Radiation dosimetry at CT fluoroscopy: physician's hand dose and development of needle holders.

The radiation dose to physicians' hands without and with use of needle holders was determined at 10 computed tomography (CT) fluoroscopy-guided transthoracic needle biopsies. As measured with ionization chambers (tube voltage, 80 kVp; tube current, 30 mA), the mean absorbed dose rate without and with holders was 1.14 mGy/sec +/- 0.02 (standard deviation) and 0.019 mGy/sec +/- 0.001, respectively. The mean duration of CT fluoroscopy was 59 seconds in 10 biopsies performed with a holder and 82 seconds in 10 biopsies performed without a holder (difference not statistically significant). The needle holders did not cause any artifacts that interfered with the biopsy procedure.

Trigeminal neuralgia associated with venous angioma--case report.

A 35-year-old male presented with trigeminal neuralgia associated with venous angioma at the root entry zone. Magnetic resonance imaging and angiography demonstrated a venous angioma with a dilated petrosal draining vein, and displacement of the anterior inferior cerebellar artery (AICA). The AICA and dilated petrosal vein were both decompressed, resulting in complete relief from symptoms of trigeminal neuralgia for 30 months. Microvascular decompression rather than resection of venous angioma is recommended for treatment of such cases. The possibility of a venous anomaly should be considered in younger patients with trigeminal neuralgia.

The reproducibility of reported height and body weight in repeated questionnaire surveys.

Reproducibility of reported height and weight were studied via repeated questionnaire surveys distributed to subjects within a year. Effects of the factors which might influence the reproducibility of these values were investigated. Although only about half of the subjects reported the same height and 30% the same weight on the second questionnaire, no change was found in average height and weight after a one year interval for both male and female subjects. The reproducibility decreased with age for both height and weight. Smokers tended to have lower reproducibility for height than non-smokers. The shorter subjects had lower height reproducibility than their taller counterparts, while heavier subjects had lower weight reproducibility than light subjects. It was observed that the lower the weight reproducibility, the lower the height reproducibility and vice versa. Reported height or weight figures ending in zero or five were more frequent than the expected 10% with low reproducibility.

[Development of real-time CT fluoroscopy].

A new CT system that permits real-time monitoring of CT images was developed. Phantom and volunteer studies revealed that the images were displayed at a rate of six per second with a delay time of 0.83 second with clinically sufficient resolution (256 x 256) using the newly developed fast image processor and partial-reconstruction algorithm. The clinical trial of stereotactic aspiration of intracerebral hematoma was successful. The initial trial with CT fluoroscopy revealed potential usefulness of the system in biopsy and other CT-guided interventions.

Oleanolic acid saponins from root bark of Aralia elata.

Three new oleanolic acid glycosides, tarasaponins I-III, were isolated as their methyl esters from the root bark of Aralia elata together with four known glycosides, the methyl esters of chikusetsusaponins IVa, IV, 28-desglucosyl-chikusetsusaponin IV and pseudoginsenoside RT1. Tarasaponins I-III were characterized as oleanolic acid 3-O-[beta-D-glucopyranosyl(1-->3)][alpha-L-arabinofuranosyl(1-->4)[- beta-D-glucuronopyranoside, oleanolic acid 3-O-[beta-D-xylopyranosyl(1-->2)][beta-D-galactopyranosyl(1-->3)]-beta- D-glucuronopyranoside and beta-D-glucopyranosyl oleanolate 3-O-beta-D-galactopyranosyl(1-->3)-beta-D-glucuronopyranoside, respectively.

Blocking effects of ethanol on stress-induced activation of rat mesoprefrontal dopamine neurons.

We investigated the effects of ethanol on stress-induced activation of the brain dopamine (DA) systems in rats. Ethanol (0.5 and 1.0 g/kg) was injected IP 25 min before sacrifice (5 min before 20-min immobilization stress). Ethanol treatment by itself did not affect the levels of either DA or its major metabolite, 3,4-dihydroxyphenylacetic acid (DOPAC), in the mesoprefrontal cortex, cingulate cortex, olfactory tubercle, or caudate putamen. Immobilization stress for 20 min caused increases in DOPAC levels in the prefrontal cortex (160% of control) and cingulate cortex (135% of control), but not in the olfactory tubercle or caudate putamen. The stress had no effects on DA levels in any of the four brain regions studied. Pretreatment with ethanol blocked, in a dose-dependent manner, the stress-induced increases in DOPAC levels in the mesoprefrontal cortex. The present data suggest that ethanol exhibits a blocking effect on stress-induced activation of the mesoprefrontal DA neurons. This blocking effect may be related to the anxiolytic action of ethanol.

Oleanolic acid saponins from root-bark of Aralia elata.

Five new oleanolic acid glycosides, tarasaponins III-VII, were isolated from the dried root-bark of Aralia elata together with stipuleanoside R2, in addition to eight saponins previously reported. They were characterized as oleanolic acid 3-O-[beta-D-xylopyranosyl(1-->2)] [beta-D-glucopyranosyl(1-->3)]-alpha-L-arabinopyranoside, beta-D-glucopyranosyl oleanolate 3-O-[beta-D-glucopyranosyl(1-->2)][alpha-L-arabinofuranosyl (1-->4)]-beta-D-glucuronopyranoside, beta-D-glucopyranosyl oleanolate 3-O-[beta-D-xylopyranosyl(1-->2)][beta-D-galactopyranosyl (1-->3)]-beta-D-glucuronopyranoside, beta-D-glucopyranosyl oleanolate 3-O-[beta-D-xylopyranosyl(1-->2)][beta-D-glucopyranosyl (1-->3)-alpha-L-arabinopyranoside, respectively.

[Aspiration cytology of salivary gland lesions--analysis of 55 cases].

Aspiration cytology of 55 salivary gland lesions (10 cases of non-neoplastic lesions, 36 cases of benign tumors and 9 cases of malignant tumors) were reviewed with histologic examination. Accuracy of benign and malignancy was 91.8% (benign lesion 97.6%, malignant tumor 57.1%), accuracy rates was almost of satisfactory for benign lesions, but not for malignant tumors. Agreement rate with histological type was 73.5%. One case (1.8%) was false positive, basal cell adenoma with adenoid cystic like pattern was falsely taken for adenoid cystic carcinoma. Three false negative cases (5.5%) consist of 1 acinic cell carcinoma, 1 adenoid cystic carcinoma with minimum cellular atypia and 1 partial malignant change (minor area) in pleomorphic adenoma. Six cases (10.9%) belong to group of insufficient material. Technical improvement of cell sampling is expected.

[Visualization of coronary arteries with helical scanning CT: diastolic reconstruction].

The purpose of this study was the visualization of coronary arteries by the diastolic reconstruction method with helical scanning (HES) CT. We chose diastolic phase images from the image data acquired with the HES technique using a continuous nutate-rotate fast CT scanner, because few motion artifacts are induced by cardiac pulsation during the slow filling diastolic phase. We assessed coronary diseases using this method based on HES-CT, and evaluated its clinical effectiveness. We also studied the clinical value of multiplanar reconstruction and three-dimensional surface reconstruction. The subjects consisted of 31 patients with coronary disease, aged 41-77 years. When a diastolic reconstruction HES-CT was employed, the average rate of visualization of the coronary arteries was 72%. An examination can be completed during a 30-second single breath-hold, permitting the acquisition of coronary artery images with excellent continuity. Calcified coronary arteries can be identified.

[Clinical usefulness of helical scanning CT (HES-CT) for orbital lesion].

Helical scanning CT (HES-CT) was applied to multiplanar reconstruction (MPR) and 3-D imaging for the diagnosis of 65 patients with orbital lesions. We employed the the Toshiba TCT-900S, a fourth-generation high-speed CT scanner equipped with a slip-ring system. In HES-CT, scanning was performed with continuous rotation of the X-ray tube for 30 s, while the couch top was simultaneously moved at a constant speed of 1 to 2 mm/s. Images were reconstructed from continuous raw data to obtain a slice pitch of 1 mm in the longitudinal direction. HES-CT was found to have some advantages over conventional thin slice CT: 1) a wide area could be scanned in a short time (30 s) without motion artifact, 2) small lesions could be easily detected because slice images could be generated with a fine pitch at arbitrary positions from continuous raw data, 3) MPR images had good continuity in the longitudinal direction, and 4) high contrast of structures was obtained in the orbital region. HES-CT was considered to be useful for the detection of orbital lesions.

Saponins from leaves of Acanthopanax sieboldianus.

Two new triterpenoid saponins named sieboldianoside A and B were isolated from the leaves of Acanthopanax sieboldianus together with five known triterpenoid saponins, kalopanax-saponins A and B, saponin A, CP3, sapindoside B, and a known flavonol glycoside, kaempferol 3-O-rutinoside. On the basis of chemical and spectral evidence, the structures of the new saponins (sieboldianoside A and B) were concluded to be alpha-L-rhamnopyranosyl(1-->4)-beta-D-glucopyranosyl(1-->6)- beta-D-glucopyranosyl esters of hederagenin and oleanolic acid 3-O-beta-D- xylopyranosyl(1-->3)-alpha-L-rhamnopyranosyl(1-->2)-alpha-L-arabinopy ranosides, respectively.

3 alpha-hydroxy-oleanane-type triterpene glycosyl esters from leaves of Acanthopanax spinosus.

Three novel 3 alpha-hydroxy-oleanane-type triterpene oligoglycosyl esters, spinoside C1, C4 and C5, were isolated from the leaves of Acanthopanax spinosus. The structures were established to be 28-O-alpha-L-rhamnopyranosyl(1-->4)-beta-D-glucopyranosyl (1-->6)-beta-D-glucopyranosyl esters of 3 alpha, 29-dihydroxy-olean-12-ene-28-oic acid, 3 alpha, 29-dihydroxy-23-oxo-olean-12-ene-28-oic acid and 3 alpha, 23,29-trihydroxy-olean-12-ene-28-oic acid.

[A case of encephalopathy accompanying Mycoplasma pneumoniae pneumonia in an adult].

We experienced a 39-year-old male who developed neurological complication during a course of Mycoplasma pneumoniae pneumonia. The diagnosis of M. pneumoniae pneumonia was made on the basis of elevation of specific antibody (CF) titer in convalescent serum. Electroencephalogram showed diffuse damage in the brain, but no other abnormalities were not found on brain CT-scan and MRI. In the cerebrospinal fluid, the number of cells did not increase and the M. pneumoniae CF titer was not elevated. From these results, we concluded that encephalopathy in this patient was raised by an allergic reaction of the brain tissue to M. pneumoniae antigen. Until now, encephalitis or meningoencephalitis accompanied with M. pneumoniae infection has been reported by many investigators, but reports on encephalopathy due to M. pneumoniae are few. Therefore, we reported herein a case of encephalopathy following Mycoplasma pneumoniae pneumonia with several references.

Meningioangiomatosis not associated with von Recklinghausen's disease--case report.

A 7-year-old girl with meningioangiomatosis not associated with von Recklinghausen's disease is described. The radiological findings were similar to meningioma, but intraoperatively, a thin septum was found between the mass and the dura mater. Microscopically, there was significant proliferation of fibroblastic spindle-shaped cells and collagenous fibers in the subarachnoid space. Proliferated cells had penetrated into the cortical tissue along the irregularly branched blood vessels. Immunohistochemically, these penetrating perivascular cells were negative for glial fibrillary acidic protein and S-100 protein, and positive for vimentin staining. These findings suggest that the histogenesis of the spindle-shaped cells is most probably meningothelial.

[Immunocytohistochemical studies of AFP producing gastric cancer--cytomorphology and characteristics of AFP positive cells].

In 24 cases (5.5%) out of 438 cases of gastric cancer showing serum AFP over 20 ng/ml, 14 cases (3.2%) showed AFP positive reaction in cancer cells by immunostaining. AFP localization in cancer cells classified into marginal type, granular type, diffuse type and inclusion type. Mixed combination of these types increased in high serum AFP level. Cytomorphology of AFP positive cells was classified to clear cell type, hepatoid cell type and round cell type. Clear cell type showed endodermal sinus like pattern in histomorphology. Clear cell type and hepatoid cell type were mixed, and transient. These types showed eosinophilic hyaline like globules, poor mucin production, and rich glycogen in cytoplasm. By immunohistochemistry showed positive rate in HCG:21.4%, PLAL:50. 0%, SP1:50.0%, CK:14.3% and alpha 1-AT:92.9%, and recognized reaction of germ cell tumor (endodermal sinus tumor) marker. We thought AFP producing gastric cancer suggested differentiation to germ cell tumor (endodermal sinus tumor). The AFP-positively stained cases were found in a high age group with advanced cancer located pylorus to corpus, medullary growth pattern, and high rate in liver metastases.

[Fundamental study of helical scanning CT--evaluation of spatial resolution in the longitudinal axis].

We evaluated spatial resolution in the longitudinal axis with helical scanning CT using a fourth-generation fast CT scanner. We made a phantom by stringing acrylic balls (65 mm phi x 8 and 9 mm phi x 6). The acquired images were processed by MPR and assessed visually to evaluate axis resolution. With the conventional scanning method, the partial volume effect varied with the starting position, but helical scanning was able to reconstruct high-resolution images using continuous raw data. During helical scanning, axis resolution varied depending on the slice width and sliding speed of the couch top. Even if the sliding speed was kept constant at 4 mm/sec, axis resolution was superior with a slice width of 2 mm than with one of 5 mm.
