[Studies of type I collagen related substances as bone metastatic markers of prostate carcinoma with special regard to serum carboxyterminal propeptide of type. I. Procollagen (PICP), cross-linked carboxyterminal telopeptide of type I collagen (ICTP) and Urinary deoxypyridinoline levels].

BACKGROUND: To investigate if serum levels of carboxyterminal propeptide of type I procollagen (PICP), cross-linked carboxyterminal telopeptide of type I collagen (ICTP) and urinary levels of deoxypyridinoline (D-Pyr) are useful markers of bone metastasis in patients with prostate carcinoma, we measured these markers in patients with untreated benign prostatic hyperplasia (BPH) and untreated prostate carcinoma (PCA). METHODS: Serum PICP, ICTP and urinary D-Pyr levels were determined in 53 patients; 16 patients with BPH, 15 patients with PCA without bone metastasis (stage A, B, C and D1) and 22 patients with PCA with bone metastasis (stage D2). At the same time correlations among these markers and serum total alkaline phosphatase (ALP) activity were studied. RESULTS: Serum PICP, ICTP and urinary D-Pyr levels in the PCA patients with bone metastasis were significantly higher than those of BPH. The serum levels of PICP in patients with PCA with bone metastasis group were significantly higher than those of without bone metastasis group. The serum levels of ICTP in patients with PCA without bone metastasis group were significantly higher than those of BPH group, while no significant difference was observed between PCA group with and without bone metastasis. In the PCA patients with bone metastasis, serum PICP and serum total alkaline phosphatase (ALP) activity were significantly correlated (r = 0.80). In these patients, serum ICTP and urinary D-Pyr levels were also significantly correlated (r = 0.70). CONCLUSION: These results suggest that serum PIPC, ICTP and urinary D-Pyr are the useful markers to quantitate bone metastasis in the patients with PCA. Moreover, the determination of serum ICTP levels may be significant for detecting occult bone metastasis in the patients with PCA.

Bonding mechanism of Ketac-Molar Aplicap and Fuji IX GP to enamel and dentin.

PURPOSE: To evaluate the bonding mechanism to enamel and dentin of two chemically-cured restorative glass ionomer cements (GICs). MATERIALS AND METHODS: Buccal surfaces of six caries-free premolars stored in a 1% chloramine solution were ground and then polished with 600 grit SiC paper to expose both enamel and dentin. Ketac-Molar Aplicap (after conditioned with Ketac-Conditioner) or Fuji IX GP (after conditioned with Cavity Conditioner) was applied on the enamel and dentin according to the manufacturers' instructions. Ketac-Molar Aplicap was coated with Ketac-Glaze and Fuji IX GP with GC Fuji Varnish. After 24-hr immersion in water, all teeth were sectioned bucco-lingually in the center of the bonded surface. Bonded interfaces of one of the halves of the sectioned specimens were etched with 35% phosphoric acid gel for 10 s and then soaked in 5% sodium hypochlorite solution for 5 min. All specimens were dehydrated with ethyl alcohol and dried with HMDS. Bonding interfaces were coated with gold-palladium and observed under the SEM. RESULTS: For both Ketac-Molar Aplicap and Fuji IX GP, an intimate adaptation between the material and the enamel was observed. Both materials bonded to dentin without gap formation. For both Ketac-Molar Aplicap and Fuji IX GP, neither resin tags nor hybrid layer was observed in any specimen.

Bonding mechanism of Compoglass to dentin in primary teeth.

This study evaluated the bonding mechanism of Compoglass compomer to dentin in primary teeth. Buccal or labial dentinal surfaces of 20 human extracted, non carious primary teeth stored in 4 degrees C physiological saline solution were obtained by grinding on silicon carbide paper (final grit 600). The specimens were divided into two groups of 10 teeth each: (1) unetched dentin, Compoglass SCA, Compoglass; and (2) dentin etched with 10% phosphoric acid (Etch-AII), Compoglass SCA, Compoglass. The Compoglass SCA and Compoglass compomer were placed according to the instructions of the manufacturer, except Group 2 were the dentin was first etched with 10% phosphoric acid for 30 seconds. Twenty-four hours after placing the compomer over the treated dentinal surface, the specimens were dehydrated with a series of alcohol and freon. Then critically point dried. The specimens were split with a chisel and the compomer/dentin interface evaluation was performed with the SEM. The results showed that when the instructions of the manufacturer were followed (Group 1) the compomer showed a very close relation to the dentin with some tag structures penetrating the dentin. When phosphoric acid etching preceded the compomer placement a hybrid layer with tags penetrating the dentin was noted in most specimens.

[Three cases of colorectal cancer with lung metastasis successfully treated with combination chemotherapy using 5'-DFUR and MMC].

Three patients with colorectal cancer and pulmonary metastasis who had previously undergone surgical treatment, were treated by combination chemotherapy with CDDP and 5-FU regimen, but the growth of the metastatic tumor could not be controlled finally. Thus, a new strategy of combination chemotherapy using 5'-DFUR and MMC were investigated in these cases, and resulted in suppressing the tumor growth successfully without admission. The procedure for treating pulmonary metastasis of colorectal cancer has not been established clearly. This new regimen may play an important role from the standpoint of not only its effectiveness against tumor growth but also the quality of life of patients.

Serum concentration of type I collagen metabolites as a quantitative marker of bone metastases in patients with prostate carcinoma.

BACKGROUND: Bone scans, widely used for the detection of bone metastases from prostate carcinoma, can neither quantitate metastatic lesions nor detect osteolytic lesions. METHODS: Serum concentrations of the carboxyterminal propeptide of Type I procollagen (PICP), the carboxyterminal pyridinoline cross-linked telopeptide of Type I collagen (ICTP), and prostate specific antigen (PSA) were measured by radioimmunoassays in 48 patients with benign prostatic hyperplasia (BPH), 25 patients with prostate carcinoma (PCA) without bone metastases, and 36 patients with PCA and bone metastases. RESULTS: Serum concentrations of PICP were significantly higher in patients with PCA with bone metastases than in patients with BPH or PCA without bone metastases. No significant differences were observed between patients with BPH and those with PCA without bone metastases. Serum ICTP concentrations were significantly higher in patients with PCA than in patients with BPH regardless of the presence or absence of bone metastases. Serum concentrations of PICP, ICTP, and PSA correlated significantly with Soloway's grading system for bone scans. The serum concentrations of PICP and ICTP in patients without bone metastases showed a significant downward trend in response to antiandrogen therapy. CONCLUSIONS: These observations suggest that the serum concentrations of PICP and ICTP are quantitative markers of bone metastases from PCA when followed serially in individual patients.

Serial lectin affinity chromatography with concavalin A and wheat germ agglutinin demonstrates altered asparagine-linked sugar-chain structures of prostatic acid phosphatase in human prostate carcinoma.

Differences between human prostate carcinoma (PCA, five cases) and benign prostatic hyperplasia (BPH, five cases) in asparagine-linked (Asn) sugar-chain structure of prostatic acid phosphatase (PAP) were investigated using lectin affinity chromatography with concanavalin A (Con A) and wheat germ agglutinin (WGA). PAP activities were significantly decreased in PCA-derived PAP, while no significant differences between the two PAP preparations were observed in the enzymatic properties (Michaelis-Menten value, optimal pH, thermal stability, and inhibition study). In these PAP preparations, all activities were found only in the fractions which bound strongly to the Con A column and were undetectable in the Con A unbound fractions and in the fractions which bound weakly to the Con A column. The relative amounts of PAP which bound strongly to the Con A column but passed through the WGA column, were significantly greater in BPH-derived PAP than in PCA-derived PAP. In contrast, the relative amounts of PAP which bound strongly to the Con A column and bound to the WGA column, were significantly greater in PCA-derived PAP than in BPH-derived PAP. The findings suggest that Asn-linked sugar-chain structures are altered during oncogenesis in human prostate and also suggest that studies of qualitative differences of sugar-chain structures of PAP might lead to a useful diagnostic tool for PCA.

Establishment of methylation-sensitive-representational difference analysis and isolation of hypo- and hypermethylated genomic fragments in mouse liver tumors.

Methylation of CpG sites in the genome, which is generally conserved during cell replication, is considered to play important roles in cell differentiation and carcinogenesis. However, investigations on changes in methylation status have been limited to known genes. To make a genome-wide search for differentially methylated genes, we developed a methylation-sensitive-representational difference analysis (MS-RDA) method. The representation of the genome was prepared using the methylation-sensitive restriction enzyme HpaII, and the mixture ratio of tester and driver DNAs was optimized to detect differences in methylation status of a single copy per diploid mammalian genome. By performing comparative MS-RDA of one hepatocellular carcinoma and of background liver tissue of one mouse treated with a food carcinogen (2-amino-3,4-dimethylimidazo[4,5-f] quinoline), we were able to identify (i) extensive hypomethylation of long interspersed nuclear element repetitive sequences in a number of hepatocellular carcinomas, (ii) reduction of the gene dosage of their mitochondrial DNA, and (iii) a hypermethylated DNA fragment of unknown origin. Furthermore, by adding the clones obtained in the first MS-RDA to the driver DNA [MS-RDA with elimination of excessive clones (MS-RDA-WEEC)], nine DNA fragments that could not be detected at the first MS-RDA were isolated as differentially methylated DNA fragments. MS-RDA, combined with MS-RDA-WEEC, is thus a promising approach to identify DNA fragments differentially methylated in two DNA sources.

Validity of electromyograms and tension as a means of motor unit number estimation.

The purpose of this study was to validate three different techniques for obtaining motor unit number estimates of the rat medial gastrocnemius muscle. These consisted of two electromyographic techniques using unprocessed and digitally averaged unitary muscle action potentials, and one mechanical technique. We also injected subunit B of cholera toxin into this muscle and counted the number of spinal motor neurons labeled by the toxin. Our results revealed that a motor unit number estimate obtained by using the unprocessed unitary muscle action potential was statistically different from the actual number of motor neurons. The other two motor unit number estimates, however, were not statistically different from the actual motor neuron number. These two methods thus seem more appropriate than the first electromyographic method for obtaining an accurate motor unit number estimate.

Study of the structure of struvite stones with scanning electron microscopy and energy-dispersive X-ray microanalysis.

Topographic features of eight struvite calculi were investigated with scanning electron microscopy (SEM) equipped with energy-dispersive X-ray microanalysis (EDX). Perpendicularly cracked fragments showed concentric laminations composed of compact and loosely packed strata alternately. Magnesium and phosphorus were detected in the compact strata with their characteristic dispersive X-ray spectra. There existed numerous spherular crystals with smooth or porous surfaces and scattered penta- or hexa-hedral coffin-lid shaped crystals in the loosely packed strata. The former crystals showed the dispersive X-ray spectra of calcium and phosphorus, and were estimated to be calcium phosphate (CaP). The latter ones were presumed to be magnesium ammonium phosphate (MAP) with EDX. The surfaces of the fragments were cracked like eggshells and displayed numerous CaP crystals and scattered MAP crystals in most cases, while in only 1 case some faces of pieces demonstrated wavy MAP phases, sundry areas which were rimmed with aggregated CaP spherulites. The mean molar ratios of Mg/P and Ca/P in each case were 0.88-1.03 and 1.25-1.52, respectively. Though EDX was inadequate to determine the accurate chemical formula of CaP and MAP crystals by detecting their molar ratios of Ca/P and Mg/P with EDX, SEM/EDX is useful to observe these urolith crystals and to surmise them to be CaP or MAP crystals by detecting their atomic elements with EDX.

Serum levels of the carboxy-terminal propeptide of type I procollagen and the pyridinoline cross-linked carboxy-terminal telopeptide of type I collagen as markers of bone metastases in patients with prostate carcinoma.

OBJECTIVES: To evaluate serum levels of the carboxy-terminal propeptide of type I procollagen (PICP) and the pyridinoline cross-linked carboxy-terminal telopeptide of type I collagen (ICTP) as possible markers of bone metastases in patients with prostate carcinoma (PCA) patients. METHODS: Levels of PICP and ICTP were measured by radioimmunoassay of serum from 29 patients with benign prostatic hyperplasia (BPH), 17 patients with PCA without bone metastases and 29 patients with PCA and bone metastases. RESULTS: Serum levels of PICP in patients with PCA and bone metastases (mean +/- SD 304.3 +/- 365.9 ng/ml) were significantly higher than those in patients with BPH (95.6 +/- 27.1 ng/ml) and in patients with PCA without bone metastases (111.1 +/- 44.6 ng/ml). No significant difference was observed between the BPH and the PCA without bone metastases groups. Serum levels of ICTP were significantly higher in the PCA with (10.4 +/- 10.3 ng/ml) and without bone metastases groups (7.7 +/- 6.0 ng/ml) than in the BPH group (3.7 +/- 2.0 ng/ml). There was no significant difference between the PCA groups with and without bone metastases. CONCLUSION: Serum PICP levels correlate well with results of bone scans, while serum ICTP levels increase in patients with PCA regardless of the presence of bone metastases.

[A case of concurrent bilateral adrenocortical adenoma causing Cushing's syndrome].

A 43-year-old woman presented with obesity and lumbago. Endocrinological examinations revealed normal plasma cortisol levels and a suppressed serum adrenocorticotropic hormone (ACTH) level. On venous sampling, markedly elevated plasma cortisol levels were observed for bilateral adrenal veins (243 and 62.3 micrograms/dl on the right and left sides, respectively). Although the computed tomogram revealed bilaterally enlarged adrenal glands, 131I-adosterol scintigram showed a strong uptake only on the right side. Right adrenalectomy successfully relieved Cushing's syndrome. Pathological diagnosis was adrenocortical adenoma, 3.5 cm in diameter. Cushing's syndrome recurred in 9 years. At that time, she underwent left subtotal adrenalectomy including a 3-cm adrenocortical adenoma. Postoperative convalescence has been uneventful with oral steroid supplementation. All 14 previously reported cases of bilateral adrenocortical adenoma (BAA) causing Cushing's syndrome as well as the present case were concurrent and dominant in females of reproductive age. This suggests that some cofactors other than ACTH, such as estrogen, contribute to the pathogenesis of BAA.

Comparison of two dentin adhesives to primary vs. permanent bovine dentin.

The purpose of this study was to compare the shear bond strengths of two adhesive systems to the primary and permanent dentin. Labial surfaces of extracted and frozen bovine mandibular primary incisors and permanent incisors were ground with #600 grit SiC paper to expose dentin. Bisco Dental Products All Bond 2 (Group AB2) or Sunmedical Co. Superbond D Liner (Group SDL) tooth surface conditioner and adhesive were applied and bonded with resin composite. A shear bond strength (SBS) test was performed and the data were analyzed by an ANOVA (P < 0.05). After the SBS test, the test surfaces of the dentin and the resin were observed using SEM. SBS on the primary dentin were significantly higher than those on the permanent dentin, both in the nonthermal cycled groups and the thermal cycled groups with the exception of the thermal cycled group of Group SDL. In the thermal cycled group of Group SDL, there was no significant difference between SBS on the primary dentin and SBS on the permanent dentin. Bond strengths on the primary dentin were found to be significantly higher than those on the permanent dentin, when using All Bond 2 or Superbond D Liner adhesive systems.

A rat genetic map constructed by representational difference analysis markers with suitability for large-scale typing.

Representational difference analysis (RDA) was applied to isolate chromosomal markers in the rat. Four series of RDA [restriction enzymes, BamHI and HindIII; subtraction of ACI/N (ACI) amplicon from BUF/Nac (BUF) amplicon and vice versa] yielded 131 polymorphic markers; 125 of these markers were mapped to all chromosomes except for chromosome X. This was done by using a mapping panel of 105 ACI x BUF F2 rats. To complement the relative paucity of chromosomal markers in the rat, genetically directed RDA, which allows isolation of polymorphic markers in the specific chromosomal region, was performed. By changing the F2 driver-DNA allele frequency around the region, four markers were isolated from the D1Ncc1 locus. Twenty-five of 27 RDA markers were informative regarding the dot blot analysis of amplicons, hybridizing only with tester amplicons. Dot blot analysis at a high density per unit of area made it possible to process a large number of samples. Quantitative trait loci can now be mapped in the rat genome by processing a large number of samples with RDA markers and then by isolating markers close to the loci of interest by genetically directed RDA.

A neural cell-type-specific expression system using recombinant adenovirus vectors.

We demonstrated neural cell-type-specific gene expression using an adenovirus vector, which is useful for delivering foreign genes into quiescent neural cells. We produced eight recombinant replication-defective adenoviruses carrying the lacZ reporter gene driven by various promoters, including those of the L7/PCP2 gene (highly restricted expression in cerebellar Purkinje cells), and the myelin basic protein (in oligodendrocytes) gene. We demonstrated in vitro and in vivo promoter-driven, neural cell-type-specific gene expression by recombinant adenoviruses. The genes were transferred into these recombinant adenoviruses and expressed with high levels of efficiency in vitro and in vivo. In primary culture, the recombinant adenoviruses AdexL7-NL-LacZ and AdexMBP-NL-LacZ appeared to be expressed in a Purkinje cell and oligodendrocyte-specific manner. Introduction of 10(8) pfu of these viruses into the adult rat cerebellum by stereotactic injection yielded neural cell-type-specific expression without apparent toxicity to the animals. Thus, adenovirus vectors are useful for cell-type-specific therapeutic uses and in studies requiring neural cell-type-specific gene expression.

Microsatellite instability and loss of heterozygosity on chromosome 10 in rat mammary tumors induced by 2-amino-1-methyl-6-phenylimidazo[4,5-b]pyridine.

Microsatellite instability (MI) and loss of heterozygosity (LOH) were examined in mammary tumors induced in Sprague-Dawley x F344 F1 female rats by 2-amino-l-methyl-6-phenylimidazo[4,5-b]pyridine (PhIP). Examination of 62 microsatellite loci revealed MI in nine of 15 (60%) PhIP-induced mammary tumors, and five of these MI-positive tumors had mutations in more than one microsatellite locus. In contrast, two of 12 (17%) 7,12-dimethylbenz[a]anthracene (DMBA)-induced mammary tumors were MI positive but had mutations at only one locus each. Further, by using 37 polymorphic markers specific LOH was observed in four of 15 PhIP induced mammary tumors on distal parts of rat chromosome 10, which is homologous to human chromosome 17q with no background level of LOH. Similarly, DMBA-induced mammary tumors showed specific LOH on the same region of chromosome 10. These data suggest that mismatch-repair deficiency and loss of chromosome 10 are involved in carcinogenesis of PhIP-induced rat mammary tumors.

A functional model for G protein activation of the muscarinic K+ channel in guinea pig atrial myocytes. Spectral analysis of the effect of GTP on single-channel kinetics.

To elucidate the functional interaction between the active G protein subunit (GK*) and the cardiac muscarinic K+ (KACh) channel, the effect of intracellular GTP on the channel current fluctuation in the presence of 0.5 microM extracellular acetylcholine was examined in inside-out patches from guinea pig atrial myocytes using spectral analysis technique. The power density spectra of current fluctuations induced at various concentrations of GTP ([GTP]) were well fitted by the sum of two Lorentzian functions. Because the channel has one open state, the open-close transitions of the channel gate represented by the spectra could be described as C2<-->C1<-->O. As [GTP] was raised, the channel activity increased in a positive cooperative manner. The powers of the two Lorentzian components concomitantly increased, while the corner frequencies and the ratio of the powers at 0 Hz remained almost constant. This indicates that G protein activation did not affect the gating of each channel but mainly increased the number of functionally active channels in the patch to enhance the channel activity. Regulation of the number of functionally active channels could be described by a slow transition of the channel states, U (unavailable)<-->A (available), which is independent of the gating. The equilibrium of this slow transition was shifted by GTP from U to A. Monod-Wyman-Changeux's allosteric model for the channel state transition(U<-->A) could well describe the positive cooperative increase in the channel availability by GTP, assuming that, in the presence of saturating concentrations of ACh, [GK*] linearly increased as [GTP] was raised in our experimental range. The model indicates that the cardiac KACh channel could be described as a multimer composed of four or more functionally identical subunits, to each of which one GK* binds.
