Genetic analyses of processing involving C-terminal cleavage in penicillin-binding protein 3 of Escherichia coli.

The processing of Escherichia coli penicillin-binding protein 3 (PBP 3) was investigated by gene manipulation for producing hybrid and truncated PBP 3 molecules. The hybrid PBP 3 was processed when the N-terminal 40 residues of PBP 3 were replaced by the murein lipoprotein signal peptide which lacked the cysteine residue for processing and followed by seven extra linker residues. In contrast, the PBP 3 molecules truncated at Thr-560 (28-residue deletion) or at Thr-497 (91-residue deletion) were not processed, and those truncated at Phe-576 (12-residue deletion) were processed at a greatly reduced rate. The results indicate that the C-terminal part, rather than the N-terminal part, is involved in the processing. This was supported by the result that the purified mature PBP 3 retained the complete N-terminal sequence with Met for translation initiation. The cleavage at the C-terminal region was shown by the loss of [35S]cysteine label when the cysteine-free hybrid PBP 3 joined to a cysteine-rich extra peptide tail was processed into the mature form. Confirmative assays for processing of PBP 3 were aided by a newly found prc mutant, defective in the processing involving the C-terminal region. A plasmid that directs PBP 3 truncated at Thr-560 complemented a thermosensitive PBP 3 mutation, but the truncated product was unstable in vivo. This suggests the importance of C-terminal hydrophobic regions that terminate at Leu-558 to PBP 3 functioning and the requirement of further-distal peptides for the stability of PBP 3.

Determination of the cleavage site involved in C-terminal processing of penicillin-binding protein 3 of Escherichia coli.

Chromatographic peptide mapping of lysyl endopeptidase digests of penicillin-binding protein 3 (PBP 3) of Escherichia coli revealed peptides that differed in retention time between the precursor and mature forms. The peptides were purified from a processing-defective (prc) mutant and a wild-type (prc+) strain. These peptides were identified as the C-terminal region of the precursor form and mature PBP 3 by amino acid sequencing. Each of the C-terminal peptides was cleaved into two fragments by trypsin digestion. By sequencing the resultant carboxyl-side fragment derived from the mature form, it was concluded that the C-terminal residue of mature PBP 3 was Val-577, and thus the Val-577-Ile-578 bond is the cleavage site for processing. This conclusion was consistent with the amino acid compositions of the relevant peptides, which suggested that the peptide from the cleavage site to the end of the deduced sequence (Ile-578-Ser-588) was present in the precursor but absent in the mature form. One lysyl peptide bond resisted both lysyl endopeptidase and trypsin and remained uncleaved in the peptide analyzed above.

Construction of an ordered cosmid collection of the Escherichia coli K-12 W3110 chromosome.

A cosmid library of the Escherichia coli K-12 W3110 chromosome was constructed in which clones were assigned to locations on the chromosome map by hybridization and genetic marker complementation tests. Approximately 70% of the genome was represented by this library. The identified clones can be maintained in the homologous system and would facilitate genetic studies of E. coli.

Temperature-sensitive sec mutants of Escherichia coli: inhibition of protein export at the permissive temperature.

Phenotypes of secY and secA temperature-sensitive mutants at permissive (low) temperature have been examined. The secY24 mutant was found to be extremely susceptible to export inhibition by a basal-level synthesis of the MalE-LacZ 72-47 hybrid protein or to overproduction of a normal secretory protein such as maltose-binding protein or beta-lactamase. Comparison of this phenotype of secY24 with those of the secY100 and secA51 mutants under similar conditions suggested that MalE-LacZ protein and overproduced secretory protein do not nonspecifically enhance the partial secretion defect but act synergistically with secY24 to inhibit protein export.

Development of drug resistance in cultured clonogenic leukemic blast cells during the clinical course of myeloblastic leukemia.

We studied changes in the drug sensitivity of clonogenic leukemic blast cells from patients with acute myeloblastic leukemia during clinical courses. These cells were assayed using the colony formation technique in methylcellulose culture. Various doses of each of two chemotherapeutic agents, daunorubicin and aclarubicin, were added at the start of culture, and drug sensitivities were measured in terms of percentage inhibition of leukemic blast colony formation. Clonogenic leukemic blast cells became less sensitive to the drugs which were administered to the patients during the course of the disease, suggesting that development of drug resistance in vivo may reflect the presence of dominant resistant populations of clonogenic leukemic blast cells in vitro.

Prognostic significance of conduction disturbance and reduction of left precordial voltage of electrocardiogram in hypertrophic cardiomyopathy.

To clarify the prognostic significance of electrocardiographic changes in hypertrophic cardiomyopathy, we retrospectively evaluated serial electrocardiograms in 77 patients with hypertrophic cardiomyopathy who were followed for more than 1 year. The electrocardiographic features analyzed were conduction disturbance and left precordial QRS voltage. There were 4 sudden deaths. Various conduction disturbances appeared in 32 (44%) of the remaining 73 patients. Intraventricular conduction delay was the most common (47%). The left precordial voltage decreased in 19 (26%), increased in 3, and did not change in 51. The left ventricular end-diastolic pressure at the initial investigation was significantly higher and clinical deterioration was more frequently seen in patients with a conduction disturbance or reduction of QRS voltage than in those without these electrocardiographic changes. Also, echocardiographic analysis showed that left ventricular dimensions increased significantly (from 4.4 +/- 0.6 to 4.8 +/- 0.7 cm in end-diastole and from 2.6 +/- 0.6 to 3.1 +/- 0.8 cm in end-systole; p less than 0.01, respectively) and left ventricular fractional shortening was reduced (from 41 +/- 8 to 36 +/- 11%; p less than 0.01) in the 32 patients with conduction disturbance during the follow-up period although absolute cavity size remained normal in 26 of these patients. These parameters did not change in those without conduction disturbance. Histopathological analysis of endomyocardial biopsies showed that myocardial fibrosis in the left ventricle was frequently associated with these electrocardiographic changes. However, such changes were not present in the sudden death patients. It is concluded that conduction disturbance and the reduction of QRS voltage are significant parameters which suggest a poor prognosis in patients with hypertrophic cardiomyopathy, but are not predictors of sudden death.

Chemotactic responses of neutrophils in cats with spontaneous feline infectious peritonitis.

The culture supernatant of peritoneal exudate cells (PEC) from cats with effusive feline infectious peritonitis (FIP) was chemotactic for peripheral blood neutrophils (PBN) from healthy cats, magnitude of the chemotactic activity being approximately 10-fold lower than that in zymosan-activated fresh serum of healthy cats (ZAS). The migration profile of PBN from healthy cats was slightly different between the PEC culture supernatant and ZAS. These findings suggest that the chemotactic activity detected in the PEC culture supernatant is distinct from that in ZAS. The chemotactic responses of PBN from FIP cats to ZAS were reduced, as compared with that from healthy controls. In contrast, the neutrophil chemotactic response and sensitivity to the PEC culture supernatant in FIP cats were not remarkably different from those in healthy controls. Furthermore, the chemotactic responsiveness of PEC from FIP cats to ZAS was slightly different from that of PEC to the PEC culture supernatant. These results suggest that neutrophils from FIP cats have altered reactivities against these chemoattractants.

[Morphological study of periodontal tissues in the initial stage of periodontal disease. Part 2. Analysis of morphological measurement and status of periodontal disease].

The morphological characteristics of periodontal tissue in periodontal disease have been interpreted differently by a number of clinical observers. Many have reported that the malposition and functional malocclusion of teeth is injurious to the periodontium. We reported in Part I that a system for evaluating periodontal status was developed for the diagnosis and management of the interproximal area at the initial stage of bone resorption. The patient group consisted of 36 adults, from 21 to 55 years of age. The severity score represented the calculated loss of periodontal support tissues: loss of alveolar bone, evaluated roentgenologically, bone level and pattern in vertical and horizontal form, periodontal pocket and gingival inflammation. Because poor oral hygiene and other factors caused swelling by gingival inflammation, we obtained study specimens from patients with chronic periodontal disease after a few tooth brushing instructions, and scalings during initial therapy in order to detect initial and established pathological changes in periodontal tissue. The purpose of this study was to clarify the relationship between periodontal disease status and morphological diagnostic indicators and different degrees of harmony and disharmony in the lower jaw. In all age groups the average percentage of bone loss and intraosseous defects tended to be higher in the groups categorized as Type III and Type F, and in the area that showed a very deep concave Spee curve to the occlusal plane in Pattern D. We considered that these morphological characteristics might be of secondary importance for diagnosis. Oral local factors were the primary extrinsic factor in the pathogenesis of horizontal and vertical interproximal bone absorption in the area of the premolars and molars.

Heterogeneous distribution of mast cells in lichenified lesions of atopic dermatitis.

The aim of the present study was to determine whether the number of mast cells in lichenified lesions of atopic dermatitis (AD) have a relationship to familial background of atopic respiratory disease (ARD). We obtained biopsy specimens of lichenified lesions from 59 consecutive patients with AD. They included 35 AD patients who had a personal history of ARD, 9 AD patients without a personal history of ARD but with a family history of ARD, and 15 "pure" AD patients without a personal or a family history of ARD. From each biopsy specimen, 4 microns-thick paraffin-embedded sections and/or 1 micron-thick Epon-embedded sections were prepared, and stained with Giemsa's reagent. With both methods of preparation, the sections from AD patients with a personal history of ARD showed significantly greater mast cell number in lichenified lesions than the "pure" AD patients. It is suggested that the increase of mast cells often seen in AD lesions may be characteristic of a subgroup of AD patients who have a predisposition for ARD.

[Maxillary sinus by waters' view-superimposed structures].

The image of the maxillary sinus obtained by Waters' view contains such superimposed structures as lateral extensions of the sphenoidal sinus and posterior ethmoidal cells. Planimetric evaluations were done for the maxillary sinus itself and the two superimposed sinuses. The medial portion of maxillary sinus is occupied by these two sinuses and the areas covered by these amount to one third of the maxillary sinus' area. To properly interpret the radiolucency of the maxillary sinus, the areas covered by these two sinuses and the areas corresponding to the zygomatic recess should be avoided. The proper radiolucency of the maxillary sinus should be evaluated at the central portion, namely at and around the point where the vertical line passing the inferior orbital canal and the horizontal line passing the foramen rotundum meet each other.

[Radiographic anatomy by waters' view on the innominate and Tuber maxillae lines].

Radiographic innominate line and TM line (Tuber maxillae line) which superimpose upon the maxillary sinus by Water's view were classified into four types. These are, no visualization, visualization of the whole length, upper half and lower half, respectively. 324 sides of maxillary sinuses by Waters' were included in this study; of innominate line, no visualization 45%, whole length 17%, upper half 27%, and lower half 11%. Of TM line, no visualization 67%, whole length 9%, upper half 1.5%, and lower half 23%. Of diagnostic significance of the present study, the innominate line, especially the lower half with it's inferior extension, might be confused with bone tips of blow-out fractures, and the TM line, when destroyed, will indicate posterior extension of the lesions. However, These lines only have limited application since about 30 to 50% of them are visualized by Waters' view.

[Types and treatments of blow-out fractures].

A total of ten cases of blow-out fractures of the orbit was investigated as to the signs and symptoms, image findings of conventional views of Caldwell and Waters' and of coronal CT. According to the locations of fractures, medial, floor and medial-floor types were classified. Fractured bone tips were classified to depressive and trapdoor types. Fractures often occur posterior to the eyeball, the inferior rectus muscle showed dislocation and cylinder-like hypertrophy. Contrary to the reports in the past, the inferior rectus muscle did not show entrapment in the herniated portion. Caldwell view is superior to Waters' view in demonstrating fractures at the medial orbital wall. As to the choice of treatment in the majority of cases, conservative methods are indicated. Images by CT are used in determining surgical methods, the depressive type without clinical improvement after two weeks is the indication for the surgical treatments.

[Introduction of the information processing system of medical examinations of yusho patients and some results obtained by the 1986 survey].

Information processing system for newly established, unified medical examinations of yusho patients which started in September 1985 was presented. Some comparative analysis was also shown on the "important findings" in yusho patients residing in Fukuoka or Nagasaki prefecture. In the 1986 survey, Fukuoka male patients showed higher percentages of pigmentation in toe nail, gingiva and palpebral conjunctiva than Nagasaki (p less than 0.01), whereas Nagasaki male patients showed higher percentages of pigmentation in buccal and lip mucosa. Average concentration of PCB in blood was higher in Nagasaki male patients (6.79 ppb) than in Fukuoka patients (4.39 ppb), the difference being significant at 1% level.

[Hepatocellular carcinoma treated with a curative segmentectomy five years after complete remission of acute myeloblastic leukemia].

Acute myeloblastic leukemia (AML) was diagnosed in a 54-year-old male who had been known to carry a chronic hepatitis B surface antigen (HBsAg) from June, 1983. Prompt remission was achieved with combination chemotherapy of BHAC-DMP. Follow-up maintenance and an intensification of this chemotherapy had been given for five years. He was readmitted to our hospital in March, 1988 because a mass was detected in the right lobe of the liver by ultrasonography. His serum alpha fetoprotein (AFP) level was found to be 180.1 ng/ml, and was diagnosed as having a hepatocellular carcinoma though there was no evidence of liver cirrhosis. A curative right hepatectomy was performed in May, 1988 after transcatheter arterial embolization and portal embolization. After resection of the tumor, the AFP level decreased to 10.7 ng/ml and no HbsAg was detected in the serum.

[Clinical effectiveness of cefoperazone for respiratory infections of elderly patients, focusing on its safety on renal function].

Clinical effectiveness of cefoperazone (CPZ) was studied for 46 elderly patients with respiratory infections. 1. Clinical efficacy rate of CPZ was 80.6% (29/36). 2. Adverse reactions occurred in 2 patients as systemic rush and fever. Laboratory abnormalities were observed in 5 patients. These included elevation of transaminase value. 3. The serum concentration of CPZ at 12 hours after administration of 2 g CPZ was not reflected in creatinine clearance and total bilirubin, and those 2 values appeared to have little effects on CPZ excretion. 4. The administration of CPZ at 4 g/day for less than 3 weeks had no effects on the renal function. As a result of this study, CPZ was found to have little undesirable effect on renal functions and to be a useful drug for the treatment of respiratory infections in elderly patients.

[A case of leiomyoblastoma of the jejunum].

Reported is a submucosal tumor of the jejunum that had developed in a 46-year-old man who manifested general fatigue and anemia. The tumor measured 3.0 x 3.0 cm and was composed of polygonal and/or round cells with a clear zone surrounding the nucleus. Five mitotic figures per 50-power fields were present. The light microscopic appearance was consistent with those of a Leiomyoblastoma. Follow-up evaluations approximately 2 years after the resection showed no evidence of metastasis or recurrence.
