[The apolipoprotein E gene polymorphism in children with steroid-resistant idiopathic nephrotic syndrome].

OBJECTIVE: To study apolipoprotein E gene polymorphism in children with steroid- resistant idiopathic nephrotic syndrome (SRINS). METHODS: Sixty children with SRINS (including 9 children with focalsclerosing glomerulonephritis) were observed in comparison with 80 age-sex-matched healthy children. Seven lipoprote in metabolism parameters including serum total-cholesterol(TC), triglyceride (TG), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C), apolipoprotein AI(apoAI), apolipoprotein B (apoB), lipoprotein (a)[Lp (a)] were detected by enzyme method, and single-strand conformation polymorphism(PCR-SSCP) was used to determine the apoE genotypes in the two groups. RESULTS: Serum TC, TG, HDL-C, LDL-C, apoAI, apoB, Lp(a) in the observation group were significantly higher than those in the healthy controls (P<0.01). After 6 months, the levels of serum TC, TG, LDL-C,apoB,Lp(a) were 81.25%, 78.13%, 84.38%, 87.50% and 78.13% respectively in 32 SRINS cases of the observation group. The apo epsilon 2 allele gene frequence was higher in the observation group than in the controls (11.66% vs 5.00%, P<0.05). The apo apsilon 2 allele gene frequence in the cases of focalsclerosing glomerulonephritis was significantly higher than that in the controls (22.22% vs 5.00%, P<0.01). CONCLUSION: The above results indicate the obvious and long term abnormalities of serum lipoprotein metabolism in the children with SRINS. The apo apsilon 2 allele gene frequence is higher in the children with SRINS. There are risk factors for atherosclerosis and cardiovascular diseases in the children with SRINS. The antilipemic drugs should be considered in use for the children with SRINS.

Effect and mechanism of electronic magnetic pulse on peripheral lymphocytes in dogs.

OBJECTIVE: To study the effects of electronic magnetic pulse (EMP) on peripheral lymphocytes in dogs and to explore the mechanisms of the biological effects of EMP. METHODS: T, TH and TS lymphocytes were estimated by acid phosphatase cytochemistry. Apoptotic lymphocytes and Bax and Bcl-2 proteins related to apoptosis were observed with in situ terminal labeling and immunocytochemistry. RESULTS: Peripheral T lymphocyte subpopulations decreased obviously after EMP irradiation with (2-12) x 10(4) V/m. Apoptotic percentages of lymphocytes increased with the elevation of EMP doses. Ten days after different intensity radiation, the Bax protein was found to be elevated in accord with the peak value of lymphocyte apoptosis. However, Bcl-2 protein decreased obviously. CONCLUSION: A definite field intensity EMP could induce injury to lymphocytes. Apoptosis induced by EMP is one of the main causes of peripheral lymphocyte death and leads to immunosuppression of the body. These results suggest that people should pay more attention to the injury caused by EMP, especially to the immunological functions of the body.

[Comparative study of expression of homeobox gene Msx-1, Msx-2 mRNA during the hard tissue formation of mouse tooth development].

OBJECTIVE: To observe and compare the expression pattern of Msx-1, Msx-2 mRNA during the different stages of hard tissue formation in the first mandibular molar of mouse and investigate the relationship between the two genes. METHODS: First mandibular molar germs from 1, 3, 7 and 14-days old mouse were separated and reverse transcription-polymerase chain reaction was performed on the total RNA of them using Msx-1, Msx-2 specific primers separately. RESULTS: Expression of both genes were detected during the different stages of hard tissue formation in the mouse first mandibular molars, but there was some interesting differences in the quantitiy between the two genes. Msx-1 transcripts appeared at the 1 day postnatally, and increase through 3 day, 7 day, then maximally expressed at 14 days postnatally; while Msx-2 mRNA was seen and expressed maximally at the 3 days postnatally, then there was a gradual reduction at 7 days, and 14 days postnatally. CONCLUSIONS: The homeobox gene Msx-1, Msx-2 may play a role in the events of the hard tissue formation. The complementary expression pattern of them during the specific stage of hard tissue formation indicates that there may be some functional redundancy between them during the biomineralization.

[The role of TGF-beta on Tca83--a tongue squamous cell carcinoma cell line].

OBJECTIVE: To investigate the role of TGF-beta on the growth of Tca83 cells. METHODS: The cell culture, MTT detection, immunohistochemistry and in situ hybridization detection methods were used. RESULTS: After 96 h, compared with the control, most groups with low concentrations of TGF-beta (0.2, 1.0, 5.0, 12.5 micrograms/L) showed growth inhibition, while the group with high concentration of TGF-beta (25.0 micrograms/L) showed no growth inhibition. Positive stain of TGF-beta 1, T beta R I mRNA and TGF-beta 1, T beta R I, T beta R II protein could be detected in Tca83 cells. CONCLUSIONS: Tca83 cell line has the potentiality to secret TGF-beta 1. Autocrine and paracrine of TGF-beta 1 could regulate cancer cell's proliferation and differentiation directly. Since the ligand-receptor conduction in Tca83 cell line is intact, external TGF-beta 1 could combine with the T beta R I and T beta R II receptor and exert its inhibitory effect on Tca83 cell line.

[Second primary lung cancer in laryngeal cancer patients: report on 36 cases].

OBJECTIVE: To discuss the development, treatment and prognosis of a second primary lung cancer in laryngeal carcinoma patients. METHODS: A total of 36 patients with a second primary lung cancer was discovered in 2,182 laryngeal cancer patients. For comparison, a group of 15,541 lung cancers was also reviewed for the presence of a second primary laryngeal cancer. All patients have been followed for over five years. RESULTS: From 1958 to 1999, out of 2,182 laryngeal carcinoma patients, 36 synchronous (3) or metachronous (33) second primary malignancies of the lung were found. This represented 1.65% of all largngeal cancers observed or 45.0% of all the second primary cancers developed. Their pathology was: squamous cell carcinoma (32-88.9%), adenocarcinoma(2-5.6%), small cell carcinoma(1-2.8%) and large cell carcinoma (1-2.8%). The c-TNM stages were: stage I (7-19.4%), stage II (12-33.3%), stage III (9-25.0%) and stage IV (8-22.2%). The 2- and 5-year survival rates were 41.7% and 8.3%, with an average survival of 23 months. The interval from the treatment of the initial laryngeal carcinoma to the development of the new second primary carcinoma of lung ranged from 1 to 14 years, with an average of 44 months. CONCLUSION: Synchronous or metachronous second malignancies of the lung are sometimes encountered in laryngeal cancer patients while the occurrence of a laryngeal second primary following a lung cancer is not observed. The development of the second primary lung cancer is not related to the treatment of the initial laryngeal malignancies. Surgery plus external radiotherapy provides better results than surgery or radiotherapy alone.

[A study of hepatitis B virus(HBV) anti-genome and its inhibitory effect on HBV replication].

OBJECTIVE: To study the combination of triplex forming oligodeoxynucleotides (TFO) with hepatitis B virus gene and its effect on replication of HBV. METHODS: An oligodeoxynucleotide (TFO20) which can form triplex with SP1 sites in HBV core promoter was synthesized and was linked with biotin. Both the liposome-TFO20 and naked TFO20 were transfected to HepG2.2.15 cells, which was detected by SABC assay and showed signals that TFO20 was combined with HBV DNA. ELISA, reverse transcript (RT)-PCR and fluorescence quantitative PCR were used to investigate the change of HBV DNA, 3.5 kb/3.4 kb RNA, 2.4 kb/2.1 kb RNA, HBsAg and HBeAg in HepG2.2.15 cells before and after treatment with TFO20. RESULTS: The ratio of liposome-TFO20 and naked TFO20 transfected to HepG2.2.15 cells were 80% and 40%, respectively. TFO20 signals mainly distributed in the nucleus and cytoplasm. After treatment with TFO20, the level of HBV DNA was reduced from 10(7.01) to 10(4.94), while 3.5 kb/3.4 kb RNA, 2.4 kb/2.1 kb RNA, HBsAg and HBeAg were also reduced by 70.2%, 31.6%, 37.0%, 78.2% respectively. CONCLUSIONS: TFO20 can be transfected into cells by packing with liposome and can be combined with HBV DNA. TFO20 can effectively inhibit replication of HBV.

[A study of mutation(s) of the epithelial sodium channel gene in a Liddle's syndrome family].

OBJECTIVE: To study the genetic mechanism of Liddle's syndrome in a family and help the diagnosis (specially ante-natal) and treatment of Liddle's syndrome clinically. METHODS: (1) Physical examination and measurement of serum potassium, plasma aldosterone and renin activity respectively by biochemical and radioimmunological assays were made for the member of the family. (2) Venous blood samples were collected from the members of the family and total genomic DNA was prepared. One set of specific primers was used for direct polymerase chain reaction, for amplifying C terminus of beta-subunit of epithelial sodium channel (hbetaENaC). Polymerase chain reaction products were subjected to single-strand conformation polymorphism and direct DNA sequence analysis. RESULTS: A new frameshift mutation (1bp, INS, 600G) of the gene of C terminus of beta-subunit of hbetaENaC was found in the Liddle's syndrome family. This mutation introduced a new stop codon at position 605 and deleted the last 34 normal amino acids from the C teminus of hbeta ENaC. Eight genetically affected subjects had severe hypertension and suppressed levels of plasma aldosterone and plasma renin activity; half of them had hypokalemia. CONCLUSION: The frameshift mutation (1bp, INS, 600G) of hbetaENaC gene is the likely cause of Liddle's syndrome in this Chinese family.

[Studies on infiltration mechanism of adenoid cystic carcinoma: haptotaxis function assay of fibronectin].

OBJECTIVE: To investigate the invasion mechanism of adenoid cystic carcinoma (ACC) by ACC haptotaxis changes in fibronectin (FN). METHODS: Three strains of ACC cells were used. The haptotaxis changes in FN, and receptor changes in the cell membrane surface were determined. RESULTS: All three strains of ACC cells had remarkably greater migration ability than SCC cell. High level of alpha 2, alpha 3, alpha 6 and beta 1 receptor subunit types was found in the cell surface of ACC and SCC; and high level of alpha 5 in ACC3. High inhibition of haptotaxis reaction was found after blocked alpha 5 and beta 1 receptor subunit in ACC and a middle inhibition after blocked alpha 4. In SCC more strong inhibition was found after blocked alpha 1, alpha 4 and beta 1. CONCLUSIONS: This study indicates that ACC cells have greater invasion ability than SCC cells in FN and haptotaxis reaction of different receptor of tested cells in FN have some feature pattern.

[Alteration of Fas/FasL gene expression in oral squamous cell carcinoma cell line Tca83 by retinoid acid].

OBJECTIVE: To study the induction of apoptosis by retinoid acid(RA) in Tca83 cell line and the expression of correlative Fas/FasL gene. METHODS: Apoptosis of Tca83 was detected by TUNEL, the expression of Fas/FasL-mRNA was detected by RT-PCR and cell immunohistochemical technique. RESULTS: The cell apoptosis number of Tca83 in RA group was significantly higher than that in non-RA groupe on day 5(P < 0.05). The expression of Fas-mRNA in RA group was also much higher than that in non-RA group on day 5(P < 0.05), but the level of FasLmRNR had no change. Immunohistochemistry also showed, after RA treatment, the Fas stain was enhanced. CONCLUSION: The result suggests that apoptosis induced by RA may be resulted from the enhancement of Fas gene transcription and translation.

Prospective survey on neurosurgical intensive care for patients with severe head injury.

OBJECTIVE: To prospectively compare the clinical outcome of intensive care therapy (ICT) with that of conventional care therapy (CCT) in severe head injured patients. METHODS: Patients with severe head injury were assigned randomly into Group ICT and Group CCT, 100 patients in each group. Patients in Group ICT accepted intensive care therapy in neurosurgical intensive care (NIC) unit for the first 2 weeks after admission, while patients in Group CCT accepted conventional care therapy in ordinary ward. The outcomes were evaluated 3 months after injury. RESULTS: There was a significant increase in good recovery (54%) (chi(2)=4.43, P<0.05) and significant decrease of death (25%) (chi(2)=4.50, P<0.05) in Group ICT compared to 39% and 39% in Group CCT respectively. The differences were also confirmed statistically in the following aspects: the patients under 50 years with good recovery pronounced a number increase (chi(2)=7.54, P<0.01), while the mortality in the same range of age was decreased in Group ICT (chi(2)=5.28, P<0.05). The mortality was reduced significantly in patients with GCS for 6-8 on admission (chi(2)=8.47, P<0.01) and in patients with the level of brain stem injured below mesencephalon (chi(2)=4.15, P<0.05). ICT would improve the outcome in patients undergoing conservative therapy only (chi(2)=13.13, P<0.01). CONCLUSIONS: NIC plays an important role in assessing the neurological state, guiding management, evaluating curative effect and estimating the outcome.

[Postoperative use of YAG laser for cataract surgery].

OBJECTIVE: To study and analyze the postoperative use of YAG laser for cataract surgery, the method, effects and complications of laser posterior capsulotomy. METHODS: Four hundred and fifty-one eyes out of 4 600 eyes with phacoemulsification in a single horizontal groove manner were dealt with YAG laser for postoperative complications, and of them, 380 eyes had a posterior capsulotomy with a two-steps method: posterior capsular circular incision and push down of the capsular chip. Forty-two eyes were treated for anterior membrane on the intraocular lens (IOL) by circular resection and peeling it off. The other 29 eyes were treated for different complications. RESULTS: The rate of YAG laser treatment for postoperative complications was 9.8% and of them, the rate of posterior capsulotomy was 8.3%. The rate of eyes with posterior capsular opacification and one or more other complications was 14.2%. The mean energy of a single pulse for posterior capsular resection was (1.21 +/- 0.31) mJ, and the mean total energy was (52.44 +/- 14.62) mJ; the mean energy of a single pulse for push down of the capsular chip was (2.12 +/- 0.43) mJ, and the mean total energy was (50.88 +/- 10.32) mJ. The average naked visual acuity after posterior capsulotomy was increased to 0.58 from 0.32 before the laser treatment, and corrected visual acuity was 0.85. The energy of a single pulse was 1.0 to 1.4 mJ for anterior membrane resection of IOL, and the mean total energy was (56.28 +/- 32.74) mJ. No obvious damage or decentration of the IOL was found. After the treatment, the IOP in 95.3% of eyes was kept in normal range. Only did 2 eyes suffer from retinal detachment in follow-up. CONCLUSIONS: YAG laser therapy for postoperative complications is easy to control, safe, effective, and has less complications. Besides the posterior capsulotomy, some other complications also can be cured with it.
