[Effects of cis-DDP in children with refractory malignant tumors].

Eighteen children with refractory malignant tumors were treated with cis-DDP. They included 10 neuroblastomas, 2 rhabdomyosarcomas, and one each of hepatoblastoma, yolk sac tumor, osteosarcoma and pinealoma. Of 7 cases treated only with cis-DDP, 2 were NC, 6 PD and none were CR or PR. Of 11 cases treated with cis-DDP combined with other agents, 7 were PR, 2 NC, 2 PD and none were CR. The major side effect of cis-DDP was renal toxicity, but this was not serious and only transient. Three cases were treated with the continuous intra-arterial infusion method. These included 2 neuroblastomas and one yolk sac tumor. This treatment was therefore highly effective for children with malignant tumors, and few side effects were observed.

[Effect of cis-platinum in neuroblastoma].

We treated six children with neuroblastomas. They were all refractory to conventional chemotherapy or at an advanced stage when diagnosed. Patients were four males and two females aged 1 to 7 years old, with a mean age of 3.7 years. Total cis-DDP dose ranged from 60 mg/m2 to 720 mg/m2, with a median dose of 240 mg/m2, and a mean dose of 260 mg/m2. Of the six children, two were PR, three were PD, and one was NC. Of three non-responses, two cases were clinically improved. Two cases had renal dysfunction but this improved within 3 weeks. It is said that cis-DDP induces irreversible renal damage, especially tubular necrosis. Our two cases had progressively decreased creatinine clearance, though values of BUN and creatinine were normal. Bone marrow suppression was mild, so there was no need to discontinue or modify the cis-DDP chemotherapy schedules. Two hypo-magnesemia were found, both cases being asymptomatic. Transient liver dysfunction occurred in one child, and nausea and vomiting were observed in almost all cases.

Advanced neuroblastoma and disseminated intravascular coagulation: report of six cases.

We have experienced 30 patients with neuroblastoma since 1975. Disseminated intravascular coagulation (DIC) developed in six of these patients. They were all in stage IV, namely disseminated neuroblastoma. These six cases with DIC proved that some advanced neuroblastomas have the potential to cause coagulopathy in the process of the disease. The plasma concentration of heparin was measured in some patients who were treated with heparin. The data revealed that the conventionally used intravenous heparin dose is not appropriate in the case of DIC. Effective treatment requires monitoring of the plasma concentration of heparin.

Radioimmunoassay of a glycoprotein associated with malignancy.

A glycoprotein associated with malignancy was purified from the 0.6M perchloric acid-soluble fraction of serum obtained from cancer patients. The purified glycoprotein contained sialic acid, which was responsible for binding to wheat-germ agglutinin-Sepharose. Gel electrophoresis showed one band with an apparent Mr of 50 000-55 000, and the isoelectric point was 4.4 +/- 0.1. The glycoprotein could be distinguished from carcinoembryonic antigen and alpha-fetoprotein. Iodination of this material with chloramine-T permitted development of a radioimmunoassay.

Radioimmunoassay of a cancer-related glycoprotein. Circulating levels.

Circulating levels of (a) tumor-related glycoprotein(s) were determined by radioimmunoassay for a variety of patients and controls, and correlated with sialic acid concentration. Levels were highest in patients with metastatic disease and progressively declined to those with localized disease receiving therapy. Values for normal, adjuvant, and cured patients were significantly lower. Sialic acid concentrations correlated best for the metastatic group but not for the normals.

Lymph follicular cholecystitis.

Ten cases of cholecystitis with severe lymphocytic reactions were selected among serially examined 131 cases of cholecystitis or/and cholelithiasis. Gram-negative bacterial infection, especially E. coli and K. pneumonia seemed to be related as the cause of these severe lymphocytic reactions, but the gallstone revealed no definite influence. We would like to separate the cases with severe lymphocytic reactions as a lymph follicular cholecystitis and consider the possibility of a gram-negative bacterial infection in bile.

[Study on mucopolysaccharides in human lung carcinoma tissue--characteristics in histological types (author's transl)].

The mucopolysaccharides were prepared from human lung carcinomas of three histologically different types and the control tissue by exhaustive proteolytic digestion, quaternary ammonium chloride fractionation and column chromatography on Dowex 1 (Cl-). They were identified by chemical, enzymic and electrophoretic methods, as hyaluronic acid (HA), chondroitin sulfate (ChS), dermatan sulfate (DS), heparan sulfate (HS) and over-sulfated ChS and/or DS. Qualitatively they were not differed in tumor and normal tissues. However, the amounts of whole mucopolysaccharide were much increased in carcinomas than those of normal control in order of squamous cell carcinoma greater than small cell undifferentiated carcinoma greater than or equal to adenocarcinoma. The increment of mucopolysaccharide contents in carcinoma are largely due to increased amounts of HA and ChS. Carcinoma-type characteristic pattern was also demonstrated in terms of relative amounts of non-sulfated (HA) and sulfated (ChS, DS, HS) mucopolysaccharides: In squamous cell carcinoma and adenocarcinoma sulfated mucopolysaccharides were predominant (73 to 78% of total mucopolysaccharides), whereas in small cell undifferentiated carcinoma sulfated ones were diminished (25% of total mucopolysaccharides). In normal lung tissue sulfated mucopolysaccharide comprised 64% of total mucopolysaccharides. The presence of over-sulfated ChS and/or DS, which have not until now been found in lung tissue, was higher in carcinoma tissue as compared to the normal control. Total glycopeptides which were derived from tissue glycoproteins and not in detail characterized in this study were decreased in carcinomas of any histological types as compared to those of normal lung tissue, when expressed by hexosamine content. Biological and clinical significance of mucopolysaccharides in carcinoma state was discussed.

Malignant melanoma of the gallbladder.

A rare case of primary malignant melanoma of the gallbladder was studied with the electron microscope. Two interesting patterns were observed: numerous melanophages were present in the tumor tissue and wrapped compound melanosomes were found in nonneoplastic mucosal epithelium. The infiltration of melanophages was considered to be a reaction to degenerated tumor cells. Concerning the mechanism of melanosome transport into the epithelial cells, a cooperative action between epithelial and melanoma cells seemed to be the most important factor.

Glycosaminoglycans in human lung carcinoma.

The glycosaminoglycans were prepared by exhaustive Pronase digestion and alkaline treatment of squamous cell carcinoma and adenocarcinoma tissues of human lung, and of tissues taken at a site distant from the tumor as a control. The glycosaminoglycan classes were characterized by chemical enzymic, and electrophoretic methods. The presence of oversulfated chondroitin-and/or dermatan-sulfates which have not up till now been found in lung tissues was also demonstrated in carcinoma and control tissues, their contents being higher in the carcinoma tissues. The levels of whole glycosaminoglycans were markedly increased in carcinoma tissue. The classes of glycosaminoglycans which increased in lung carcinoma tissue were predominatly chondroitin-4-and/or-6-sulfates and hyauronic acid.

Mass screening for neuroblastoma by HPLC and by non-HPLC: a review.

There have been a number of contradictory views concerning the efficacy of mass screening for neuroblastoma. Three Japanese and one cohort study from Quebec, and three Japanese cross-sectional studies were reviewed. The four cohort studies revealed that mass screening using high performance liquid chromatography (HPLC) reduced the incidence of this disease by about half in children aged from 1 to 4 years while little reduction was noted in screening utilizing non-HPLC methods. There is a large difference in the efficacy of mass screening depending on the method used. In the three cross-sectional studies, there was no discrimination between screened and unscreened cases, or between subjects screened by HPLC and non-HPLC methods. The percentage of children in the cross-sectional studies screened by HPLC was low. Cross-sectional studies seem inappropriate in assessing the effectiveness of current HPLC mass screening.
