Immunohistochemical studies on the distribution of nerve fibers in chronic liver diseases.

Innervation of the liver with or without chronic liver disease was immunohistochemically studied. In normal liver tissues, gamma-enolase [neuron-specific enolase (NSE)]- and S-100 protein (S-100)-positive nerve fibers were found around hepatic arteries, portal veins, and bile ductules in the portal area, along the sinusoid, and around central veins inside the hepatic lobule. Neuropeptide Y (NPY)-immunoreactive (IR) nerve fibers were detected in close contact with hepatic arteries and portal veins in the portal area and along the sinusoid in the parenchyma. Vasoactive intestinal polypeptide (VIP)-IR nerve fibers were found in the portal area but were scarce in the parenchyma. VIP-IR nerve fibers were much fewer than NPY-IR fibers. In patients with chronic active hepatitis, NSE- and S-100-positive nerve fibers, as well as NPY-IR nerve fibers, proliferated in the enlarged portal area. In cirrhotic liver, NSE- and S-100-positive nerve fibers and NPY-IR fibers increased remarkably in the fibrous septa, whereas they decreased or vanished inside the pseudolobules. These findings suggest that, in chronic liver disease, intrahepatic nerve fibers change their distribution, accompanying the structural changes in the hepatic lobules.

[Pre- and post-operative adjuvant chemotherapy of colorectal cancer. Part 1. Drug concentration in tissues following UFT administration].

The concentration of Tegafur, 5-FU and Uracil in tumor and normal tissue were measured in 47 colorectal cancer patients who had been administered UFT (400/mg) for seven days before operation. The concentration of Tegafur, 5-FU and Uracil in tumor was higher than in normal tissue (p less than 0.01), and the concentration of 5-FU was correlative to the concentration of Tegafur and Uracil in both tissues. The Dukes group and histological type were not related to the concentration of Tegafur, 5-FU and Uracil in either tissue. The uptake of each drug was good and high in tumor tissue.

[Correlation of microangiographic findings of colorectal carcinoma with the level of various tumor markers in portal blood].

In order to elucidate the factors responsible for evaluation of various tumor markers (CEA, CA-19-9, TPA, Ferritin) in colorectal cancer, correlation between portal blood values of these markers and microangiography of the tumor was examined. 1) Mean values of these markers in portal blood were higher than those in peripheral blood. 2) Microangiography of tumor coincided well with cancer infiltration. A significant extramural vascular change was found in ulcerative cancers which showed a vertical invasion for colonic wall than in those with horizontal invasion or protuberant cancers. 3) The elevated portal CEA was noted in ulcerative cancers with vertical invasion extending through muscular vessels and/or in tumors with vascular disarray and histological necrotic foci. 4) Elevated portal TPA relationship between portal TPA levels and vascular patterns of tumor. 5) Portal CA19-9 levels changed within normal value in correlation with the degree of vascular change. Portal ferritin values showed wide variations as well as peripheral ones. The present study indicates that the extra or intramural vascular change and vascular pattern of tumor may be a major contributing factor to transfer these markers from tumor into blood circulation.

[Diurnal variation of the QT intervals in idiopathic mitral valve prolapse].

Diurnal variation of QT intervals was analyzed using Holter monitoring for 44 patients with mitral valve prolapse (MVP), and the relationship between ECG changes and autonomic nervous dysfunction was assessed by measuring blood catecholamine during bicycle ergometer exercise and various autonomic functional tests. The following results were obtained. 1. The QT intervals in MVP patients had greater diurnal variations and the averaged QTc was in a high level all the time. 2. Positivity of the autonomic functional tests and the catecholamine level after exercise were significantly high in patients of MVP. 3. After catecholamine depletion by beta-blocking agent (carteolol: 15 mg a day, 5 to 12 months' duration), the variation of QTc became less marked. These data indicate the presence of autonomic dysfunction in patients of MVP.

[Studies of 5-FU concentrations associated with microangiography in colorectal cancer tissue].

In order to elucidate the effect of supplying vessels on 5-FU distribution within a tumor, the correlation between 5-FU tissue concentration and tumor microangiography was examined in 31 patients with colorectal cancer after administration of 5-FU (500 mg/day for 3 days). The 5-FU concentration at the tumor margin was significantly higher than that in the other two tissue areas (p less than 0.05). A low 5-FU concentration was noted in the central region of ulcerative cancer showing vertical invasion with vascular disarray and/or histological necrotic foci. This type of tumor also showed a low RNA index by flow cytometry. The present study indicates that the vascular change and vascular pattern of a tumor may be a major contributing factor to 5-FU transfer from the blood circulation into a tumor.

Pulmonary elastic fiber degradation in paraquat toxicity. An electron microscopic immunohistochemical study.

To study the morphologic alterations of pulmonary elastic fibers in cynomolgus monkeys with paraquat toxicity, peroxidase- and ferritin-labeled antielastin antibodies were used for the light and electron microscopic localization of elastin. One week after paraquat, alveolitis, tissue damage and alveolar dilatation were present; elastic fibers were frayed and more diffusely and intensely stained than those of control animals. In the latter, staining was localized in peripheral regions of the amorphous components and, to a lesser extent, in some microfibrils of elastic fibers. At 3 to 4 weeks, diffuse staining was evident in damaged interstitial elastic fibers and in newly formed elastic fibers in areas of intraalveolar fibrosis. At 8 weeks, the interstitium contained many elastic fibers which showed staining only in peripheral regions of the amorphous components. These observations suggest that: 1) preembedding immunohistochemical staining for elastin is localized in peripheral regions of normal elastic fibers because the antielastin antibody can penetrate into mature and undamaged amorphous components only to a very limited extent; 2) in early stages of paraquat toxicity this staining is more diffuse and intense because elastase from inflammatory cells partially degrades the elastic fibers and permits greater penetration of the antibody into the amorphous materials; 3) in later stages the staining pattern returns to normal as inflammation subsides and elastic fibers are repaired; however, newly formed elastic fibers in areas of intraalveolar fibrosis stain diffusely, reflecting increased penetration of the antibody because of immaturity and incomplete cross-linking, and 4) degeneration of elastic fibers of alveolar walls in paraquat lung may lead to alveolar dilatation, which is associated with irregular fibrosis and constitutes one of the processes of pulmonary structural remodeling in paraquat lung.(ABSTRACT TRUNCATED AT 250 WORDS)

Intranuclear rod-shaped actin filament bundles in poorly differentiated axillary adenosquamous cell carcinoma.

An axillary cystic poorly differentiated adenosquamous cell carcinoma was studied by electron microscopy and immunohistochemistry for keratin, vimentin, desmin, and actin. Many tumor cells had rod-shaped intranuclear fibrillary inclusions which consisted of 60 A microfilaments. These intranuclear inclusions reacted with antiactin antibody and were considered to be F-actin bundles. The cells having intranuclear inclusions were frequently found in areas close to the cystic space, and inclusions of tumor cells close to the cyst were thicker. However, mitotic figures were predominantly found in the periphery of the tumor. Intranuclear actin is known to be mainly soluble G-actin, and one of its important functions is reported to be the chromosome condensation in mitosis. It is speculated that the cells having intranuclear fibrillary inclusions of actin have impairment of mitosis.
