[Antitumor effect of 5'-DFUR and PyNPase activity in colorectal cancer].

Following preoperative administration of 5'-deoxy-5-fluorouridine (5'-DFUR) in patients with colorectal cancer, histologic antitumor effects as well as pyrimidine nucleoside phosphorylase (PyNPase) activities in cancerous tissues were examined. Fifty-five patients were randomly divided into two groups: Twenty-five patients received 5'-DFUR orally at a daily dose of 800 mg for 3 weeks before operation (Group A) and twenty-nine received no medication (Group B). PyNPase activities in cancerous tissues obtained from resected specimens in Group A and B were 49.7 +/- 21.6 and 66.6 +/- 40.0 U/mg prot, respectively, which were significantly higher than 25.5 +/- 14.1 and 22.1 +/- 11.2 U/mg prot in normal colorectal tissues. No significant difference was observed in PyNPase activities in cancerous tissues between Group A and B. PyNPase activities in cancerous tissues in Group A were no different either from 59.2 +/- 40.0 U/mg prot in biopsy specimens before administration of 5'-DFUR. Histologic anti-tumor effects were as follows: Grade 2, 1 case Grade 1b, 5; Grade 1a, 10; and Grade 0, 9. Although no correlation was observed between antitumor effects and PyNPase activities in cancerous tissues, antitumor effects of 2 cases in which PyNPase activities in cancerous tissues before administration of 5'-DFUR showed more than 100 U/mg prot were Grade 2 and 1b, respectively.

[Assessment of blood flow by Doppler method].

There are two categories including Doppler sound and color-duplex sonography to assess blood flow using Doppler principle. Ankle pressure measurement has been a useful method to assess the severity of lower limb by Doppler sound. Color-duplex sonography could recognize total occlusion with no signal and significant stenosis with jet and turbulence, which could also differentiate embolism from thrombosis. Internal carotid lesion could be delineated to provoke minor stroke, and transient ischemic attack by this method. Ruptured aneurysm couldn't be diagnosed precisely by the color-duplex sonography. Doppler sound has been used to evaluate deep vein patency; however, recent available color-duplex sonography as well as gray scale ultrasonography could recognize thrombus formation precisely around the femoral, popliteal, and infrapopliteal deep vein.

Intraspinal extradural cysts communicating with adjacent herniated disks: imaging characteristics and possible pathogenesis.

We report two cases of intraspinal extradural cysts communicating with an adjacent herniated disk that we term "disk cysts." These cysts were well defined and homogeneous, and were present in the ventrolateral extradural space adjacent to a lumbar herniated disk. They had rim enhancement on contrast-enhanced MR images, and communication with a herniated disk was revealed by diskography.

Common diagnostic test panels for clinical evaluation of new primary care outpatients in Japan: a cost-effectiveness evaluation.

BACKGROUND: The Japan Society of Clinical Pathology (JSCP) has developed a guideline for common diagnostic test utilization in new primary care outpatients. To determine the scientific and economic validity of the JSCP panel testing system, we analyzed cost-effectiveness parameters of test panels advocated. METHODS: The "Essential Laboratory Tests" panel (2) [ELT(2) panel], a package of common diagnostic tests added to the ELT(1) baseline health-status screening panel, was applied to 540 new outpatients who visited the Comprehensive Medicine Clinics in an academic medical center during 1991 to 1997. A "useful result" (UR) of testing was defined as a finding that contributed to a change in a physician's diagnosis- or decision-making, relating to a "tentative initial diagnosis" (TID) obtained from history and physical examination alone. RESULTS: Clinical usefulness was demonstrated in 259 patients with ELT(2), in whom 398 URs were generated. Clinical effectiveness (UR/TID) ranged from 1.65 (hematological) to 0.088 (neurological disease), with a cost disparity from yen1251 ( approximately $10) to yen23 037 ( approximately $200) per UR. A total of 1137 tests generated URs. We further assessed the clinical effectiveness and economic efficiency (cost/UR) of ELT(1) and restructured panels. Use of the ELT(1) alone generated 244 URs in 167 patients. The poor efficiency of the ELT(1) panel was markedly improved with the addition of certain ELT(2)-specific tests in liver/pancreatobiliary, metabolic/endocrine, and cardiovascular disease groups. CONCLUSIONS: A wide disparity in the utility of ELT panels in different patient groups does not support the JSCP recommendation of their routine use for new outpatients. Selective test combinations should be used in selected patient groups.

T-cell-mediated immune responses in alloepithelial rejection after murine keratoepithelioplasty.

PURPOSE: To evaluate the role of delayed-type hypersensitivity (DTH) and cytotoxic T-lymphocyte (CTL) responses on alloepithelial rejection in a murine keratoepithelioplasty model. METHODS: C3H/He mouse corneal lenticules were grafted around the limbus in BALB/c mice, and alloepithelial rejection was assessed by microscopic evaluation. The relation between rejection scores and DTH or CTL responses to donor antigens was assessed by Spearman correlation analysis. Suppression of DTH responses by induction of anterior chamber-associated immune deviation (ACAID) was used to evaluate the contribution of DTH responses to allograft rejection. CTL responses were evaluated by in vitro and in vivo depletion of CD4+ or CD8+ cells. RESULTS: DTH responses, which developed 2 weeks postoperatively, correlated significantly with rejection scores (correlation coefficient r = 0.55). ACAID induction by anterior chamber inoculation of C3H/He splenocytes significantly suppressed allospecific DTH responses and alloepithelial rejection. While allospecific CTL responses also developed 2 weeks postoperatively and increased by 4 weeks, CTL responses did not exhibit positive correlation with rejection scores (r = -0.36, P = 0.076). The CTL responses were mediated exclusively by CD8+ cells. Although in vivo depletion of CD8+ cells abolished the induction of CTL responses, it did not prevent allograft rejection. Immunohistochemistry showed infiltration of CD4+ and CD8+ cells into the alloepithelium. Positive staining for interferon gamma but not interleukin 4 further implicated the participation of a DTH response. CONCLUSIONS: Allograft rejection after keratoepithelioplasty appears to be mediated primarily by DTH responses, with CTL responses playing only a minor role perhaps in modifying the development of rejection.

HTLV-I-associated myelopathy: acute progression and atypical MR findings.

We describe serial MR imaging findings in a patient with HTLV-I-associated myelopathy. The patient had acute progression of neurologic symptoms and exhibited swelling of the entire length of the spinal cord with increased T2 signal and contrast enhancement on MR imaging. The spinal cord became atrophic a few years later.

[A case of lung large cell carcinoma that survived for 8 years after resection of a solitary metastatic lymph node].

A 54-year-old man who underwent Miles operation for primary rectal cancer, had undergone right upper lobectomy for large cell carcinoma of the right lung 2 years before. In the Miles operation, the large lymph node was palpable in the mesenterium near the small intestine. Small bowel resection with lymphadenectomy was performed. Histological examination showed lymph node metastatic large cell carcinoma of the lung. The patient has survived for 8 years after the second operation without recurrence. Large cell carcinoma of the lung tends to metastasize to digestive organs, so careful follow-up is necessary after surgery. Surgical treatment for a solitary metastatic lesion of lung cancer is considered to be effective and to have a satisfactory prognosis.

Augmentation and suppression of action potentials by estradiol in the myometrium of pregnant rat.

The purpose of this study was to investigate the actions of estradiol on spontaneous and evoked action potentials in the isolated longitudinal smooth muscle cells of the pregnant rat. Single cells were obtained by enzymatic digestion from pregnant rat longitudinal myometrium. Action potentials and currents were recorded by whole-cell current-clamp and voltage-clamp methods, respectively. The acute effects of 17beta-estradiol on action potentials and inward and outward currents were investigated. The following results were obtained. The average resting membrane potential of single myometrial cells was -54 mV (n = 40). In many cells, an electrical stimulation evoked a membrane depolarization, and action potentials were superimposed on the depolarization. In some cells, spontaneous action potentials were observed. Estradiol (30 microM) slightly depolarized the membrane (ca. 5 mV) and attenuated the generation of action potentials by reducing the frequency and amplitude of the spikes. Afterhyperpolarization was also attenuated by estradiol (30 microM). On the other hand, in 5 of 35 cells, estradiol increased the first spike amplitude and action potential duration, while frequency of the spike generation and afterhyperpolarization were inhibited. In voltage-clamped muscle cells, estradiol inhibited both inward and outward currents. Acute inhibition or augmentation of spike generation by estradiol is due to the balance of inhibition of inward and outward currents. Inhibition of both currents also prevented afterhyperpolarization, causing potential-dependent block of Ca spikes.

[Local complications of the intestinal tract in patients with ulcerative colitis].

Various types of local complication of the intestinal tract are recognized in patients with ulcerative colitis. Among those local complications, massive bleeding, perforation and toxic megacolon require emergent operation in order to avoid fetal condition. Stricture of the colon, fustura formation and intra-abdominal abscess are rare complication in patients with ulcerative colitis, but sometimes require surgical treatment. Colitic cancer is another serious complication which develops highly advanced stage. Surveillance of the colon by endoscopy is necessary to obtain early diagnosis of the colitic cancer.

Lumbar spinal synovial cyst presenting with neurological deficit. A case report and review of the literature.

A spinal synovial cyst is a rare condition. We reported a case of lumbar spinal synovial cyst presenting neurological deficit. A 78-year-old woman was admitted to our hospital with the low back pain radiating to the left buttock. A myelography with computed tomography and magnetic resonance imaging revealed an extradural cystic lesion at the L5 and S1 level. At the time of surgery, a standard posterior approach was used to expose the posterior elements from L5 to S1. An en bloc laminectomy and total removal of the cyst was performed at the L5 to S1 level. The postoperative recovery was uneventful except for a slight pain in the left leg which persisted for some time.

Effect of water, saliva and blood contamination on bonding of metal brackets with a 4-META/MMA/TBB resin to etched enamel.

PURPOSE: To investigate the influence of contamination by water, human saliva, and blood on the bonding of metal brackets with a 4-META/MMA/TBB resin to etched enamel. MATERIALS & METHODS: For compressive shear bond strength measurements, the surfaces of bovine enamel were prepared either by etching with 37% phosphoric acid solution for 10, 30, or 60 s and then dried with oil-free compressed air for 10 s, or by contaminating with water, human saliva, and blood. Brackets were applied with Super Bond under loads of 200, 400, or 600 g. The bonded samples were immersed in water for 1 d or thermo-cycled for 500 cycles, and the mean shear bond strengths were compared using two-way ANOVA and Scheffé's multiple comparisons test at P = 0.05. RESULTS: The bond strengths to enamel etched for 60 s were independent of the variable load, regardless of the type of contamination. A short etching duration provided higher bond strengths than extended etching of samples contaminated with saliva and blood. The bond strengths to enamel etched for 10 s after thermal stress and immersion in water were from 11.4 to 30.4 MPa. The samples contaminated by saliva showed the lowest bond strength, and thermal stress did not reduce the bond strengths.

Non-convulsive status in the ring chromosome 20 syndrome: a video illustration of 3 cases.

We report the clinical history and the video-EEG recordings of three young adults with r(20), who exhibited repeated episodes of non-convulsive status epilepticus, often triggered by emotional events. This recognizable clinical entity associates a slight mental deficiency, behavioural disorder and epilepsy, often resistant to treatment and with a particular clinical presentation. We demonstrate with video EEG recording, atypical absence status triggered by emotional behaviour with ictal EEG characterized by bifrontal slow waves, often rhythmic, or sharp waves. Our report highlights the characteristic features of non-convulsive status in this electro-clinical syndrome.

Phospholipase cbeta4 is specifically involved in climbing fiber synapse elimination in the developing cerebellum.

Elimination of excess climbing fiber (CF)-Purkinje cell synapses during cerebellar development involves a signaling pathway that includes type 1 metabotropic glutamate receptor, Galphaq, and the gamma isoform of protein kinase C. To identify phospholipase C (PLC) isoforms involved in this process, we generated mice deficient in PLCbeta4, one of two major isoforms expressed in Purkinje cells. PLCbeta4 mutant mice are viable but exhibit locomotor ataxia. Their cerebellar histology, parallel fiber synapse formation, and basic electrophysiology appear normal. However, developmental elimination of multiple CF innervation clearly is impaired in the rostral portion of the cerebellar vermis, in which PLCbeta4 mRNA is predominantly expressed. By contrast, CF synapse elimination is normal in the caudal cerebellum, in which low levels of PLCbeta4 mRNA but reciprocally high levels of PLCbeta3 mRNA are found. These results indicate that PLCbeta4 transduces signals that are required for CF synapse elimination in the rostral cerebellum.

Right ventricular responses to vagus stimulation of fibers to discrete cardiac regions in dog hearts.

To investigate the role on the right ventricular function of the parasympathetic nerves in discrete cardiac regions, we studied the effects of stimulation of the intracardiac parasympathetic nerves to the atrioventricular (AV) nodal area (AVPS) or sinoatrial (SA) nodal area (SAPS), and stimulation of the cervical vagus nerves (CVS) on the right atrial and ventricular function in the autonomically decentralized heart of the anesthetized dog. AVPS prolonged AV conduction time (AVCT) and decreased the ventricular rate without changes in atrial rate, right atrial 'a' wave pressure (RAP), its first derivative (RAd P/dt), right ventricular pressure (RVP) and its first derivative (RVd P/dt) in the spontaneously beating heart. On the other hand, AVPS prolonged the AVCT in the atrium paced electrically and further decreased the ventricular rate with decreases in ventricular pressure responses but not atrial responses. SAPS decreased atrial and ventricular rates without AVCT changes and attenuated the right atrial and ventricular pressure responses. In the atrial paced heart, SAPS decreased the ventricular responses less than those responses in the spontaneously beating heart with similar decreases in the atrial pressure responses, but it changed neither the AVCT nor ventricular rate. On the other hand, CVS decreased the atrial and ventricular rate or pressure responses, and increased the AVCT. CVS in the paced heart also decreased the atrial and ventricular pressure responses with the prolongation of AVCT. The cardiac responses to parasympathetic stimulations were abolished by atropine. These results suggest that AVPS decreases ventricular rate without direct right ventricular pressure effects but the negative chronotropic effects of AVPS and SAPS decrease the ventricular pressure responses rate-dependently in the dog heart.

Studies on selectin blockers. 7. Structure-activity relationships of sialyl Lewis X mimetics based on modified Ser-Glu dipeptides.

We have previously found that heterochiral fucodipeptides, L-Ser-D-Glu (3a) and D-Ser-L-Glu (3b), exhibited up to 20-100 times more potency than a sialyl Lewis X (sLeX, 1) and a 3'-sulfated Lewis X analogue (2) toward E-selectin binding and have also proposed, from molecular dynamics calculation, that their strong activities would depend on a possible formation of the type II and/or type II' beta-turn of compounds 3a,b (Tsukida, T.; Hiramatsu, Y.; Tsujishita, H.; Kiyoi, T.; Yoshida, M.; Kurokawa, K.; Moriyama, H.; Ohmoto, H.; Wada, Y.; Saito, T.; Kondo, H. J. Med. Chem. 1997, 40, 3534-3541). To clarify our hypothesis, we synthesized several analogues of compounds 3a,b and investigated their structure-activity relationships. As a result, it was indicated that the type II and/or type II' beta-turn conformation would be a comparatively tight form and would play important roles in favorable binding to E-selectin. These findings indicate that sLeX mimetics with type II and type II' beta-turn dipeptides could be a useful methodology for the design of an active selectin blocker.
