[Dip-shaped hearing loss of Békésy audiogram in high school students].

Small dip-shaped hearing loss may be frequently observed on Bekesy audiograms in ears without any hearing loss on pure tone audiograms. Hearing tests and otoacoustic emission measurements were performed in club member of 2 senior high schools (K-D School and S-D School), in order to determine any relationship between inner ear impairments and the small dips on Bekesy audiograms. In the kendo, soccer, tennis and orchestral music clubs of K-D School micro-dips in depth over 10 dB were found to have incidences between 40% and 53%, and the micro-dips were distributed in frequencies above 5 kHz. In the of kendo, tennis, brass band and tea ceremony clubs of S-D School the incidences of micro-dips ears were between 30% and 48%, and the micro-dips tended to be distributed in higher frequencies except in the tea ceremony club. The micro-dips in the ears of the tea ceremony club members were distributed over all the frequencies examined, but most of them disappeared when counted for dip size above 15 dB remaining an ear at 7.5 kHz. Incidences of earphone or headphone users as a hobby were in the range of 30 to 70% in members of all clubs. There were no characteristic findings of clubs in the incidences of the earphone or headphone users. The degree of association between the micro-dip ears and the use of earphone or headphone was low. C-EOAEs which had a 6 ms or longer duration of evoked otoacoustic emissions were detected with an incidence of 40 to 65% and there was a significant association between the micro-dip ears and the C-EOAE ears. Incidences of spontaneous otoacoustic emissions were between 26% and 28% in all clubs and there were differences in the incidences between sexes. The frequency distribution of the spontaneous otoacoustic emissions were remarkably different to that of the micro-dips. To determine whether the C-EOAEs express a predisposition to inner ear susceptibility to noise-induced hearing loss and to ascertain that the micro-dip is a precursor notice to noise-induced hearing loss, further follow-up surveys are needed.

[The efficacy of ATP sensitivity assay on intravesical prophylactic instillation for superficial bladder cancer].

BACKGROUND: The object of this study is to evaluate the usefulness of the ATP sensitivity assay for appropriate selection of anticancer drugs for prophylactic bladder instillation therapy in patients with superficial bladder cancer. METHODS: The ATP assay was performed using the method reported previously. The anticancer drugs examined were ADM, MMC, THP-ADM and EPI. The 5-year rate of non-recurrence in group A (32 cases) which had been treated using the results of ATP assay, was compared with that of group B (37 cases), for which the ATP assay was not performed. RESULTS: The most sensitive anticancer drug was THP-ADM. The 5-year rate of non-recurrence in group A (80.9%) was significantly higher than that in group B (39.4%) (p < 0.001). Tumor recurrences in group A was observed within 2 years post-operatively in all cases. When the ATP assay was re-performed in 3 cases with recurrent disease, altered antitumor sensitivity was observed. CONCLUSION: These findings suggested that ATP assay was useful for choosing effective anti-cancer drugs for prophylactic instillation, especially for patients with primary or solitary tumor, grade 2 disease or tumor of a certain size (> or = 1 cm). It also appeared that the ATP assay should be performed for patients with primary superficial bladder cancer as a screening test for the selection of drugs for prophylactic instillation therapy.

[Intermittent intra-arterial chemotherapy for the treatment of patients with bladder cancer or prostate cancer using implantable injection pump].

BACKGROUND: The objective of this study is to evaluate the usefulness of intra-arterial chemotherapy using an implantable injection system for the treatment of bladder or prostate cancer. METHODS: Twenty-four patients with bladder cancer and 13 with prostate cancer were treated with intermittent intra-arterial chemotherapy using a totally implantable injection pump. A heparinized catheter was percutaneously fixed through the femoral artery with the tip 2 or 3 cm above the bifurcation of the common iliac artery, and the pump was placed in a subcutaneous pocket. RESULTS: The total number of infusion sessions for bladder cancer ranged from 4 to 38 times (mean 20.8), and 15 of the 24 patients were injected from 2 to 19 sessions (mean 8.5) as outpatients. The tumor response for 15 patients with newly diagnosis muscle-invasive bladder cancer were 12 patients in CR, 1 in PR, and 2 in NC, and 9 patients with recurrent bladder cancer were 6 patients in CR, 1 in PR, and 2 in NC. The total number of infusions for prostate cancer ranged from 6 to 35 times (mean 20.2), and 10 of the 13 patients were injected from 1 to 11 times (mean 6.3) as outpatients. All 13 patients with prostate cancer were treated with hormone therapy and irradiation, and achieved PR. Complications of this drug delivery system were obstruction of the catheter or pump observed in 7 patients, dislocation of the catheter in 2, and obstruction of right external iliac artery in 1. CONCLUSION: These findings suggest that favorable therapeutic efficacies and an improved quality of life for patients can be obtained by intra-arterial chemotherapy using an implantable injection pump.

[Clinical evaluation of grade 3 superficial bladder cancer].

PURPOSE: To evaluate the treatment of grade 3 superficial bladder tumor, retrospective analysis of superficial bladder tumors was performed with special references to tumor progression and prognostic factors. MATERIALS AND METHODS: From 1976 to 1994, 247 cases with pTa-pT1 superficial bladder tumor were treated. Mean duration of follow up 77.3 months. These patients were divided into pTa (196), pT1 (52), grade 1 (61), grade 2 (196) and grade 3 (62). The prognostic factors were calculated with multivariate and univariate analysis. Tumor progression was defined as muscle invasion or distant metastasis. RESULTS: G3 tumor showed poor prognosis and was more frequent in tumor progression compared with G1 and G2 tumors (19.4% vs 0% and 1.6%). According to multivariate analysis, significant variables for actual survival rate were patient age and tumor grade. Tumor grade, recurrence and tumor configuration were also significant risk factor for cause-specific survival rate. By univariate analysis, patient age, tumor configuration, tumor size, multiplicity and concomitant CIS in G3 group were different from the other two G groups. In the G3 group, only recurrence was the predictable factor for progression. Analysis of prognosis and therapeutic modality revealed that G1 and G2 tumors were sufficiently controlled by endoscopic treatment. On the other hand, 21 cases (33.9%) of G3 tumor required total cystectomy after all. 11 cases of G3 group died of bladder cancer. Lymphatic involvement was detected in some cases of G3 tumor even if superficial. This factor showed a little relevance to poor prognosis of G3 tumor. CONCLUSION: Tumor grade was thought to be the most important risk factor of superficial bladder tumor. These results suggested that total cystectomy should be considered for the treatment of superficial G3 bladder cancer when recurrence occurs or conservative treatment is though to be failed.

[Two cases of long-term survival with hepatocellular carcinoma following targeting therapy with SMANCS/lipiodol].

We performed arterial infusion of SMANCS/Lipiodol in nineteen cases with hepatocellular carcinoma (HCC). We report two of these cases who survived for more than five years after the initial treatment. In case 1, HCC responded very well to the initial subsegmental infusion of SMANCS/Lipiodol with a prominent decrease in AFP level. In case 2, a 63-year-old male, repeated subsegmental infusion of SMANCS/Lipiodol for the local recurrence controlled the tumor well. In both cases, an approximately three-year period of complete remission passed until the tumor recurred. Arterial infusion of SMANCS/Lipiodol is expected to be a potent treatment for HCC. Its administration should be subsegmental, if possible, and should be repeated for local recurrence with a careful follow-up study.

[Application of laser spectroscopy for 13C-breath tests].

A new device Tunable Diode Laser Spectroscopy (LS) was developed for the analysis of isotope ratios of 13CO2/12CO2. Its applicability for breath tests was validated. The exhaled breath is collected in a 2 l aluminum bag, of which CO2 is separated by cryogenic system and introduced into LS. Repeat measurements (8 times) of two kinds of gases (delta 13C:-2.62 and - 1.14%) revealed 0.045% and 0.065% variation (IS.D.), from which precision of measurement was estimated as 0.2% (3S.D.). Seven healthy volunteers were given orally different dose of 13C-glycine (50 mg-200 mg in 5 subjects) and 13C-methacetin (30 mg and 150 mg in 2 subjects). One patient with gastric ulcer with helicobacter pylori (HP) infection was given 100 mg of 13C-urea. Serial breath samples (9-15) were taken and measured both by LS and mass spectrometer (MS) thereafter. Appreciable peaks were obtained at 30-50 min. after glycine and methacetin ingestions. The height of the peaks were dose dependent. Increased excretion of 13CO2 characteristics to HP infection was obtained in 13C-urea breath test. Measured values with LS were identical with those measured with MS (60 samples) with the range of difference within 0.2% (3S.D.). [Y (LS) = 1.02X (MS) + 0.55, r = 0.996] We conclude the LS is suitable for various 13C-breath tests.

[Polyangiitis overlap syndrome with multiple pulmonary aneurysms and renal vascular hypertension].

A 37-year-old man with multiple nodules on a chest X-ray film, hypertension, and proteinuria was admitted to our hospital in September, 1993. Thirteen years earlier, he had been admitted to another hospital because of bloody sputum, fever, and dyspnea. He also had slight oral aphthae and slight iridocyclitis. A chest X-ray film at that time showed bilateral diffuse reticular shadows, and his illness was diagnosed as acute interstitial pneumonitis. Since that time, he had been continuously receiving tapering doses of prednisolone. His chest symptoms were relieved, and the findings on the chest X-ray film resolved with that treatment. In 1993, he also had positive tests for HBs antigen and for HLA-B51, and he was found to have renal vascular hypertension. Chest CT films and a pulmonary angiogram showed multiple aneurysms and occlusions of pulmonary arteries. A renal angiogram showed multiple intraparenchymal aneurysms of renal arteries. A specimen taken by open-lung biopsy showed healed endoarteritis obliterans of muscular arteries and no alveolitis. His illness was diagnosed as polyangiitis overlap syndrome, because of the unclassified systemic necrotizing vasculitis.

Prediction of maximal oxygen uptake by squat test in men and women.

To examine the reproducibility of simple tests including step, squat and double quick tests, the respective tests were performed twice in 242 college women. The step test for 1, 2 or 3 min and the squat test for 1 or 1.5 min were adopted as simple endurance tests with superior reproducibility. Then 30 men (18-26 yr) and 32 women (18-34 yr) participated in a maximal O2 uptake (VO2max) test and the simple endurance tests. The scores (the sum of heart beats for 30-60, 90-120, 150-180 seconds in a sitting posture following the exercise) in the respective simple endurance tests were significantly correlated (P < 0.001) with VO2max per kilogram body weight in the men and women. For example, the equation relating VO2max to the score (X) of the squat test for 1.5 min in men was: VO2max (ml. kg-1. min-1) = -0.261X + 85.19 (r = -0.820, P < 0.001). The discrepancies between VO2max predicted by using the respective estimation equations and that determined by the direct method were 6.3% (by the step test for 3 min) approximately 8.1% (by the squat test for 1 min) in men and 4.7% (by the step test for 2 min) approximately 6.1% (by the squat test for 1 min) in women. Significant correlations were observed between the respective scores in the simple endurance tests (P < 0.001) and between % body fat and the scores (P < 0.01) in both men and women, but not for height vs. the scores. These results suggest that VO2max can be estimated not only by the step test for 1, 2 or 3 min but also the squat test for 1 or 1.5 min.

[Usefulness of estriol for the treatment of bone loss in postmenopausal women].

To assess the effect of estriol (E3) on bone loss in Japanese postmenopausal women, twenty-five women within 10 years after menopause received 2 mg/day of E3 and 2 g/day of calcium lactate (Ca) daily (n = 17; Group-E3 + Ca) or 2 g/day of Ca daily (n = 8; Group-Ca) for one year. Bone mineral density (BMD) at the lumbar spine L2 - L4 was evaluated by dual energy X-ray absorptiometry (DXA). In Group-Ca, BMD was significantly reduced (p < 0.05 vs. pretreatment) at the end of the study period. In contrast, in Group-E3 + Ca, the mean percent increase in BMD was 1.66%, which was significantly higher than that of Group-Ca (-3.08%; p < 0.001). As for the biochemical markers of bone metabolism, bone specific alkaline phosphatase (ALPIII) reflecting bone formation, and pyridinoline (Pyr) and deoxypyridinoline (D-Pyr) reflecting bone resorption, were significantly decreased (p < 0.001) in Group-E3 + Ca after six months of treatment. These data indicate that the acceleration of bone turnover usually observed after menopause was prevented by treatment with E3. As to the side effects of E3, only one of the 17 patients experienced a slight increase in vaginal discharge, but no other side effects were observed. Accordingly, reasonable compliance can be expected among postmenopausal women treated with E3 to prevent bone loss.

Ocular and orbital imaging.

The development of magnetic resonance imaging technology has proven to be a great breakthrough in diagnostic medical imaging, ophthalmologic imaging, and biomedical research. The anatomic detail demonstrated by an magnetic resonance image is a representation of at least three physical properties of static tissue: photon density and T1 and T2 relaxation times. Intrinsic differences in proton density and proton relaxation times of tissues allow excellent image contrast between various normal structures and high sensitivity for detecting pathologic states. This article discusses magnetic resonance imaging features of common ocular and orbital lesions, with emphasis on the potential of magnetic resonance imaging in ophthalmology.

[A case report--congenital esophago-bronchial fistula with esophageal diverticulum].

Congenital esophago-bronchial fistula with diverticulum is rare, and in Japan only 29 cases have been reported. A 59-year-old man had suffered from recurrent pulmonary infections and often complained of coughing while drinking water during the previous two years. An UGI series for follow-up of gastric ulcer revealed an esophago-bronchial fistula, located between the mid-esophagus and B10 of the right lung. We resected the diverticulum and the fistulous tract with a partial excision of S10. Histological examination revealed that the squamous epithelium with the muscularis mucosa of the esophagus was smoothly connected to the columnar epithelium of the bronchus. Thus, we diagnosed this fistula as congenital, not acquired. Comparison of the previous UGI series before presenting symptoms with that performed at this admission showed the lengthening and thickening of this congenital fistula during the three-years period. We suspected that the influence of acquired factors may cause a congenital fistula to manifest various clinical symptoms.

[Pulmonary lymphangiomyomatosis with tuberous sclerosis and endolymphatic stromal myosis of the uterus].

Pulmonary lymphangiomyomatosis is a rare disorder that affects only women of reproductive age. The lung shows numerous cystic changes and striking proliferation of smooth muscle around the airways, blood vessels, and lymphatic channels. We encountered a patient with pulmonary lymphangiomyomatosis who had repeated pneumothorax and bloody sputum over 10 years. The patient also had other lesions, including intracranial calcification, renal angiomyolipoma and endolymphatic stromal myosis of the uterus. The first two findings are known as tuberous sclerosis, and lymphangiomyomatosis is considered to be a form fruste of tuberous sclerosis. However, we know of no previous reports of uterine lesions in this disease. Lymphangiomyomatosis may be a systemic disorder of smooth muscle.

[A successful staged complete repair in a neonate with coarctation of aorta, ventricular septal defect, and left ventricular outflow tract obstruction--a new palliative operation].

An eight day old neonate with coarctation of aorta (CoA), ventricular septal defect (VSD), and severe left ventricular outflow tract obstruction (LVOTO) underwent a successful staged repair following a new palliative operation, which comprised aortic arch reconstruction, PTFE bypass graft between main pulmonary artery and descending thoracic aorta, and pulmonary artery banding (PAB) distal to the graft. Insertion of the bypass graft was aimed to allow enough systemic blood flow and reduces after load of ventricles after PAB. The second staged definitive operation was performed 21 days after the first operation and the procedure comprised partial resection of posterioly deviated infundibular septum via VSD, patch closure of VSD, and division of the graft. No evidence of progression of LVOTO was noted until the second stage operation despite the tight PAB. This new palliative operation, which does not need aid of cardiopulmonary bypass, is particularly effective for those with LVOTO, and can be safely applied even to seriously ill neonate. Thus, for the staged repair in aortic arch obstructive disease with VSD and severe LVOTO, this procedure can be a good alternative for palliative operation.

Interleukin 11 levels in patients with disseminated intravascular coagulation.

We assessed the blood interleukin 11 (IL-11) levels in patients with disseminated intravascular coagulation (DIC). The IL-11 level exceeded the detection limit, i.e., IL-11 was positive, in 14 of the 21 patients in the group with DIC complicated by sepsis, with a mean value of 20.4 pg/ml. In the group with DIC uncomplicated by infection, IL-11 levels were above the detection limit in 8 of the 17 patients, and the average level was 10.6 pg/ml. The difference between the IL-11-positive rate in the two groups was not significant. Although IL-11 levels tended to be higher in the group with sepsis as a complication, the difference was not significant. The elevated IL-11 levels observed in patients with DIC, with or without complication by infection, seem quite appropriate as a biological response to increased platelets.

Correlation of the OSR/ZRCI gene product and the intracellular glutathione levels in Saccharomyces cerevisiae.

The OSR (oxidative-stress-resistance) gene was cloned as the gene which enhances resistance against oxidative stress caused by lipid hydroperoxide in Saccharomyces cerevisiae. The nucleotide sequence of the gene was determined and found to be identical with the zinc-resistance-conferring (ZRCI) gene, which was cloned as the gene enhancing the resistance against zinc and cadmium. A knockout mutant of the OSR gene showed increased sensitivity to lipid hydroperoxide. The intracellular glutathione content in the knockout mutant decreased approx. 40% compared with that of wild-type cells, whereas it increased 3-fold in yeast overexpressing the OSR gene. Therefore the OSR/ZRCI gene product seems to regulate the intracellular glutathione content.
