Level of anastomosis and anorectal manometry in predicting function following anterior resection for adenocarcinoma.

The clinical and physiological consequences of sphincter preservation after resection of rectal carcinoma at various levels were evaluated. Thirty-two patients (mean age 59.2 years; range 31-79 years) who had undergone curative surgery at least one year previously and were not given adjuvant therapy, were studied with 19 normal controls. Three blinded observers independently assessed clinical sphincter function, level of anastomosis (ANL) and anorectal manometry. 85.7% had an increase in stool frequency but their normal lifestyles were unaffected. The mean stool frequency was 3.1 (range 2 to 8) times/day. The anal maximal basal and maximum squeeze pressures, rectal volume of first sensation (VIS) and urge, maximum tolerable volume and compliance were all significantly impaired (P < 0.05). Patients with poorer function had significantly lower ANLs (P < 0.05). On multiple regression analysis, the ANL and the VIS significantly predicted stool frequency (P < 0.05). ANL below 6 cm was significantly associated with impaired frequency of stools (P < 0.05). This may be a subgroup of patients whose residual rectal reservoir function is compromised and may theoretically benefit from colonic pouches.

A simple and effective treatment for hemorrhagic radiation proctitis using formalin.

Radiation proctitis is a common complication of radiotherapy for pelvic malignancy. In the more severe form, it leads to intractable or massive hemorrhage, which may require repeated hospital admissions and blood transfusions. Medical therapy in patients with radiation proctitis is usually ineffective, whereas surgery is associated with a high morbidity and mortality. Eight patients (seven females and one male) with hemorrhagic radiation proctitis were treated over a six-month period with endoluminal formalin. The technique used ensured minimal contact with formalin. The median age of the patients was 68 years (range, 42-73 years). Seven patients had had cancer of the uterine cervix, and one patient had had cancer of the prostate treated with radiotherapy at a median time of 30 months (range, 9-46 months) previously. The median duration of time of symptomatic rectal hemorrhage before formalin therapy was eight months (range, 1-12 months). The median number of units of blood transfused previously per patient was four (range, 2-32). The time taken for formalin therapy was 20 minutes (range, 10-70 minutes). One patient required repeat formalin application at two weeks. Bleeding ceased immediately in seven patients after formalin treatment. No further bleeding was noted, nor was any blood transfusion needed, at follow-up at four months (range, 1-6 months). Formalin therapy is a simple, inexpensive, and effective treatment for hemorrhagic radiation proctitis.

Preliminary study on the anti-radiation effect of jen-sheng-yang-yung-tang.

Six to seven week old male mice of ICR strain were exposed to different doses of x-rays to determine if jen-sheng-yang-yung-tang could be a modifier in the elimination of radiation damage. Colony forming units of bone marrow cells in the spleen (CFUs) were measured before and after x-ray irradiation with intraperitoneal injection of 10 mg/20 g or 20 mg/20 g body weight of jen-sheng-yang-yung-tang, once a day for seven consecutive days. The recovery of CFUs and hemocytes counts by 4 Gy irradiation with jen-sheng-yang-yung-tang administration was faster for a concentration of 20 mg/20 g than 10 mg/20 g. The measurement of 10-day CFUs showed an increase of radiotolerance in the treatment of 20 mg/20 g administration before x-ray irradiation. The injection of jen-sheng-yang-yung-tang accelerated the recovery of hemocyte counts in mice irradiated with 4 Gy x-ray; the effect was especially profound for leukocytes with 20 mg/20 g jen-sheng-yang-yung-tang administration after irradiation.

Commissioning Varian 2100C medical linear accelerator.

One Varian 2100C medical linear accelerator (LINAC) was installed at Kaohsiung Medical College Hospital last summer. The outputs of this LINAC are 6 and 10 MV X-ray beams, and 6, 9, 12, 16, and 20 MeV electron beams. A commissioning procedure including acceptance tests, dose data, environmental radiation survey was executed when the LINAC installation was completed. The acceptance tests organized as a sequential were performed by the medical physicist and Varian's engineers. Dose determination and environmental radiation survey were measured with ion chamber, phantom and survey meters. Radiation dose was calculated following the protocal of the Task Group 21, Radiation Therapy Committee of the American Association of the Physicists in Medicine (AAPM). After the commissioning procedure, the license application to operate the LINAC was submitted to the Atomic Energy Council (AEC) of Executive Yuan. When the license was authorized by the AEC last November, a quality assurance (QA) program was established and performed continuously in order to keep the Varian LINAC with high quality of radiation therapy.

Radiotherapy in the treatment of hepatocellular carcinoma and its metastases.

A study was conducted to evaluate the effect of external radiation therapy on hepatocellular carcinoma (HCC) and its metastatic lesions. A total of 33 patients with cytopathologically proven HCC were subjected to radiation therapy over a 4-year period, and treatment was discontinued in 8 cases due to jaundice, severe discomfort, or early mortality. Thus, 25 patients with 28 lesions underwent irradiation with a total dose ranging between 3000 and 5600. Of these, seven were irradiated for liver tumors, and the results showed that two lesions decreased in size, the symptoms improved in 1 case, and another patient maintained stable disease for 4 months. Among the 21 metastatic lesions treated, only 2 patients failed respond to the treatment. Nine subjects were irradiated for bone metastases, and the bone pain subsided in all but one case. The survival for bone metastasis was as long as 23 months when the primary tumor was treated effectively. Three of the four cases of irradiated skin nodules disappeared and had not recurred after 5 months, 1 year, and 4 years, respectively. Tumor shrinkage or symptoms of relief were noted for three abdominal lymph nodes, one neck lymph node, one pleural tumor, and one lung tumor. Clinical improvement associated with a stable lesion was observed in two patients with brain metastasis. Follow-up revealed regrowth of the tumor or recurrence of symptoms in most of the patients. However, none of the patients died as a direct result of a metastatic lesion. Although external radiation therapy is palliative in intent, it appears to be useful in the treatment of HCC and its metastatic lesions.

Prospective, randomized trial comparing pain and clinical function after conventional scissors excision/ligation vs. diathermy excision without ligation for symptomatic prolapsed hemorrhoids.

Forty-nine consecutive patients with symptomatic prolapsed hemorrhoids were prospectively randomized for conventional scissors excision with ligation (Group A; n = 16) or diathermy excision without ligation (Group B; n = 33). The median time taken to complete the procedure was 20 minutes (range, 10-40 minutes) and 10 minutes (range, 5-35 minutes) in Groups A and B, respectively (P < 0.05). Length of hospital stay was similar in both groups, with a median of three days and a range of two to five days. The median length of follow-up was 35 weeks (range, 20-50 weeks) and 35 weeks (range, 20-51 weeks) for Groups A and B, respectively. There was no statistical difference in the severity of postoperative pain between the two groups. The use of postoperative oral analgesics was significantly lower in Group B (P < 0.02), but there was no significant difference in the demand for intramuscular or topical analgesics. Diathermy excision of hemorrhoids is significantly faster than scissors excision, there is less bleeding, the vascular pedicles need not be ligated, and there is significant reduction in the requirement for oral analgesics postoperatively without any increase in early or late postoperative complications.

Duplication of the proximal colon mimicking volvulus: a case report.

The case of a 56 year old female with intermittent pain, weight loss, anaemia, and a palpable tympanic abdominal mass is reported. Barium enema showed a very redundant loop of proximal colon, which was thought to have undergone recurrent volvulus. At colonoscopy, the findings seemed normal, but a much shorter length of colon was intubated to the caecum than expected from the barium findings. The duplicated colon was discovered only intra-operatively. This unusual diagnosis should be considered when a barium enema shows a long redundant colon which is not confirmed on colonoscopy.

Computerised 3-dimensional vector volume analysis--the role of a new method for assessing anal sphincter competence.

The results of manometric assessment (using conventional parameters) in idiopathic faecal incontinence overlap with normals, especially if the impairment is not severe. Computerised three-dimensional vector volume analysis is a recently introduced method of integrating the pressure profile along the anal canal in multiple radial directions, which may detect minor sphincter weaknesses more accurately. We compared the two methods in 47 subjects (normals = 25, patients with idiopathic faecal incontinence = 22), to analyse the same data derived from a station pull-through manometric technique. The conventional parameters of mean resting and maximum voluntary contraction pressures did not differ significantly between normal and incontinent subjects. The anal canal length in incontinent patients was significantly shorter (mean 2.9 SEM 0.2 cm in normals and 2.0 SEM 0.3 cm in incontinent; p less than 0.5). The computer calculated vector volumes and pressure symmetries (both at rest and during contraction) were not significantly different between the two groups. Although local areas of weaknesses may be identified on computerised vector diagrams, the vector volume calculations gave little additional objective information to the conventional indices to discriminate milder degrees of idiopathic faecal incontinence.

Preliminary study on antiradiation effect of kuei-pi-tang.

In order to evaluate the potential action of Kuei-Pi-Tang as an antiradiation agent, colony forming units of bone marrow cells in the spleen (CFUs) were used. Different sequences of X-ray irradiation with or without Kuei-Pi-Tang administration in the groups of ICR strain mice were intraperitoneally injected 10mg/20g or 20mg/20g, once a day, for consecutive seven days before or after 4Gy X-ray irradiation. After the different treatments, whole blood was collected from the tail endings to observe the fluctuation of leukocytes, erythrocytes and thrombocytes. The administration of 20mg/20g was more effective than that of 10mg/20g. Lower radiosensitivity was observed with the treatment of 20mg/20g of Kui-Pi-Tang than that of 10mg/20g. The injection of Kuei-Pi-Tang accelerated the recovery of blood cell counts of leukocytes, erythrocytes and thrombocytes in mice irradiated with 4Gy, especially for leukocytes under the treatments with 20mg/20g of Kuei-Pi-Tang administered after irradiation.

Obstructed obturator hernia in 90 year olds--a management dilemma.

We present two 90 year-old women with small bowel obstruction from obturator hernias. A higher incidence of such obstruction has been reported in Chinese and the diagnosis should be suspected in elderly women with no previous surgery presenting with small bowel obstruction. Although the diagnosis was suspected in both cases, early surgical intervention was prevented by the patients' and relatives' fears of incomplete recovery with poor quality of life in advanced age. In such situations, we suggest on observation period of not more than two days, and if the general health does not deteriorate significantly, surgery should again be strongly advised.

Blunt abdominal trauma causing a 'degloving injury' to the colon.

A case of an unusual degloving injury of the descending colon resulting from a crushing injury to the mid-torso is reported. The diagnosis was made at laparotomy for associated injuries. The colon was stripped of its muscle layers and mesentery for 10 cm without perforation.

Epstein-Barr virus (EBV)-associated undifferentiated carcinoma of the parotid gland.

A case of undifferentiated carcinoma of the salivary gland occurring in the parotid gland of a southern Chinese was reported. Tumour cells showed immunofluorescence for Epstein-Barr virus (EBV)-associated nuclear antigen, and DNA hybridization demonstrated the presence of EBV-DNA in tumour tissue. The findings in this case, together with previous reports, suggest a causal relationship between EBV and salivary gland carcinoma. The relationships between EBV and undifferentiated epithelial tumours of the salivary glands, nasopharynx and thymus are also discussed.

Treatment of residual and recurrent cervical metastasis from nasopharyngeal carcinoma.

Eleven patients with cervical metastases from nasopharyngeal carcinoma were studied. Even after adequate radiotherapy treatment, viable tumour cells were detected in all patients. Nine patients underwent radical neck dissection and the histological study of the specimens confirmed the operative findings. The extent of the disease was far greater than the clinical assessment suggesting that simple lymphadenectomy is an inadequate form of treatment.

A feasibility study of the in vivo prompt gamma activation analysis using a mobile nuclear reactor.

A facility for in vivo prompt gamma activation analysis using moderated neutron beams from a 0.1 W mobile nuclear reactor is described. The low-power nuclear reactor provides total neutron flux of 3.3 X 10(4)n cm-2 s-1 on the surface of a vertical beam tube to which a liquid phantom is positioned. The capability of such a partial-body irradiation facility is demonstrated by measuring trace amounts of toxic cadmium in kidney. The detection limit of Cd in kidney for a skin dose of 1.66 mSv (166 mrem) is 1.34 mg under 500 s irradiation. This facility therefore combines the advantages of mobility with high sensitivity of detection of a toxic element under low neutron and gamma doses.

Posttraumatic acute arterial subdural hematoma manifested during anesthesia.

Traumatic acute arterial subdural hematomas are rare and are associated with a high mortality rate. The authors report a case in which the initial computed tomographic (CT) scan examination failed to reveal a hematoma. Within the subsequent 4 hours, while recovering from anesthesia given so that hemostasis of bleeding soft tissue injuries could be obtained, the patient suddenly developed bilaterally dilated pupils, both nonreactive to light. A repeat CT scan then revealed large subdural and intracerebral hematomas. The case is discussed and the pertinent literature is reviewed.
