Linear cutaneous neuromas (dermatoneurie en stries): a limited phakomatosis with striated pigmentation corresponding to cutaneous hyperneury (featuring multiple endocrine neoplasia syndrome?).

A 60-year-old woman developed a progressive linear pigmentation on the trunk. Skin biopsy demonstrated an increase in the number and size of neural fibers in the dermis. Clinical and paraclinical investigations of this unusual disease showed findings similar to the hereditary type of Sipple syndrome (multiple endocrine neoplasia syndrome, Type 2b). Such findings as Marfanoid habitus, abnormal electromyography and hypertrophy of the corneal nerves suggest that our patient's disease and Sipple syndrome are identical. However, such other symptoms of Sipple syndrome as presence of endocrine tumors are absent. Questions of long-term prognosis and physiopathogenicity are raised, since no nerve growth factor was detected in this patient. The name of linear cutaneous neuromas is suggested for the patient's lesions, and a possible association with the pigmentation and cutaneous neuromas of the MEN 2b syndrome.

Biopsy

Solitary Langerhans cell histiocytoma.

Biopsy of a solitary tumor of the buttock in a 3-month-old girl was diagnosed as a histiocytic proliferation suggestive of histiocytosis X. Electron microscopy showed Birbeck granules and dense myelinlike bodies within the cytoplasm of the tumor cells. An immunoperoxidase study, using a panel of monoclonal antibodies (OKT6, OKT4, and OKM1) and a polyclonal anti-S100 protein antibody, showed positive staining for OKT6 and OM1 and moderately positive staining for OKT4 and S100. After surgery to remove the tumor, visceral involvement could not be demonstrated during a 20-month follow-up.

Buttocks

Increased resistance to Salmonella infection of hypoferremic mice fed a low-protein diet.

BALB/c and CBA/CA mice fed a protein-deficient diet developed a plasma hypoferremia corresponding to a 30 percent lowering of serum iron concentration. This hypoferremia persisted as long as the diet was maintained. Hypoferremic CBA/CA mice had increased resistance to Salmonella typhimurium C5 infection, as shown by the reduced lethal activity and the decreased growth of the bacteria in the spleen and in the peritoneal exudate of the deficient animals. This induced resistance was abolished after injection of iron or Desferal into the restricted animals. Such resistance was not observed with BALB/c mice fed a protein-deficient diet, in spite of the plasma hypoferremia. The growth of S. typhimurium C5 in the spleen and in the peritoneal exudate of these animals did not differ from the growth observed in control animals fed a protein-sufficient diet. This study suggests that hypoferremia induced by a protein-deficient diet is probably involved in the enhancement of resistance of CBA/CA mice to Salmonella infection, and that the phenomenon is host-strain dependent.

[Role of iron and a siderophore in the development of bacterial infections in the mouse: the study of two bacteria and three routes of inoculation].

We have studied the influence of ferric iron and the influence of a siderophore on the lethal activity of two Bacteria injected in Mice intraperitoneally intravenously or orally. Iron and the siderophore significatively increase the lethal activity of an avirulent Bacteria, Enterobacter cloacae, injected intraperitoneally or intravenously. The virulence of a Bacteria pathogen for Mice, Salmonella typhimurium, is also increased when injected intraperitoneally. Therefore we have noted a decrease in virulence when this Bacteria was injected intravenously or orally in Mice treated with the siderophore.

[The "bad mother" to the vulnerability of the parent child relationship].

This paper summarizes the results of a longitudinal study on young asthmatic children and particularly on the mother-child relationship in this illness: At both observation times--14 to 30 months and 4 to 6 years--the majority of the children appeaed to be developing in a very adequate manner, both in the area of autonomy and opposition. Similarly, in most situations, the mothers appeared quite adequate in the face of their child's illness, and mother-child relationships, generally, seemed harmonious. However, a certain vulnerability in the area of opposition was more marked in a few cases, more frequently between 4-6 years than between 14-30 months. The author attempts a brief history of the development of the concept of the "bad mother" who has become a scapegoat not only in childhood asthma but also in many illnesses encountered in child psychiatry. In questioning the role which may have been played by dynamic theories in this evolution, psychoanalytic writings are reviewed (for example, S. Freud, M. Klein, Spitz, Brody) and it is hypothesized that psychoanalytic theory has had more of a catalytic than a causal influence in relation with a strong tendency in our culture to the eclosion of negative attitudes toward the mother in her relationship with her child. The emphasis placed by North American culture on the nuclear family, the hypertrophy of the maternal function coupled with a paternal role much reduced in time, as well as the almost complete disappearance of maternal substitutes, have all probably greatly influenced the development of this concept of the "bad mother". The author notes however, that happily, in recent years some child development theories insist on the concept of mother-child interaction and on the mutual role that mother and child have on one another in their exchanges. A review is made of the work of Sander, Bowlby, Schaffer, Lewis and Rosenblum which demonstrates this new trend. The author also studies the question of vulnerability within parent-child relationships, leading to the appearance of child psychopathology, and discusses the development of methods which could help in its early detection. Recent attempts in this direction are reviewed, particularly those of Broussard, Gray et al., Lézine. The importance of Thomas and Chess' work is particularly underlined, which emphasizes the specific interaction between the temperamental characteristics of the child and the parents' specific responses to them, at all levels of development. More recent studies by Brazelton et al. and by Korner are also reviewed, suggesting a very early interaction between mother and child. The author concludes by briefly developing the preventive implications of these works and stressing the importance of closely following the efforts made at pre- and post-natal levels which may have much influence in the early prevention of psychopathology in parent-child relationships.

Asthma

[Ulstrastructural study of various evolutive stages of pityriasis versicolor (author's transl)].

Small size, less numerous and sometimes clustered melanosomes, were seen within the melanocytes. The others organites alos showed abnormalities: smaller mitochondria, abundant cytofilaments. Sometimes total degeneration of the melanocyte was seen. These alterations are variable, depending upon the evolutive stage of pityriasis versicolor: either hypopigmented undergoing treatment or not, or hyperpigmented. Reactions of several cellular types of the dermoepidermal unit could be observed: more numerous Langerhans cells located near the base of the epidermis, abundant nerve fibers in the superficial dermis and active fibroblasts of foetal type. A slight inflammatory infiltrate was present in some cases. It is suggested that a toxic effect of the fungus is responsible for the changes of the morphological aspects of the melanocyte.

Adult

[Hypomelanosis of the skin and malignant melanoma (author's transl)].

8 patients suffering from malignant melanoma have been found to have various degrees of hypomelanoses of the skin. Depigmentation of the skin and/or hair associated with malignant melanoma may take several forms. Schematically six clinical types are described: - vitiligo-like hypomelanoses distant from cutaneous tumors; - vitiligo-like depigmentation associated with uveitis (incomplete Vogt-Koyanagi); - hypomelanoses in the primary tumor and/or its metastases; - "halo melanoma" in which the hypomelanoses encircles the primary tumor and/or its metastases; - halo nevi around preexisting benign nevocellular nevi; - "non-specific depigmentary phenomen" distant from the primary tumor or its metastases. Histological and ultrastructural studies of the depigmented skin show an absence of melanocytes. The association of hypomelanoses of the skin with malignant melanoma is probably not coincidental. Various hypothesis are discussed as well as the prognostic significance of such an association.
