Three new variants of pyrimidine 5'-nucleotidase (P5N) found in Japan were studied. They are characterized by slow electrophoretic mobility and a high Michaelis constant for cytidine 5'-monophosphate as has been described in previously reported cases, but are unique with respect to the thermostability test and in pH optima. P5N Kumamoto was thermostable and showed a markedly basic shift in the pH optimum. P5N Nagano was thermolabile and had a normal pH optimum. P5N Kurume was thermostable and showed a basic shift in the pH optimum. These data suggest that these variants have structural gene mutations and that they are clearly distinguished from previously reported cases.
The long-term survival of alveolar rhabdomyosarcoma of the perianal region in a 13-month-old infant is described. Local excision with chemotherapy consisting of vincristine, actinomycin D, and cyclophosphamide over a period of two years has resulted in eight years of tumor-free survival with normal growth and development. Our case and review of the literature suggest that local excision with chemotherapy may be an appropriate treatment for Group I perianal rhabdomyosarcoma in children.
The effects of adriamycin and daunomycin on cardiac function were studied in 33 patients with acute leukemia (16 cases), neuroblastoma (5 cases), osteosarcoma (4 cases), malignant lymphoma (3 cases), rhabdomyosarcoma (3 cases) and malignant histiocytosis (2 cases). The left ventricular function was evaluated by serial echocardiographic assessment. Ejection fraction (E.F.) and shortening fraction (S.F.) of left ventricule were calculated from echocardiographic measurements. Seven of 33 cases (21.2%) revealed the decrease of E.F. and S.F. There was the significant correlation between total doses of daunomycin and E.F. Three patients died of severe congestive heart failure probably due to daunomycin administration. Usually, cardiac dysfunction caused by these drugs has improved within 3 months after the discontinuation.

During a ten-year period, from November 1, 1961, to October 31, 1971, 339 residents aged 40 years or over at death were nonselectively autopsied in a Japanese community, Hisayama town (mean autopsy rate: 84%). One or more standard 12-lead ECGs plus V4R and V7 electrocardiograms taken at periodic medical examinations were available for 308 of them. In 46 persons, QS waves were localized in one or more leads from V1 to V4. By transverse sectioning of the hearts, old myocardial infarction extending into the interventricular septum was found in nine of these 46 persons. Frequency of myocardial infarction cases in each category for QS localization was as follows: Lead V1 to V4, three of three; Leads V1 to V3, six of nine; Leads V1 and V2, three of 15; Leads V2 and V3, none of two; Lead V3, none of one; Lead V2, one of eight; and Lead V1, one of 25.
