Structure-activity relationship within a series of okadaic acid derivatives.

Okadaic acid (OA) is a potent non-12-O-tetradecanoyl-phorbol-13-acetate (non-TPA) type tumor promoter on mouse skin. OA acts on cells through inhibiting the activity of protein phosphatases and results in the increase of phosphorylation of proteins. Seventeen OA derivatives were evaluated as possible tumor promoters by means of three biochemical tests: inhibition of specific [3H]OA binding to a particulate fraction of mouse skin containing protein phosphatases, inhibition of protein phosphatase activity, and induction of ornithine decarboxylase in mouse skin. Potency in each of these biochemical tests correlated well for each of these derivatives. We present results indicating that the carboxyl group as well as the four hydroxyl groups at C-2, C-7, C-24 and C-27 of OA are important for activity. Acanthifolicin, which gave positive responses in these three biochemical tests as strong as those of OA and dinophysistoxin-1, is predicted to be an additional member of the OA class of tumor promoters.

Postoperative lumbar spinal instability occurring or progressing secondary to laminectomy.

The manifestations and pathomechanism of postoperative lumbar spinal instability, occurring or progressing secondary to laminectomy, was clarified by means of functional radiographic analysis in a series of 46 patients over 40 years of age. The relation between instability and the clinical symptoms also is discussed. In patients under 60 years of age, instability at the operated level tended to appear in cases of wide laminectomy more often than in cases of partial laminectomy. Occurrence or progress of instability seems to be promoted by resection of the posterior spinal elements rather than the disc. It is further considered that the postoperative aggravation of clinical symptoms may be influenced not only by instability, but also by the other factors.

Presence of both stimulating and blocking types of TSH-receptor antibodies in sera from three patients with primary hypothyroidism.

A case report of three patients with primary hypothyroidism who had potent TSH-binding inhibitor immunoglobulins (TBII) and both thyroid stimulating (TSAb) and thyroid stimulation-blocking antibodies (TSBAb) has been described. Two patients displayed symptoms and signs indistinguishable from those in primary myxoedema (cases 1 and 2), and another patient had a history of Graves' disease (case 3). TBII, TSAb and TSBAb activities were 90.0, 1084.2 and 94.5% in case 1, 91.5, 826.6 and 95.8% in case 2, 76.0, 230.0 and 95.0% in case 3, respectively (normal range, less than 11.0%, less than 145.0 and less than 22.0%, respectively. The results indicate that both stimulating and blocking types of TSH-receptor antibodies exist in these patients. The possible mechanism whereby hypothyroidism developed has been discussed.

An immunohistochemical study of copper, zinc-containing superoxide dismutase detected by a monoclonal antibody gastric mucosa and gastric cancer.

The immunohistochemical localization of copper, zinc-superoxide dismutase (Cu,Zn-SOD) in human gastric mucosa and gastric cancer was studied using a monoclonal antibody. In gastric mucosa, parietal cells, pyloric glandular cells and foci of intestinal metaplasia showed positive staining in the cytoplasm and/or nucleus. The wide distribution of Cu, Zn-SOD in the gastric mucosa suggests cell function may be vulnerable to active oxygen species. In gastric cancer, 34 of 70 cases showed a positive reaction for Cu, Zn-SOD. There was a relationship between the grade of Cu,Zn-SOD immunoreactivity and the histological type of gastric cancer, well-differentiated types of gastric cancer being more frequently positive. The positive cases of poorly-differentiated adenocarcinoma were characterized by a pattern of diffusely infiltrative invasion. These results suggest that some types of gastric cancer are resistant to active oxygen species.

Hepatic neoplasms: effects of transcatheter arterial embolization on coagulation and fibrinolysis.

Forty-eight patients with hepatic malignancy (47 with hepatocellular carcinoma, one with metastatic colon carcinoma), who underwent transcatheter arterial embolization (TAE) for treatment of hepatic neoplasms, were investigated to determine the effects of TAE on coagulation and fibrinolysis. TAE was followed by a significant decrease in the platelet count (P less than .001); a prolongation of prothrombin time (P less than .001); an early increase in levels of fibrinopeptide A (P less than .01), fibrinopeptide B beta-15-42 (P less than .001), and fibrin(ogen) degradation products (P less than .001); and a delayed increase in the fibrinogen level (P less than .001), without a significant prolongation of the activated partial thromboplastin time. In the three patients who developed disseminated intravascular coagulation (DIC) after TAE, a reduction of both the platelet count and fibrinogen level occurred significantly earlier and in a more severe form than in the other patients without DIC; this reduction preceded the onset of the characteristic symptoms of DIC. Data suggested that close monitoring of platelet count and fibrinogen level is important for early detection of DIC following TAE.

Combinative improving effect of increased solubility and the use of absorption enhancers on the rectal absorption of uracil in beagle dogs.

An improvement of the rectal absorption of uracil was examined by the application of absorption enhancers in addition to the increased solubility of uracil. Uracil was ground with additives such as MgO, sodium 2,6-dihydroxybenzoate, human serum albumin or hydroxypropylmethylcellulose acetate succinate. Aqueous, oily and powdery formulations, which consisted of the ground mixtures, nicotinamide, urea and absorption enhancers such as polyoxyethylene (23) cetylether (BC-23) or sodium caprate, were prepared. Uracil solubility in the aqueous formulations was increased about 4-13 times that in the corresponding control formulations. When rectally administered to beagle dogs, marked increases in the plasma uracil level were observed in some of the cases of aqueous and oily formulations. In the powdery formulations and formulations containing macromolecular additives, however, absorption improvements was not observed. The results indicated that an improvement in the absorption of uracil was caused by the combinative improving effect of the increased uracil solubility and the promoting effect of absorption enhancers.

Simultaneous measurement of TSH-binding inhibitor immunoglobulin and thyroid stimulating autoantibody activities using cultured FRTL-5 cells in patients with untreated Graves' disease.

A new and practical assay was developed using cultured FRTL-5 cells for simultaneous assessment of TS-ab and TSH-binding inhibitor immunoglobulin, allowing direct comparison of these two activities under the same conditions. Subsequent to the TS-ab assay in which extracellular cAMP concentration in Hanks' medium without NaCl was determined, [125I]b TSH in this medium was added to observe the ability of serum Ig to inhibit the binding of [125I]b TSH to FRTL-5 cells. We found a much higher specific binding of [125I]b TSH to FRTL-5 cells and a much greater inhibition of [125I]bTSH binding to the cells exposed to Graves' Ig in hypotonic NaCl-free than in NaCl containing Hanks' medium, indicating that the binding of both TSH and Graves' Ig to the TSH receptor was salt-sensitive. The inhibitory activity of [125I]bTSH binding to the cells was 0.2 +/- 4.6% (mean +/- SD) in 45 normals. Inter-assay coefficients of variation in two positive controls with the mean values of 18.0 and 65.8% were 15.8 and 16.5%, respectively. Among 46 patients with untreated hyperthyroidism owing to Graves' disease, 45 (97.8%) were positive for TS-ab; 35 (76.1%) and 40 (87.0%) were positive for TSH-binding inhibitor in Ig assays using FRTL-5 cells and solubilized porcine thyroid membranes, respectively. TS-ab activities correlated less closely with TSH-binding inhibitory activities determined using FRTL-5 cells (r = 0.576, p less than 0.001) than with those determined using porcine thyroid membranes (r = 0.745, p less than 0.001).(ABSTRACT TRUNCATED AT 250 WORDS)

[Changes in the partial pressure of oxygen of the nasal mucosa caused by stellate ganglion block].

Stellate ganglion block (SGB) is clinically used for the treatments of various lesions diseases. SGB gives rise to nasal obstruction of transient duration. It is also suggested that derangement of the autonomic nervous system underlies nasal allergy. In order to study the hemodynamics of the nasal mucosa after SGB, two groups of the subjects, i.e. healthy volunteers and patients with nasal allergy, were included for evaluation of the partial pressure of oxygen of nasal mucosa (PtsO2) after SGB. Simultaneous evaluations were done to assess the nasal patency in the normal group and the temperature at the anterior tip of the inferior nasal turbinate in the nasal allergy group. The results thus obtained are summarized as follows: 1) In the normal group, SGB was resulted in a reduction in nasal patency on the blocked side and a significant increase in PtsO2 when compared with the non-blocked side. The PtsO2 remained higher than normal after although nasal patency was restored. 2) The PtsO2 at rest was lower in the nasal allergy group than in the normal group. 3) Changes in PtsO2 following SGB were similar in the two groups. 4) A parallelism was noted between changes in the temperature at the tip of inferior nasal turbinate and those in PtsO2 during SGB. These results suggest that SGB augments the arterial blood flow in the nasal mucosa. The compromised nasal mucosa of nasal allergy patients is in a state of hypoxia but showed virtually the similar improvement in circulation after SGB as noted in the nasal mucosa of normal subjects.

Nucleotide sequence analysis of human beta-globin gene by the quantification method: mutations in 3'-splice junction sequence and beta-thalassemia.

The nucleotide sequence at the intron-exon junction in the human beta-globin gene was analyzed by the quantification method (categorical discriminant analysis) proposed previously. Using the sample score of a 16-nucleotide sequence at a 3'-splice junction, we studied to what extent such a sequence contains the 3'-splice signal. To examine the applicability of our method, we further studied several mutants of beta-thalassemia, where nucleotide changes exist at 3'-splice junction sequences of the first and second introns. Other mutants involve point mutations which generate new 3'-splice signals within the first intron. Experimental results on the abnormal splicing in those mutants could be explained in terms of the sample scores of 16-nucleotide sequences and their locations relative to the branch point.

Quantification analysis of human alpha-globin gene. Mutation in 5'-splice junction sequence and alpha-thalassemia.

Nucleotide sequence of the exon-intron junction in human alpha-globin gene was analyzed by quantification method proposed previously. Using sample score of 9-nucleotide sequence at 5'-splice site, we examined strength of the splice signal. We further studied a mutant of alpha-thalassemia, where pentanucleotide deletion occurs around 5'-splice junction of the first intron. This mutation abolishes the normal 5'-splice site completely, but activates a cryptic site lying in the first exon. Such a behaviour was well explained in terms of our sample scoring scheme.

[Prosthetic valve implantation without removal of tricuspid apparatus for an old case of corrected transposition of the great arteries].

Prosthetic valve implantation was performed for the systemic atrioventricular regurgitation in an old patient with corrected transposition of great arteries. SJM27M prosthesis was implanted without removing posterior and septal leaflet, chordae and papillary muscles with respect to preserve the function of the morphological right ventricle. It is important to preserve the function of the morphological right ventricle because it must continuously play a role of systemic ventricle even after surgical procedure.

[A successful surgical case of aortic arch aneurysm associated with aortic regurgitation].

A 57-year-old male patient with aortic regurgitation and aneurysm of the ascending aorta and the aortic arch underwent aortic valve replacement and graft replacement from the ascending aorta to the aortic arch. The operation was done using cardio-pulmonary bypass and selective cerebral perfusion with deep hypothermia. Postoperative DSA revealed no dilatation of the sinus Valsalva and a good configuration of the anastomosis. It seems that selective cerebral perfusion with deep hypothermia is a safe method to prevent cerebral damage in a case of arch aneurysm.

[Definition of the adaptation of endoscopic surgery for early gastric cancer. Special examination on the relationship between the histological differentiation of the intramucosal gastric cancer and the lymph node metastasis].

Out of five hundred and nineteen surgically resected lesions which were histologically shown to be intramucosal gastric cancer, 247 were well differentiated, 98 were moderately differentiated, and 176 were poorly differentiated lesions. The incidence of lymph node metastasis in the 247 well differentiated, 98 moderately differentiated, and 176 poorly differentiated intramucosal adenocarcinomas were 0.8% (= 2/247), 2.1% (= 2/98), and 8% (14/176), respectively. The cumulative survival rate of 5 years after gastrectomy was 94.2% for the 247 well, 93.3% for the 98 moderately, and 93.7% for the 176 poorly differentiated adenocarcinomas. A pathological study on the atypism of the gland of the cancer and the condition of the proprial space under the cancer cells was conducted on 100 lesions of well differentiated adenocarcinomas in order to define the criteria of non-surgical endoscopic removal resection for early gastric cancer. Out of the 100 lesions mentioned above, there were only two well differentiated adenocarcinomas with lymph node metastasis in which the boundary between the epithelium of the cancer and the proprial space under the cancer was very unclear and the interstitial space in the propria under the cancer was very small. On the other hand, we have resected 73 intramucosal gastric cancers using the endoscopic surgical method called strip biopsy since 1984, and have followed up these cases. The cancers resected by strip biopsy, and were histologically intramucosal well differentiated adenocarcinomas shown to have a moderately maintained interstitial space under the cancer cells. And, up to the present, there has been no incidence of recurrence after strip biopsy for the 73 lesions mentioned above. According to these results, we have defined the criterion for early gastric cancer endoscopic surgery, as follows. Intramucosal cancer which is shown to be well differentiated adenocarcinoma with a moderately maintained interstitial space under the cancer cells is considered to be sufficient for non-surgical endoscopic resection.

Quantification analysis of 5'-splice signal sequences in mRNA precursors. Mutations in rabbit beta-globin gene.

Concerning the signals which direct excision of introns from mRNA precursors in genes of higher eukaryotes, a consensus sequence composed of nine nucleotides, CAAG/GTAGAGT, has been proposed for the 5'-splice site, but actual 5'-splice site sequences differ from it to a greater or lesser degree. In the present paper, the 5'-splice site sequence of the rabbit beta-globin gene was analyzed using a quantification method (categorical discriminant analysis). In this method, each 9-nucleotide sequence in the pre-mRNA was characterized by its sample score, which shows the extent to which the sequence contains the signal. This approach could explain not only the location of the 5'-splice site, but also the experimental results of various point mutations in the 5'-splice region, as reported by Aebi et al. Our method further predicted the positions of cryptic 5'-splice sites, which are activated when the authentic 5'-splice site is abolished.

Electrophysiological properties of isolated rat liver cells.

The electrophysiological properties of isolated rat liver cells were studied using the patch clamp method in whole-cell configuration. The membrane potential in isolated hepatocytes was -42 +/- 7 mV (n = 20). The input resistance (Rin) and the time constant (tau m) were 51 +/- 17 M (the range of 34 to 180 M omega) (n = 20) and 4.2 +/- 1.0 msec (the range of 3 to 16.5 ms) (n = 20). Assuming that the specific membrane capacitance is 1 microF/cm2, the membrane resistance and membrane capacitance were 42. +/- 9.0 K omega cm2 and 87 +/- 27 pF. These values indicate that isolated rat hepatocytes are not abnormally permeable or leaky. The current-voltage relationship was linear with no rectification. The depolarizing pulse from the resting potential did not induce fast or slow inward currents even when norepinephrine or high Ca2 (3.6 mM) were applied. This indicates that there is no voltage-sensitive Ca2+ channel in the isolated hepatocytes.

Nucleotide sequences of the 5.8S rRNA gene and internal transcribed spacer regions in carrot and broad bean ribosomal DNA.

Nucleotide sequences of the first and second internal transcribed spacers (ITS1 and ITS2, respectively) of ribosomal DNA (rDNA) from two dicot plants, carrot and broad bean, were determined. These sequences were compared with those of rice, a monocot plant, and other eukaryotic organisms. Both types of ITS region in some species of Angiospermae were the shortest among all eukaryotes so far examined and showed a wide range of variation in their G+C content, in contrast to a general trend toward very high G+C content in animals. Phylogenetic relationships of plants with animals and lower eukaryotes were considered using the nucleotide sequences of carrot and broad bean 5.8S rDNA that were determined in the present study, together with that of wheat 5.8S rRNA, which has been reported previously.
