Two hundred sixty-six patients took part in a multicenter comparative study of doxazosin and prazosin. Both drugs produced a significant reduction (p less than 0.001) in blood pressure and no increase in heart rate. Blood pressure was considered "markedly decreased" or "decreased" in 70.8% of patients treated with doxazosin and 70.0% of patients treated with prazosin. No statistically significant between-group differences in antihypertensive efficacy were observed. Both doxazosin and prazosin were well tolerated; seven patients (5.6%) in each group had the therapy withdrawn.
A patient with an extra digit arising from the forearm is reported. This is the first such case reported in the English-language literature.
Carbonic anhydrase (CA) is an extremely basic zinc metalloenzyme with a wide phyletic distribution, and the enzyme is important for the regulation of acid-base status. A novel autoantibody reactive with carbonic anhydrase was demonstrated. Several different classes of CA are known in mammals. Using the immuno blotting method and and immun-dot analysis, we found this autoantibody to be reactive with CA in the sera from patients with Sjögren's syndrome (20.8%), including a patient with Sjögren's syndrome and renal tubular acidosis, and in patients with systemic lupus erythematosus (31.6%). The autoantibody varied in the extent of its cross-reactivity among human CA I (or B), human CA II (or C), bovine CA I, bovine CA II, rabbit CA, and dog CA. The titers continued to float and tended to parallel disease activity. Positive reactivity of autoantibody was observed on eccrine sweat glands and the distal tubules of the kidney by the indirect immunofluorescent method.

Polyclonal activation of lymphocytes due to an unknown cause is considered to be one of the most important findings of systemic autoimmune disorders including systemic lupus erythematosus (SLE). In order to confirm the expression of the C-Myc protooncogene in lesional skin, tissue specimens from SLE were examined by the histo in situ hybridization method and a histochemical method using a specific antibody reactive with C-Myc related products. Twenty-two cases of SLE, six cases of DLE, one case of lupus erythematosus profundus, two cases of lichen planus, and five skin specimens from healthy volunteers were selected for the examination. In the SLE group, further comparative examination of diseased skin and normal skin from the same patient, and of diseased skin in an active stage and a stable stage in the same SLE patient with renal involvement, were carried out. In most of the active SLE cases, protooncogene expression had apparently increased as compared with the expression in the groups of inactive and treated SLE, active DLE, active lichen planus, and those with healthy skin. Even in normal-appearing skin from active SLE without other organic failure, the protooncogenes were not expressed very strongly.
Circulating antibodies against certain extractable nuclear antigens (ENA) have been shown to have diagnostic and prognostic importance in connective tissue diseases. We described here an antibody against ENA found in the sera of patients with idiopathic thrombocytopenic purpura (ITP). The antigen, tentatively called WK according to the patients' initials, was distinct from U1 RNP/Sm, SSA/SSB, Scl-70, PCNA, PM-Scl, Jo-1, and Ku by immunodiffusion. On immunoblotting, the anti-WK serum recognized polypeptides of 99 kd, 98 kd, and 96 kd in rabbit thymus extracts and a 99 kd polypeptide in KB cell extracts. The anti-WK antibody was detected in the sera of 2 out of 360 ANA positive patients, both children with clinical features of ITP. One patient developed systemic lupus erythematosus eight years after the onset of ITP.
Among 1.041 clinical isolates (77 serovars) of Salmonella which had been derived from cases with acute enterocolitis, 601 (58%) contained one or more plasmids. Large serovar-specific plasmids were seen in 95 of 307 isolates (31%) of Salmonella typhimurium, in 34 of 34 isolates (100%) of Salmonella enteritidis and in 36 of 38 isolates (94.7%) of Salmonella braenderup: the sizes of which were 100, 60 and 106 kilobases (kb), respectively. In order to determine the role of these plasmids in pathogenicity for enterocolitis, the plasmids were eliminated from some strains of S. braenderup and S. typhimurium and the pathogenicity of the plasmid-less strains was compared with that of the parent strains by invasiveness to HeLa cells, fluid accumulation in the rabbit ligated ileal loop, lesion of mucosal tissue and the Sereny test. The virulence of all the plasmid-less strains was as strong as that of the plasmid-bearing strains in these pathogenicity assay systems. We therefore concluded that the 106-kb plasmid of S. braenderup and the 100-kb plasmid of S. typhimurium are not necessary for their pathogenicity in the experimental models: invasiveness to HeLa cells, fluid accumulation in the rabbit ligated ileal loop, and Sereny test.
A new two-step deprotection/cleavage procedure for t-butoxycarbonyl (Boc) based solid phase peptide synthesis is reported. First the protective groups are removed from 4-(oxymethyl)-phenylacetamidomethyl (PAM) resin attached peptide with the weak hard acid, trimethylsilyl bromide-thioanisole/trifluoroacetic acid (TFA). In the second step, the peptide is cleaved from the resin with a stronger hard acid such as trimethylsilyl trifluoromethanesulfonate in TFA or with HF. The method is also shown to deformylate Nin-formyltryptophan moiety efficiently. The usefulness of this procedure for practical solid phase peptide synthesis is demonstrated by comparison with other deprotection methods in the synthesis of urotensin II and human endothelin.
Syntheses are described for the putative human and porcine biosynthetic precursors (hET-38 and pET-39) of endothelin, with the sequence previously deduced from human- and porcine-cDNA coding for preproendothelin. The Boc based solid phase synthetic method was applied, followed by weak hard acid, trimethylsilyl bromide, cleavage. The peptide removal from the resin was optimally accomplished with hydrogen fluoride. Disulfide bridges were formed by air-oxidation, and the linkage modes determined by enzymic (Endoproteinase Asp-N) digestion and HPLC. Five additional C-terminally elongated endothelin homologs were also synthesized. For alternative synthesis of pET-39, the use of trimethylsilyl trifluoromethanesulfonate for the removal of peptide from the resin generated a major side product, which was characterized. hET-38 was found to be less effective in vitro, when compared to endothelin. The vasoconstrictor activity in vitro of other related peptides was comparable to that of hET-38.

The blood pressure response to exercise loading, Na concentration in the erythrocyte and Li-Na countertransport (Li-Na CT) system in the erythrocyte membrane in 40 male volunteers and 98 patients with mild essential hypertension were investigated. Subjects were divided into a juvenile group (less than 36 years) and a middle-aged (greater than 36 years and less than 65 years) group. Exercise-loaded blood pressure in patients with mild essential hypertension was followed up for more than 5 years. 2. Systolic blood pressure (SBP) at 6 min during exercise loading was significantly higher in the mild hypertension group compared with the control group. 3. Patients with high SBP at exercise loading in the mild hypertension group transferred more often to the moderate hypertension group within 5 years. 4. No difference in the haemodynamic parameter at rest was noted in the transferred group for moderate hypertension and the non-transferred group. 5. A higher blood pressure response to noradrenaline was noted in the transferred group compared with the non-transferred group of the juvenile group. 6. High Na concentration in the erythrocyte and acceleration of the Li-Na CT system in the erythrocyte membrane were noted in the mild hypertension group compared with the volunteer group. 7. A correlation between SBP at exercise loading and Li-Na CT system in the erythrocyte membrane was noted. This correlation was strongly noted in subjects less than 50 years old. 8. From these results it was concluded that SBP at exercise loading and Li-Na CT system in the erythrocyte membrane are useful as indicators for the prognosis of mild essential hypertension.
We determined the minimum inhibitory concentrations (MICs) of rokitamycin (TMS-19-Q, RKM), a macrolide antimicrobial agent, against strains of various bacterial species isolated from enteritis patients, and compared them with those of josamycin (JM), erythromycin (EM) and ofloxacin (OFLX). MIC90 of RKM against 147 strains of Campylobacter jejuni, and each 25 strains of Shigella spp., Salmonella spp. and diarrheagenic Escherichia coli were 1.56, 200, 800 and 200 micrograms/ml, respectively. There was only one RKM resistant (MIC greater than 100 micrograms/ml) C. jejuni strain, while most of the strains of the other species were resistant to RKM. MIC values of the other drugs were all similar to those of RKM. MIC90 of OFLX against 147 strains of C. jejuni was 0.78 micrograms/ml, lower than other drugs.

Postoperative course including serial changes in values of liver function tests and occurrence of hepatic failure was investigated in 59 patients with hepatocellular carcinoma. Eleven cases out of 59 patients were associated with chronic hepatitis (CH group), while remained 48 cases were with liver cirrhosis (25 cases of group A and 23 of group B in Child's classification). There was no significant difference in survival rate or recurrence rate after operation among these three groups. Hepatic failure within one year after hepatectomy was observed in 9%, 28% and 91% of patients in groups CH, Child's A and Child's B, respectively, and the frequency of occurrence of hepatic failure in Child's B group was significantly higher than those of groups CH and Child's A. Hepaplastin test and serum albumin levels recovered from the decline after hepatectomy in groups CH and Child's A. On the other hand, no recovery was found in Child's B group as well as lower levels before operation and that was thought to be the major reason for the frequent occurrence of hepatic failure in Child's B group.
In primary Sjögren's syndrome, various cutaneous manifestations may become apparent: annular erythema or eruptions reminiscent of erythema multiforme, lichen planus or erythema nodosum (dermo-panniculitis), chilblain-like erythema, purpura, cheilitis, and xerosis, are some of them. In the clinical diagnosis, the following laboratory tests are of value: Schirmer test, rose bengal test, fluorescence staining techniques, gum test, sialography, sialoscintigraphy, lip biopsy. In some cases, dryness can be absent though the above-mentioned skin manifestations are observed. Mothers with Sjögren's syndrome can deliver babies with neonatal lupus erythematosus. Serological testing almost always reveals anti-Ro/SSA-autoantibody and the more specific anti-La/SSB in the sera of patients. The sensitivity of detection can be enhanced by using the immunoblotting technique and the two-dimensional immunoblotting method. In some cases, a new antibody against carbonic anhydrase has been detected.
