[Two cases of perioperative coronary vasospasm following ASD closure and OMC].

Two cases of perioperative coronary spasm following ASD closure and OMC are reported. A 36-year-old female had been diagnosed of vasospastic angina and another 47-year-old female patient was diagnosed of effort angina for 99% stenosis of the right ventricular branch preoperatively. Both patients suffered from coronary vasospastic attack and ventricular fibrillation several times within 17 hours postoperatively. Although the coronary dissolution was obtained finally following aggressive cardiac massage, administration of spasmolytic agents, such as NTG, lidocaine, DBcAMP and the start of IABP, the resolution was stormy due to the hemodynamic derangement. To prevent the coronary spastic episode, it is suggested, for patients of possible coronary spasm, to avoid arousal stimulation and to administer spasmolytic agents prophylactically within the first postoperative day.

[Familial cardiac myxoma with multiple and contralateral recurrence].

This report described a familial recurrent cardiac myxoma involving mother and daughter. The mother, at 27 year of age, had developed recurrent multiple myxomas in both left and right atrium and right ventricle 4 years after surgical excision of left atrial myxoma. Excision was successful and remains well without signs of recurrence 9 years postoperatively. In an asymptomatic 13-year-old daughter, a recurrent left atrial myxoma was found 3 years after the excision of right atrial myxoma by echocardiographic follow-up at 6 month intervals. Excision of left atrial myxoma was performed and histology showed the essentially the same findings as primary myxoma without signs of malignancy. From an experience of this familial recurrent myxoma and a review of 38 cases of 17 familial cardiac myxoma, it is recommended that wide excision of tumor including surrounding tissues, thorough search for multiple heterotopic tumors at surgery, close postoperative echocardiographic follow-up for at least 5 years, and examination of skin and breast tumor, and endocrine disorder for "complex" myxoma.

Elevation of the petrous bone caused by hyperplasia of the occipital bone presenting as hemifacial spasm: diagnostic values of magnetic resonance imaging and three-dimensional computed tomographic images in a bone anomaly.

A case of elevation of the petrous bone due to hyperplasia of the occipital bone presenting as hemifacial spasm is reported. A 44-year-old man sought treatment for twitching of the buccal muscles on the right side that progressed rapidly in severity within 2 weeks of the onset. The anatomical details of the petrous and occipital bones were delineated clearly by computed tomographic scans of a bone window level. Details of the brain stem were shown by magnetic resonance images. The bone anomaly was displayed more realistically by three-dimensional computed tomographic reconstructions. The faithful representation of structures with these radiological studies should be mandatory, to prepare the surgical planning of such a complicated bone anomaly.

[Significance of raphe in congenitally bicuspid aortic valve].

The decrease of rheumatic aortic regurgitation (AR) is observed due to the relative increase of non-rheumatic aortic valvular diseases since 1980. Among 240 patients who had undergone aortic valve replacement (AVR) including combined valvular diseases up to March 1989, the congenitally bicuspid aortic valve was responsible for AR and stenosis (AS) in 33 patients (13.8%) and were divided according to the presence of a raphe. In the raphe (+) group (n = 15), infective endocarditis (IE) (n = 5), prolapse of the aortic valve, mainly non-coronary cusp (n = 5), and thickening with contraction of cusp (n = 4) were the cause of AR. Calcification of the cusp was seen in 2 older (greater than 59 yrs) patients. In the raphe (-) group (n = 18), IE (n = 2), contraction of cusps (n = 2) in the relatively younger (less than 48 yrs) were the cause of AR. Rest of the patients exhibited severe AS due to the calcification of cusps except a case who showed IE with AR in the calcified cusp. Although not generally recognized, the bicuspid valve with a raphe, less tendency to deposit calcium, is an important cause of pure AR severe enough to warrant AVR. The bicuspid valve without raphe, as already recognized, prones to develop severe calcification and AS in later life.

[Left coronary ostial stenosis in aortitis syndrome--patch mounted graft technique for proximal anastomosis].

Late restenosis or total occlusion of proximal aortic anastomosis frequently occurs due to intimal proliferation in aortitis CABG patients although successful early postoperatively. A Gore-Tex patch (diameter 20 mm) mounted grafts to LAD and CX with a single ostium was sutured in the inflammatory aorta to prevent late occlusion in a 40-year-old female patient. Postoperative CAG a month after showed a patent graft to LAD and improved exercise treadmill test. Graft to Cx occluded due to competed flow from LAD. No aneurysm was found in the proximal anastomosis. Tissue nonpermeable Gore-Tex mounted graft is considered to be suitable for CABG in aortitis patients with the high incidence of late occlusion.

Integration and expression of murine retrovirus-related sequences in schistosomes.

Antibodies against the retrovirus envelope glycoprotein (gp70) of mouse xenotropic retrovirus, BALB virus 2 (Bv2) reacted with the adult worms of Schistosoma japonicum and S. mansoni. This reaction was completely inhibited after adsorption of the antibodies with virions of retrovirus. The reactive schistosome antigen was located in the subtegumental layer of the adult male fluke and in the vitelline gland of the adult female of S. japonicum and S. mansoni. Proteins extracted from both parasites were examined by immunoblot analysis. Anti-Bv2 gp70 antiserum reacted with those proteins from both schistosomes and the band patterns were different among sexes and species. Southern hybridization of the DNA extracted from adults of S. japonicum and S. mansoni demonstrated the presence of sequences homologous to the env gene of mouse ecotropic and xenotropic retroviruses. DNA sequences homologous to the gag and pol regions of the ecotropic murine leukaemia virus were also detected in the DNAs of schistosomes.

Production of 2',3'-dideoxyadenosine and 2',3'-dideoxyinosine from 2',3'-dideoxyuridine and the corresponding purine bases by resting cells of Escherichia coli AJ 2595.

A novel microbial method for the production of 2',3'-dideoxynucleosides by transdideoxyribosylation has been developed. By screening microorganisms producing 2',3'-dideoxyadenosine (DDA) from 2',3'-dideoxyuridine (DDU) and adenine, Escherichia coli AJ 2595 was selected as the best producer. Optimal pH and temperature for the DDA-producing reaction were ca. 6.5 and 50 degrees C, respectively. Pi seemed to be an essential factor for the reaction, and its optimal concentration was ca. 25 mM. Moreover, polyethylene glycol had a notable effect on DDA production. Under the best conditions established, 52 mM DDA was obtained from 100 mM DDU and 100 mM adenine after 48 h of incubation from resting cells of E. coli AJ 2595. This strain could also produce 2',3'-dideoxynucleosides, such as 2',3'-dideoxyinosine (DDI), 2',3'-dideoxyguanosine, and 2',3'-dideoxythymidine, from DDU and the corresponding bases. In particular, this strain could produce DDI in high yield (ca. 32 mM from 100 mM DDU and 100 mM hypoxanthine) after 24 h of incubation. However, 2',3'-dideoxycytidine was not produced from DDU and cytosine by resting cells of E. coli AJ 2595.

A comparison of the antischistosomal effect of levo- and dextro-praziquantel on Schistosoma japonicum and S. mansoni in mice.

Praziquantel (PZQ) is a racemic compound composed of equal proportions of its optical isomers, levo- and dextro-PZQ. The efficacy of these compounds was compared with that of PZQ in mice infected with Schistosoma japonicum or S. mansoni. Mice were given 50, 2 x 50 (on consecutive days), 500, or 2 x 250 mg/kg of each compound orally 5 weeks after infection. Significant reduction of worm recovery was observed in S. japonicum infection 30 days after treatment with 2 x 50, 500, or 2 x 250 mg/kg of levo-PZQ, whereas no therapeutic effect was demonstrated with dextro-PZQ. Percent reduction in worm burden in mice treated with levo-PZQ was significantly higher than in those with PZQ at a dosage of 2 x 50 mg/kg (67.9% vs. 34.5%). Neither eggs in feces nor miracidial hatching of eggs from the livers and intestines were observed in mice treated with levo-PZQ. In S. mansoni infection, levo-PZQ showed no significant schistosomicidal effect compared with PZQ and dextro-PZQ, although there was reduction in egg counts.

Schistosoma japonicum and S. mansoni: ultrastructural damage in the tegument and reproductive organs after treatment with levo- and dextro-praziquantel.

Ultrastructural observations were made of changes in the tegument and reproductive organs of Schistosoma japonicum and S. mansoni from ICR mice after treatment with praziquantel (PZQ), levo-PZQ, and dextro-PZQ at a single oral dose of 500 mg/kg body weight. No marked difference in types and extent of lesions of the tegument of S. japonicum was found between the compounds regardless of the time of worm recovery after treatment. This was equally true of S. mansoni. Degeneration of the testis, ovary, and vitelline gland of S. japonicum was more prominent in worms administered PZQ and levo-PZQ than in those receiving dextro-PZQ. In S. mansoni, extensive regression of the reproductive organs was observed in male and female worms treated with PZQ and dextro-PZQ, while no serious damage was seen in worms treated with levo-PZQ.

Schistosomiasis on Bohol Island, Philippines, with special emphasis on the successful discovery of new habitats of the vector snail, Oncomelania quadrasi, and area-wide mollusciciding.

In the endemic area of schistosomiasis on Bohol Island, we succeeded in locating seven new colonies of the vector snail, Oncomelania quadrasi, four in Trinidad and three in Talibon, in addition to six locations from which the snails were previously recorded. We were led by the proximity of the snail habitat to the residence of the infected individuals. In northern Bohol the continuous wet season would appear to make the major differences between the places that do and those that do not support O. quadrasi. Twice-yearly weed clearance and applying chemical molluscicides (niclosamide and phebrol) for the control of O. quadrasi have been extensively in effect over the snail colonies, since July 1986. Although it has not yet yielded satisfactory results in certain colonies, snails have been eliminated from six habitats in a period of two and a half years.

Ultrastructural observations on damage to schistosomula of Schistosoma japonicum by mouse neutrophils in vitro.

Ultrastructure of schistosomula of Schistosoma japonicum was studied in relation to the effect of mouse neutrophils in vitro. In the presence of antibody and complement, damage to the schistosomular tegument covered with many neutrophils began to appear within 1 h incubation. Exfoliation of the granular cytoplasm of the tegument, disorganization of the muscle layers and vacuolation in the inner tissues were seen in a part of neutrophil-attached parasites by 2 h incubation. However, the majority of schistosomula with fewer cells retained their integrity till 16 h. The cytochemical examination of schistosomula incubated with mouse neutrophils for 1 and 2 h demonstrated clearly the localization of the peroxidase activity on the surfaces of the cells and the parasites.

[Simplified operative technique for thoracoabdominal dissecting aneurysm].

A graft with branches is used to reconstruct the extensively dissected thoracoabdominal aorta. Anastomoses of the proximal end of the main graft distal to the left subclavian artery and the side branch to the distal aneurysmal descending thoracic aorta were first undertaken after the preparation of both ends under the iliac vein-artery partial bypass. A main graft with a Y graft extension was anastomosed distally to iliac arteries. This permits the blood flow within the aneurysm and the main graft to the extremities until splanchnic and spinal cord arteries have individually been transferred to the main graft. Ischemic time of the splanchnic and spinal cord arteries, using this technique, can be minimized within 10 minutes to each in the total replacement of descending thoracic and abdominal aorta.

Schistosoma japonicum: the effect of serum fractions precipitated by ammonium sulfate on viability of schistosomula.

The effect of serum fractions precipitated by a saturated ammonium sulfate solution on viability of Schistosoma japonicum schistosomula was investigated. Axenized cercariae were incubated for 10 days at 37 degrees C in NCTC 109 supplemented with the 34%, 40%, 50%, 62% and 68% fractions of human or rabbit serum in an equivalent concentration to 10% serum. Only larvae cultured in media with the 68% fraction of human or rabbit serum retained the transparent feature and active motility, while the larvae cultured in other media became opaque and immotile. This was also true in media with human serum albumin or fatty acid-free human serum albumin at concentrations of 1.0 and 10.0 mg/ml of NCTC 109. On 10-day culture a part of larvae in the medium with the 68% fraction of human serum began to feed on red blood cells and the intestine was nearly closed by 21 days. These results suggest that serum fraction of 68% ammonium sulfate consisting of albumin is an important constituent in supporting the further growth of schistosomula in vitro.

[Interposition graft method in the treatment of aortic regurgitation associated with aneurysm of the ascending aorta].

Interposition graft method, total replacement of the ascending aorta with a valved conduit and reimplantation of the coronary artery by an interposed graft with some modifications, had been exclusively used for the treatment of aortic regurgitation associated with an aneurysm of the ascending aorta in consecutive 7 cases. Clinical improvement is apparent and postoperative angiographic study revealed a satisfactory coronary and aortic appearance with no leakage, pseudoaneurysm formation nor compression of the graft. This technique proved to be a reliable and safe method for the treatment of aortic regurgitation associated with an aneurysm of the ascending aorta with no mortality nor modality.
