[Allograft-prosthesis combination in limb salvage surgery].

OBJECTIVE: To study allograft-prosthesis combination in limb salvage and the rational of composite prosthesis. METHODS: Of 16 patients with bone tumors age from 19 to 60 years, 6 women 12 men, osteosarcoma was found in 4 patients, chondrosarcoma in 2, malignant giant cell tumor in 3, fibrosarcoma in 3, and other malignant bone tumor in 4. Allograft-prosthesis combination in included allograft proximal femurs with total hip arthroplasty (3 patients), allografts bipolar femoral head replacement (8), allografts with total knee arthroplasty prosthetic replacements (7). All patients received adjuvant therapy for tumor control. RESULTS: The length of follow-up ranged from 18 months to 5 years during which time 1 patients died, 3 survived as tumor-carriers, and 12 were disease free. All patients obtained better functional result in their extremities and had good quality of life. CONCLUSIONS: Both prosthetic replacement and allograft advantages, the use of large bone allografts with metallic prosthesis for the treatment of bone tumors near or involving joints can be applied with good results in selected patients having limb salvage surgery.

[Flow cytometry for testing chemosensitivity of malignant bone tumors].

OBJECTIVE: To study chemosensitivity and multidrug resistance of malignant bone tumors and to make an individual protocol of chemotherapy with sensitive antitumor agents. METHODS: Apoptosis induced by MTX, ADM, MMC, VP-16, VCR and CTX, and chemosensitivity in 32 fresh specimens of malignant bone tumors were analyzed by flow cytometry (FCM) and quantitative immunofluorescence. The expression of P-glycoprotein (P170) in the specimen and the relation between P170, multidrug resistance, and chemosensitivity were detected. RESULTS: The rate of apoptosis was significantly different in malignant bone tumor cells treated with different drugs: MTX (30.50 +/- 10.22)%, ADM (26.28 +/- 9.35)%, MMC (23.11 +/- 7.38)%, VP-16 (18.17 +/- 6.14)%, VCR (4.44 +/- 2.55)%, and CTX (1.22 +/- 0.59)%. The individual variation of cell apoptosis was prominent in the specimens of same pathological type treated with same agent. There was lower chemosensitivity in the malignant bone tumors with a high expression of P170. CONCLUSIONS: Chemosensitivity testing by flow cytometry is a simple, quick and sensitive assay. Quantitative analysis of expression of P170 could be used to predict multidrug resistance and its chemosensitivity of malignant bone tumors.

[Digital subtraction angiography of cerebrum for diagnosis of cavernous sinus fistula and its intravascular treatment].

OBJECTIVE: To evaluate the diagnosis of digital subtraction angiography (DSA) of cerebral arteries and the intravascular treatment for carotid artery-cavernous sinus fistula (CCF) mainly manifesting as pulsating exophthalmos. METHODS: Digital subtraction angiography (DSA) of cerebral arteries was carried out for 15 patients with pulsating exophthalmos. Of them, detectable balloon and tungsten filament microcoil were used for intravascular embolism treatment in 12 cases, and the 3 cases with CCF of the branch of external carotid artery were not treated. RESULTS: Of the 15 cases, 12 were fallen in the CCF of high flow type of unilateral internal carotid artery, and 3 in the CCF of low flow type of unilateral external carotid artery. The symptoms and signs of intravascular embolism disappeared after treatment in 11 cases. CONCLUSION: Cerebral artery DSA and intravascular therapy are the ideal methods for the diagnosis and treatment of CCF mainly manifesting as pulsating exophthalmos.

[Studies on relationship between occupation and pregnancy outcome].

OBJECTIVE: To study the effects of maternal occupation on reproductive outcomes, including pregnancy outcome and neonatal status, and complications of pregnancy and delivery. METHODS: Data were extracted from 12 224 obstetric records of eight hospitals in Beijing, and all the subjects recruited in the study were interviewed before discharge during from May 1992 to April 1993. Pregnancy and reproductive outcomes were analyzed according to their occupational classifications, adjusted for the potential confounding factors, such as age, smoking and drinking during pregnancy and history of spontaneous abortion. RESULTS: Incidence rates of pregnancy induced hypertention, low birth weight, intrauterine growth retardation and postpartum hemorrhage were higher in the workers than that of the average levels. Incidence rates of threatened abortion, gestational anemia, stillbirth and uterine atony in professional persons, of prolonged pregnancy and neonatal asphyxia in farmers, of low birth weight in housewives, of gestational anemia and of uterine atony in administrative persons all were higher than the average levels. CONCLUSION: Occupation undertaken by pregnant women was associated with the adverse pregnancy and reproductive outcomes to certain extent.

[Effects of occupational exposure to formamidines on cardiovascular functions].

OBJECTIVE: To understand the possible effects of occupational exposure to formamidines on human cardiovascular function, this study was carried out. METHODS: Sixteen farmers spraying chlordimeform, 14 packers packaging chlordimeform and 23 packers packaging mono-formamidine were followed-up pre- and post-exposure. Their urinary excretion of formamidine or its metabolite, as well as air concentrations of formamidine at their work places and their skin contamination with it were measured to estimate the exposure level. Furthermore, 24-hour urinary level of vanillinmandelic acid (VMA) was analyzed among mono-formamidine packers. RESULTS: Their whole body skin contaminated with 3.240 and 2.142 g/cm(2) of chlordimeform in the sprayers and packers, respectively. Their hand skin contaminated with mono-formamidine at 6.59 g/cm(2) in the packers. It indicated that the major route of exposure to formamidine was skin contamination. Urinary levels of formamidines or their metabolites increased significantly after exposure, reaching 6.194 and 3.378 micromol/L for the sprayers and packers exposed to chlordimeform, respectively, and 2.760 - 3.427 micromol/L for mono-formamidine in the packers. Their heart rates slowed down, P-R and Q-T intervals prolonged and blood pressure reduced after exposure, as compared with those before exposure. Consistency of the results in several studies demonstrated that formamidines could induce changes in the indices for cardiovascular functions under the relatively low exposure levels at present. Decrease of urinary VMA from 0.068 micromol/L pre-exposure to 0.040 - 0.055 micromol/L post-exposure suggested that catecholamine could play a role in these effects. CONCLUSION: Formamidines has certain effects on human cardiovascular function. It is suggested that attention be paid to the changes in cardiovascular functions of those exposed in their health surveillance.

[Relationship between bone mineral density and blood and urine biochemical indices in women].

OBJECTIVE: To study the relationship between bone mineral density (BMD) and some biochemical indices in their blood and urine of women. METHODS: Fasting urinary calcium (Ca) and creatinine (Cr), ratio of Ca to Cr, urinary hydroxyproline (Hyp), ratio of Hypr to Cr were determined in 163 healthy women, and activity of serum alkaline phosphatase (AKP) were determined in 182 healthy women. BMD at the 1/10 lower end and 1/3 middle-lower site of the radius were measured with single photon absorptiometry for all of them. RESULTS: Serum AKP and ratio of urinary Hyp/Cr increased with age, especially postmenopause, and correlated negatively with BMD. Fasting urinary Ca/Cr ratio also increased with age, especially postmanopause, but not correlated with BMD. CONCLUSION: Serum AKP and fasting urine Hyp/Cr ratio could be used as indicators to reflect bone remodeling and absorption, respectively.

[Experimental and clinical study to remove lead and to prevent lead damage of kidney with salt of high dose KI].

OBJECTIVE: To study and make a salt prevent lead poisoning. METHODS: SD rats were given 0.25% lead acetate by food intake and were given 10 mg.kg-1.d-1, 20 mg.kg-1.d-1 and 30 mg.kg-1.d-1 potassium iodide from drinking water in respective groups, in order to look for the dose of KI which prevents lead poisoning. The salt to prevent lead poisoning was made, 50 workers were given the salt as to flavouring of daily food intake. PbB, Pbu, ZPP, delta-ALA, NAG(N-acetyl-beta-D-glucosaminidase), RP(Retinol protein) were observed. RESULTS: The dose of KI to prevent experimental lead poisoning of SD rats was 10 mg.kg-1.d-1 by oral intake. People who contacted lead had an increase of urine lead in the third day, and had most urine lead on ninth day after taking the salt with KI. Normal urine lead on the fourth week. Pre- and post-oral intake of the salt with high dose KI, Hb, ZPP, delta-ALA, PbB, NAG and Rp the people mentioned above had the following indexes, (101.2 and 128) g.L-1 (P = 0.05), (33.5 and 7.7) micrograms.g-1 Hb(P < 0.01), (123.12 and 65.35) mumol.L-1 (P < 0.05), (2.123 and 0.85) mumol.L-1 (P < 0.01), (20.9 and 6.7) U.L-1 (P < 0.01), (1.5 and 0.34) mg.L-1 (P < 0.05) respectively. CONCLUSION: The salt with 1% KI may remove lead and prevent lead damage of the kidney.

[Studies on the stability of sodium taurocholate in different media and in the course of sterilization].

OBJECTIVE: To investigate the stability of the main active constituent of snake gall sodium taurocholate, TCANa in 25% alcoholic solution and artificial gastric juice, and in the course of hot pressurized sterilization. METHOD: The stability of the solution was predicted by an accelerated isothermal test, and the content of TCANa was determined by TLC-colorimetry. RESULT: The half-time(t1/2) of TCANa in 25% alcoholic solution is 60.57 d at 25 degrees C, and in artificial gastric juice 11.37 d at 37 degrees C, the loss rate under the condition(115 degrees C, 30 min) of sterilization about 3%. CONCLUSION: TCANa in 25% alcoholic solution is unstable. Therefore it is suggested that in order to raise the stability of TCANa the alcoholic concentration be increased and the solution be stored at temperatures below 0 degree C.

[The selection of tympanoplastic type in treating chronic suppurative otitis media].

OBJECTIVE: To investigate the effect of different type of tympanoplasty on chronic suppurative otitis media (CSOM). METHOD: Myringoplasty, focuses clearance and myringoplasty, focuses clearance and ossiculoplasty and myringoplasty were performed in 189 cases of CSOM. RESULT: The cure rate of perforation of tympanic membrane with myringoplasty was above 99% accompanied by the varying improvement in acoustic sensibility. CONCLUSION: Different types of tympanoplasty should be used according to the CSOM. The slighter the pathological changes of CSOM, the more the hearing improve. The better effects was in tympanoplasty of simple type. The tympanic exploration is necessary to be included in tympanoplasty.

[Malignant melanoma of external ear: 14 cases report].

OBJECTIVE: To improve the diagnosis and treatment level of malignant melanoma in external ear. METHOD: 14 cases with malignant melanoma of external ear were reported. All patients were treated by surgical excision. RESULT: 13(in 14) cases were followed up. 7 patients were survival with no tumor, the others died. The 5-year survival rate for the whole series was 50.0%. CONCLUSION: The malignant melanoma of external ear has rapid progress and poor prognosis, early detection and treatment are necessary. Extensive excision of malignant melanoma was principle method of the treatment. Recurrent tumor should be treated by combined operation with radiation.

[Neurilemmoma of the nasal cavity and paranasal sinuses (report of 12 cases)].

OBJECTIVE: To further understand the neurilemmoma of nose. METHOD: 12 cases of neurilemmomas of the nasal cavity and paranasal sinuses were treated successfully by operation, 2 cases of malignant change lesions were treated by extensive resection and radiation. RESULT: 11 in 12 cases were followed up. 9 cases were cured and no any serious complications, 2 cases of malignant tumor were died. CONCLUSION: Early stage diagnosis is important. CT scan should be made as essential examination, complete surgical resection is the choice of treatment for primary lesions; malignant change lesions should be treated by combined operation with radiation.

[Determination of triptolide in tripterygium preparations by gradient high performance liquid chromatography].

A gradient elution method was developed for the separation and determination of triptolide in Tripterygium wilfordii Hook preparations by high performance liquid chromatography (HPLC) using YWG column made in China. Methanol with 0.05 mol/L KH2PO4 was used as mobile phase, and the photodiode array detector was used at lambda = 218 nm. The sample of tripterygium oral preparation was extracted with ethanol and then by chloroform. The extract solution was evaporated and the residue was dissolved in 10 mL of ethanol as a sample solution. The spectrum was obtained by the subtraction of spectrum of ethanol from that of sample solution. In comparing with isocratic elution, the gradient operation used in this paper had higher resolution and sensitivity. The calibration curve was linear over the range of 1.32-21.10 mg/L (r = 0.9999). The average recovery of the method was 97.59%. The method was applied to separate and determine the triptolide in tripterygium tablets and the results indicated that the triptolide content by this method was less than that by isocratic and label-claimed one.

[Study on high performance liquid chromatography/indirect photometric detection of fosfomycin with acridine as detection reagent].

Fosfomycin is a broad-spectrum antibacterial agent which has strong polarity and weak UV absorption. In this work, a method using standard ODS column and conventional UV detector set at 254 nm to analyse fosfomycin was investigated. Firstly, several UV detection reagents were tested and acridine(A) was found to give the highest indirect detection response and proper modification on retention of fosfomycin. With acridine as the UV detection reagent, the mobile phase parameters which affect retention and detection response of solute are discussed, e.g. pH, concentration of organic solvent and concentration of acridine. The mechanism is interpreted for each case. After optimization, we adopted mobile phase as V(CH3OH):V(H2O) = 50:50, pH 3.18, [HAc] = 2 mmol/L, [A] = 1 x 10(-5) mol/L. A new chromatographic system monitored by indirect photometry for detection of fosfomycin was established. This method was used for quantitative analysis, the minimum detectable level was 10 mumol/L, the relative deviation was 4.51%.

[A study on distribution modeling of the electrical stimulation under surface nerve stimulation electrodes for somatosensory evoked potential].

The detection of somatosensory evoked potential (SEP) has been an important approach to determining the function of spinal cord and peripheral nerve. Transepidermal Electrical Neuromuscular Stimulation (TENS) on the posterior tibial nerve for SEP is one of the applications and can improve the effectiveness. This paper theoretically studies the distribution of potential field and current density field for human tissue under TENS. A mirror method is used and the analytical solution of the potential distribution is obtained.

[Antibiotic resistance and plasmid profiles of Vibrio isolates from cultured Sparus sarba].

A total of 51 potential pathogenic vibrios were isolated from moribund silver seabream Sparus sarba, which were collected from local fish farms of Hong Kong. All the isolates were classified and identified as 7 species by the API 20 E system and the scheme of Alsina & Blanch. These species were Vibrio alginolyticus (24 strains), Vibrio vulnificus (12 strains), Vibrio parahaemolyticus(7 strains), Vibrio logei(4 strains), Vibrio pelagius II(2 strains), Vibrio fluvialis (1 strains) and Vibrio meditterranei (1 strains). Among these isolates, the three predominant species (V. alginolyticus, V. vulnificus and V. parahaemolyticus) were confirmed to be virulent to sea bream by experimental challenge. All isolates were also screened for plasmid DNA by agarose gel electrophoresis and tested for susceptibility to 16 antimicrobial agents by the agar dilution method. Of the 51 isolates examined, all strains were sensitive to ceftriaxone, streptomycin, nalidixic acid and rifampicin, and almost all were sensitive to ceftazidime, netilimicin, chloramphenicol and sulfamethoxazole except one or two strains. Most isolates were resistant to ampicillin (60. 8%), cefuroxime (66.7%), amikacin(55%), kanamycin(58.8%) and trimethoprinm (76.5%). Fifteen of the 51 isolates harboured 1-4 plasmids, with sizes ranging from 9 to 123 kb. Both the plasmids and the associated antimicrobial resistance (ampicillin, cefuroxime and trimethoprim) of 9 isolates could be transferred to recipient by single-step conjugation, however, the frequencies were very low, ranging from 10(-11) to 10(-9). The present results indicate that resistance to these antibiotics is chromosomal.

[SDS-PAGE and EITB analysis of the protein components of different isolates of Schistosoma japonicum in China and Japan].

AIM: To make a supplementary observation on the protein components prepared from adult S. japonicum of 4 different isolates from Zhejiang, Jiangxi, Taiwan Provinces of China and Japan in origin and to observe antigen reactivity of the above-mentioned isolates together with adult worms from Anhwi, Hubei, Sichuan and Yunnan isolates against heterologous anti-Oncomelania h. hupensis (collected from Guangxi, China and Japan) sera by EITB. METHODS: SDS-PAGE and EITB. RESULTS AND CONCLUSION: SDS-PAGE showed that by Coomassie blue staining, male S. japonicum from Jiangxi, Zhejiang, Taiwan Provinces and Japan isolales revealed 7-17 bands while female worms revealed 1-6 bands. The protein patterns of Taiwan and Zhejiang male worm were similar, but slight difference could be seen above 81 kDa. By silver staining, male worms of the 4 isolates revealed 10-23 bands while female worms revealed 1-19 bands. Male worms from Japan not only showed less bands but also differed in their pattern as compared to those of other Chinese isolates. Results of EITB revealed that each of the 4 isolates had slightly different pattern but all of the tested isolates had common antigens with their heterologous snail hosts i.e., Oncomelania h. hupensis from Guangxi, China and Japan.
