnormal. The physician's speciality influenced the choice of the ambulatory medical examinations and their response to abnormal tests. Physicians tend to be more sensitive to the abnormality of tests related to their specialty. Hence, it is necessary to constitute guidelines on the response to abnormal "essential laboratory tests".

[A recovered case of SLE with central nervous system involvement who relapsed presenting new symptoms associated with development of serum anti-Sm antibody].

A rare SLE patient with central nervous system involvement (CNS-SLE) who relapsed presenting new symptoms associated with the development of serum anti-Sm antibody and was then successfully treated with cyclophosphamide (CY) pulse therapy is presented here. A 47-years old housewife was admitted to Kushiro City General Hospital because of fever, limb erythema and drowsy consciousness in September 1995. On the basis of convulsion, proteinuria, leukopenia, thrombopenia, serum positive tests for both anti-nuclear antibody and anti-SSA antibody and low complement levels, as well as elevations of IgG index and IL-6 in the cerebrospinal fluid (CSF), she was diagnosed as having CNS-SLE. Serum tests for anti CL-beta 2 GPI antibody and lupus anticoaglant was negative. Serum test for HBs antigen was positive. She was treated successfully with methylprednisolone (mPSL) pulse therapy and plasma exchange (PE). Prednisolone was gradually tapered to the dosage of 17.5 mg per day and she was discharged in April 1996. She was re-admitted because of fever, an exacerbation of skin eruption and arthralgia in October 1996. Serum anti-Sm antibody was found to be positive. mPSL pulse therapy was not effective. On the basis of hallucination and elevations of IgG index and IL-6 in the CSF, a diagnosis of relapsed CNS-SLE was made. However the level of IFN-alpha in the CSF was normal. Although PE was not effective, CY pulse therapy was markedly effective.

[Acute promyelocytic leukemia relapse as leukemia cutis shortly after complete remission with all-trans retinoic acid].

A 56-year-old man was admitted to our hospital in September, 1996. Chromosomal translocation (15; 17) and a PT-PCR study for PML-RAR alpha mRNA were positive in bone marrow aspirates, and acute promyelocytic leukemia was diagnosed. After CR was obtained with all-trans retinoic acid (ATRA) followed up with chemotherapy, the RT-PCR became negative. When he was readmitted in April, 1997, skin eruption on his chest and extremities were observed. Specimens taken for biopsy revealed leukemia cutis, and RT-PCR became positive in the same specimen. Bone marrow PT-PCR was also positive without abnormal promyelocytes. Although he was treated with oral ATRA 80 mg/day again, no significant improvement in leukemia cutis was noted. After combined therapy with Ara-C and acularubicin, skin eruption disappeared and bone marrow RT-PCR became negative. A second CR was then obtained. Although it is unknown whether the administration of ATRA is related to extramedullary relapse or not, we recommend combined chemotherapy for such cases.

Matrix metalloproteinases and their inhibitors in human vitreous.

PURPOSE: To conduct zymographic analysis to study the matrix metalloproteinases (MMPs) and tissue inhibitor of metalloproteinases (TIMPs) in vitreous samples of patients undergoing pars plana vitrectomy as part of the treatment of vitreoretinal disease. METHODS: Forty-two vitreous samples were collected at the time of pars plana vitrectomy. Diagnoses included severe (exudative) age-related macular degeneration (AMD) (12), macular hole (10), presumed ocular histoplasmosis syndrome (6), proliferative diabetic retinopathy (PDR) (5), epiretinal membrane (4), vitreomacular traction syndrome (2), macroaneurysm with subretinal hemorrhage (1), central retinal vein occlusion with vitreous hemorrhage (1), and proliferative vitreoretinopathy (1). Gelatin zymography, reverse gelatin-zymography, carboxymethylated transferrin zymography, and sodium dodecyl sulfate-polyacrylamide gel electrophoresis were performed on the liquid vitreous samples to assess for MMP and TIMP activity. RESULTS: Progelatinase A occurred in all vitrectomy samples. In addition, a band consistent with TIMP-2 occurred in all samples on reverse zymography. An inhibitor of MMP of a lower molecular weight than TIMP-1 was found in all the samples. A serine proteinase with a broad band around 180 kDa was found in 2 of the 11 AMD vitreous samples. A 75-kDa metalloproteinase was found in several AMD samples, but it was much more abundant in the PDR samples. CONCLUSIONS: Metalloproteinases and their endogenous inhibitors are present in human vitreous and may be involved in the pathogenesis of PDR and other vitreoretinal diseases.

Trigeminal neuralgia caused by the metastasis of malignant lymphoma to the trigeminal nerve: a case report.

The authors cared for a patient suffering from left trigeminal neuralgia who had systemic malignant lymphoma which had been treated with chemotherapy. The lesion of the left trigeminal nerve was not detected by brain CT scans or by the magnetic resonance images made at the onset of trigeminal neuralgia. However, metastasis to the left trigeminal nerve root was disclosed 6 months after the onset of the systemic lymphoma. Open biopsy confirmed lymphomatous involvement of the left trigeminal nerve root. In this case the site of the metastasis was mainly the trigeminal nerve root entry zone in the cerebellopontine cistern, and the lesion caused left trigeminal neuralgia and sensory impairment.

[Epitope analysis of anti-serum protein antibodies and its application for amyloidosis research].

To obtain antibodies specific to desired portions in proteins, synthetic peptides are widely used as immunogens, however, generated antibodies often react improperly with native proteins. Monoclonal antibodies (MoAb), epitopes of which are well defined, would be useful. In this review, we present epitope analysis and its application for immunohistochemical studies on four MoAbs against serum amyloid A(SAA), the serum precursor of fibrillar component in reactive amyloid deposits. Multipin-ELISA (enzyme-linked immunosorbent assay), the epitope mapping system which is based on reactivity with small amount of synthetic peptides immobilized on plastic pins, divided 13 clones of rat-mouse hybrid anti-SAA MoAbs into four. Two were determined epitopes as a portion around residue 18 and 30 of SAA while the other two could not be determined. These epitopes were analyzed using truncated recombinant SAA proteins, which were produced in E. coli, and determined as a portion around residue 90 and 100 (C-terminus), respectively. Negative immunohistochemistry of anti-SAA C-terminus MoAb for human reactive amyloid deposits suggests that SAA C-terminus is removed before or during amyloid deposition.

[A case of systemic lupus erythematosus developed with intestinal perforation].

A case of intestinal perforation associated with SLE is presented. A 54-year-old woman was diagnosed as having SLE twenty-five years ago when she had facial erythema, photosensitivity, oral aphtha, polyarthraliga, leukopenia, positive LE cell and positive antinuclear antibody. She had been treated with prednisolone and admitted to Kushiro City General Hospital because of one month history of fever and anorexia in February 1996. Laboratory findings did not reveal activity of SLE, and a diagnosis of urinary tract infection was made based on the findings of urinalysis. After severe diarrhea, disseminated intravascular coagulation (DIC) developed. A rectal perforation was revealed by endoscopic and radiological examination. An emergency laparotomy revealed necrosis of the rectum and sigmoidostomy was performed. The biopsied specimen of the rectum were diagnosed as gangrene of ischemic colitis histologically. Because of a penetration to the urinary bladder, an ureterocutaneostomy was performed. She died of sepsis and DIC on the 127th day of admission. Only 11 cases of intestinal perforation associated with SLE have been reported in Japan, and the association of vasculitis has been considered. In the present case, the prolonged use of prednisolone might cause the necrotizing ischemic colitis.

[The examination of apoE phenotypes in diabetic patients with peripheral neuropathy].

Apolipoprotein E (apoE), which is reported to recognize the low density lipoprotein receptor and remnant receptor, mediates the delivery of cholesterol and other lipids to the cells all over the body. There are several phenotypes such as apoE2, apoE3 and apoE4. Recently, it is reported that apoE plays an important role in neurite outgrowth. To determine whether apoE phenotype is concerned in diabetic peripheral neuropathy, we investigated the incidence of apoE phenotypes in diabetic patients with peripheral neuropathy. The occurrence of retinopathy and nephropathy were not different in apoE2, apoE3 and apoE4. However, the frequency of diabetic neuropathy was higher in apoE4 than apoE2 and apoE3 (p < 0.05). Furthermore, as the stage of diabetic neuropathy advanced, the incidence of apoE4 increased. From these results we conclude that apoE phenotype influences the progress of diabetic peripheral neuropathy and that apoE4 contributes to the deterioration of diabetic peripheral neuropathy.

[Typing of DNA using the primer extension preamplification (PEP) method--studies of reliability of typing and detection limits].

Primer extension preamplification (PEP), which can amplify sequence-independently a limited quantity of DNA as a whole, allows multiple DNA analyses by polymerase chain reaction (PCR). This technique may be applicable to forensic cases, especially in cases where only small amounts of DNA are available. To define the accuracy and sensitivity of PEP, the DNA typing results of nine loci (TH01, MCT118, HLA-DQ alpha, amelogenin, LDLR, GYPA, HBGG, D7S8, GC) by PCR with PEP (PEP-PCR) were compared with those by PCR without PEP. Both results were identical to each other for each sample tested. Furthermore, amplification of an initial genomic DNA by PEP was found to range from 15 to 600 times of the initial quantity and the efficiency of PEP may depend on the sequences flanking the loci tested. These results indicate that PEP can increase typing potential of PCR from forensic samples.

[Composite pheochromocytoma with ganglioneuroma in the adrenal gland: a case report].

A 67-year-old male demonstrated a right adrenal tumor at another hospital, and consulted our hospital for surgical treatment. Abdominal computed tomography revealed a 13 x 12 cm mass in the right adrenal region. Serum and urinary adrenaline levels were high, and the catecholamine levels in the blood sample of the selective adrenal vein were also high. The tumor was 1,325 g in weight and 13 x 9 x 18 cm in diameter. Pathological diagnosis was a mixed neuroendocrine-neural tumor. It was composed of pheochromocytoma and ganglioneuroma. This combination in the adrenal gland is rare. We reviewed 3 previously reported cases of composite pheochromocytoma with gangloineuroma in the adrenal gland in the Japanese literature, and this is considered to be the fourth case.

[Serum selenium contents and the risk of cancer].

To investigate the relationship between serum Se contents and the risk of cancer, 4857 serum samples were obtained from cancerous (2730) and non-cancerous patients (2127). In this cohort, the incidence of cancer in the non-cancerous patients was followed for the subsequent 3 years. The serum Se level of non-cancerous patients who later developed cancer during the 3 years was determined and compared with that of the non-cancerous patients. A high incidence of cancer was observed in the lower serum Se patients of the non-cancerous group. The serum Se levels of cancerous patients were significantly lower than in non-cancerous patients. These results suggest that the low serum Se level in cancerous patients may not be induced by the tumor but it was more likely already present before the tumor. To calculate the odds ratio, cancerous and non-cancerous patients were divided into group of less than 110 ppb and more than 111 ppb group in serum Se. The odds ratio was estimated to be 1.95, suggesting the high risk of cancer in the low serum Se group.

Antibiotic-loaded porous hydroxyapatite blocks for the treatment of osteomyelitis and postoperative infection. A preliminary report.

Hydroxyapatite blocks (HAB) can be used to administer antibiotics or anticancer drugs because its porous structure allows the gradual administration of the pharmacologic agents. A novel drug delivery system using hydroxyapatite blocks was developed for osteomyelitis and postoperative infections occurring after joint replacement. To load the antibiotics, hydroxyapatite blocks were mixed with an antibiotic solution and centrifuged at 1500 rpm for 15 minutes or decompressed in vacuum container at 5 to 10 in. Hg for 20 minutes. Fifteen patients with osteomyelitis including one with tuberculosis and four with infections subsequent to joint replacement were treated with antibiotic-loaded hydroxyapatite blocks in combination with intravenous injection. Except in one case, all of the foci had completely healed at follow-up (range: 13 to 71 months; average: 39.7 months). These new methods are simple and can safely treat osteomyelitis in a one-stage operation.

Exposure of the hip by anterior osteotomy of the greater trochanter.

A new exposure for the direct lateral approach to the hip through the gluteus medius by anterior half osteotomy of the greater trochanter is described. The skin incision is made with the greater trochanter longitudinally at its midpoint and extends parallel to the shaft of the femur along its anterior border. The anterior two thirds of the tendinous gluteus medius muscle is divided. The incision continues to the middle of the greater trochanter and through the distal half of the vastus lateralis muscle. An anterior thin slice osteotomy of the greater trochanter is performed. The gluteus minimus is exposed and split in the line of its fibers or detached from the greater trochanter and the acetabular roof is exposed. In order to provide a wide exposure, a Charnley transverse retractor is used and a Calipers Dual Pin Retractor is inserted over the edge of acetabulum by elevating the soft tissues from the ilium. The Calipers Dual Pin Retractor is used to precisely position the prosthesis in an anatomic and functional position and eliminate leg length discrepancy.
