Molecular analysis of the glycoasparaginase gene was performed on two Japanese siblings with aspartylglucosaminuria. The cDNA from one patient contained 7 additional bases between exons 3 and 4 (3'-terminal sequence of intron 3). This insertion resulted in a frame shift, and a termination codon appeared at amino acid 146. Amplification and sequencing of genomic DNA detected a single base transition (A-->G) at the 5' side adjacent to the insertion sequence. This mutation created a consensus AG dinucleotide in the splice acceptor site, and produced almost exclusively an abnormal mRNA containing the insertion by alternative splicing. The calculation of the sample score of the acceptor site supported this analytical result. BsmAI restriction site analysis of amplified cDNA and genomic DNA showed that these patients were homozygotes for this mutation. We conclude that the splicing defect in intron 3 causes glycoasparaginase deficiency in these patients.
BACKGROUND: We assessed the usefulness of magnetic resonance imaging (MRI) in identifying nonmalignant intraductal papillary mucinous tumors (IPMTs) of the pancreas. METHODS: Thirty-three patients with branch duct-type IPMT diagnosed by endoscopic retrograde cholangiopancreatography were prospectively examined with magnetic resonance cholangiopancreatography followed by dynamic gadolinium-enhanced MRI examinations, and patients with no findings suggestive of malignancy, including a solid mass, mural nodules, a main pancreatic duct wider than 5 mm in diameter, and stenosis of the main pancreatic duct, were prospectively followed up with sequential MRI examinations once or twice a year. RESULTS: Twenty-six (79%) patients showed no findings suggestive of malignancy in the initial MRI examination. The diameter (mean +/- standard error) of the main pancreatic duct was 3.9 +/- 0.7 mm and that of the ectatic branch pancreatic duct was 36.0 +/- 9.1 mm. Twenty-three patients were prospectively followed for more than 36 months and 22 of them showed no findings suggestive of malignancy during follow-up periods ranging from 39 to 77 months (mean = 55 months). CONCLUSION: MRI was useful to identify nonmalignant IPMTs of the branch duct type, and close follow-up observation with serial MRI examinations may be appropriate in the management of such patients.
A case of a hemothorax that occurred after thoracic epidural anesthesia is described. This situation might have been caused by accidental puncture of the intercostal vessel and visceral pleura by a Tuohy needle. The risk of causing a pneumothorax and/or hemothorax must be kept in mind when attempting thoracic epidural anesthesia.

Blood was taken from 56 selected newborn babies whose mothers had Graves' disease, to assess the relation between their thyroid function and the presence of thyrotropin (TSH) binding inhibitor immunoglobulins (TBII) and thyroid stimulating antibodies (TSAb) in maternal serum. All the mothers of the thyrotoxic babies had both these TSH receptor antibodies in their serum. However, most of the mothers whose thyroid function had been well controlled in pregnancy gave birth to normal babies. 15 babies had a transient syndrome of low serum thyroxine (T4) and free T4 with normal TSH and this tended to be associated with TSH receptor antibodies in maternal serum (TBII 9/15, TSAb 4/15). 2 infants had transient hyperthyroxinaemia without hyperthyroidism and both their mothers showed strong TSAb activity without TBII activity.
A primary leiomyosarcoma of the thyroid gland in a 72 year old Japanese woman is described. This is the second case reported in the English literature. The patient presented with a 7 month history of a gradually expanding tumor in the right neck. The surgical specimen taken by thyroid lobectomy revealed a relatively well demarcated tumor, 2 x 2 x 3 cm in size, confined to the right lobe. Histologically, the tumor showed a classical leiomyosarcomatous appearance of interlacing fascicles of spindle-shaped cells with occasional blunt-ended nuclei and a high frequency of mitotic figures. Immunohistochemistry of the tumor cells clearly showed smooth muscle differentiation; the cells were positive for desmin, muscle-specific actin and vimentin and negative for cytokeratin, epithelial membrane antigen, carcinoembryonic antigen, thyroglobulin and calcitonin. The patient was free of disease for 3 years and 11 months without further treatment when evidence of multiple bone metastases appeared on bone scintigraphy. She died of pneumonia 4 years and 3 months after the lobectomy.
Sonographic characteristics were analyzed in 141 histologically confirmed thyroid nodules including 84 benign and 57 malignant tumors. Seven items were examined: Presence of a 'halo'; presence of cystic components; presence of calcification; hypoechogenicity; heterogeneity of the internal echo pattern: irregular or obscured margin; and massive extrathyroidal extension. Only the last-mentioned item appeared exclusively, but not very often, in malignant lesions, and the other characteristics were encountered in both benign and malignant lesions. Although ultrasonography is a method of choice for the evaluation of thyroid nodules, its ability to distinguish benign from malignant lesions is limited.
