Effect of 1alpha-hydroxycholecalciferol on the metabolism of calcium and phosphorus was studied using Ca-labelled and non-labelled rats. With an oral daily dose of 2.5 or 12.5 microgram/kg of 1alpha-hydroxycholecalciferol, serum calcium, urine volume, urinary calcium excretion, urinary calcium concentration and water consumption increased, while fecal concentration, urinary phosphorus concentration and food consumption decreased. There was a lag time (1 or 2 days) between the increase of serum calcium and that of specific activity of serum calcium, and bone resorption was stimulated later than was intestinal calcium transport. After 10 days treatment, calcium concentration in the femur decreased, while a remarkable calcification was noted in soft tissues such as kidney, intestine, aorta, heart and muscle, although these effects were reduced with lower doses of the drug. With a smaller oral daily dose (0.1 or 0.5 microgram/kg), for 6 months, bone calcium and phosphorus concentration increased without soft tissue calcification. Thus, 1alpha-hydroxycholecalciferol may be an effective drug for patients with metabolic bone diseases.

The effect of amikacin on the neuromuscular junction was studied in man during anesthesia and surgery. Amikacin alone did not show any neuromuscular blocking action in normal man in therapeutic doses (100-200 mg). However, during recovery from either d-tubocurarine block or phase II block with succinylcholine, the intravenous administration of 200 mg of amikacin caused a decrease in twitch tension. This neuromuscular block aggravated by amikacin was antagonized by edrophonium (10 mg) or calcium chloride (400 mg). Data obtained from the peroneal nerve-anterior tibial muscle preparation of rabbits suggested that the neuromuscular blocking potency of amikacin was one-twelfth of that of kanamycin in therapeutic doses. Although amikacin is thus less potent than kanamycin, caution should be taken in its use for patients with myasthenic state.

Biological activity of 1alpha-hydroxycholecalciferol was studied in rats. 1alpha-Hydroxycholecalciferol was found to be more potent and rapidly active than vitamin D in stimulating intestinal calcium transport and calcium mobilization from bone both in normal and vitamin D deficient rats. 1alpha-Hydroxycholecalciferol was also active in nephrectomized and/or thyroparathyroidectomized rats both in intestine and bone. Although it is well known that 1alpha-hydroxycholecalciferol is metabolized to 1alpha, 25-dihydroxycholecalciferol in the liver, there is the possibility that the former is active without further metabolism. In rats in which hepatitis was induced by CCl4, 1alpha-hydroxycholecalciferol was active both in the intestine and in the bone, while it was inactive in hepatectomized rats. These data clearly demonstrate that 1alpha-hydroxycholecalciferol is not active by itself and must be metabolized in the liver. This idea also shows the lag time in response of rats to 1alpha-hydroxycholecalciferol has more potent antirachitic activity than vitamin D and does not lose its activity with chronic oral administration. In view of these findings, 1alpha-hydroxycholecalciferol appears to have a good potential for clinical application in cases of renal failure and metabolic bone diseases.

Changes in the degree of fluorescence polarization in the fluorescent cytoplasm of lymphocytes from patients with malignant diseases, patients with nonmalignant diseases, and normal donors were investigated by the fluorescence polarization technique. In order to confirm Cercek's cancer test, two kinds of parameters, the degree of fluorescence polarization of lymphocytes stimulated with phytohemagglutinin (PHA) and cancer basic protein, were obtained for each person. Patients with malignant diseases were easily differentiated from the others without exception by an index, the response ratio of the degree of fluorescence polarization. Usefulness of this technique was confirmed for the detection of cancer-bearing patients.

The comparative studies in the short term intermittent toxicity of pepleomycin (NK631) and bleomycin (BLM) were performed on 10 beagle dogs of 13 approximately 15 months old. Two dogs per group were injected intravenously with NK631 and BLM in doses 5.0 and 2.5 mg/kg body weight every fourth day for 11 treatment. Two dogs served as control and were injected with saline solution. In the group of 5.0 mg/kg of NK631, one dog was sacrificed on day 37 of the treatment and another one dog died on day 33 of the treatment. All the dogs of other group survived until the end of the treatment. The toxicity to the hepatic and renal damage caused by NK631 was stronger than the BLM. On the contrary, the toxicity to lung and various mucocutaneous regions was weaker than the BLM. The grade of pulmonary fibrosis of NK631 was about 1/2 and 2/3 of the BLM in dose of 5 mg/kg and 2.5 mg/kg, respectively. Other toxicological changes such as anorexia and weight loss were almost similar to the BLM.

Granular cell indices (GCI: Dunihue and Robertson, '57) in kidneys of fetal rats one day after maternal nephrectomy of fetal unilateral nephrectomy on day 21 of gestation were determined by examining sections stained with Bowie's technique. Maternal nephrectomy induced an extreme increase of GCI in fetal kidneys with increased granularity of cells in the juxtaglomerular apparatus. Fetal unilateral nephrectomy did not cause any significant change.
