[The effect of removable partial denture on the composition of bacteria on the apt-to-decay site of abutment teeth].

This study was aimed at the composition of plaques which were on the base teeth's carious sensitive sites, just after the insertion of the removable partial denture (RPD). The results showed that the ratios of bacterial detection in different teeth at the same period and those on the different sites of same teeth at different periods were not significantly different. There was a tendency that the detection ratio of the stomatococcus at the early period was higher than that of other bacteria, but at the middle period the ratios declined, and at the late period it was higher again. The detection ratio of the bacillus was on the contrary in the study. At the same time, the composition of the plaques changed significantly after the dentures had been used for 7, 14, and 21 days and 3 months. After 3 months, the ratios of the stomatococcus and the bacillas returned to their normal levels. The 3 main cariogenic bacteria were all detected in this study. The Streptococcus mutant was the dominant bacterium in the plaques and its ratio went up with the time duration. The ratio for the lower second molar was higher than that for the mandibular second premolar (P < 0.05). The detection ratios of the Actinomyces viscosus and lactobacillus were lower. In conclusion, after the insertion of RPD, the oral microbial ecosystem would be changed and the cariogenic bacteria began to implant on the surface of the base teeth. So RPD is a potential factor to cause the caries.

[Prognostic significance of P53 protein expression in patients with colorectal adenocarcinoma].

In order to investigate the prognostic significance of P53 protein expression in the patients with colorectal adenocarcinoma, we used the P53 monoclonal antibody and ABC immunohistochemical method combined with the repairing method for antigen by microwave oven to study the relationship between the overexpression of P53 protein and the prognosis in 40 cases of colorectal adenocarcinoma. The results showed that 65% (26/40) cases of colorectal adenocarcinoma were positive for overexpression of P53 protein. The 5-year survival rate for positive cases was 50%, but 87.5% for negative cases. These findings suggest that the overexpression of P53 protein closely associates with the prognosis in colorectal adenocarcinoma (P < 0.05), thus it might become a new prognostic indicator. The prognosis in colorectal adenocarcinoma could be predicted thoroughly by considering the expression of P53 protein in combination with the traditional prognostic factors.

[The role of lens epithelium in cataract formation in diabetic rats].

This study was designed to evaluate the changes and the role of lens epithelium in sugar cataract formation, in regard to the fact that the highest level of aldose reductase is found in this layer of lens. By light and electron microscopy, we examined the histological changes of central epithelium in lens of rats made diabetic with streptozotocin (STZ) with or without AL1576, an aldose reductase inhibitor, at varying periods of time ranging from 5 to 40 days after intraperitoneal injection of STZ. Also, we examined Na-K-ATPase activity in lens epithelium of rats with diabetes, diabetes plus AL1576 and normal controls at the time of 30 days. The results showed that the first detectable abnormalities occurred after 15 days of STZ injection and were limited to the lens epithelium; cell edema, intracellular vacuoles and extention of rough endoplasmic reticulum pool were remarkable; that AL1576 could prevent almost all of the lesion mentioned above; and that Na-K-ATPase activity in lens epithelium of rats with diabetes increased at the time of 30 days. The findings suggest that lens epithelium may play an important role in sugar cataractogenesis.

[Clinical study of childhood acute myelogenous leukemia].

OBJECTIVE: To investigate the clinical characteristics, treatment responsiveness and prognostic factors of childhood acute myelogenous leukemia. METHODS: Two hundred and seventy-four children with acute myelogenous leukemia diagnosed between 1978 and 1995 were retrospectively analyzed. RESULTS AND CONCLUSIONS: Male:female is 1.6:1. The median age was 7.25 years. M2 and M3 were the commonest subtypes(80%). Twenty cases were hyperleukocytosis and 24 with extramedullary involvement. Thirty two percent of the patients was Auer rods positive. Two hundred and forty-four patients received combination chemotherapy. Complete remission (CR) rates varied from 55.88% to 92.31%. Intensive chemotherapy(including DA, DA + Vm26 or Vp16) was the cornerstone for high CR rate. Other factors influencing CR rate were initial WBC count and hepatosplenomegaly.

The correlation between immune rejection and osteoinduction of allogeneic bone grafting.

OBJECTIVE: To evaluate the relationship between the immune rejection and the osteoinductive potential of bone allograft. METHODS: Allogeneic and syngeneic fresh bone, autolyzed antigen-extracted bone, bone matrix gelatin and demineralized bone matrix were implanted into the muscle of mice, and immunological tests, histological observation and alkaline phosphatase assay were performed. RESULTS: Three and 6 weeks after implantation, all kinds of allogeneic implants activated immune rejection, among them, fresh bone induced the most vigorous immune rejection and bone matrix gelatin caused the weakest response. Allogeneic autolyzed antigen-extracted bone, bone matrix gelatin and demineralized bone matrix inhibited proliferation of the lymphocytes in vitro and bone matrix gelatin had the most powerful inhibiting effect. Both allogeneic and syngeneic autolyzed antigen-extracted bone, bone matrix gelatin, and demineralized bone matrix induced heterotopic osteogenesis in vivo and bone matrix gelatin had the best osteoinductive capacity. CONCLUSION: There is a negative correlation between immune rejection to bone allograft and osteoinductive capacity of the graft.

[Carcinosarcoma of the larynx (with a case report)].

Carcinosarcoma of the larynx is a rare disease. In this article one case is reported. Combined with references we also discuss pathologic characteristic, clinical manifestation, treatment and prognosis in the article.

Supraglottic carcinoma: does preoperative radiotherapy reduce the incidence of cervical metastasis?

OBJECTIVE: To compare surgery (S) alone with combined radiotherapy and surgery (R + S) in the management of patients with supraglottic laryngeal cancer. METHODS: Between 1981 and 1994, patients were stratified according to stage and randomised to either surgery (S) or 4000cGy of radiotherapy and surgery. There were 102 patients in the S group and 99 in the R + S group who completed at least 3-year follow-up. RESULTS: Using Kaplan-Meier survival method showed no significant difference between the two groups. When the patients were grouped according to tumour stage, a significant reduction in the regional recurrence was noted in the R + S group for stage I-III disease (Cox multivariate analysis, P < 0.02). They had an increased relative risk of 1.8 (95% confidence 1.1-2.9) for neck recurrence. There was no significant difference in neck recurrence rates in the two groups for stage IV disease. When Cox proportional hazard model was used, only TNM stage (P < 0.02) and histological nodal status (positive lymph nodes, P < 0.01) were found to be independent risk factors for regional control. CONCLUSION: Preoperative radiotherapy can improve regional cervical control of stage I-III supraglottic cancer as compared with surgery alone.

[Clinical analysis of hoarseness after thyroidectomy].

The present paper reports 86 cases of hoarseness after thyroidectomy. In 37 cases, glottic paralysis was confirmed. Among them the injury of recurrent laryngeal nerve were 89.91% (33/37). In 33(36 side) cases of recurrent laryngeal nerve paralysis, left injury was 20 and right was 16. Referring to the literature author consider that: 1. the recurrent laryngeal nerve was injured easy by thyroidectomy because that thyroid gland was located closely with recurrent laryngeal nerve in neck; 2. recurrent laryngeal nerve injury after thyroidectomy was related to the character of thyroid gland tumor and times of operations; 3. incidence of superior laryngeal nerve injure in thyroidectomy was rare; 4. following up 16 cases of glottic paralysis, most of all (13/16) hoarseness was improved with the health side vocal cords overcompensation.

[The exchange and significance of the glucocorticoid receptor of circum-circulative blood lymphocyte of cases with allergic rhinitis].

It is found that the exceptional unity of blood Gr (Glucocorticoid Receptor) and F (Hydrocortisone) decreased obviously (P < 0.01) after the determination on them of circum-circulative blood lymphocyte of cases with allergic rhinitis. They showed positive interrelation, which means that the decrease of F and GR inhibited the anti-allergic action so that allergic rhinitis may occur.

[The investigation of children's Helicobacter pylori infection by 13C-urea breath test in Yangzhong area of Jiangsu province].

OBJECTIVE: Yangzhong area in Jiangsu province is one of the places in China with high gastric cancer incidence (73.19/1000,00). In order to investigate the relations between gastric cancer and helicobacter pylori infection rate. METHODS: 13C-urea breath test was conducted for 119 children aged from 4 to 12 years old. RESULTS AND CONCLUSIONS: Total H. pylori infection rate was 73.11%, and 81.08% for boys, 60.00% for girls.

[Determination of quercetin in changyanning tablets by RP-HPLC].

The quercetin in Changyannning Tablets was determined by reverse phase HPLC on Shim-Pack CLC-ODS column(150 mm x 4.6 mm) with UV detector and methanol-water-phosphoric acid(55:44.6:0.4) as mobile phase (1.0 ml/min). The average recovery relative standard deviation were 99.7% and 1.01% respectively.

[Gene expression of fibrous main collagen in the lumbar disc].

OBJECTIVE: To study the patterns of gene expression of main collagen, type I and type II in lumbar discs. METHODS: The level of collagen gene mRNA expression of type I and type II was investigated by in situ hybridization on fetal, adults and pathologic specimens with cDNA probe. Collagen in various specimens was observed by staining of sirius scarlet. RESULTS: Positive mRNA hybridization signals of collagen type I were concentrated around outlayer of annulus fibrosus. Type II collagen was highly expressed in nucleus pulposus. In adult and pathologic specimens, no hybridization signal of type II collagen was observed, and the expression level of type II collagen mRNA decreased. Positive signals appeared in nucleus pulposus. CONCLUSION: Obvious changes took place in the regulation of collagen gene expression with aging. Expression of collagen in adult and pathological disc was decreased. Abnormal appearance could be seen between collagen gene type I and type II. Expression of collagen type I mRNA appeared in nucleus pulposus.

[The changes of suppressor T-cells function in patients with RAU].

OBJECTIVE: To study the change of suppressor T-cell function in patients with RAU. METHODS: Samples of 12 patients with RAU active phase were studied by it suppressor assay. RESULTS: The T-cell suppress rates at ConA 1,2,4 and 8 mg/L were 60%, 40%, 27% and 20% respectively, and were evidently lower when compared with normal controls' 72%, 56%, 41% and 34% (P < 0.05). CONCLUSION: The suppressor T-cell function may be depressed in RAU patients.

[The effect of Vit. B12 injection into acupoints in the treatment of verruca plana].

OBJECTIVE: To study the therapeutic effect of combined western and traditional Chinese medicines for the treatment of verruca plana. METHODS: Acupoint injection of Vit. B12 was used to treat 91 patients suffering from verruca plana after unsuccessful therapy of more than 3 methods. RESULTS: Complete cure, 35 cases; significantly improved, 56 patients. The total effective rate was 100%. Compared with the Vit. B12 i.m. injection group and polyinosinic cytidy lic acid i.m. injection group, the difference was highly significant (P < 0.001). The results were also proved by histologic examination. CONCLUSION: The Vit. B12 acupoints injection is a simple and effective method for the treatment of facial verruca plana.

[Fate of massive retrieved human bone allografts].

OBJECTIVES: To study retrieved massive human allograft specimen after they had been in situ for four to sixty-five months, and to investigate the changes union, bone replacement and degradation of articular cartilage. METHOD: BMP gene expression in the specimen was also studied with in situ hybridization, immunohistochemical methods, and the observations were correlated with the clinical and isotope scan date. RESULT: The union between the allograft and the host was achieved by the formation of an external callus derived from the cortex of the host. New bone replacement took place slowly. Analysis of the articular cartilage revealed evidence that some chondrocytes had survived in non bearing area of the articular. The expression of BMP in the surface of the cortex and united callus was positive 4 months after implantation. But it was negative in the center of cortex after it had been implanted for over 12 months. An external callus formed from the cortex of the host. Some chondrocytes survived in non-bearing area of the articular. Neovasculature penetrated into osteoarticular. CONCLUSION: Bone absorption may be the cause of collapse of articular surface. The results suggest that large allografts in humans are osteo-conductive rather than inductive. Incomplete repair of osteon may be related to slowed replacement of massive human allograft.

[Distribution of B-lymphocyte subgroup CD20 and its clinical significance in patients with myasthenia gravis].

OBJECTIVE: To assess the distribution and clinical significance of B-lymphocyte subgroup CD20 in patients with myasthenia gravis. METHOD: B-lymphocyte subset CD20 was studied in patients with thymomas associated with or without myasthenia gravis using immunohistochemistry technique. RESULT: The moderate and obvious aggregation of CD20 positive cells was found in patients with myasthenia gravis, there was a significant difference when compared with those patients without myasthenia gravis (chi(2) = 26.032, P < 0.01). There was negative correlation between the distribution of CD20 positive cells and the staging of thymomas (gamma = 0.07), and no correlation between the distribution and clinical type of myasthenia gravis (gamma < 0.4). CONCLUSION: The results of this study indicated that pathological changes of thymus are more important than those of thymomas itself in the pathogenesis of myasthenia gravis. The degree of B-lymphocyte subgroup aggregation can not truly reflect clinical status because of the heterogeneity of antibody against acetylcholine receptor.

[The osteoinductivity and the dose-effect relationship with implantation of reconstituted bone xenograft: experimental study].

OBJECTIVE: To observe osteoinductivity and the dose-effect relationship with administration of reconstituted bone xenograft (RBX). METHOD: A posterior thigh muscle pouch model with RBX transplantation was established in BALB/C mice. Samples were treated at regular postoperative intervals for radiologic, histomorphologic and ALP examinations. RESULT: (1) In the groups that had been implanted with RBX, the osteoinductivity was positively correlated with the bBMP contents in RBX (r = 0.7204, P < 0.01), showing a dose-dependent relationship; (2) satisfactory osteogenesis was noted in the RBX I group, whereas no substantial bone formation was seen in the group implanted with bBMP alone apart from some evidence of osteogenic effect; (3) the alkaline phosphatase activity attained its peak on the 7th day postoperatively and kept still higher to the 42nd day. CONCLUSION: RBX is a highly osteoinductive grafting material with the treated homologous cancellous bone being a good slow-delivery carrier which can enhance the osteoinductive capacity of bBMP.
