Potassium perchlorate, potassium iodide, and propylthiouracil: promoting effect on the development of thyroid tumors in rats treated with N-bis(2-hydroxypropyl)-nitrosamine.

The effect of 1000 ppm potassium perchlorate (KClO4), 1000 ppm potassium iodide (KI) or 1000 ppm propylthiouracil (PTU) in the diet on the development of thyroid tumors was studied histologically and biochemically in Wistar rats given a single ip injection of 280 mg of N-bis(2-hydroxypropyl)nitrosamine (DHPN) per 100 g body weight. Basal diet containing 100 ppm KClO4, 1000 ppm KI or 1000 ppm PTU was given for 19 weeks from week 2 to week 20. The incidence of thyroid adenomas at the end of week 20 of the experiment was 100% (20/20) in rats treated with DHPN followed by KClO4, 85% (17/20) in rats given DHPN followed by KI, 95% (19/20) in rats given DHPN followed by PTU, and 5% (1/20) in rats given DHPN alone. The incidence of thyroid cancers was 100% (20/20) in rats treated with DHPN followed by KClO4, 65% (13/20) in rats treated with DHPN followed by KI and 0% (0/20) in rats treated with DHPN followed by or not followed by PTU. Rats given KClO4, KI or PTU alone and untreated rats had no thyroid tumors. The mean values of TSH in serum were 2.94 +/- 0.79 ng/ml in rats treated with DHPN followed by KClO4, 9.40 +/- 16.0 ng/ml in rats treated with DHPN followed by KI and 60.94 +/- 20.60 ng/ml in rats treated with DHPN followed by PTU. It was confirmed that (1) KClO4, PTU and KI promote the development of thyroid tumor in rats treated with DHPN, (2) the promoting effect of KClO4 or KI is stronger than that of PTU and (3) the value of TSH in serum is not parallel to the promoting effect on the development of thyroid tumor.

Effects of castration before and after treatment with N-bis (2-hydroxypropyl)-nitrosamine (DHPN) on the development of thyroid tumors in rats treated with DHPN followed by phenobarbital.

The effect of castration on the development of thyroid tumors was studied histologically and biochemically in Wistar rats given a single ip injection of 210 mg of N-bis (2-hydroxypropyl)-nitrosamine (DHPN) followed by phenobarbital (Pb). Castration was performed one week after, or one week before the injection of DHPN. The injection of DHPN was given at the end of the first week, and the rats were fed 500 ppm Pb in the basal diet for 38 weeks from week 3 to week 40. The incidence of thyroid adenomas and cancers was 20% (4/20) and 10% (2/20) in rats treated with DHPN alone. It was 75% (15/20) and 40% (8/20) in rats treated with DHPN and Pb; 30% (6/20) and 15% (3/20) in rats treated with DHPN and Pb, and castrated after DHPN; 20% (4/20) and 0% in rats treated with DHPN and Pb, and castrated before DHPN and Pb; and 0% and 0% in rats castrated either before or after receiving DHPN. Castration thus inhibited the development of thyroid tumor in rats treated with DHPN. The inhibition of tumor development in rats treated with DHPN and Pb, and castrated before receiving DHPN, was greater than in the rats castrated after receiving DHPN. Castration inhibited the secretion of TSH in rats treated with DHPN and Pb.

Effects of physical exercise training on cardiac function and graft patency after coronary artery bypass grafting.

The effects of physical exercise training after coronary artery bypass grafting on recovery of cardiac function and graft patency were studied in 115 patients. The patients were divided into Group I (N = 60) with and Group II (N = 55) without a scheduled rehabilitation program. The rate of graft patency was 98% in Group I and 80% in Group II. After operation, the exercise cardiac index and stroke index were significantly higher in Group I than in Group II. The exercise stroke index increased significantly in Group I, but not in Group II. The variables affecting graft patency, examined by a stepwise method, included double product, cardiac index, stroke index, and left ventricular end-diastolic pressure during postoperative exercise conditions, and postoperative exercise training. The multiple coefficient for these five variables was 0.459 (p less than 0.01). Our findings suggest that physical exercise training should be started as early as possible after coronary artery bypass grafting to improve graft patency and recovery of cardiac function.

Deficiency of apolipoproteins A-I and C-III and severe coronary heart disease.

A 55-year-old woman with severe coronary arteriosclerosis and skin xanthomas is described. The patient had a normal total cholesterol level of 168 mg/dl. Her level of high-density lipoprotein cholesterol was markedly reduced (3 mg/dl). On apolipoprotein analysis, apolipoproteins A-I and C-III were not detectable, and the level of apolipoprotein A-II was found to be at a low level (3.5 mg/dl). This case may possibly belong to a distinct new disease entity of deficiencies of apolipoprotein A-I and C-III in which coronary arteriosclerosis appears prematurely and severely.

Malignant "triton" tumor with metastatic hemangiopericytoma in a patient associated with von Recklinghausen's disease.

An autopsy case of malignant schwannoma in a 31-year-old female associated with von Recklinghausen's disease with unusual elements is reported. The patient who was diagnosed as von Recklinghausen's disease from a biopsy specimen of a subcutaneous tumor in her shoulder and multiple café au lait spots in her chest and back, complained of gradual increase in pain in her lower abdomen. A large mass measuring about 18 cm in diameter was located in her left retroperitoneum. Malignant schwannoma associated with von Recklinghausen's disease was indicated by the cellularity of the tumor, intersecting bundles of spindle cells, alternating cellular and less cellular myxoid areas in the peripheral portion of the tumor, whereas a large part of the tumor had a hemangiopericytoma-like arrangement with gaping blood vessels. In addition, however, foci of rhabdomyosarcomatous differentiation and of angiosarcomatous components were found in some areas of this tumor. Moreover, intravascular schwannian growth, which was presumably related to von Recklinghausen's disease, was observed in the heart and thyroid. This tumor should be classified as a malignant triton tumor because of the presence of rhabdomyoblasts.

Malignant fibrous histiocytoma with widespread metastasis and pulmonary cancer.

An autopsy case of malignant fibrous histiocytoma (MFH) with widespread metastases and lung carcinoma in a 64-year-old Japanese woman is reported. The initial signs were cough and sputum, followed by hemosputum. A chest X-ray photo showed a right pleural tumor, which could not be identified from a biopsy specimen, but was identified as MFH by light and electron microscopic studies on biopsy specimens of tongue tumors. Autopsy examination revealed metastases of the MFH to the brain, lung, liver, kidney, adrenal, pancreas, retroperitoneum, and some bones, and pulmonary adenocarcinoma.

Long-term in vivo carcinogenicity tests of potassium bromate, sodium hypochlorite, and sodium chlorite conducted in Japan.

Long-term in vivo carcinogenicity tests of potassium bromate (KBrO3), sodium hypochlorite (NaClO), and sodium chlorite (NaClO2) have been conducted in Japan from 1977 to 1985. In these investigations, groups of approximately 50 male and 50 female F344 rats or B6C3F1 mice were given solutions of the compounds as their drinking water ad libitum at two dose levels determined on the basis of preliminary 13-week tests. Control animals were given distilled water. The carcinogenic potential of KBrO3 was tested by administering doses of 500 or 250 ppm to rats for 110 weeks. Significantly elevated incidences of renal cell tumors in males and females and mesotheliomas of the peritoneum in males as compared to controls were observed. When female mice were given KBrO3 at doses of 1000 or 500 ppm for 78 weeks, no significant differences in tumor incidences between experimental and control groups were apparent. NaClO was administered to male and female rats, respectively, at doses of 1000 or 500 ppm and 2000 or 1000 ppm for 104 weeks. In mice, NaClO was given at doses of 1000 or 500 ppm to either sex for 103 weeks. The incidences of tumors in NaClO-treated and control animals of both sexes were not significantly different in both rat and mouse studies. NaClO2 was given to rats of both sexes at a dose of 600 or 300 ppm for 85 weeks. No statistically significant differences were observed in the incidences of tumor formation between NaClO2-treated and control groups of both sexes. NaClO2 was administered to mice at a concentration of 500 or 250 ppm for 85 weeks. In males, the combined incidences of hyperplastic nodules and hepatocellular carcinomas of the liver in a low-dose group, and adenomas and adenocarcinomas of the lung in a high-dose group, were marginally increased compared to controls (p less than 0.05). However, these incidences in treated males were within the range of values of historical control data in our program. We concluded that KBrO3 was carcinogenic in rats of both sexes. NaClO was not carcinogenic in either rats and or mice under the conditions of the present studies. Although NaClO2 was shown to be noncarcinogenic in rats, the results for mice were evaluated as inconclusive. Also the results of two-stage mouse skin carcinogenesis using KBrO3, NaClO, and NaClO2 are presented. The necessity for further testing of oxidant chemicals to determine potential carcinogenic and/or promoting effects is suggested in view of the recently proposed role of active oxygen species in carcinogenesis.

Effects of sodium chloride and alcohol on experimental esophageal carcinogenesis induced by N-nitrosopiperidine in rats.

The effects of sodium chloride and alcohol on esophageal carcinogenesis induced by N-nitrosopiperidine (NNP) were studied in male F-344 rats. Sodium chloride given concomitantly with NNP during the first 8 weeks of the experiment and followed by sodium chloride increased the incidence of esophageal tumors in week 20. Sodium chloride given after NNP during the second 12 weeks of the experiment increased the incidence of esophageal hyperplasia. In the present experiment, no clear effect of alcohol on esophageal carcinogenesis was demonstrated, but alcohol slightly increased the incidence of preneoplastic esophageal tumors in groups given alcohol with and after NNP. These results suggest that sodium chloride may be able to be a potent promoter of the development of esophageal tumors in rats if the conditions of initiation are appropriate. Examination of the incidences of tumors in 9 segments of the esophagus gave no evidence of any portion that was especially susceptible to induction of esophageal tumors.

[Indications for aortocoronary bypass operation in elderly patients: medical point of view].

Aorto-coronary (A-C) bypass operations were performed in 20 patients aged 68 to 78 years, and the indications for this operation were discussed retrospectively. The subjects consisted of 14 patients successfully operated and six patients unsuccessfully operated (death 3, graft occlusion 2, perioperative infarction 1). The results were as follows: Average age: There was no difference between the successful group (71.1 +/- 3.3 years; mean +/- SD) and the unsuccessful group (70.3 +/- 2.9 years). Coronary arteriographic findings: The average number of narrowed branches (greater than or equal to 75% stenosis) was 4.3 +/- 3.0 in the successful group (triple vessel disease in 12 cases and double vessel disease in four) and 2.7 +/- 0.5 in the unsuccessful group (triple vessel disease in four and double vessel disease in two), showing no significant difference between the two groups. Nine patients (64.3%) in the former group and one (16.7%) in the latter group had significant left main coronary artery disease (greater than or equal to 50% stenosis). Hemodynamic findings: Ejection fractions of less than 40% existed in three patients (50%) solely in the unsuccessful group; two of whom died. Left ventricular end-diastolic pressures of 30 mmHg or more existed in two cases (33.3%) only in the latter group. Other cardiac findings: Left ventricular end-diastolic dimension of 55 mm or more was found in one (7.1%) successful case and four (66.7%) unsuccessful cases. 201Tl-scintigrams showed a dilated left ventricular cavity in one case (7.1%) in the former group and two (33.3%) in the latter. Functions of other organs: Forced expiratory volume 1.0% or % vital capacity less than 70% was recognized in two cases (33.3%) only in the latter group. A level of serum creatinine more than 1.5 mg/dl was seen in only one case in the latter group. A case with advanced diabetes mellitus was present in the unsuccessful group, and this patient died of diabetic coma. A-C bypass operations are indicated for elderly patients when they have good cardiac function and have no complications of the kidneys, lungs and other organs. It is presumed that grafting the main coronary branches, thereby shortening the time of cardiopulmonary arrest prevents postoperative complications.

Carcinogenicity study of a commercial sodium oleate in Fischer rats.

Sodium oleate was added to the drinking-water of groups of 50 male and 50 female F344 rats at levels of 2.5 and 5.0% for 108 wk. The mean liver weight in males given 5% oleate was lower than that of the males given 2.5% oleate or distilled water alone. The mean thymus weight of females given 5% oleate was higher than that of females given 2.5% oleate or distilled water. There were no statistically significant differences between treated and control rats in the results of the urine and serum analyses or haematological determinations or in the incidence of tumours, apart from pancreatic tumours. The latter, in the males, showed some increase over the concurrent controls but did not differ significantly from the reported spontaneous incidence in this strain. It was concluded that sodium oleate does not induce tumours when given orally to F344 rats.

Dose-related effect of trisodium nitrilotriacetate monohydrate on renal tumorigenesis initiated with N-ethyl-N-hydroxyethylnitrosamine in rats.

The development of renal tumors was examined in rats given 1000 p.p.m. N-ethyl-N-hydroxyethylnitrosamine (NEELA) and then 30 000, 10 000 or 3000 p.p.m. of trisodium nitrilotriacetate monohydrate (Na3 X NTA X H2O) in their food. The incidences of renal tumors in rats treated with NEELA for 2 weeks and then 30 000, 10 000 and 3000 p.p.m. Na3 X NTA X H2O for 30 weeks were 100, 86 and 22%, respectively, while they were 16 and 0% in rats treated with NEELA or 30 000 p.p.m. Na3 X NTA X H2O alone. Na3 X NTA X H2O at 30 000 or 10 000, but not 3000 p.p.m., also increased the size and number of renal tumors in rats treated with NEELA, and Na3 X NTA X H2O at 30 000 p.p.m., but not 10 000 or 3000 p.p.m., also induced hydronephrosis in rats irrespective of whether they were treated with NEELA. Thus, Na3 X NTA X H2O has a dose-related effect on the kidney.
