Molecular analysis of genetic diseases: an overview for clinicians.

The identification of fetal genetic disease has, for the most part, relied on examination of an end product, such as analysis of factor VIII levels obtained from cord blood in fetuses at risk for hemophilia. Advances in molecular genetics have shifted our focus in prenatal diagnosis away from protein product analysis toward etiology, making new discoveries gleaned from the Human Genome Project relevant to clinicians. This review discusses the basic principles involved in gene-based diagnosis, highlighting the complexities of current approaches to molecular diagnosis of fetal genetic disease. Given an understanding of both the theory and practice of genetic analysis, the review covers the fundamental principles of molecular biology (structure, function, packaging, and regulation) and discusses recombinant DNA techniques presently used for the analysis of mutations. Clinical examples are presented to introduce the techniques most commonly employed in service laboratories: direct detection assays, where the specific mutation is recognized, and indirect detection assays, useful for the deduction of an inheritance pattern where the actual mutation or its gene is not known but may be closely linked to known DNA polymorphisms.

The biological effectiveness of radon-progeny alpha particles. III. Quality factors.

Domestic radon risk estimates are typically based either on data for uranium miners or on data derived from A-bomb survivors; comparison of domestic radon risk estimates derived from these two disparate sources represents an important test of their reliability. There is currently a significant discrepancy of about a factor of three between domestic radon risk estimates generated with these two independent methods. To base such risk estimates on the data for A-bomb survivors, who were exposed mainly to low-LET radiation, requires a quality factor for alpha particles from random progeny; the final risk estimate is then directly proportional to this quality factor. We have used the most extensive quantitative in vitro data set currently available at high LET for an oncogenic end point, to make the best estimate we can that could be used as a basis for a quality factor. Our best estimates of values appropriate for the quality factor for radom progeny are significantly lower than those currently used (20-25) in estimating lung cancer mortality due to randon. Specifically, our best estimate for home dwellers is around 10. In addition, because of the different geometry in the bronchial epithelia of nonsmokers compared to smokers, our best estimate of an appropriate quality factor for home dwellers is about 18% greater than that for miners; thus our best estimate of the "effective K factor" to convert to effective dose/WLM in home dwellers from effective dose/WLM in miners would be increased by this factor. Based on a quality factor of approximately 10, the dosimetrically based estimate of radon-induced mortality would be approximately 35,000 per year in the U.S. rather than the value of approximately 70,000 obtained using a quality factor of 20. The value of 35,000, while larger than the values based on data for miners (approximately 20,000), is much smaller than previous estimates of approximately 70,000 based on dosimetric methods; thus risk estimates based on the two approaches, dosimetric and epidemiological, may be partially reconciled. Finally, a quality factor of 10 would reduce the proportion of the collective effective dose caused by radon progeny from the currently accepted value of 55% down to about 38%.

Prevention of tumor formation in a mouse model of Burkitt's lymphoma by 6 weeks of treatment with anti-c-myc DNA phosphorothioate.

BACKGROUND: Transgenic mice bearing a murine immunoglobulin enhancer/c-myc fusion transgene (Emu-myc) provide a useful model for Burkitt's lymphoma. MATERIALS AND METHODS: Groups of 12 Emu-myc mice were treated prophylactically for 6 weeks after weaning with anti-c-myc DNA phosphorothioate (20 mg/kg/day), scrambled control DNA, or saline, delivered by micro-osmotic pumps. RESULTS: Half of the mice treated with saline or scrambled control DNA displayed palpable tumors by 8-9 weeks after birth, and 95% of them did so by 16 weeks, but 75% of the mice treated with antisense DNA were still free of tumors at the age of 26 weeks. Antisense therapy ablated MYC antigen in the spleens of tumor-bearing mice. Plasma physiological parameters indicated no acute toxicity. CONCLUSIONS: Long-term tumor resistance after anti-c-myc DNA therapy implies induction of a host response. Prophylactic anti-c-myc DNA therapy might prevent lymphoma in asymptomatic individuals displaying c-myc translocations.

[Intra- and extra-cranial arterial lesions in TIA patients].

Ninety-six patients with transient ischemia attacks were examined by transcranial Doppler (TCD), carotid color-coded ultrasonic imagine, brain computed tomographic scan (CT), compared with digital subtract angiography (DSA), MRI and MRA were done in some cases. The abnormality of CT and MRI was 30.2% and 74.1%, respectively. The most common findings were lacunar infarction or cerebral atrophy. Sixty-two cases fulfilled the TCD criteria of definitive arterial stenosis (64.5%) and three cases of arteriovenous malformation, among whom 27 patients (43.5%) carried out the examination of DSA. There was an excellent coincidence between the DSA and TCD findings, although DSA showed more vessels involved in some patients. Of the 96 cases, the intracranial arterial stenosis or occlusion was in 48.5% and the extracranial in 17.8%. The younger patients (age < 60 years) were subject to involvement of unilateral intracranial artery, but the older had predominantly the bilateral intracranial arterial and extracranial carotid or subclavian arterial lesions.

[DNA polymorphisms of the apolipoprotein AI-CIII-AIV gene cluster in coronary artery disease].

Two polymorphic sites of the apolipoprotein AI (ApoAI) gene, PstI and SstI, were examined in a sample of 43 unrelated Chinese patients with coronary artery disease (CAD) and 60 unrelated healthy individuals. Allele frequencies were compared between patients and controls and their impact on lipid and oxygen free radicals metabolism were also studied. The frequencies of the uncommon P2 and S2 alleles in CAD cases were found as 0.14 and 0.22 respectively, which were significantly higher than 0.05 and 0.13 in controls. The more the number of vessel disease in CAD cases, the higher the frequencies of S2 alleles which were the highest in the group of 3 vessel diseases. Some serum lipid and oxygen free radicals levels differed between CAD cases of different genotypes. These data suggest that genetic variation in P2 and S2 alleles of APOAI gene may contribute to the development and extent of CAD in Chinese subjects.

[Effects of thymectomy and glucocorticoid therapy on peripheral lymphocytes in myasthenia gravis].

Clinical manifestations and peripheral blood lymphocyte subset changes were studied in patients with myasthenia gravis (MG) to elucidate the mechanism of clinical improvement following treatment, with thymectomy (Tx) or glucocorticoid (GC) therapy. The changes found were: 1. There was a significant increase in percentages of CD3+, CD29+ CD4+ cells and CD4/CD8 ratio and a significant decrease in percentages of CD8+ and CD16,56+ cells in patients who had never been treated with any immune therapy. 2. After Tx or GC therapy, CD3+ and CD4+, CD29+ cells were decreased, but the number CD19+ and CD16, CD56 cells did not change. 3. Tx had a special effect on CD8+ cells. In most of the patients who showed clinical improvement after Tx, CD8+ cells were increased and CD4/CD8 ratio wad decreased. 4. Anti-acetylcholine receptor (AChRAb) titers were markedly decreased after GC therapy. These results indicate that there were obvious abnormalities in cell-mediated immunity in addition to those in humoral immunity in myasthenia gravis. These abnormalities tended to be normalized after Tx or GC therapy.

[The experimental observation on the procedure of atherosclerotic restenosis following PTCA].

Angioplasty of stenotic iliac artery was performed in 23 Japanese white rabbits. Angiography and histopathology examinations were performed one day (6 rabbits), 3 days (5 rabbits), one week (5 rabbits) and one month (7 rabbits) after dilation. TXB/PGF in blood was analyzed with radio-immunologic method. TXB/6-K-PGF increased at 1st day, began to reduce at 3rd day, became almost normal after one week and returned to normal level week after one month. Restenosis occurred in all animals. Platelet aggregation and mural thrombi were observed at 1st and 3rd day. New endothelial cells covered the surface of denuded endothelium at 3rd day. At 7th day, repairing of endothelium was completed and proliferation of smooth muscle cells were prominent in new intimia. Stenosis of lumen occurred again after 1 month. The results suggested that intimia injury during the procedure initiates the process of restenosis and aggregation of platelets, imbalance of between TXA and PGI is and proliferation of smooth muscle cells play important roles in the formation of restenosis.

[Sequencing of polyhedrin gene of Leucania separata multiple capsid nuclear polyhedrosis virus by silver sequencing system].

The genome of LsNPV was digested with EcoRV and cloned into pBluescript. With the partial polyhedrin gene of Autographa californica as a probe, positive recombinants were screened by colony in situ hybridization, further characterized by sequencing, and designated as pLsEV and pLsPH5. The intact 741 bp polyhedrin gene of LsNPV is located on the two recombinants revealed by Silver Sequencing System, and shares 80.0% and 97.0% nucleotide identity of the coding sequence, 87.8% and 97.5% amino acid identity with AcMNPV and MbMNPV respectively. Glu is the richest amino acid, and Gln and Trp the poorest amino acid in the polyhedrin of LsNPV. The codons ended with pyrimidine were higher in the codon usage. There was a signal-peptide-like hydrophobic region of 26 amino acids in the N end of polyhedrin.

[Effect of deoxynupharidine on immune function in vitro].

Deoxynupharidine (DON) is an alkaloid isolated from rhizome Nuphar pumilum which has been extensively used in the treatment of rheumatoid arthritis and back and leg pains as a folk remedy in China. Data presented in this paper indicate that the DON has potent immunosuppressive activities. Mitogens or allogen induced lymphoproliferative responses of murine splenocytes or human tonsillar mononuclear cells (hTMNC) were markedly reduced when DON was added in the cultures. The trypan blue exclusion test showed that this inhibition was not exerted through a toxic effect. IL-2 activity in the supernatants was evaluated for its ability to support IL-2 independant cell line (CTLL-2) proliferation and the results showed that DON had no marked effect on IL-2 level, but inhibited the capacity of murine peritoneal macrophages to produce IL-1 and TNF, which play very important roles in the process of inflammation and immune response. This immunosuppressive actions of DON may partly account for some of its potential in the treatment of chronic inflammatory diseases, where immunological mechanisms are known to play a major pathogenic role.

[Value of surgical treatment in patients with gastrointestinal metastatic tumors].

A retrospective review was completed on 43 patients with metastatic lesions to the gastrointestinal tract between 1980 and 1993. The most commonly involved organs were stomach and small intestine. The main features were abdominal pain (70%), acute abdomen (42%), alimentary bleeding (21%), massive in 11.6%). Operations were performed on 42 patients (palliative resection in 40 patients, enteric bypass in 2). The results support surgical intervention for patients with complaints of gastrointestinal metastases, especially in emergency patients.

[Significance of diagnosis and prognosis for breast carcinoma with examination of Ag-NORs].

The argyrophilic nucleolar organizer regions (Ag-NORs) was measured in the tissue sample aspirated with fine needle for 17 cases of breast benign lesion and 46 cases of breast cancer, by analysis of their micro-image with light microscope and computed. The results indicated that the amount of Ag-NORs in the nuclei of breast cancer was increased while the percentage of average areas occupied by Ag-NORs was decreased. It was significant statistically in comparing with benign lesion (P < 0.05). In addition, the amount of Ag-NORs in the nuclei of breast cancer with and that without metastasis was not different markedly (P > 0.05). However the percentage of average areas occupied by Ag-NORs was different statistically (P < 0.05). The conclusion of Ag-NORs is of benefit to distinguish from benign and malignant tumours of breast as well as significant for evaluating the prognosis for breast cancer.

[An analysis of burn cases in the last decade].

The clinical data of 3,911 burn patients admitted to our institute from January, 1983 to December, 1992 were analyzed mainly on the distribution of age, causes of burns, burn area, combined injuries, common complications, incidence of burn shock, mortality and cause of death. Results showed that 91.7 per cent of deceased had suffered from inhalation injury, internal organ complications and systemic infection, suggesting that inhalation injury, visceral complication and systemic infection still remain to be the three major causes of death in burn patients.

[Effect of lipid peroxide on lymphocytes and its role in postburn immunosuppression].

PURPOSE: Mechanisms of postburn immunosuppression are complicated and remain unclear. In the present experiment the effect of linoleic acid hydroperoxide (LOOH) on lymphocytes was evaluated in vitro, and then the changes in postburn lipid peroxide (LPO) levels and lymphocyte functions were measured, in order to investigate the mechanism of immunosuppression following burn. METHODS: Proliferation and IL-2 production and LPO of lymphocytes were assayed after the incubation with LOOH for in vitro study. On day 6 after induction of 11%-12% TBSA full-thickness burn, the animals were sacrificed, and proliferation and IL-2 production of splenic lymphocytes, LPO levels of the plasma, livers and spleens were measured. RESULTS: LOOH inhibited proliferation and IL-2 production and induced lipid peroxidation of lymphocytes in vitro, and Vit E could attenuate the effects of LOOH. LPO levels increased and proliferation and IL-2 production decreased after burn, but those changes in Vit E or SOD group showed no statistical significance compared with normal control group. CONCLUSION: Enhancement of lipid peroxidation and increased LPO after burn may be one of the mechanisms of postburn immunosuppression.

[Alternation of intracellular Ca2+ caused by streptomycin in the isolated cochlear OHC].

Stained with fluorescence, the intracellular free calcium concentration (Ca2+) of the cochlear out hair cell (OHC) isolated from guinea pigs was measured using ACAS 570. Streptomycin caused a remarkable rise in Ca2+ and reached peak value in 60s, decreased gradually after lasting for 60s in that value. The Ca2+ fell back to primary level in another 100s and followed by a slight drop. The distribution of free calcium in the OHC was showed. The rise of intracellular free calcium in the OHC was assumed to be a period in the ototoxicity caused by aminoglycoside antibiotics.

[A research of acute hemodynamics and hormonal changes of intravenous captopril in patients with cor pulmonale and pulmonary hypertension].

In 20 patients of cor pulmonale with heart failure and respiratory failure and in 11 healthy men, after intravenous captopril injection, acute hemodynamic and hormonal change were observed. The results indicated that intravenous captopril injection rapidly reduced cardiac preload and afterload, reduced pulmonary arterial pressure, increased cardiac output, inhibited renin system activity, decreased plasma level of thyroxin and atrial natriuretic peptide. A rapid symptomatic improvement in patients with acute or severe cor pulmonale was shown and there was no significant effect on blood gas immediately.
