Effects of enzyme inhibitors of catecholamine metabolism and of haloperidol on the pancreatic secretion induced by L-DOPA and by dopamine in dogs.

1. Effects of inhibitors of DOPA decarboxylase, dopamine beta-hydroxylase and monoamine oxidase, and haloperiodol on the secretion of pancreatic juice induced by L-DOPA and dopamine were studied in preparations of the isolated blood-perfused canine pancreas.2. The increased secretion induced by the infusion of L-DOPA (100 mug/min) was completely antagonized by Ro 4-4602 (300 mug), a DOPA decarboxylase inhibitor.3. The secretogogue effect of dopamine (1-10 mug) intra-arterially was not affected by Ro 4-4602, but was enhanced by the infusion of fusaric acid (100 mug/min), a dopamine beta-hydroxylase inhibitor.4. The increase in the secretion induced by dopamine (1-10 mug) was enhanced by treatment with nialamide (100 mg/kg), a monoamine oxidase inhibitor, given intravenously.5. Haloperidol (1 mg) intra-arterially attenuated the dopamine-induced pancreatic secretion.6. It is concluded that L-DOPA is converted to dopamine in the acinar cells which causes an increase in the secretion of pancreatic juice, thus the intracellular level of dopamine may be controlled by enzymatic equilibrium.

Complete Clq deficiency associated with IgG multiple myeloma.

We report a case of IgG multiple myeloma with selective complete Clq deficiency. The patient was a 75-year-old Japanese woman who exhibited urticaria on the arm and an absence of serum hemolytic complement activity (CH50). Further studies revealed no vasculitis in the urticarial lesion but showed selective complete deficiency of Clq without low molecular weight Clq precipitin. Addition of highly purified Clq restored the CH50 level of the patient's serum to normal. It is suggested that this abnormality was a primary Clq deficiency. We discussed a relationship between the Clq deficiency and myeloma and reviewed the literature.

The tripeptide glycyl-prolyl-glycine amide does not affect the early steps of the human immunodeficiency virus type 1 replication.

OBJECTIVE: To determine whether the peptide glycyl-prolyl-glycine amide (GPG-NH2) corresponding to a conserved motif in the tip of the third hypervariable region of gp120 affected the early events in the human immunodeficiency virus type 1 (HIV-1) replication. DESIGN/METHODS: Glycyl-prolyl-glycine amide was tested for its effect on HIV-1 adsorption, co-receptor usage, proviral DNA synthesis, messenger RNA (mRNA) synthesis and splicing, translation, tat/TAR transactivation, and virus protease activity. RESULTS: Glycyl-prolyl-glycine amide did not appear to affect the early events of the virus replication. HIV-1 having glycine-leucine-glycine instead of GPG in the V3 loop and the mutants deleted of the GPG motif were still inhibited by the peptide. Glycyl-prolyl-glycine-NH2 had no discernible effect on any of the other steps in the virus replication cycle tested. The only effect observed was an increased sodium dodecyl sulfate polyacrylamide amide gel electrophoresis mobility of gp160/120 at high concentrations of GPG-NH2. CONCLUSIONS: The tripeptide GPG-NH2 is a nontoxic compound that inhibits the replication of HIV-1 by an apparently new mode of action.

Haemopathological changes in recurrent deep venous thrombosis of the extremities.

We observed 42 (11.7%) patients with multiple or recurrent thrombosis in 358 patients with deep venous thrombosis of the extremities (DVT) and its sequelae. In those patients with recurrent DVT, following characteristics were found in comparison with patients with recurrent DVT, following characteristics were found in comparison with patients who experienced only one episode of DVT. Collagen disease including Behçet disease are often observed as an important predisposing factor. Relatively high incidence in the young generation. Pulmonary embolism occurred about 4 times as frequent as in patients with single episode of DVT. The platelet adhesiveness increased in 7 (38.9%) cases and the hyperaggregability was also frequently found (34.8% by ADP, 26.1% by collagen), though no significant differences was noted from patients with single episode of DVT. Meanwhile, beta-thromboglobulin increased an antithrombin III decreased with a significant difference. DVT with those characteristics should more carefully be managed.

[Outcome of coronary angioplasty and coronary artery bypass grafting in patients over 75 years old].

Mortality, morbidity, and 3-year survival rates were evaluated in patients aged over 75 years undergoing initial revascularization by percutaneous transluminal coronary angioplasty (PTCA) or coronary artery bypass grafting (CABG). The groups of 74 patients undergoing PTCA and 27 undergoing CABG had similar clinical characteristics including age, sex, emergency operation, prior myocardial infarction, and ejection fraction. The PTCA group contained significantly more patients with single vessel disease (44% vs 8%, p < 0.01) while the CABG group had more three-vessel or left main trunk disease (30% vs 70%, p < 0.01). The patients in the PTCA group demonstrated more prior cerebral vascular events, renal insufficiency, and abdominal aortic aneurysms. Angiographic revascularization was achieved in 112 of 130 lesions (86%) and in 63 of the 74 (84%) patients in the PTCA group. Hospital mortality for the PTCA group was 5.4% (two cardiac deaths and two non-cardiac deaths), but 0% for the CABG group. Myocardial infarction occurred in 1.3% and 3.7%, respectively (p = NS). Three-year survival, excluding hospital deaths, was 90% for patients with PTCA and 96% for those with CABG (p = NS). All these deaths were of non-cardiac origin. Both PTCA and CABG are safe and effective for selected patients over the age of 75 years.

Ambulatory blood pressure and heart rate during once-daily, randomized, crossover administration of carteolol and atenolol.

This study evaluated the effect of beta-adrenoceptor blocking agents (beta-blockers) with or without intrinsic sympathomimetic activity on the 24-hour blood pressure profile of 15 untreated patients with essential hypertension. After a 4-week run-in period, subjects were randomly assigned to an 8-week treatment period of once-daily carteolol (15 mg/d) or atenolol (50 mg/d). The groups were crossed over at week 8. Office blood pressure and heart rate were recorded every 2 weeks and 24-hour ambulatory blood pressure monitoring was performed immediately preceding and at the conclusion of each period. Both drugs significantly reduced (P < 0.01) office blood pressure and heart rate throughout the two treatment periods. The 24-hour ambulatory blood pressure monitoring at 0.5- and 1-hour intervals revealed that systolic blood pressure in 2 of 8 sleeping hours and diastolic blood pressure in 4 of 8 sleeping hours were significantly higher (P < 0.05) after carteolol treatment than after atenolol treatment. The average values for both daytime and nighttime blood pressures, however, were significantly lower at the end of both periods. Although atenolol lowered heart rate throughout the 24-hour period, there was a smaller reduction in heart rate with carteolol than with atenolol during daytime (-5.4 +/- 4.9 beats/min vs -12.7 +/- 6.6 beats/min, P < 0.005, respectively). Heart rate increased during nighttime (P < 0.02) and was significantly greater than with atenolol treatment (5.0 +/- 7.2 beats/min vs -5.7 +/- 8.0 beats/min, P < 0.001, respectively). These results suggest that the different effects of the two beta-blockers on heart rate and nighttime blood pressure may be attributed to the presence or absence of intrinsic sympathomimetic activity.
