[A case of advanced seminomatous germ cell tumor cured with PVB combination chemotherapy].

A case of stage IIB seminomatous tumor treated with the combination chemotherapy regimen of cis-platinum, vinblastin and bleomycin is presented. The patient was a 42-year-old man who was admitted to our hospital with the chief complaint of the right intrascrotal mass and edema in right lower extremity. Right radical orchiectomy was immediately performed under the diagnosis of testicular tumor. Histological diagnosis was pure seminoma. Paraaortic bulky metastasis and right hydronephrosis were found in the abdominal CT-scan. PVB therapy for initial treatment was started. After four courses of this therapy, retroperitoneal lymphadenectomy was performed. Because no live tumor cells were found histologically in any of the dissected material, complete response was considered to have been achieved by PVB therapy. Considering the poor survival and high relapse rate of advanced seminomatous tumor treated with radiotherapy, this PVB therapy should be the initial treatment for this disease.

[Clinical evaluation of ceftizoxime in the treatment of complicated urinary tract infections].

Ceftizoxime (CZX) was given clinically to 80 patients with urogenital tract infections, in which 34 cases of complicated UTI satisfied the criteria of the UTI committee. CZX was administered for at least five days at a daily dose of 1 to 4 g in two divided doses by intravenous drip infusion. The overall clinical efficacy of CZX in 34 cases of complicated UTI was 59%, estimated by the criteria of the UTI committee. The overall clinical efficacy rate according to group of infection was: 64% for the 11 patients in the 1st group, 56% for the 9 patients in the 2nd group, 100% for the 5 patients in the 3rd group, 50% for the 4 patients in the 4th group and 20% for the 5 patients in the 5th group. No serious side effects were observed in the clinical or laboratory findings except for slight elevation in GOT & GPT values in two cases (2.5%), which returned to normal after CZX treatment. It is therefore suggested that CZX is a clinically useful and safe drug in the treatment of complicated UTI.

[Indication and result of renal autotransplantation for renovascular hypertension].

Twenty patients with renovascular disease in 22 kidneys were treated surgically by the method of renal autotransplantation at our department during the 13 years from 1970 to 1982. Etiological disorders included 11 cases of fibromuscular dysplasia, 3 cases of atherosclerosis, 3 cases of aortitis syndrome, two cases of renal artery aneurysm and one case of renal artery thrombosis. Age distribution ranged from 13 to 64 years old (average 28.9 years), and male to female ratio was 1 to 3. Ex vivo surgery was carried out in 11 of these 20 cases. Angioplasty in 9 cases, endarterectomy and partial nephrectomy in one case each were performed extracorporeally. Reconstruction of the urinary tract was needed for 14 kidneys in 13 patients, 12 kidneys were treated by UCN, and PU anastomosis was performed in 2 kidneys in 2 patients. Outcome of renal autotransplantation was "cured" in 17 cases and "improved" in 3 cases during follow-up period from 8 months to 13 years. Revision was successful for the postoperative complication such as urine leakage, bladder tamponade, perirenal fluid collection and ileus.

Loss of J chain during primary immune responses in chicks.

J chain positive cells (JPC) were studied by direct immunofluorescence in embryonic and newly hatched chicks. The results indicated a decrease in the amounts of JPC in the embryonic spleen and bursa of Fabricius after in ovo antigenic stimulation with sheep erythrocytes (SRBC) compared with that of unstimulated lymphocytes. The level of thymic JPC in the control chicks and those subjected to antigenic stimulation was always about the same. Partial re-expression of the J chain in splenic lymphocytes was detected in newly hatched, antibody producing chicks, while the percentage of JPC in non-antibody producing chicks did not recover to the control level. Further evidence obtained indicated that the JPC decreases did not depend on the antigen dosage. After antigenic stimulation, J chain re-expression in cells of embryos and newly hatched non-antibody producing chicks was found to be essentially the same. These findings imply that the re-expression of J chain molecules is associated with immunoglobulin production. Furthermore, it seems plausible that the non-re-expression of the J chain occurred at the time of immunological unresponsiveness.
