Noninvasive diagnosis of thrombus in the heart and large vessels--usefulness of two-dimensional echocardiography and X-ray CT.

The usefulness of two dimensional echocardiography (2-D echocardiography) and x-ray computed tomography (CT) for the diagnosis of thrombi in the cardiac cavity and large vessels was studied by comparing them with the findings of invasive methods. Among 56 subjects with mitral stenosis, left atrial thrombi were noted in 12 cases (16 regions) by CT and 8 cases (9 regions) by 2-D echocardiography. In 16 subjects who underwent operations, one false negative case by CT and 3 false negative and one false positive cases by 2-D echocardiography were found. In 80 subjects with myocardial infarction 2-D echocardiography, CT and left ventriculography (LVG) were performed at approximately the same time. Thrombi were detected in 10 subjects (12.5%) by 2-D echocardiography, in 15 (18.8%) by CT and in 14 (17.5%) by LVG. Although mural thrombi in abdominal aortic aneurysm were detected very easily, thin thrombi surrounding the false lumen of the dissecting aneurysm were not detected ultrasonographically. These thrombi were only detected by the enhanced CT. Our results show the usefulness of both methods for detecting thrombi in the heart and large vessels. CT can distinguish the thrombi more clearly than 2-D echocardiography, but 2-D echocardiography is performed more easily, safely and economically than CT.

Repeated micro-monocyte adherence inhibition assay: a new technique of leukocyte adherence inhibition to detect tumor immunity in patients with hepatoma.

A microplate leukocyte adherence inhibition (micro-LAI) assay was performed with peripheral blood mononuclear cells obtained from patients with hepatoma and control subjects (including healthy donors and patients with other diseases). Cell extracts of human hepatoma cells (HCC-M) and human hepatic cells (Chang liver cell) in tissue culture were prepared by sonication followed by centrifugation. The supernatants of these two cell lines were used as a specific antigen and a nonspecific antigen, respectively. It was found that monocytes were major indicator cells and that monocytes produced an LAI reaction in the absence of lymphocytes. Therefore, a repeated microplate monocyte adherence inhibition (MAI) assay was developed, in which the monocyte population of adherent cells is increased by removing nonadherent cells after an initial assay in fetal calf serum-containing medium without test antigens, and monocytes are counted selectively as peroxidase-positive cells in a subsequent second assay with test antigens. With regard to sensitivity and reproducibility, the repeated micro-MAI assay is superior to a micro-MAI assay in which the initial assay is omitted although monocytes are selectively counted. With this simple and sensitive technique a hepatoma-associated immune response to the extract of HCC-M was detected in 16 out of 22 patients (73%) with hepatoma, whereas the false-positive rate was 7% (3/41) in all control subjects.

Detection of antibodies to Chang liver cell in sera from patients with chronic liver diseases by 125I-labelled protein A binding assay and the effect of prednisolone and 6-mercaptopurine treatment on the level of the antibodies.

Antibody binding to living Chang liver cell was measured in sera from 71 patients with various chronic liver diseases using 125I-labelled protein A binding assay. The level of antibody binding to Chang liver cell was significantly elevated in sera from patients with chronic active hepatitis (CAH), chronic persistent hepatitis (CPH) and liver cirrhosis as compared to those from healthy donors, but not in sera from patients with fatty liver. There was no detectable antibody binding to HeLa cells in those sera. The antibody binding to Chang liver cell was blocked by a human liver specific protein (LSP) preparation. The levels of antibody binding to Chang liver cell were significantly higher in patients with CAH than patients with CPH. On the other hand, the level of antibody binding to Chang liver cell was significantly decreased in sera from patients with CAH after a treatment with prednisolone (PSL) for 2 months and a subsequent combined administration of 6MP and a maintenance dose of PSL for 1 month. These results suggest that antibodies to Chang liver cell are closely correlated with the activity of chronic liver disease and that PSL and 6MP treatment can reduce the level of the antibodies.

[Echocardiographic manifestations of postinfarction perforation with extensive dissection of the interventricular septum: report of an autopsied case].

This is a report of the echocardiographic features of postinfarction perforation associated with dissection of the interventricular septum (IVS) in a 61-year-old woman. She had acute myocardial infarction and was admitted to a nearby hospital, and later admitted to our CCU because hemodynamic deterioration. On admission, she was somnolent and hypotensive (90/64 mmHg), with sinus tachycardia, marked cyanosis, and peripheral edema. On auscultation there were a harsh holosystolic murmur over the LLSB and moist rales in both lung fields. An ECG revealed extensive anterior and inferior infarctions. Catheterization data confirmed O2 step-up in the right ventricle. Two-dimensional echocardiograms demonstrated extensive dissection of the IVS characterized by an echo-free lumen extending from the apex to the cardiac base in the long-axis and partly reaching the left ventricular free wall in the short-axis with the concomitant paradoxical movement of the IVS. Autopsy disclosed marked hemorrhagic infarction and extensive dissection of the IVS forming a lumen (5 X 5.5 X 2 cm3) corresponding exactly to the echocardiographic free space. In addition, a shunt between the right and left ventricles was confirmed by the presence of two perforations near the apex on the right and left sides of the IVS, whose diameters were 6 and 10 mm, respectively.

Decreased natural killer activity in patients with liver cirrhosis.

Natural killer (NK) activity of peripheral blood mononuclear cells from patients with non-alcoholic liver cirrhosis (LC) (30 cases), patients with other, non-malignant diseases (41 cases), and healthy subjects (36 cases) were investigated using 51Cr-labelled CCRF-CEM and K562 target cells. NK activity in patients with LC was lower than that in healthy subjects and that in patients with other, non-malignant diseases. The differences were statistically significant by Student's t-test and the Wilcoxon rank test. There was no difference of NK activity between serologically hepatitis B surface antigen (HBsAg)-positive and -negative patients.

Establishment of a cell line (HCC-M) from a human hepatocellular carcinoma.

A continuous human cell line was established from a hepatocellular carcinoma of an HBsAg-positive Japanese male. The cell line, designated HCC-M, grows as an adhering monolayer with a doubling time of 24 h in medium RPMI-1640 supplemented with 10% FCS and grows with 30% clonal efficiency in soft agar. The cells have been shown by light and electron microscopy to be of epithelial type. When they were transplanted subcutaneously into the back of athymic nude mice (BALB/c, nu/nu), tumors developed at the sites of inoculation, which were shown to be hepatocellular carcinoma, similar in morphology to the original tumor from which they were derived. HCC-M had a chromosome mode of 63 with five identifiable marker chromosomes. HCC-M produced albumin at the 10th passage but this property was lost by the 30th passage. This cell line has not secreted alpha-fetoprotein. Hepatitis B viral particles or HBsAg have not been demonstrated in the cells from the primary culture nor in several subsequent subcultures tested.

Evaluation of cardiac rehabilitation in relation to prognosis.

We designed a rehabilitation program for patients with post-myocardial infarction by modifying the 14-step program of Emory University into a more suitable form for a Japanese. The usefulness of this rehabilitation program was evaluated by comparing the clinical course of our patients with that of the patients in our affiliated institutions, where patients had no systematic rehabilitation therapy. The following results were obtained: 1) Most patients of the rehabilitation-completed group were living a non-restricted life 6 months after discharge. The life style of the rehabilitation group even including the rehabilitation-non-completed group, was far better than that of the non-rehabilitation group. 2) Many patients of the rehabilitation group were working at the same job as before infarction 3 years after discharge, while a substantial number of the patients of the non-rehabilitation group had changed their job or retired within 3 years after discharge. 3) The reason for changing job or retiring was primarily subjective symptoms or objective findings in the rehabilitation-completed group, while it was mostly fear in the non-rehabilitation group. 4) Patients who could not complete our program were mainly old patients of over 70 years of age, patients with extensive anterior infarction and subendocardial infarction and ones with complications such as shock and cardiac failure.

Combination chemotherapy in retroperitoneal poorly differentiated myxoid liposarcoma: a report of a case.

We describe a case of a 47-year-old man with retroperitoneal poorly differentiated myxoid liposarcoma. The retroperitoneal invasion of the sarcoma caused hydroureter-nephrosis, rectal stenosis and also exaggerated steroid induced diabetes mellitus. A remarkable regression was achieved by systemic combination of chemotherapy with prednisolone, cyclophosphamide, vincristine sulfate and actinomycin D. This combination chemotherapy is worth trying if a patient has inoperable liposarcoma. It can also be used to reduce local recurrence rate after surgical or radiation therapy, or both, against liposarcoma.

[Positron computed tomography for the assessment of myocardial infarction].

Positron computed tomography (PCT) was performed on 2 normal subjects and 3 patients with myocardial infarction. The PCT device "Positologica-II" developed at the National Institute of Radiological Science Japan was used in this study (Fig. 1). This is a whole body positron CT consisting of 3 detector rings that provide 5 sliced images simultaneously. The radiopharmaceutical 13N-ammonia (13NH3) synthesized by the cyclotron at the institute was used as an indicator of myocardial blood flow. Immediately after the intravenous administration of 13NH3, five serial 1-min PCT scans were performed with 3 additional scans that provided 15 static images spaced 6 mm apart. Cardiac blood pool images were obtained from the first scan. A patient with extensive anterior myocardial infarction revealed an outward bulging of the blood pool compatible with aneurysmal formation (Fig. 2). Three additional late scans provided high quality cross-sectional images of the distribution of 13N in the left ventricular myocardium. Although 13N distribution in myocardium was uniform in normal subjects (Figs. 3 & 4), the patients with myocardial infarction revealed defects in the accumulation of 13N at the sites corresponding to the infarction (Figs. 5, 6, 7 & 8). The conclusions are as follows: (1) serial PCT scans with 13NH3 provide cardiac blood pool images, and (2) myocardial 13N tissue concentration seems to reflect myocardial blood flow.

Membranous lipodystrophy (lipomembranous polycystic osteodysplasia). Two case reports.

Membranous lipodystrophy, a hereditary disease, is characterized by symmetric multiple cystic bone lesions and progressive neuropsychiatric symptoms. In two patients, a 22-year-old woman and a 30-year-old man, as in others reported in the literature, neither the etiology nor the pathogenesis was apparent. Histologically, the lesions consisted of convoluted membranes interlaced with lipoid structures.
