Effects of penetrating keratoplasty rejection on the endothelium of the donor cornea and the recipient peripheral cornea.

PURPOSE: To determine the effects of penetrating keratoplasty rejection on the recipient endothelium. METHODS: Twenty transplanted corneas of 20 keratoconus patients were enrolled in this study. Ten of the corneas had undergone allograft rejection and the other 10 grafts had an uneventful postsurgical course. The endothelium of the donor cornea and the endothelium of the recipient peripheral cornea were evaluated by wide-field specular microscopy. RESULTS: The transplanted corneas that underwent allograft rejection showed a marked decrease in endothelial cell density not only in central donor cornea but also in the recipient peripheral cornea compared with that in the uneventful transplanted corneas. Additionally, the recipient endothelium had significant pleomorphism in the rejection group. There were no morphologic differences in the peripheral donor cornea between the control group and the rejection group. CONCLUSIONS: Our findings indicate that the recipient peripheral endothelium is also affected by allograft rejection, perhaps because of the contribution of recipient endothelium to the wound healing process after allograft rejection.

The mitogenic activity of hepatocyte growth factor on rat hepatocytes is dependent upon endogenous transforming growth factor-alpha.

Both transforming growth factor-alpha (TGF-alpha) and hepatocyte growth factor (HGF) induce DNA synthesis in hepatocytes in vitro and in vivo. Hepatic and circulating levels of HGF have been reported to increase before an increase in TGF-alpha levels in several rat models of liver regeneration. In addition, serum TGF-alpha levels increase after an increase in serum HGF levels in patients with either partial hepatectomy or acute hepatitis. In this study, we investigate the significance of TGF-alpha in hepatocyte proliferation. TGF-alpha contents and DNA synthesis in cultured rat hepatocytes increased in response to HGF addition to the culture medium in a dose-related manner. These increases were suppressed by the addition of anti-sense TGF-alpha mRNA oligonucleotide. Furthermore, the addition of anti-TGF-alpha rabbit IgG suppressed the increase in DNA synthesis. When the anti-TGF-alpha antibody was administered to rats after partial hepatectomy, the number of mitotic hepatocytes was reduced in comparison to rats treated with normal rabbit IgG. These results were observed even though hepatic HGF levels were increased equally in rats given either anti-TGF-alpha antibody or normal rabbit IgG. Our results suggest that HGF stimulates TGF-alpha production in rat hepatocytes, and that the mitogenic activity of HGF depends on endogenous TGF-alpha activity.

Exogenous aspartate neurotoxicity in the spinal cord under metabolic stress in vivo.

BACKGROUND: Considerable evidence exists that neurotoxicity of excitatory amino acids is related to the neuronal injury, including paraplegia. However, little is known about aspartate neurotoxicity in the spinal cord in vivo. We evaluated the detrimental effects of exogenous aspartate on spinal cord neurons under metabolic stress. METHODS: New Zealand white rabbits underwent an infrarenal aortic isolation. Group A animals (n = 7) received segmental aspartate 50 mmol/L) infusion for 10 minutes. Group B animals (n = 7) received saline as a negative control. Group C animals (n = 5) received segmental aspartate 100 mmol/L) infusion for 5 minutes. Group D animals (n = 7) were pretreated with segmental infusion of (+)-5-methyl-10,11-dihydro-5H-dibenzo[a,d]cycloheptan-5,10-imine (MK-801) (6 mg/kg), a noncompetitive N-methyl-D-aspartate (NMDA) receptor antagonist for 1 minute, followed by segmental infusion of aspartate (50 mmol/L) for 9 minutes. Group E animals (n = 7) received vehicle only, followed by aspartate (50 mmol/L) infusion as a control of group D. Neurologic status was assessed at 12, 24, and 48 hours after operation using the Tarlov score. RESULTS: Group A animals exhibited paraplegia or paraparesis with marked neuronal necrosis. Group B and C animals recovered fully. Group D animals showed significantly better neurologic function (p = 0.0007) compared with group E animals that exhibited paraplegia or paraparesis. CONCLUSIONS: Exogenous aspartate can have detrimental effects on spinal cord neurons under metabolic stress. This model may be useful in assaying neuronal injury mediated by NMDA receptor in vivo.

Continuous pulmonary perfusion during cardiopulmonary bypass prevents lung injury in infants.

BACKGROUND: Lung injury after cardiopulmonary bypass is a serious complication for infants with congenital heart disease and pulmonary hypertension. Excessive neutrophil sequestration in the lung occurring after reestablishment of pulmonary circulation implies that interaction between neutrophils and pulmonary endothelium is the major cause of lung injury. METHODS: Thirty infants with either ventricular septal defect or atrioventricular septal defect and with pulmonary hypertension were enrolled in this study. We performed continuous pulmonary perfusion during total cardiopulmonary bypass on 16 patients (perfused group) and conventional cardiopulmonary bypass on 14 patients (control group). PaO2/FiO2 and neutrophil counts were assessed from immediately before surgery to 24 hours after termination of cardiopulmonary bypass. RESULTS: PaO2/FiO2 was higher in the perfused group than in the control group, and the difference was significant throughout the study period. Neutrophil counts decreased below prebypass values in both groups at 30 minutes after aortic unclamping, and the difference was significant in the control group but was not in the perfused group. Duration of postoperative ventilatory support was significantly less in the perfused group. CONCLUSIONS: Our study demonstrates that arrested pulmonary circulation during cardiopulmonary bypass is the major risk factor of lung injury and that continuous pulmonary perfusion is effective in preventing lung injury.

A high incidence of Staphylococcus aureus colonization in the external eyes of patients with atopic dermatitis.

OBJECTIVE: To determine the frequency distribution of bacteria on the external surface of eyes of patients with atopic dermatitis (AD) and to investigate the relationship between the frequency of bacterial colonization and the grade of atopy or ocular diseases associated with AD. DESIGN: Comparative cross-sectional study. PARTICIPANTS: Thirty-six AD patients (mean age, 24.5 years) and 16 nonatopic, age-matched control participants (mean age, 25.5 years). INTERVENTION: The eyelid margins and conjunctival sacs were scraped with sterile swabs. These samples were inoculated into aerobic and anaerobic culture media. MAIN OUTCOME MEASURES: The frequency distribution of bacteria isolated from the eyelid margins and conjunctival sacs. RESULTS: Bacteria isolated from AD patients were: Staphylococcus aureus in 21 of 36 patients (including methicillin-resistant Staphylococcus aureus in two patients); Staphylococcus epidermidis in two patients (including methicillin-resistant Staphylococcus epidermidis in one patient); other coagulase-negative Staphylococcus in six patients;alpha-streptococcus in three patients; Corynebacterium species in three patients; Neisseria species in two patients; and Propionibacterium acnes in one patient. From the nonatopic control participants, we isolated S. aureus in one patient, S. epidermidis in two patients and alpha-streptococcus in one patient. S. aureus was isolated from 67% of the AD patients, and any type of bacteria was isolated from 86% of the patients. These rates were significantly higher than those of nonatopic control participants (6% S. aureus and 25% any bacteria). There was no significant relationship between the frequency distribution of bacteria and the grade of atopy or associated ocular diseases. CONCLUSIONS: High rates of bacterial colonization, especially S. aureus, were found in the conjunctival sacs and eyelid margins of AD patients. In case management of AD patients, this unique distribution of bacteria must be carefully considered.

Comparison of topographic indices that correlate with visual acuity in videokeratography.

OBJECTIVE: To evaluate the relationship between the best spectacle-corrected visual acuity (BSCVA) and two quantitative indices of the anterior corneal surface obtained by videokeratography. DESIGN: Prospective, single center, comparative, observational study. PARTICIPANTS: Eighty-nine normal eyes and 52 eyes with keratoconus with contact lens-corrected visual acuity of 20/20 or better. INTERVENTION: Videokeratography was performed with the TMS-2 and the CAS system 2000. MAIN OUTCOME MEASURES: The relationship between the BSCVA recorded in log minimal angle of resolution (logMAR) units, the surface regularity index (SRI), and the predicted corneal acuity (PCA) were assessed by linear regression analysis. RESULTS: The BSCVAs for all eyes ranged from 0.82 to -0.30 logMAR units. BSCVA was highly correlated with the SRI (r = 0.70, P < 0.0001) and the PCA (r = -0.61, P < 0.0001). There was no statistical difference in the regression slopes and the intercepts for the estimated BSCVA using the SRI and measured BSCVA, and the estimated BSCVA using PCA and measured BSCVA. CONCLUSIONS: Two quantitative topographic indices, SRI and PCA, are useful for estimating the effect of irregular astigmatism on visual acuity even though both indices quantify different aspects of the anterior surface of the cornea.

The effect of disparity on the very earliest ocular following responses and the initial neuronal activity in monkey cortical area MST.

Movement of the visual scene evokes tracking movement with the eyes (ocular following response, OFR) at short latency (Miles, F.A., Kawano, K., Optican, L.M., 1986. Short-latency ocular following responses of monkey. I. Dependence on temporospatial properties of visual input. J. Neurophysiol. 56, 1321-1354). We examined the effect of binocular disparity on the initial OFR. The dependence of the OFR on horizontal disparity steps was studied in three monkeys (Macaca fuscata), and the associated unit discharges in the medial superior temporal area (MST) were studied in two of these. Based on the changes in eye position over the period 50-83 ms (measured from stimulus onset), the initial OFR showed clear dependence on the disparity imposed during the preceding centering saccade. The disparity tuning curves were S-shaped with a peak at a small crossed disparity and a trough at uncrossed disparities. Based on the changes in discharge rate over the period 40-73 ms (measured from stimulus onset), almost all OFR-related MST neurons (80/83, 96.4%) showed significant dependence on the disparity step (Student's t-test, P < 0.05). The early neuronal responses of the majority of units (41/75, 55%) had disparity tuning curves resembling those for the OFR, which peaked at small crossed disparities. These findings suggest that the neurons in the MST contain information on binocular disparity in their short-latency discharges, and are involved in the neural basis of the OFR's dependence on horizontal disparity.

Molecular modelling and conformational analysis of novel glycoprotein (Gp) IIb/IIIa antagonists. Molecular orbital calculation and the condensed heterocyclic derivatives.

A naphthalene compound was chosen as lead compound to develop a new series of fibrinogen receptor antagonists. Eight new compounds with different condensed heterocyclic parts were prepared and their in vitro activities were evaluated. 5-Amidinobenzofuran compound 2, 6-amidinobenzothiophene 7, and 5-amidinofuro[2,3-b]pyridine 8 were more active than lead compound 1. Molecular orbital calculation studies suggested the presence of a preferred spatial orientation between the amidine and the carboxylic acid.

Long-term viability of cryopreserved cultured epithelial grafts.

Human cultured epithelial grafts are frozen for long-term preservation. To assess the viability of these stored grafts, their cell survival rate and colony-forming efficiency of grafts cryopreserved at -135 degrees C and at -80 degrees C were followed over time. Flow cytometry showed that the cell survival rate of the grafts cryopreserved at -135 degrees C for 1 month, 6 months and 1 year averaged 89.3%, 61.7% and 61.6%. Cryopreservation at -80 degrees C maintained cell survival rate as well for 1 month, but after 6 months of cryopreservation survival was reduced at -80 degrees C (35.2%) compared with that of -135 degrees C. In histological examination, the cell structure and basal layer were very well preserved after 6 months of storage at -135 degrees C, but not at -80 degrees C. Cell survival rate at -135 degrees C was also assessed by colony-forming efficiency. Colony-forming efficiency of the grafts cryopreserved for 1 month, 6 months and 1 year averaged 66.1%, 58.5% and 55.1% of control (noncryopreserved) grafts. These findings suggest that, even when cultured epithelial grafts are subjected to long-term cryopreservation, cell viability remains sufficient, reculturing is possible, and that graft banking could be used for clinical applications.

The long-term outcome of hearing preservation following vestibular schwannoma surgery.

OBJECTIVE: The aim of this study is to describe the long-term outcome of preserved hearing after vestibular schwannoma surgery. METHODS: Subjects are 20 unilateral vestibular schwannoma patients whose class A hearing of the AAO-HNS classification was preserved successfully after tumor removal. Hearing preservation surgery was attempted via the middle cranial fossa (MCF) or the extended MCF approach. The follow-up periods ranged from 2 to 16 years. The outcome measures included the pure tone average (PTA) and speech discrimination score (SDS). RESULTS: PTA was maintained in less than 30 dB in 11 out of 20 patients within 2 years follow-up, six out of 13 patients within 4 years follow-up, and two out of five patients within 6 years follow-up, respectively. SDS was maintained in more than 70% in 17 out of 20 patients within 2 years follow-up, ten out of 13 patients within 4 years follow-up, and three out of five patients within 6 years follow-up, respectively. CONCLUSIONS: The preserved hearing maintained about more than 40% of the patients with the class A hearing in every 2 years follow-up. From this result, we can conclude that the hearing preservation surgery could be one of the best treatments for vestibular schwannoma patients with class A hearing. However, further study will be needed to clarify the mechanism of the deterioration in hearing after hearing preservation surgery.

Minimizing radial-keratotomy-induced diurnal variation in vision using contact lenses.

A 41-year-old man with 16 radial keratotomy (RK) incisions in each eye reported a paradoxical diurnal variation in vision in both eyes with low Dk/L soft contact lenses. After rk, the patient experienced the conventional diurnal change a morning-to-evening mean (right and left eyes) myopic change of -1.80 diopters (D). However, while wearing low Dk/L contact lenses, a paradoxical morning-to-evening mean hyperopic change of 2.30 D was found. The diurnal variation was minimized (0.50 D) by wearing high Dk/L contact lenses. These results suggest that contact lenses can be used to treat diurnal variation in manifest refraction after RK by inducing appropriate stress.

Absence of simple partial seizure in temporal lobe epilepsy: its diagnostic and prognostic significance.

The diagnostic and prognostic significance of the absence of simple partial seizures (SPS) immediately preceding complex partial seizures (CPS) was examined in patients with temporal lobe epilepsy. The status of self-reported SPS in 193 patients with temporal lobe epilepsy who had surgical therapy more than 2 years ago was reviewed. Before surgery, 37 patients never experienced SPS before CPS (Group A), 156 patients either always or occasionally had SPS before CPS (Group B). The frequency of mesial temporal sclerosis (MTS) was lower and the age at onset of epilepsy was higher in Group A. The seizure focus was in the language-dominant temporal lobe in 73% of the cases in Group A, compared with 40% in Group B. The surgical outcome did not differ between the two groups. The findings suggest that temporal lobe seizures without preceding SPS tend to originate in the language-dominant temporal lobe that contains a pathologic etiology other than MTS, especially in the lateral temporal lobe. The surgical outcome in patients without SPS is similar to that in patients with SPS.

Comparison of skin prick test with serially diluted wild-type and genetically engineered recombinant Der f2.

BACKGROUND: C8/119S, a genetically engineered less allergenic mutant of group II allergen (Der f2) of house dust mite, Dermatophagoidesfarinae, was constructed in order to reduce the risk of anaphylactic reactions of allergen specific immunotherapy. OBJECTIVE: To further evaluate, with a larger number of mite-allergic patients, the safety of C8/119S for the treatments of humans. METHOD: We tested the dose-dependent responses of 20 mite-allergic volunteers to a skin prick test with C8/119S and wild-type recombinant Der f2 and compared the biologic potentials of these allergens to induce type I allergic reactions. In a separate experiment, we compared IgE binding capacities to C8/119S and to wild-type recombinant Der f2 in individual sera from 34 mite-allergic donors. RESULTS: The concentration of C8/119S needed to induce positive skin prick test (SPT) reaction was at least 100 times more than that of recombinant Der f2 in 95% of the volunteers tested. Consistent with this result, IgE binding data showed that 85% of the mite-allergic donors had little or no detectable IgE bound to C8/119S. Our data also shows that a minority of mite-allergic patients responded in a similar manner to both C8/119S and wild-type recombinant Der f2. CONCLUSION: Our data confirms that C8/119S is much less allergenic and thus can be used safely for immunotherapy of most of mite-allergic patients. Care should still be taken because, in a minority of patients, C8/119S may cause similar type I allergic reactions as does wild-type recombinant Der f2.

Water extract of defatted rice bran suppresses visceral fat accumulation in rats.

Rice bran has been reported to inhibit pancreatic lipase activity in vitro. This action shows that administration of rice bran may result in a decrease in plasma triglyceride levels and suppress accumulation of fat in vivo. We administered water extract of defatted rice bran (WED-rice bran) to rats to determine its effects. Single administration of WED-rice bran at a dose of 1 g/kg body weight caused a decrease in plasma triglyceride levels in fat emulsion induced hypertriglyceridemic rats. Four week administration of WED-rice bran suppressed accumulation of visceral fat and body weight gain without influencing food consumption, liver function, and renal function. These results indicate that a reduction of plasma triglycerides and suppression of visceral fat accumulation may be induced by pancreatic lipase inhibition caused by administration of WED-rice bran.
