[Mid-systolic ejection murmur with thrill caused by right ventricular outflow tract obstruction secondary to septal aneurysm following myocardial infarction: a case report].

A 71-year-old woman with a history of previous myocardial infarction was transferred to our hospital for evaluation of chest pain and ventricular tachycardia. On admission, a loud mid-systolic ejection murmur accompanied by a thrill was found at the left sternal border in the third intercostal space, and it was significantly accentuated in the post-extrasystolic beat. Abnormal Q waves and ST elevations were noted in leads I, aVL and V5,6 on electrocardiograms. Echocardiograms, confirmed a septal-to-apical aneurysm, and a thin interventricular septum (IVS) with paradoxical motion. Right ventricular (RV) catheterization showed a pressure gradient of 21 mmHg between the outflow tract (RVOT) and the apex, and a mid-systolic ejection murmur was recorded in the RVOT on an intracardiac phonocardiogram. Coronary arteriograms revealed total occlusion of the left anterior descending artery in its proximal portion, and a 90% stenosis of the circumflex artery. A left ventriculogram demonstrated a septal-to-apical aneurysm with a markedly reduced ejection fraction of 0.16. A right ventriculogram showed obstruction to RVOT caused by systolic ballooning of the IVS. In this patient, the mid-systolic ejection murmur was probably caused by the obstruction of the outflow tract secondary to septal aneurysm following old myocardial infarction.

[Basic study on interferon-beta: Part IV. Antitumor effect on nude mouse-transplanted human tumors].

The effects of human interferon-beta (IFN-beta, MR-21) on the growth of xenografted human tumors in nude mice were examined. IFN-beta was administered to mice with malignant melanoma (SK-MEL-28 and Sk-14) intratumorally at a dose of 1 X 10(5)-3 X 10(5) IU/mouse, with acute leukemia (CCRF-HSB-2) intratumorally at a dose of 3 X 10(5) IU/mouse, with glioblastoma (U-373 MG) intravenously or intratumorally at a dose of 1 X 10(5)-6 X 10(5) IU/mouse, or with uterine cervical tumor (HeLa S3) intravenously at a dose of 0.3 X 10(5)-1 X 10(5) IU/mouse. IFN-beta inhibited the growth of all of these tumors in a dose-dependent manner.

[Opsoclonus-polymyoclonia syndrome suppressed with reserpine].

A 38-year-old man was admitted to Iwakuni National Hospital on July 6, 1978, with the complaints of difficulty seeing and walking. Two weeks before admission, he first experienced dizziness and it slowly progressed to uncontrollable tremor-like movements of the whole body. On admission, he was alert, oriented and afebrile. He had not experienced nausea, vomiting nor headache. He showed irregular horizontal oscillations of the eyes. Electronystagmographic study showed that this jerky eye movement appeared especially with changes of fixation of the eyes. It was also recorded during conjugate eye movement, and while he closed his eyes. He was ataxic, unable to walk, but no other abnormalities in cerebellar functions were observed. Spinal tap was performed and yielded watery clear cerebrospinal fluid containing 9/mm3 mononuclear cells. Clonazepam was given, 1.5 mg per day, for three days followed by doses of 3 mg per day. Improvement in walking was observed one week after starting the medication, when reserpine was started at a dose of 1 mg per day and increased to a dose of 1.5 mg per day in three days. One week after starting reserpine, opsoclonus improved markedly and he became able to read again. He was discharged home on September 3, 1978. Six months after admission, reserpine was decreased to 0.5 mg per day. Difficulty in reading developed within a month. Reserpine was given 1.0 mg per day and the doses was continuously given for next three months. One year after admission, he is back to his former occupation without medication. He complains of slight difficulty in reading for more than an hour, and in watching TV.(ABSTRACT TRUNCATED AT 250 WORDS)

[Evaluation of cis-diamminedichloroplatinum (II) in the multidisciplinary treatment of head and neck cancers].

Thirty-eight cases of head and neck cancer treated with cis-diamminedichloroplatinum (II) (CDDP) with/without the combination of Peplomycin (PEP) from 1982 to 1985 were evaluated. The overall response rate was 55%. Eleven of 16 patients who had received no prior therapy obtained CR and 4 patients obtained PR, the response rate being 94%. In contrast, only 6 of 22 patients who had been previously treated achieved CR and PR, the response rate being 27%. According to the pharmacokinetics of CDDP and clinical observation of measurable recurrent tumors after the administration of CDDP alone, it was revealed that the volume of CDDP-sensitive and-resistant cells were balanced at 3 weeks after the injection of CDDP. After this time, CDDP-resistant cells exceeded CDDP-sensitive cells in volume. From viewpoint, in the case of multidisciplinary treatment with CDDP, other therapies should be induced within 3 weeks after CDDP injection.

[Study on human lymphoblastoid interferon-alpha (MOR-22): Part III. Antitumor effect on nude mouse-transplanted human tumors].

The effects of human lymphoblastoid interferon-alpha (MOR-22) on the growth of xenografted human tumors in nude mice were examined. IFN-alpha was administered subcutaneously to mice with renal tumor (ACHN), intratumorally for glioblastoma (U-373MG) or intravenously with uterine cervical tumor (HeLa S3). The dosages of IFN-alpha were 2 X 10(4)-5 X 10(5) IU/mouse for ACHN tumor, 1 X 10(5)-5 X 10(5) IU/mouse for U-373MG tumor and 3 X 10(4)-1 X 10(5) IU/mouse for HeLa S3 tumor. IFN-alpha inhibited the growth of these tumors in a dose-dependent manner.

[Reversed Hegglin syndrome in Takayasu arteritis].

To study the "reversed Hegglin syndrome" in Takayasu arteritis, 57 patients were selected, and divided into four groups; Group I: 12 with Takayasu arteritis with aortic regurgitation (AR), Group II: 27 with Takayasu arteritis without AR, Group III: six with annuloaortic ectasia and severe AR, and Group IV: 12 with severe AR of other miscellaneous etiologies. QII-QT intervals were compared among the four groups. The reversed Hegglin syndrome (QII-QT greater than or equal to 40 msec) was observed in 58 percent of Group I patients and 37 percent of Group II patients. This phenomenon was not seen in any patients in Group III or IV. Sixteen of the 17 patients who had the reversed Hegglin syndrome belonged to the thoracic aortic type or diffuse type of Takayasu arteritis. Most patients who did not have this syndrome either had a localized lesion or a prolonged QT interval. The reversed Hegglin syndrome, therefore, is related to widespread lesions involving the thoracic aorta due to Takayasu arteritis, and may be attributed to a delay in closure of the aortic second heart sound due to a decrease in aortic impedance of the diseased proximal aorta.

Intracellular structure of the outer hair cell of the organ of Corti.

The intracellular structure of the outer hair cells of the normal guinea pig organ of Corti was investigated three dimensionally by scanning electron microscope. Freeze fracturing technique followed by maceration with a 0.1% OsO4 solution (osmic maceration method) was used. Among the cell organelles, the endoplasmic reticulum (ER) showed the most interesting features, such as subsurface cisternae and lamellar bodies. The subsurface cisterna which formed a stratiform network covered the inner surface of the cell membrane and the stratiform structure disappeared at the infranuclear region. Variously shaped mitochondria (spherical, cylindrical and branched) were found on the innermost layer of the subsurface cisterna. The lamellar body which was located beneath the cuticular plate consisted of dilated cisternae and tubular ER and was surrounded by mitochondria. The tubular ER of the lamellar body were continuous with the subsurface cisterna.

Central chemosensitivity to H+ and CO2 in the rat respiratory center in vitro.

The brainstem, cervical cord and attached phrenic nerve were excised from newborn rats and superfused in vitro. Respiratory output was measured by integration of phrenic nerve discharges. Respiratory output was enhanced by an increase in pCO2 at constant pH as well as by decreased pH with constant pCO2. It is concluded that the adequate stimulus to central chemoreceptors is not restricted only to H+.

An experimental study on cupular function: mapping of the cupula by direct stimulation.

In the present study, the posterior semicircular canal of the frog was isolated and the entire cupula was exposed after cutting the ampullary wall. The surfaces of the utricle and the lateral cupula were depressed by a fine glass pipette. Action potentials were taken from the posterior ampullary nerve and were compared by using nine points of stimulation on the cupular surface of the utricular side and two points on the lateral surface. The maximum action potential was recorded at stimulus to the lowest point on the utricular side surface. This potential proportionally decreased as the stimulation point approached the top of the cupula. Depression of the lateral surface evoked only a small potential. In contrast to the stimulus applied to the utricular surface, a stimulus to the upper point of the lateral surface yielded a greater potential than did a stimulus to the lower point.
