A cytochemical study of dense granules in the rat junctional epithelium.

To clarify the details of intracytoplasmic granules in the junctional epithelium (JE) of the rat gingiva, phosphotungstic acid (PTA) staining, acid phosphatase cytochemistry and zinc iodide-osmium (ZIO) staining were carried out. Condensing granules containing homogeneous electron-dense materials, measuring about 260 nm in diameter, have been observed at the peripheral cytoplasm of the JE. These granules reveal PTA-positive, ZIO-positive and acid phosphatase-positive staining. These results indicate that the granules contain glycoprotein, phospholipid, lipoprotein and lysosomal enzymes. From their composition and morphology, it is suggested that these granules in the JE may contribute secretory function and secrete glycoproteins or glycolipids. Although it is unclear whether the dense granules correspond to membrane-coating granules by our staining methods, it is surmised that, if the dense granules can engage in establishment of the barrier function, then this role is impeded by the large intercellular space in the JE.

Effectiveness of peanut agglutinin-binding glyco-related antigen and cyclophosphamide on metastasis of Lewis lung carcinoma.

Soluble tumor-associated antigens were isolated from Lewis lung carcinoma (3LL) cells by detergent solubilization and purified by affinity chromatography on peanut agglutinin (PNA)-agarose. Subcutaneous injection of PNA-binding glyco-related antigen into 3LL-bearing male C57BL/6 mice significantly augmented delayed-type hypersensitivity (DTH), and also inhibited the growth of the primary 3LL tumors. The effects of combined immunotherapy and chemotherapy with this antigen and cyclophosphamide (CY) were examined following excision of 3LL by determining the number of pulmonary surface nodules and wet weight of the lungs. Injection of PNA-binding glyco-related antigen and CY significantly inhibited pulmonary metastasis and prolonged the survival of the mice. The spleen cells obtained from mice with combined treatment showed higher neutralizing activity against 3LL cells than other groups. Therefore, combined immuno and chemotherapy may be useful in preventing metastasis.

Involvement of the nipple in early carcinoma of the breast.

Cancerous involvement of the nipple and subareolar tissue (nipple and areolar complex, NAC) was confirmed histopathologically in 24 of 65 patients with gross tumors measuring 2.5 centimeters or less. Erosion of the nipple as a clinical manifestation of involvement of NAC was seen in only two patients. The other 22 were all subclinical. The histologic form of involvement of NAC included intraductal-1 in 19 patients, stromal in one patient, lymphatic in two patients and intraductal-1 as well as stromal in two. Stepcut and serial observation of the whole breast suggested that both intraductal spread and stromal invasion of carcinoma were continuous processes from the underlying tumor. Involvement of NAC was more frequent if the patient was aged 50 years or less and if the proximity of the tumor to the nipple was less than 4 centimeters, regardless of the size of the tumor.

[Experimental study of materials used for intra-atrial baffle for the Mustard operation].

An experimental model was developed to study the fate of prosthetic materials used for the construction of an intra-atrial baffle for the Mustard operation. Total atrial septa were replaced in sixteen puppies with autologous pericardium (n = 3), glutaraldehyde-preserved canine pericardium (n = 4), glutaraldehyde-preserved porcine pericardium (n = 3), EPTFE reinforced sheet (Gore-Tex sheet, n = 3) and EPTFE reinforced cardiovascular patch (Gore-Tex patch, n = 3). Septal replacement was performed under total circulatory arrest and surface induced deep hypothermia. All animals were sacrificed six months after the septal replacement. Pathological examination revealed that all septal grafts were well healed, completely covered by neoendothelium and had contracted to approximately 60% of their original size. There were no significant difference between groups. Histologic examination showed the good tissue adaptation of autologous pericardium and Gore-Tex patch materials. The thinnest layers of neoendothelium covered autologous pericardial baffle. Glutaraldehyde-preserved pericardium and Gore-Tex sheet did not adapt well to host atrial tissue. These materials were easily detached from their neoendothelial substrates. It was concluded that the autologous pericardium as well as a Gore-Tex patch may be superior to other prosthetic materials used for the construction of an intra-atrial baffle for the correction of transposition of the great arteries.

[A case of acute thoracic aortic occlusion due to blunt trauma].

A rare case of complete thoracic aortic occlusion secondary to blunt trauma was reported. The patient was a 27-year-old policeman who was involved in an automobile accident while driving a patrol car. Hypertension of the upper extremities, edema of the upper body and anuria were found at emergency room examination. Emergent aortogram demonstrated traumatic occlusion of the aorta at the level of isthmus. Urgently an axillo-femoral bypass was performed, and two days later radical aortic repair was carried out. At the radical operation, disruption of the aortic intima was encountered in the area of ligamentum arteriosum. The distal torn edge was enfolded and completely occluded the aortic lumen. The lacerated intima was sutured, and the intimal defect of the aorta was restored with woven Dacron patch. Postoperative course was uneventful. On subsequent follow-up 3 years after injury, the patient was asymptomatic showing normal thoracic aortogram.

[Statistical study of epulis, especially in general pathology].

This report contains a statistical study of 874 cases of epulis diagnosed by the Department of Pathology of Tokyo Dental College from 1966 to 1986. 1. Of the 874 cases, 344 were epulis fibrosa, 217 were epulis granulomatosa, 78 were epulis fibromatosa, 74 were epulis fibrosaosteoplastica, 51 were epulis hemangiomatosa, 43 were epulis fibrosa teleangiectaticum, 15 were epulis cementoplastica, 14 were epulis osteomatosa, 3 were congenital epulis, 2 were giant cell epulis, and 1 was epulis cementomatosa. 2. As has been reported in other literature, there is a marked tendency for this condition to occur in females (331 male cases and 539 female cases). 3. Our data indicate a higher occurrence rate in people in their fifties, although the occurrence rates were similar in people in their twenties and in people in their sixties. 4. The epulis was observed most frequently in the maxillary incisor region.

[A successful surgical treatment of acute dissecting aneurysm of the ascending aorta: repair of the proximal intimal tear necessitating bypass for the right coronary artery].

This is a case report of the successful surgical treatment of acute dissecting aneurysm of the ascending aorta. The patient was a 65-year-old female, who admitted to the hospital 3 hours after the onset of severe back pain. Enhanced CT of the chest and abdomen revealed acute dissection of the ascending aorta from its origin to the bilateral common iliac arteries. Thirteen hours initiation of the symptoms, operation was carried out using pump-oxygenator with hypothermia. Transverse incision of the ascending aorta revealed acute dissection in anterior aspect extending to two third of the circumference. The proximal small intimal tear was found 3 mm apart from the right coronary artery ostium. Upon closure of the tear resulting in obliteration of the right coronary artery, it required bypass procedure between the right coronary artery and the innominate artery using the saphenous vein graft. The aortic incision was directly closed using Dacron felt bolsters. Postoperative studies revealed disappearance of the proximal aortic dissection, patency of the bypass graft and existence of distal aortic dissection. Patient is in active life 6 months after operation.

[Emergency aortic valve replacement in active infective endocarditis in a chronically hemodialyzed patient].

A 36-year-old housewife, who had been treated with hemodialysis for 15 years is reported. She had suffered from active infective endocarditis of the aortic valve caused by blood access infection for several weeks. She rapidly fell into severe cardiac failure. Five days after the onset of the cardiac failure she was transferred to our hospital. The aortic valve was replaced using #21 Björk-Shiley aortic valve prosthesis shortly after the arrival to the hospital. Postoperatively she was treated with vigorous antibiotics and was anticoagulated. Seven months after the surgery, patient is back to an active life with hemodialysis 3 times a week.

[Experimental study on vascularized fresh whole-joint allograft transplantation].

The vascularization of fresh whole knee and hip joints was done as part of an allograft transplant procedure involving forty rats (12 weeks old). Cyclosporin and prednisolone were injected into thirty-five recipient rats; five rats did not receive any drugs and were used as the control group. All control group rats with necrotic limbs died two to three weeks after the operation. Eleven of the twenty rats with transplanted knee joints and ten of the fifteen rats with transplanted hip joints began walking with full weight-bearing and the transplanted joints exhibited good mobility one month after the operation. Autopsy findings four, eight and sixteen weeks after the operation showed no degenerative or necrotic changes of the articular cartilage or subchondral bone of the joints. We concluded that this technique of vascularized whole-joint transplantation in rats was successful. This may be clinically applicable to reconstructive surgery after resection of diseased joints.

[Clinical experiences with a new membrane oxygenator with low priming volume (D702 MASTER FLO 51), studies during pulsatile and constant flow perfusion].

The newly developed oxygenator "D702" is a compact hollow fiber membrane oxygenator with a priming volume of 170 ml. The maximum flow allowance is 4 liters per/minute. We used this oxygenator in 16 patients (11 infants and children, and 5 adults) undergoing various open heart surgery, and function of this oxygenator was studied. Pulsatile cardiopulmonary bypass was performed in 8 patients and nonpulsatile constant flow perfusion was employed in the remaining 8 patients. Our clinical experience showed excellent maintenance of PaO2 and PaCO2 during both pulsatile and constant flow bypasses. A low pressure drop was encountered across the membrane oxygenator, and therefore, this oxygenator is applicable for pulsatile cardiopulmonary bypass. The D702 is a very useful and applicable for a wide range of patients from infants to adults with a small body structure.

[Successful surgical treatment of infective endocarditis complicated with DIC and intracerebral hemorrhage].

This is a case report of a 61-year-old man, suffering from infective endocarditis of mitral valve in association with DIC and intracerebral hemorrhage. He was treated symptomatically and conservatively for 80 days after the onset of the DIC. His multiple cerebral hematomas gradually disappeared 2 months after conservative management. Then the diseased mitral valve was replaced with a Duromedics valve. He is doing well at present. In a review of the literature it appears that there is some room for surgical intervention in endocarditis despite the complication of coagulopathy. We feel that one of the most serious complications of infective endocarditis is DIC. Operative management of endocarditis with this complication is recommended to be followed by medical management.

[A case report of pulmonary arterio-venous fistula complicated with hereditary hemorrhagic telangiectasis].

A 5-year-old girl with multiple pulmonary arterio-venous fistulae was reported. Both her mother and grandmother have been diagnosed as the same disease previously. Her grandmother was treated with resection of the right basal segment for arterio-venous fistulae. Her mother is well with fistulae for more than 20 years. The patient was operated upon to enucleate two larger fistulae in segments 3 and 6 of right lung. Her postoperative course was uneventful. She was discharged from the hospital on the 22nd postoperative day. Pulmonary arteriovenous fistulae are thought to be a part of the hereditary hemorrhagic telangiectasis. Enucleation of the larger fistulae of the lung are preferable even for multiple lesions.

[Surgical management of coarctation of the aorta in early infants].

Thirteen infants under three months old were repaired aortic coarctation between 1980 and 1988. The mean age at operation was 2 +/- 0.8 months, the mean weight was 3.7 +/- 0.6 kg. Patients were simple coarctation in 1, coarctation with VSD in 7, and coarctation with complex heart disease in 5. As the operative method, the prosthetic patch plasty was done in 11 patients, end to end anastomosis in 1 patient and remaining one was primary VSD closure and secondary coarctation repair (patch plasty). Three patients died perioperatively, the overall early mortality rate was 23%. Since the introduction of PGE1 therapy in 1985, operative mortality rate of the operation (patch aortoplasty) was improved dramatically. Two of 10 patients died from LOS after the secondary operation (one was TGA and another was congenital MS with VSD). Eight patients are alive and clinical course after operation had been good. The measurement of arm-leg pressure difference didn't reveal significant residual gradient. Postoperative isthmus index was 90 +/- 13% in follow up measurement. Recoarctation has not been occurred in patients undergoing operations with patch plasty. Postoperative hypoplastic arch index was 93 +/- 19% in 1 to 2 years after operation compared with 70 +/- 16% of the preoperation. Therefore, it is concluded that prosthetic patch aortoplasty and end to end anastomosis of the aorta are both available as reasonable and/or adequate operation for the early infants with coarctation.

Structural requirements of salicylanilides for uncoupling activity in mitochondria: quantitative analysis of structure-uncoupling relationships.

The uncoupling activities of more than 20 salicylanilides were measured in rat liver mitochondria. The activities, expressed as the minimum concentrations required for full release of state-4 respiration, ranged over three orders of magnitude. The acid dissociation constant, pKA, and the partition coefficient between octanol and water (Poct) of some of the salicylanilides were determined. These two parameters were found to be well expressed in terms of the Hammett constant, sigma, and the hydrophobic substituent coefficient, II, respectively. The pKA and log Poct values of all the salicylanilides were predicted according to these relationships. Furthermore, the capacity factor, k', on high-performance liquid chromatography was determined on glyceryl-coated-controlled pore glass (gly-CPG). Values of log k' correlated well with those of log Poct. The uncoupling activities of the salicylanilides were analyzed in terms of these three parameters. Both hydrophobic and electron-withdrawing properties were found to be essential for induction of potent uncoupling activity. The correlations using log k' were better than those using log Poct.

Primary structure of chicken skeletal muscle and fibroblast alpha-actinins deduced from cDNA sequences.

The complete 897-amino-acid sequence of chicken skeletal muscle alpha-actinin and the 856-amino-acid sequence (97% of the entire sequence) of chicken fibroblast alpha-actinin have been determined by cloning and sequencing the cDNAs. Genomic Southern analysis with the cDNA sequences shows that skeletal and fibroblast alpha-actinins are encoded by separate single-copy genes. RNA blot analyzes show that the skeletal alpha-actinin gene is expressed in the pectoralis muscle and that the fibroblast gene is expressed in the gizzard smooth muscle as well as in the fibroblast. The deduced skeletal alpha-actinin molecule has a calculated Mr of 104 x 10(3), and each alpha-actinin can be divided into three domains: (1) the NH2-terminal highly conserved actin-binding domain, which shows similarity to the product of the Duchenne's muscular dystrophy locus; (2) the middle rod-shaped dimer-forming domain, which contains the spectrin-type repeat units; and (3) the COOH-terminal two EF-hand consensus regions. Comparison of the skeletal alpha-actinin sequence with the fibroblast and smooth muscle alpha-actinin sequences demonstrated that the EF-hand structure was conserved in all of these alpha-actinin sequences, despite the reported variability of the Ca2+ sensitivities of the actin-gelation by various alpha-actinin isoforms.

Amino acid sequence of the calcium-binding light chain of myosin from the lower eukaryote, Physarum polycephalum.

We have established a new method for preparing Physarum myosin whose actin-activated ATPase activity is inhibited by micromolar levels of Ca2+. This Ca2+-inhibition is mediated by the Ca2+ binding to the myosin rather than by the Ca2+-dependent modification of the phosphorylated state of the myosin (Kohama, K., and Kendrick-Jones, J. (1986) J. Biochem. (Tokyo) 99, 1433-1446). Ca2+-binding light chain (CaLC) has been suggested to be primary importance in this Ca2+ inhibition (Kohama, K., Takano-Ohmuro, H., Tanaka, T., Yamaguchi, T., and Kohama, T. (1986) J. Biol. Chem. 261, 8022-8027). The amino acid sequence of CaLC was determined; it was composed of 147 amino acid residues and the N terminus was acetylated. The molecular weight was calculated to be 16,131. The homology of CaLC in the amino acid sequence with 5,5'-dithiobis-(2-nitrobenzoic acid) light chain and alkali light chain of skeletal muscle myosin were rather low, i.e., 25% and 30%, respectively. Interestingly, however, the CaLC sequence was 40% homologous with brain calmodulin. This amino acid sequence was confirmed by sequencing the cloned phage DNA accommodating cDNA coding CaLC. Northern and Southern blot analysis indicated that 0.8-kilobase pair mRNA was transcribed from a single CaLC gene. This is the first report on the amino acid sequence of myosin light chain of lower eukaryotes and nucleotide sequence of its mRNA.

Recurrence of myocardial infarction related to different vessels in a patient with diffuse coronary artery spasm without underlying severe organic stenosis.

A 59-year-old man had a first myocardial infarction in the right coronary artery, followed by an another infarction in the left anterior descending coronary artery. Coronary thrombolysis for both occasions of acute myocardial infarction was successfully performed without severe residual stenosis. In the chronic phase after the second myocardial infarction, ergonovine test showed diffuse coronary artery spasm. The etiology of myocardial infarction is briefly discussed.
