Reduction of blood loss from diagnostic sampling in critically ill patients using a blood-conserving arterial line system.

STUDY OBJECTIVE: To demonstrate the utility of a new blood-conserving arterial line system in reducing blood loss associated with blood drawing in the critical care setting. DESIGN: Prospective, randomized, crossover comparison between two arterial line systems. SETTING: Medical intensive care unit (ICU); tertiary care teaching institution. PATIENTS: Thirty-one patients who required invasive arterial blood pressure monitoring throughout their ICU course. INTERVENTIONS: For ICU days 1 to 2, patients were randomized to receive either a conventional arterial line system or a new blood-conserving arterial line system. On ICU days 3 to 7, patients with a conventional arterial line were crossed over to the blood-conserving arterial line, and vice versa. Laboratory blood volumes, mixed discard volumes, and blood discard volumes were then recorded to document how much blood loss is associated with each aspect of the blood sampling process. RESULTS: The mean total volume of blood sent to the laboratory for testing was 257.4 ml. As a result of "clearing the line" over the 7-day period, patients with the conventional arterial line system lost a mean volume of 340.2 ml of blood mixed with heparinized saline solution more than patients with the blood-conserving arterial line. In terms of the blood component of the blood-heparinized saline solution mixture, use of the conventional arterial line was associated with an average of 156.8 ml more blood discarded than with the blood-conserving arterial line. CONCLUSION: The new blood-conserving arterial line system provides a simple and effective method for reducing blood loss related to diagnostic sampling in the critical care setting.

[The observation on analgesic effect of moxibustion in rats].

Radiant heat exposure on Ciliao point was taken as radiant heat moxibustion (RHM). When temperature of surface of point was modulated within the range of 38-39 C and 43-44 C, the latency of tail flick threshold (LTH) in rats was increased by 17.8 +/- 2.1% and 22.2 +/- 2.5% after 5 min. and by 16.1 +/- 2.9%, 22.1 +/- 3.4% and 21.9 +/- 3.2% (50-52 degrees C) after 10 min. respectively. LTH was increased by 19.8 +/- 3.1% after 10 min. with needing plus moxibustion. The change of latency after RHM in every group and needing with moxibustion group had statistical significance (P < 0.05). There was no difference of analgesic effect among all groups (P > 0.05). The analgesic effect of RHM on Ciliao point was more powerful than Zusanli and Zhiyang (P < 0.05).

Coloanal anastomosis in radical surgery for low rectal carcinoma by intra-anal circular ligation.

Using self-made special supporting tube, coloanal anastomosis was performed by intra-anal ligation for radical surgery in 48 patients with low rectal carcinoma. The anastomoses were completed without suturing, and the results were quite satisfactory. Follow-up after operation revealed that the bowel movement and the sphincteric function were excellent in all patients. The anal canal and the anal sphincter were all kept intact and abdominal colostomy was not required for decompression. Anastomosis without suturing, least trauma, and short operation time made blood transfusion unnecessary during the operation. This technique is actually derived from Park's operation, and is considered a promising procedure for coloanal anastomosis in the radical surgery for low rectal carcinoma.

[Clinical observation of the superoxide dismutase (SOD) and lipid peroxide (LPO) changes in oral-maxillofacial surgical patients].

This article reports examination of superoxide dismutase (SOD) activity in RBC and lipid peroxide (LPO) content in the plasma of the oral-maxillofacial operation patients. The results showed that the LPO content in postoperation ascended remarkably compared with pre-operation (P < 0.001) and that the SOD activity rose also (P < 0.05). It was discovered that the difference of LPO content between the pre-operation and post-operation was closely relative with the difference of SOD activity, the operative time and the operation method (intra- or extra-oral approach). The results demonstrated that the operative damage resulted in the increasing of the reactivity of superoxide radical and LPO and that the rising of SOD activity was probably the compensation of the body.

[Effects of thyroxine and methimazolum on monamine transmitters in preoptic/anterior hypothalamic area and peripheral serum in rats].

The contents of monamine transmitters in preoptic/anterior hypothalamic (PO/AH) area and in the serum of rats injected with thyroxine (T4, 1 mg/100 g body wt/d, for 10 d, hypodermic i.) and fed with methimazolum (10 mg/100 g body wt/d, for 15 d) were assayed by high pressure liquid chromatography with electrochemical detector (HPLC-ECD). It was found that the content of dopamine and homovanillic acid in PO/AH area showed significant increase (P less than 0.01) associated with a slight rise (P greater than 0.05) of 5-hydroxytryptamine (5-HT) and 5-hydroxyindoleacetic acid (5-HIAA), and with no change of norepinephrine in the thyroxine group. After fed with methimazolum, the content of norepinephrine decreased significantly (P less than 0.05), but no obvious changes occurred in dopamine, homovanillic acid, 5-HT and 5-HIAA. By synchronous analysing of monamine transmitters in peripheral serum, it was showed that there was no significant linear relationship between the changes of monamine transmitters in the brain and in the serum. The correlation between thyroxine and methimazolum on the content of monamine transmitters and on the change of body temperature was also discussed.

[Studies on the effect of immunoenhancing reagent "425" on egg granuloma formation in mice infected with Schistosoma japonicum].

Fourteen--34 days after Schistosoma japonicum-infected mice were injected intraperitoneally with immunoenhancing reagent, Chinese angelica root extract ("425"), the specific antibody (IgG) levels were significantly higher in Group III (362.67 +/- 162.21 - 480.00 +/- 289.45) than the control group (66.67 +/- 39.68 - 245.33 +/- 101.56) in experiment one, and in Group II (488.00 +/- 320.38 - 768.00 +/- 267.38) than the control group (256.00 +/- 189.07 - 394.67 +/- 141.06) in experiment two. The egg granulomatous formation were markedly diminished in the injected mice. The ratios of granulomas vs. eggs' mean diameters of Group III of mice were 5.24 +/- 1.04 - 3.95 +/- 0.77 and those of the control group were 6.59 +/- 1.19 - 5.29 +/- 0.94 in experiment one, those of the group II were 3.75 +/- 0.71 - 4.15 +/- 0.73 and those of the control group were 4.94 +/- 0.81 - 5.36 +/- 0.97 in experiment two. Meantime the egg antigen levels in the injected mice might be lower. This study shows that the control of immunomodulation of granulomatous formation in the hosts injected with immunoenhance reagent "425" can be induced.

[Comparison of the detecting mutagenic effect of two short microbiological tests].

With pre-incubation and plate-incorporation, the responses of Arar forward mutation assay with S. typhimurium SV50 strain to 12 chemical mutagens were compared with those of four standard test strains (TA 97, TA 98, TA 100 and TA 102) in Ames test. The positive mutagenic results obtained from strain SV50 were in accordance with those from the four strains in Ames test. The mutagenicity of glutaraldehyde is the first reported in China. It is recommended that Arar forward mutation assay with strain SV50 can be regarded as one of the method of choice to screen chemical mutagens.

[Imaging analysis of antiulcer action and the active constituent of Atractylodis rhizoma].

Examination of the sizes of gastric mucosal injury has been conducted by visual means. However, the human visual-eye examination may sometimes involve personal biases and errors. In order to reduce these errors, automated analytical apparatuses have been developed. The equipment is generally expensive. In the present study, a low-cost imaging analytical method to examine the antiulcer action of drugs is reported. By using our imaging analytical method, an antiulcer effect of herbal medicine was examined on hydrochloric acid-ethanol induced gastric mucosal injury. Acetone extract of Atractylodis Rhizoma was found to have antiulcer action. In addition, the examination of fractions of the acetone extract, obtained through column chromatography, indicated that atractylon, at 17.5 mg/kg, p.o. significantly inhibits gastric mucosal injury.

[Localization of antigen and antibodies in the liver and intestine of mice with Schistosoma japonicum].

Indirect fluorescent antibody test was used to localize the antigen in the livers and intestines of mice with Schistosoma japonicum. No antigen was detected 4 weeks p.i.. There was a significant rise in antigen level 6 weeks p.i. which reached peak level 10-12 weeks p.i., the GMRT being 512 and 483, respectively. The eggs in the tissues showed strong fluorescent reaction 6 weeks p.i.. The mean diameter of schistosome egg granulomas in the livers and intestines of infected mice was remarkably enlarged 6 weeks after infection. The mean diameter of egg granulomas in the livers was markedly larger than that in the intestines. The antibodies were localized in the livers and intestines of infected mice as shown by the direct fluorescent antibody test (DFAT) and IFAT. The results indicated that the levels of IgG, IgM and IgA antibodies appeared 6 weeks p.i. which went on a significant increase at the 10-12 weeks. The IgG antibody level was higher than that of other two antibodies. It is suggested that the IgG may be the main antibody in the immunologic response of Sj egg granulomas.

[Studies on dynamic of antibodies detected in sera from jirds infected with Brugia malayi before and after treatment].

Indirect immunofluorescent antibody test (IFAT) and immunoenzymatic staining technique (IEST), using frozen section of Brugia malayi and Setaria cervi adult worms, were applied to detect levels of IgG and IgM antibodies in jirds infected with Brugia malayi before and after treatment. Both methods could show the dynamic of antibodies during the course of infection. The peak of IgG and IgM antibodies were at the 12-14th week and 2-6th week after infection respectively. A high correlation was observed between the levels of IgG antibody and the period of infection, whereas the antibody titer had no relation with the density of infection. During 6 months after treatment, the levels of antibodies detected by IFAT and IEST using two antigens decreased to low titre. It is considered that IFAT and IEST using heterologous and homologous antigens could be equally used for the serodiagnosis and the evaluation of cure in filariasis.
